A Case Report : Complications of Chemotherapy after Curative Resection in Patient with Ovarian Carcinoma IV (근치적 절제술 시행한 난소암 환자에서 항암치료 후유증 치험 1례)

  • Objectives: We designed this study to evaluate the effect of Korean medical treatment on complications of chemotherapy after curative resection in patient with ovarian carcinoma IV. Methods: The patient got total abdominal hysterectomy and bilateral salpingo-oophorectomy (TAHBSO) on 9/4 and received chemotherapy on 10/7, 10/28, 11/18. During this period, the patient suffered from anorexia, dyspepsia, nausea, weight loss and insomnia. We treated the patient with herbal medicine and acupuncture. The efficacy of treatment was evaluated with visual analog scale (Vas), weight and 36-item short form health survey instrument (SF-36). Results: After treatment, although the scale values went ups and downs according to chemotherapy schedule, abdominal pain Vas changed 7 to 0, weight changed 46 kg to 51 kg. Also, SF-36 scores increased. Conclusions: This case report shows that the Korean medical treatment is useful in the treatment of complications of chemotherapy after curative resection in patient with ovarian carcinoma IV.

A Case Report on Symptom Improvement in a Polycythemia Vera Patient Treated with Acupuncture (침치료로 임상증상이 호전된 진성적혈구증가증 환자 치험 1례)

  • Objectives: This study investigated the effectiveness of acupuncture for improving polycythemia-vera-related symptoms and quality of life. Methods: A 56-year-old woman diagnosed with polycythemia vera received acupuncture treatment between February 19, 2021, and August 26, 2021. We observed the changes in subjective symptoms and conducted myeloproliferative neoplasm symptom assessment form total symptom score (MPN-SAF TSS) questionnaire. Results: After 13 acupuncture treatment sessions over six months, symptoms of polycythemia vera, such as fatigue, bone pain, itching, and headache, improved. Conclusion: This study suggests that acupuncture may be a helpful treatment strategy for polycythemia vera patients suffering from significant symptom burdens and reduced quality of life.

  • Renal Fanconi syndrome (RFS) is caused by generalized proximal tubular dysfunction and can be divided into hereditary and acquired form. Adult-onset RFS is usually associated with drug toxicity or systemic disorders, and modern molecular genetics may explain the etiology of previous idiopathic cases of RFS. Here, we report the case of a 52-year-old woman with RFS whose etiology could not be identified. She presented with features of phosphaturia, renal glucosuria, aminoaciduria, tubular proteinuria, and proximal renal tubular acidosis. Her family history was unremarkable, and previous medications were nonspecific. Her bone mineral density was compatible with osteoporosis, serum intact parathyroid hormone level was mildly elevated, and 25(OH) vitamin D level was insufficient. Her blood urea nitrogen and serum creatinine levels were 8.4 and 1.19 mg/dL, respectively (estimated glomerular filtration rate, $53mL/min/1.73m^2$). Percutaneous renal biopsy was performed but revealed no specific renal pathology, including mitochondrial morphology. No mutation was detected in EHHADH gene. We propose the possibility of involvement of other genes or molecules in this case of adult RFS.

Portal Hypertension of a Delayed Onset Following Liver Abscesses in a 12-Month-Old Infant: A Case Report and Review of the Literature

  • We report a 12-month-old female infant who had a history of neonatal sepsis with liver micro-abscesses that resolved with intravenous antibiotics during neonatal period. During her neonatal admission period, no umbilical vein catheter was inserted. Also, she did not undergo any abdominal surgeries or had a postnatal history of necrotizing enterocolitis. However, the child developed upper gastrointestinal bleeding in form of hematemesis and melena secondary to esophageal varices at the age of 12 months with an extra-hepatic portal vein obstruction with cavernous transformation and portal hypertension subsequently. The child underwent a successful endoscopic injection sclerotherapy. She is now 20-month-old and has portal hypertension but otherwise asymptomatic. We are proposing the possibility of a delayed-onset portal hypertension as a complication of liver abscess and neonatal sepsis.

  • Pigmented villonodular synovitis (PVNS) is a proliferative disease that affects the synovial joint, tendon and bursa. PVNS can form a nodular structure in any joint, but it most commonly affects the knee joint and is rare in the foot and ankle joint. PVNS is divided into two types. Localized-type PVNS exhibits focal involvement with a nodular mass, while diffuse-type PVNS involves the entire synovium. Synovitis of the affected joint can also destroy cartilage and bone. Diffuse type accounts for 75% of PVNS and has a reported recurrence rate of 12.2% to 46%; aggressive synovectomy is recommended as the most effective treatment. In localized-type PVNS, only arthroscopic partial synovectomy is effective with a lower recurrence rate. We report a patient with severe ankle joint arthritis induced by diffusetype PVNS. The patient was treated by lateral malleolar ostectomy and ankle arthrodesis with a plate and screws via a lateral approach.

Application of Complex Korean Medicine Therapy to a Patient with Behavioral and Psychological Symptoms of Dementia from Suspected Alzheimer's Disease: Case Report (알츠하이머 치매 의심 환자의 행동심리증상에 대한 복합한의치료 1례: 증례 보고)

  • Objectives: The purpose of this study was to report the clinical effectiveness of the complex Korean medicine therapy on a patient with BPSD (Behavioral and Psychological Symptoms of Dementia) due to suspected Alzheimer's disease. Methods: We experienced an age 73 female who complained of a depressive mood, insomnia, and accompanying symptoms such as abnormal behavior. During treatment, the patient received acupuncture and herbal medicine. The clinical effects were evaluated through KGDS (Korean Form of Geriatric Depression Scale) and subjective statements of the patient. Results: After the complex Korean medicine therapy, the treatments ameliorated overall symptoms that the patient complained about. The patient was more positive and slept better after the treatment. No specificities or adverse effects were reported during treatment. Conclusions: This study suggests the possibility of treating patients with BPSD with complex Korean medicine therapy.

Use of a buccinator myomucosal flap and bilateral pedicled buccal fat pad transfer in wide palatal fistula repair: a case report

  • Primary palatoplasty for cleft palate places patients at high risk for scarring, altered vascularity, and persistent tension. Palatal fistulas are a challenging complication of primary palatoplasty that typically form around the hard palate-soft palate junction. Repairing palatal fistulas, particularly wide fistulas, is extremely difficult because there are not many choices for closure. However, a few techniques are commonly used to close the remaining fistula after primary palatoplasty. Herein, we report the revision of a palatal fistula using a pedicled buccal fat pad and palatal lengthening with a buccinator myomucosal flap and sphincter pharyngoplasty to treat a patient with a wide palatal fistula. Tension-free closure of the palatal fistula was achieved, as well as velopharyngeal insufficiency (VPI) correction. This surgical method enhanced healing, minimized palatal contracture and shortening, and reduced the risk of infection. The palate healed with mucosalization at 2 weeks, and no complications were noted after 4 years of follow-up. Therefore, these flaps should be considered as an option for closure of large oronasal fistulas and VPI correction in young patients with wide palatal defects and VPI.

  • Knotting of a pulmonary artery catheter (PAC) is a rare, but well-known complication of pulmonary artery (PA) catheterization. We report a case of a double-knotted PAC with a large loop in a patient with hepatocellular carcinoma (HCC) undergoing liver transplantation, which has been rarely reported in the literature. A PAC was advanced under pressure wave form guidance. PAC insertion was repeatedly attempted and the PAC was inserted 80 cm deep even though PAC should be normally inserted 45 to 55 cm deep. However, since no wave change was observed, we began deflating and pulling the balloon. At the 30-cm mark, the PAC could no longer be pulled. Fluoroscopy confirmed knotting of the PAC after surgery (The loop-formed PAC was shown in right internal jugular vein); thus, it was removed. For safe PA catheterization, deep insertion or repeated attempts should be avoided when the catheter cannot be easily inserted into the pulmonary artery. If possible, the insertion of PACs can be performed more safely by monitoring the movement of the catheter under fluoroscopy or transesophageal echocardiography.

First Report of Leaf Spot Disease Caused by Cladosporium pseudocladosporioides on Morus alba in South Korea

  • Morus alba, known as White Mulberry, is one of the most common species of mulberry found in South Korea, along with M. australis, known as Korean Mulberry. During a routine survey to investigate fungal diseases on deciduous broad-leaved trees in 2020, leaf spots were consistently observed on the White Mulberry in Sejong-si (36°30'12.8"N 127°17'34.5"E) and Wonju-si (37°15'29.6"N 128°11'37.9"E), South Korea, with a disease incidence of approximately 70 to 80%. Symptoms included circular, tan or necrotic lesions surrounded by a dark margin on leaves, which, in some cases, the lesions coalesced to form relatively large necrotic regions. The pathogen was successfully isolated from M. alba, and was identified as Cladosporium pseudocladosporioides based on the phylogenetic analysis and morphology. To the best of our knowledge, this is the first report of leaf spot disease on M. alba caused by C. pseudocladosporioides in South Korea.

A Case Study of a Taeeumin Patient with Chronic Diarrhea treated with Cheongsimyeonja-tanggamibang (삼차신경통 태음인 환자의 만성 설사 치험례)

  • Objectives This study was aimed to report a significant improvement of chronic diarrhea patient, diagnosed as 'Taeeum type liver heat-lung dryness pattern' based on Sasang medicine. Methods The patient had treatment with Sasang constitutional medicine. The treatment was executed three times everyday from August 27th to September 3rd and two times everyday from September 3rd to September 16th of 2021. Clinical improvement was evaluated with Bristol stool form scale, the degree of abdominal discomfort and nausea compared to the day of hospitalization and the number of defecations and nocturia per day. Results The symptoms of diarrhea, abdominal discomfort and nausea were improved. The number of night urination was decreased from 3-4 times to once a night. Conclusion After the treatment with Sasang constitutional medicine, diarrhea, abdominal discomfort, nausea and the general condition of the patient also improved.
