Cystic Lung Disease: a Comparison of Cystic Size, as Seen on Expiratory and Inspiratory HRCT Scans

  • Objective: To determine the effects of respiration on the size of lung cysts by comparing inspiratory and expiratory high-resolution CT (HRCT) scans. Materials and Methods: The authors evaluated the size of cystic lesions, as seen on paired inspiratory and expiratory HRCT scans, in 54 patients with Langerhans cell histiocytosis (n = 3), pulmonary lymphangiomyomatosis (n = 4), confluent centrilobular emphysema (n = 9), paraseptal emphysema and bullae (n = 16), cystic bronchiectasis (n = 13), and honeycombing (n = 9). Using paired inspiratory and expiratory HRCT scans obtained at the corresponding anatomic level, a total of 270 cystic lesions were selected simultaneously on the basis of five lesions per lung disease. Changes in lung cyst size observed during respiration were assessed by two radiologists. In a limited number of cases (n = 11), pathologic specimens were obtained by open lung biopsy or lobectomy. Results: All cystic lesions in patients with Langerhans cell histiocytosis, lymphangiomyomatosis, cystic bronchiectasis, honeycombing, and confluent centrilobular emphysema became smaller on expiration, but in two cases of paraseptal emphysema and bullae there was no change. Conclusion: In cases in which expiratory CT scans indicate that cysts have become smaller, cystic lesions may communicate with the airways. To determine whether, for cysts and cystic lesions, this connection does in fact exist, paired inspiratory and expiratory HRCT scans are necessary.

Augmentation of respiratory muscle activities in preterm infants with feeding desaturation

  • Purpose: Frequent desaturation due to immature incoordination of suck-swallow-breathing in preterm infants can influence multiple organs such as the heart, lungs, and brain, which can then affect growth and development. Most notably in preterm infants, feeding desaturation may even affect pulmonary function during gavage feeding. Because respiratory muscle activities may reflect the work required during respiration, we evaluated the differences in these activities between full-term and preterm infants with feeding desaturation, and investigated the correlations with clinical variables. Methods: Nineteen preterm infants with feeding desaturation (group 1) and 19 age-matched full-term infants (group 2) were evaluated. Oromotor function was evaluated using video recording. The root-mean-square (RMS) envelope of the electromyography signal was calculated to quantify the activities of muscles involved in respiration. The differences in RMS between both groups and the correlation with clinical variables including gestational age (GA), birth weight (BW), and Apgar scores (AS) at 1 and 5 minutes after birth were evaluated. Results: The RMS values of the diaphragm (RMS-D) and rectus abdominis (RMS-R) were significantly greater in group 1 compared to group 2, and the 1- and 5-min AS were significantly lower in group 1 compared to group 2. RMS-D and RMS-R were inversely correlated with GA, BW, 1- and 5-min AS in all infants. Conclusion: This study showed that respiratory muscle activities were augmented during feeding in preterm infants compared to full-term infants. Additionally, respiratory muscle activities were inversely correlated with all clinical variables.

  • Many previous studies based on respiratory characteristics of Idiopathic Parkinson's Diseases (IPD) patients have not controlled related factors appropriately. Accordingly, these studies produced discordant results. Furthermore, there is currently a lack of studies that can provide precise explanations on the characteristics of respiration and phonation. This study included a total of 40 subjects: 20 mixed gender de novo IPD patients ranging in age from 50 to 80 (Hoehn & Yahr stage 1~3), and 20 normal subjects with similar matches for age and gender. All participants were controlled based on their gender, age, height, weight, vocal fold function, cognitive abilities, and depression factors. K-MMSE (Korean-Mini Mental State Examination), nVHI-10 (new Voice Handicap Index), and KGDS (Korean Form of Geriatric Depression Scale) were evaluated to select this study subjects. In order to compare respiratory functions between the two groups, FVC, FEV1, and FEV1/FVC were measured using microQuark, a PC-based spirometer. CSL was used by measure MPT and PAS was used to measure MFR. To investigate the characteristics of phonation ability, CSL was used to measure jitter and shimmer, while PAS was used to measure Psub. In order to compare the respiratory function averages and phonation ability between the two groups, statistical analysis was conducted using SPSS (version 12.0). The results of this study showed that most de novo IPD patients were included in the normal average range of respiratory and phonatory ability. But the respiratory and phonatory ability of de novo IPD patients showed lower tendency as compared with the normal group. When the average of respiratory and phonatory ability among the gender was compared, the difference of males was greater than the difference of females.

How Does the Filter on the Mask Affect Your Breathing?

  • Objective: The purpose of this study was to determine the effect of the difference in mask filters on the respiration rate of healthy people. Design: A randomized cross-over design. Methods: A total of 15 subjects were selected for this study (n=15). After filling out the Physical Activity Readiness Questionnaire, the selected participants abstained from caffeinated beverages and meals 30 minutes before and sat in a chair 10 minutes before stabilizing their breathing. Afterwards, the lung function test was performed 3 times for each mask, and the maximum value was used. The provided masks were Mask Free, Dental Mask, KF80, and KF94. Exhalation was measured for 6 seconds for each mask, and breathing was stabilized by repeating inhalation and exhalation until the next time. Results: In this study, the difference in respiratory function according to the mask type was statistically significant except for FEV1 and FVC (p<0.05). As a result of post-hoc analysis, FVC, FEV1, PEF, and FEF values were significantly lower than those of the control group not wearing a mask (p<0.05). When wearing KF94, FVC, FEV1, PEF25-25%, and FEF were significantly lower than when wearing a dental mask (p<0.05). When wearing a KF80 mask, it was significantly lower in FVC and FEV1 than when wearing a dental mask (p<0.05). In FEV1/FVC, the difference by mask type was not statistically significant (p<0.05), but it was lower than the spirometry standard of COPD patients (FEV1/FVC<0.7). Conclusions: As Now that wearing a mask is essential, it has been confirmed that the mask affects the respiratory rate.Therefore, in the case of healthy adults, it is recommended to rest after wearing a mask if attention deficit or headache occurs. People with low breathing capacity are recommended to have low-intensity activities and frequent rest periods after wearing a mask.

Ginsenosides attenuate bioenergetics and morphology of mitochondria in cultured PC12 cells under the insult of amyloid beta-peptide

  • Background: Mitochondrial dysfunction is one of the significant reasons for Alzheimer's disease (AD). Ginsenosides, natural molecules extracted from Panax ginseng, have been demonstrated to exert essential neuroprotective functions, which can ascribe to its anti-oxidative effect, enhancing central metabolism and improving mitochondrial function. However, a comprehensive analysis of cellular mitochondrial bioenergetics after ginsenoside treatment under Aβ-oxidative stress is missing. Methods: The antioxidant activities of ginsenoside Rb1, Rd, Re, Rg1 were compared by measuring the cell survival and reactive oxygen species (ROS) formation. Next, the protective effects of ginsenosides of mitochondrial bioenergetics were examined by measuring oxygen consumption rate (OCR) in PC12 cells under Aβ-oxidative stress with an extracellular flux analyzer. Meanwhile, mitochondrial membrane potential (MMP) and mitochondrial dynamics were evaluated by confocal laser scanning microscopy. Results: Ginsenoside Rg1 possessed the strongest anti-oxidative property, and which therefore provided the best protective function to PC12 cells under the Aβ oxidative stress by increasing ATP production to 3 folds, spare capacity to 2 folds, maximal respiration to 2 folds and non-mitochondrial respiration to 1.5 folds, as compared to Aβ cell model. Furthermore, ginsenoside Rg1 enhanced MMP and mitochondrial interconnectivity, and simultaneously reduced mitochondrial circularity. Conclusion: In the present study, these results demonstrated that ginsenoside Rg1 could be the best natural compound, as compared with other ginsenosides, by modulating the OCR of cultured PC12 cells during oxidative phosphorylation, in regulating MMP and in improving mitochondria dynamics under Aβ-induced oxidative stress.

  • The vocal fold has three major function-phonation, respiration and protection, and is richly innervated. The vocal 1314 its autonomic innervation-adrenergic and cholinergic from superior cervical ganglion and the vagus nerve, respectively. The action of both system account for vasoregulation and glandular activity. In e vocal fold several kin of neuropeptides, including SP, CGRP, VIP, TH, NPY, ENK have been reported at the animal including cat or dog. But information regarding the distribution of autonomic nerve fibers containing neuropeptides in the human vocal fold is lacking. Two neuropeptides are of special interest : 1) vasoactive intestinal polypeptide(VIP)that is known to be contained in the parasympathetic(cholinergic) neuron. 2) tyrosine hydroxylase(TH)is located in the cytoplasm of noradrenergic neuron and is the rate-limiting enzyme in noradrenaline synthesis. To understand specific autonomic function of vocal fold we did immunohistochemical examination of VIP and TH in the human vocal fold.

  • In the "Somun(素問) Youngranbijeonron(靈蘭秘典論)", that describes the lung as "the office of assisting Heart, the administration come out Lung(相傅之官, 治節出焉)". The means of "the office of assisting Heart" is that Lung assist Heart and execute the Heart's order. The administration come out Lung has two means. The first, Lung administrates and controls the body. The second, Lung controls the Gi and blood(氣血). In the "Somun(素問) Gyeongmaekboulron(經脈別論)", that describes the creation of pectrol Gi(宗氣). The Essence derived from food(穀氣) digested in Stomach comes to Heart, and mixed Lung's Gi of respiration(呼吸之氣), than becomes a pectrol Gi(宗氣). The pectrol Gi(宗氣) controls the Gi and blood(氣血), and we can say that function is the administration come out Lung.

안면부 및 경부 화상 환자의 삼킴 및 의사소통능력 선별 프로토콜 개발을 위한 예비 연구 (Preliminary Study on Developing Test Items of Swallowing & Communication Screening Protocols for Patients with Head and Neck Burns)

  • 안면부 및 경부 화상 환자에게 적절한 영양 공급을 위해 일차적으로 고려하여야 할 요소는 삼킴기능의 평가와 재활치료이며, 이와 더불어 타인과의 효과적인 의사소통과 심미적 기능을 만족시키기 위해 말능력에 대한 자세한 평가와 재활 역시 고려하여야 한다. 본 연구의 목적은 안면부 몇 경부화상 환자의 삼킴장애 및 말운동장애, 음성 장애 등을 종합적으로 파악할 수 있는 의사소통장애 선별프로토콜의 예비 문항을 정리하는데 있다. 화상으로 초래되는 여러 가지 의사소통장애를 다루고 있는 국외 연구들을 참고하여 '구강기제(oral mechanism)', '호흡/음성(respiration/voice)', '삼킴(swallowing)', '조음(articulation)'을 포함한 총 4개의 평가영역으로 나누고 하위범주와 이에 필요한 문항들을 작성하였고, 전문가 5인에 의한 내용타당도 검증 후 예비 문항을 선정하였다. 내용타당도 범위는 .50~.84로 나타났다. 구강기제 및 조음 평가 영역에 포함된 대부분의 선별평가 문항들이 적절하게 나타난 것과 다르게 호흡/음성 및 삼킴 평가 영역의 경우, 전문가마다 조금씩 다른 의견을 보이기도 하였다. 안면부 및 경부 화상 환자가 보이는 의사소통장애의 이질적인 특성 상, 본 연구를 통해 정리된 선별 프로토콜 항목을 바탕으로, 향후 화상 환자의 유형 및 중증도별 평가를 통해 각 환자에게 맞춘 평가 양식으로 적절히 수정하여 활용할 수 있을 것으로 예상한다. 이로써 구강조음기관의 기능적 해부와 생리에 대한 지식이 있고 이에 적합한 재활훈련이 가능한 언어재활사들이 화상 환자의 의사소통장애에 대해 더욱 종합적인 평가와 치료 적용이 가능할 것으로 기대한다.

단감의 최적 Modified Atmosphere포장 규격 설정 (Designing Modified Atmosphere Packaging for Persimmon (Diospyros kaki cv. Fuyu) Fruit Based on Respiration Modelling)

  • 수확후 연화, 흑변, 저온장해 등의 생리장해로 품질이 급속히 저하되는 단감을 장기간 신선도를 유지하기 위하여, $0^{\circ}C$에서 비경쟁억제효소반응속도식으로 조사된 단감 과실의 호흡특성과 저밀도폴리에틸렌필름의 두께별 가스특성을 조사하여 적정수준의 저산소와 고이산화탄소농도를 유지하도록 MA 포장규격을 설정코자 본시험을 수행하였다. 40, 52, 60, 70 ${\mu}m$ 두께의 samples LDPE film의 산소와 이산화탄소 가스투과도는 각각 $Q_{O2}=(2,540{\times}1/L)-16ml{\cdot}hr^{-1}{\cdot}atm^{-1}{\cdot}m^{-2}$, $Q_{CO2}=(13,742{\times}1/L)-70ml{\cdot}hr^{-1}{\cdot}atm^{-1}{\cdot}m^{-2}$이었다. 개당 평균 과중이 $225{\pm}5$ g인 단감을 두께가 각각 40, 52, 60, 70 ${\mu}m$인 저밀도폴리에틸렌필름으로 한개씩 포장하여 $0^{\circ}C$에서 저장하였을 때 효소반응속도식으로 조사된 단감 과실의 호흡특성과 저밀도폴리에틸렌필름의 두께별 가스투과도를 고려하여 시간에 따른 포장내 공기조성의 변화를 예측한 값과 저장 70일후 조사한 실측값은 대체로 일치하는 결과를 보였고, 과육내 Acetaldehyde와 Ethanol의 축적과 생리장해과의 발생에 있어서도 최적 포장조건으로 예측된 두께 52 ${\mu}m$의 포장조건이 실제 저장에서도 적합한 것으로 조사되었다. 이 같은 방법으로 과실의 무게에 따른 적정 MA포장조건을 설정하였다. 포장작업 특별히 주의할 점은 필름의 접착을 완벽하게 하여 핀 홀의 발생이 없도록 해야 한다.

A Study on Ability of Cognitive Impairment According for Decreased Balance Function of Pneumoconiosis Patients

  • Purpose: Pneumoconiosis is a chronic lung disease in which the lungs become hardened due to the accumulation of fine mineral dust that has been breathed in at industrial sites. The lungs are unable to function properly, resulting in complications of several respiratory diseases. Methods: The subjects were 36 elderly adults (pneumoconiosis patients 18, healthy elderly 18) over the age of 65 years. The respiratory function was assessed using a spirometer to measure forced vital capacity (FVC), first second of forced expiration (FEV1), FEV1/FVC, and peak expiratory flow n(PEF). The static balance function test was determined by the center of mass (COM), ankle angle range, hip angle range, sway parameters, and reciprocal compensatory index (RCI). Cognitive function was measured by applying MoCA-k. Results: Comparison of respiration functions between the two groups showed statistically significant differences in FVC, FEV1, FEV1/FVC, and PEFs (p<0.05). Cognitive abilities showed statistically significant differences due to higher cognitive skills in the control group as compared to the patient group (p<0.05). Most results of the static balance capability measurements showed statistically significant differences between the groups (p<0.05). However, no statistically significant differences were obtained for COM AP Range, Angle AP Range, RCI ML Range, and AP Range (p>0.05). Conclusion: The results of the current study indicate that patients suffering from pneumoconiosis have less cognitive ability and balance function as compared to healthy elderly people. Therefore, we expect an improvement in the balance ability and cognitive function through systematic management and strengthening lung function training to improve the quality of life of pneumoconiosis patients.
