Activation and Ratio of Shoulder Stabilizer Muscles on Variations of Manual Resistance during Three Dimensional Shoulder Rehabilitation Exercises (3차원적 어깨재활운동 시 도수 저항의 강도에 따른 어깨안정근의 활성도 및 비율)

  • Purpose: The aim of this study was to compare the activation of shoulder stabilizer muscles to variations of manual resistance during three-dimensional shoulder rehabilitation exercises. Methods: A total of 13 participants were included in this study. To normalize each muscle's activity, a maximal isometric voluntary contraction was performed by all participants. After receiving 30 minutes of training in three-dimensional shoulder rehabilitation exercises, participants randomly performed PNF arm and scapular patterns according to the intensities of manual resistance. The activities of the upper trapezius, lower trapezius, and serratus anterior were measured during these patterns. All exercises were performed for five seconds, and the average of three seconds, excluding the first and last seconds, was used for data analysis. Results: Lower trapezius activity was significant among manual resistance intensities. In both the PNF arm and scapular patterns, using 80% manual resistance of maximum resistance showed higher activity of the lower trapezius muscle compared to 20% of the maximum resistance. Conclusion: It is expected that PNF arm and scapular patterns, with varying intensities of manual resistance, can be used for early rehabilitation of patients with shoulder impingement syndrome.

Difference between absolute and relative muscle strength according to resistance exercise proficiency

  • In this study, the absolute and relative strength of six upper extremity resistance exercises were measured by classifying resistance exercise experts and non-experts. As a result, the skilled group showed higher absolute and relative muscle strength than the unskilled group in the 6 upper extremity resistance exercises. These results are judged to be the hypertrophy of fast-twith muscles, the mobilization of motor units, and the increase in the speed of nerve conduction while the skilled person consistently performs resistance exercise. Experts use intermuscular coordination efficiently to stably perform the load according to the movement and exercise intensity performed during exercise, whereas the inexperienced person uses relatively large muscle groups rather than efficiently using intermuscular coordination. It is considered that exercise motion and load were performed by mobilizing. In addition, as a result of comparing the absolute and relative muscle strength between the 6 types of upper limb resistance exercises, there was a difference between the 6 types of upper limb resistance exercises in the two groups. It can be judged that greater muscle strength and endurance were created through liver coordination.

Effects of Sling and Resistance Rotation Exercises on Pelvic Rotation and Pain in Patients with Chronic Low Back Pain

  • Purpose: This study compared the different sling and resistance exercises on pelvic rotation during active straight leg raises (ASLR) and on pain in patients with chronic low back pain (CLBP). Methods: Twenty subjects were divided randomly into a sling group (SG) and a resistance exercise group (REG). Internal oblique (IO), external oblique (EO), rectus abdominis (RA), and rectus femoris (RF) muscle activity; pelvic rotation angle during ASLR; and visual analogue scale, pressure pain threshold were measured. Sling and resistance exercises were then performed for 30 minutes and the measurements taken again. Results: Both groups showed significantly lower RF muscle activity and significantly higher EO and IO muscle activity (p<0.05). The RA muscle activity decreased significantly in the SG, but increased significantly in the REG (p<0.05). The pelvic rotation angle was significantly lower in the SG (p<0.05). The pain press threshold increased significantly in both groups (p<0.05). The visual analogue scale decreased significantly in the SG (p<0.05). Conclusion: Both exercises appear to be beneficial for modifying the muscle activity and pain control in the intervention of CLBP. On the other hand, the sling was more effective in increasing the pressure threshold than resistance exercise, and the pelvic rotation angle was reduced. Therefore, both exercises can help patients with CLBP change their muscle activity and control pain. CLBP patients should use a sling for short periods of time to learn to reduce the pain and control pelvic rotation.

Effects of Plank Exercises with Resistance of One-Sided Hip Adduction on the Abdominal Muscle Thickness

  • Purpose: The purpose of this study was to examine the effects of plank exercises with resistance of one-sided hip adduction on abdominal muscle thickness. Methods: Thirty subjects were divided into a group that underwent plank exercises with one-sided hip adduction resistance (n=15) and a group that underwent plank exercises only (n=15). Their changes in abdominal muscle thickness before the experiment (n=15) and three and six weeks after the experiment were analyzed using a two-way repeated analysis of variance at a statistical significance level of ${\alpha}=0.05$. When there was any interaction between the time of measurement and each group, post hoc t-tests were conducted at a statistical significance level of ${\alpha}=0.01$. Results: The results of the experiment showed statistically significant differences in the thickness of the rectus abdominis, internal oblique muscle, and transversus abdominis, depending on the time of measurement and the interaction between the time of measurement and each group (p<0.05). Statistically significant differences were observed in the thickness of the external oblique, depending on the time of measurement, the interaction between the time of measurement and each group, and variances between the groups (p<0.05). Conclusion: The results of this study indicated that plank exercises with resistance of one-sided adduction are effective for increasing abdominal muscle thickness. The study's overall findings will likely be used as basic data for lumbar stabilization exercises and rehabilitation treatment.

Effect of lower extremity resistance exercise on gait performance in a patient with systemic lupus erythematosus with cerebral infarction and lower extremity vasculitis: a case study

  • Objective: This study was conducted to investigate the effects of resistant exercise on the gait performance of a patient with systemic lupus erythematosus (SLE) patient. Design: A case study. Methods: A 30-year-old male adult who had been diagnosed with systemic lupus erythmatosus (SLE) in April 2013, right middle cerebral artery infarction, and with left hemiplegia agreed to participate in this case study. Patient was unable to walk due to being affected with adynamia. Due to developing necrotizing vasculitis on the left lower extremity, patient underwent a myotomy on the left thigh. The patient was trained with a progressive resistant exercise program for 8 weeks. An intensity of 15 RM was used for the resistant exercises and the resistance level was increased progressively in order to improve the muscle power of the patient. Methods used to increase resistance included changing positions, providing mechanical resistance instead of manual resistance, transitioning from open kinetic chain to closed kinetic chain exercises, and changing the colors of the theraband to those with increase level of resistance. Outcome measures included the 5-repetition sit-to- stand test (5RSST), Timed Up & Go (TUG), and 10-meter walk test (10MWT). In addition, the GAITRite was used to assess the spatio-temporal gait variables, including gait speed, cadence, stride length of the left side, and double limb support pre and post-intervention. Results: The patient was able to perform sit-to- stand after two weeks of performing the resistant exercises. The patient was able to walk after 4 weeks, and the patient's overall gait performance had improved after 8 weeks. All of the variables had improved after each week. Conclusions: The results of this case study may be used to enhance future efforts to objectively evaluate resistant exercises during gait performance in persons affected by SLE.

Effect of Exercises on the Cerebral Blood Flow (운동이 뇌 혈류 변화에 미치는 영향)

  • The purpose of this study was to analyze the effect given to the variable details which disturb the flow of blood in brain artery disease through Aerobic exercises. It chose the subjects of study : 5 persons in an exercise group and 5 persons in a non-exercise group. Peak brain blood velocity, mean velocity, and resistance & artery stricture ratio were measured with TCD, measure machine for brain artery blood flow. The conclusion of the study was as follows: 1. At pre-test of an exercises group and a non-exercise group, PBV, MBV, BRI, ASI of a non-exercise group were showed much lower in the variation of left common carotid artery(LCCA). 2. At post-test of an aerobic exercises group and a non-exercise group, PBV, MBV, BRI, ASI of a non-exercise group were showed just a little decrease ratio but were not significant different in the variation of left common carotid artery(LCCA). Viewing on the base of these result, continuing exercises promote the functional improvement of the heart blood system and were showed the positive variation of artery stricture ratio according to brain blood flow velocity, the pulse and resistance or blood velocity. Therefore programs for prescriptions through aerobic exercises must be developed in many ways.

The Effect of Lower Trapezius Resistance Exercises on Shoulder Alignment and Muscle Activity in Patient with Rounded Shoulder Posture (아래등세모근 저항운동이 둥근 어깨를 가진 성인의 어깨 정렬과 근활성도에 미치는 영향)

  • Purpose : The aim of study was to determine the effect of lower trapezius muscle strengthening exercises on the height of rounded shoulder posture and muscle activity in patients with rounded shoulder posture. Methods : Thirty-one subjects with rounded shoulders were selected and assigned, with the use of a random number table, to an experimental group (EG, n=16) that received resistance exercises of the trapezius (lower) muscles or a control group (CG, n=15) that received isometric exercises of the lower trapezius muscles. In the EG, with the subject in a prone position, both arms were abducted at the angles suggested for each posture (145 °, 45 ° abduction), with the hands holding dumbbells to assume a functional posture. The CG performed only isometric exercises without dumbbells in the same conditions. At the start signal, with the thumb raised, the arm was raised to the level of the ear. Both groups were assessed by the height of the rounded shoulder posture (RSP) and muscle activity (EMG). The intervention was performed three times a week. Results : The RSP significantly decreased during the intervention period in both the EG and CG (p<.05), and a significant difference was determined between the RSP of the two groups (p<.05). In the EG, the EMG significantly increased during the intervention period (p<.05), but the CG did not show any significant increase (p>.05). The final EMG was significantly higher in the EG than in the CG (p<.05). Conclusion : Both the EG and CG showed positive improvement in rounded shoulder posture during the intervention period. In particular, lower trapezius resistance exercise appears to reduce shoulder height and positively affects the activity of muscles around the shoulder.

Modern Interpretation on Kinesiology of Yangsaeng-Doinbub Presented in "Zhu-Bing-Yuan-Hou-Lun.Yao-Bei-Bing-Zhu-Hou" ("제병원후론(諸病源候論).요배병제후(腰背病諸侯)"에서 제시된 양생도인법(養生導引法)의 현대운동학적 이해)

  • Objectives The objective of this study is to interpretate Yangsaeng-Doinbub presented in "Zhu-Bing-Yuan-Hou-Lun Yao-Bei-Bing-Zhu-Hou" in a modern kineologic approach Methods Based on the interpretation of "Zhu-Bing-Yuan-Hou-Lun Yao-Bei-Bing-Zhu-Hou" and implementation of its kinesiology, this study presents similar kineologies and their purposes, with the reference to various documents on modern kinesiology. Results 1) Yangsaeng-Doinbub presented in "Zhu-Bing-Yuan-Hou-Lun Yao-Bei-Bing-Zhu-Hou" is similar to stretching, active exercise and resistance exercise. 2) Exercises in Yangsaeng-Doinbub presented in "Zhu-Bing-Yuan-Hou-Lun Yao-Bei-Bing-Zhu-Hou", which are similar to resistance exercise, can be used for isometic exercise of cervical extensor. 3) Exercises in Yangsaeng-Doinbub presented in "Zhu-Bing-Yuan-Hou-Lun Yao-Bei-Bing-Zhu-Hou", which are similar to Stretching exercise, has its purpose to stretch quadratus Lumborum, lateral side of body, gluteus Maximus, quadriceps femoris, shoulder extensor, hamstrings, hip joint, ankle dorsi flexor, thoracic rotator,inferior shoulder joint. 4) Exercises in Yangsaeng-Doinbub presented in "Zhu-Bing-Yuan-Hou-Lun Yao-Bei-Bing-Zhu-Hou", which are similar to active exercise, can be used for strengthen exteral oblique. 5) Doctors can make various applications of Yansaeng-Doinbub. For example, it can be used to correct improper low back and neck exercise patterns. 6) Yangsaeng-Doinbub also describes breathing methods, which help normalization of breathing exercises and increase the efficiency of spine exercises. Conclusions The modern interpretation on kinesiology of Yangsaeng-Doinbub presented in "Zhu-Bing-Yuan-Hou-Lun Yao-Bei-Bing-Zhu-Hou" leads to a conclusion that Yangsaeng-Doinbub consists of numourous exercises for various body parts. In particular, breathing methods increase efficiency of such exercises. Plus, the exercises in Yangsaeng-Doinbub can be applied to various uses by doctors.

The Effects of Sling and Resistance Exercises on Muscle Activity and Pelvic Rotation Angle During Active Straight Leg Raises and Pain in Patients with Chronic Low Back Pain (만성 허리통증 환자에게 슬링과 기구저항운동이 통증과 능동 뻗은발올림 동안 근활성도, 골반 회전각에 미치는 영향)

  • PURPOSE: This study was conducted to identify a more effective intervention in sling and resistance exercise for chronic low back pain patients. METHODS: Seventy (70) subjects were randomly divided into the sling group (SG) and resistance exercise group (REG). Muscular activity of the internal oblique (IO), external oblique (EO), rectus abdominis (RA), and pelvic rotation angle during active straight leg raise (ASLR), a pressure pain threshold (PPT) and a visual analog scale (VAS) were measured. Sling and resistance exercises were conducted for 12 weeks. Intermediate measurements were taken after 8 weeks and final measurements were taken after 12 weeks. RESULTS: Both groups showed significantly decreased RA muscle activity and significantly increased IO muscle activity (p<.05). Additionally, EO muscle activity was significantly decreased in the REG, but significantly increased in the SG (p<.05), while the pelvic rotation angle and VAS were significantly decreased in the SG (p<.05). The pressure pain threshold was significantly increased in both groups (p<.05). CONCLUSION: Based on the results of this study, a 12-week intervention seems to be effective at improving back pain in both groups. However, a lower VAS was seen in the sling group after 8 weeks of intervention. Therefore, it is recommended that the sling be applied first when establishing a chronic back pain treatment program to shorten the treatment period and reduce the pain period.

A Comparison of Various Exercises for Scapular Stabilization (견갑골 안정화를 위한 다양한 운동의 비교연구)

  • PURPOSE : This study of 20 healthy male subjects by applying various scapular stabilization exercise to compared Serratus anterior and lower trapezius is change in ultrasound images. METHOD : Thirty healthy subjects voluntarily participated in this study. Ultrasound imaging was recorded from the increasing the activity of Serratus anterior(SA) and Lower trapezius(LT) muscles using Push-up plus, Wall slide, Scapular plane shoulder elevation with resistance exercise. Thickness changes in the Serratus anterior(SA) and lower trapezius(LT) muscles between the relaxed and contracted states in the each exercises. To identify statistical significance, one-way ANOVA with repeated measures was used with the significance level of .05. RESULT : The results of this study were as follows : 1) There were statistically significant difference in thickness changes in the Serratus anterior(SA) and lower trapezius(LT) muscles between the relaxed and contracted states in the each exercises. 2) The Scapular plane shoulder elevation with resistance is more effective to Strengthening in the scapular stabilization muscles than Push up-plus and Wall slide. CONCLUSION : The Scapular plane shoulder elevation with resistance may be used to effectively that patient with various shoulder pain.
