Clinical evaluation of a new extraction method for intentional replantation (의도적 재식술을 위한 새로운 발치법의 임상 평가)

  • Purpose: Intentional replantation (IR) is a suitable treatment option when nonsurgical retreatment and periradicular surgery are unfeasible. For successful IR, fracture-free safe extraction is crucial step. Recently, a new extraction method of atraumatic safe extraction (ASE) for IR has been introduced. Patients and Methods: Ninety-six patients with the following conditions who underwent IR at the Department of Conservative Dentistry, Seoul National University Bundang Hospital, in 2010 were enrolled in this study: failed nonsurgical retreatment and periradicular surgery not recommended because of anatomical limitations or when rejected by the patient. Preoperative orthodontic extrusive force was applied for 2-3 weeks to increase mobility and periodontal ligament volume. A Physics Forceps was used for extraction and the success rate of ASE was assessed. Results: Ninety-six premolars and molars were treated by IR. The complete success rate (no crown and root fracture) was 93% (n = 89); the limited success rates because of partial root tip fracture and partial osteotomy were 2% (n = 2) and 5% (n = 5), respectively. The clinical and overall success rates of ASE were 95% and 100%, respectively; no failure was observed. Conclusions: ASE can be regarded as a reproducible, predictable method of extraction for IR.

Selection of Replantation Species in Roadside Cutting-slope Area of the Baekdu Range (백두대간 도로 절토비탈면의 녹화수종 선정)

  • This study was carried out to select proper species for early stage replantation in cutting-slope area of the Baekdu Range roadside. In the cutting-slope area and upper forest area of the cutting-slope area, sample plots of 46 were selected and their vegetations and environmental factors were investigated. We found total 90 species in the 46 plots. We found Festuca arundinacea in 27 plots of the 46 plots. We also found Lespedeza bicolor in 26 plots, Eragrostis curvula in 22 plots, Artemisia princeps var. orientalis in 19 plots, Erigeron annuus and Pinus densiflora in 15 plots, Lactuca indica var. laciniata in 14 plots, Miscanthus sinensis in 13 plots, Oenothera odorata, Commelina communis, and Humulus japonicus in 11 plots, Lespedeza cuneata in 10 plots, Salix koreensis and Salix hulteni in 9 plots, Festuca rubra, Youngia denticulata, Aster scaber, and Festuca arundinacea in 8 plots, Chenopodium album var. centrorubrum in 7 plots, Patrinia villosa and Erigeron canadensis in 6 plots, and Setaria viridis, Digitaria sanguinalis, and Dactylis glomerata in 5 plots.The dominancy of Festuca arundinacea, Eragrostis curvula, Lespedeza bicolor, Miscanthus sinensis, Pinus densiflora, Festuca arundinacea, Festuca rubra, Artemisia princeps var. orientalis, Erigeron annuus, Lespedeza cuneata, Dactylis glomerata, Salix koreensis, Alnus hirsuta, Pueraria thunbergiana, Medicago sativa, Lactuca indica var. laciniata, and Digitaria sanguinalis were 13.41% 11.87%, 4.88%, 4.56%, 3.38%, 2.29%, 2.14%, 2.14%, 1.99%, 1.95%, 1.54%, 1.47%, 1.44%, 1.37%, 1.15%, 1.13%, 1.07% in the roadside cutting-slope, respectively. The dominancy of Rhododendron mucronulatum, Lindera obtusiloba, Rhus trichocarpa, Quercus mongolica, Lespedeza bicolor, Rhododendron schlippenbachii, and Quercus serrata 22.15%, 13.36%, 13.14%, 12.03%, 11.74%, 11.20%, and 11.14% in shrub layer of the upper forest area, respectively. The species of Rhododendron mucronulatum, Lindera obtusiloba, Rhus trichocarpa, Rhododendron schlippenbachii, Zanthoxylum schinifolium, Weigela subsessilis, Stephanandra incisa, Rhamnus davurica, Ligustrum obtusifolium, Lindera erythrocarpa, Rosa multiflora, Rubus coreanus, Symplocos chinensis for. pilosa, Smilax china, Prunus sargentii, and Staphylea bumalda may be more helpful in stabilizing of the cutting-slope area and making harmony with the surrounding forest area.

EXPERIMENTAL STUDY ON HEALING PROCESS OF AUTOGENIC DEMINERALIZED BONE (자가 탈회골의 조직반응에 관한 실험적 연구)

  • Many surgeons are on the point of bone excision and reconstruction of the bone defects by autograft. xenograft, and allograft in the treatment of begin and malignant tumors of bone. Of all type of bone grafts, we received the autograft as the best ideal bone graft. Of autogenic bone graft, replantation of excised autogenic bone for reconstructiong the bone defects has been the ideal method until now, but early bone healing reponses and tumor cell devitalization after replantation of excised autogenic bone have not been identified for clinical applications. So, to evaluate bone healing response after replantation in rabbit's calvarial bone, we divided the experimental group into three groups. Group 1 is a fresh autogenous bone group. Group 2 is a deep frozen group. Group 3 is freeze-dried demineralized group. Obtained result were as followed: 1. Inflammatory cell infiltration appeared at I week and disappeared at 4 weeks in all experimental group, Especially, severe inflammatory cell infiltration showed in fresh autogenous bone group at 2 weeks. Group 3 is the least showing group on the point of inflammatory cell infiltration. 2. Osteoblastic activity evenly increased upto 4 weeks and maintained to 6 weeks and decreased after this period, especially osteoblastic activity in group 2 is less than group 1 and group 3. We can't discriminate between osteoblastic activity of group 1 and that of group 3. 3. In new bone formation, group 3 was more active than any other groups at early stage, but there were little differences among three experimental groups at later state. 4. Bone resorption around the grafted bone slightly appeared at 1 week and disappeared at 4 weeks in all experimental groups. We can find the more bone resorption in group 2 at 2 weeks than any other groups. We could suggest, as appears from our results, that freeze-dried deminiralized bone graft is the useful bone graft in the clinical applications of excised autogenic bone.

Analysis on the Vegetation Structure of the Decomposed Granite Cut-slope - In case study on forest road of Gyeongsang-do and Jeolla-do - (화강암질 풍화토 절토비탈면의 식생구조 분석 - 경상도와 전라도 임도를 중심으로 -)

  • This study was carried out to select proper species for early stage replantation in granite forest roads cut-slope. From one to fourteen year elapsed forest roads in eight regional areas of Gyeongsang-do and Jeolla-do, sample plots were selected, and their vegetations and environmental factors were investigated. Soil organic matter, total nitrogen, soil moisture content, and bulk density in the granite weathering soils were below 0.4%, 0.03%, 12.4%, and above 1.2g/$cm^3$, respectively. The result indicated that the soil was not good for plant growth. The environmental factors affecting distribution of species in the granite forest roads were the elapsed year, elevation, and cut-slope aspect. Invaded species with high frequency in the forest road cut-slope ordered Pinus densiflora, Lespedeza bicolor, Rubus crataegifolius, Alnus hirsuta, Youngia denticulata, Patrinia villosa, Artemisia princeps var. orientalis, Zanthoxylum schinifolium, and Dactylis glomerata. Herbs seeded in earth stage such as Dactylis glomerata, Eragrostis curvula, Festuca arundinacea, Lespedeza sp., and Amorpha fruticosa had changed into perennial herbs and shrubs such as Youngia denticulata, Patrinia villosa, Aster scaber, Patrinia scabiosaefolia, Melica onoei, Astilbe chinensis var. davidii, Chrysanthemum zawadskii var. latilobum, Pueraria thunbergiana, Stephanandra incisa, and Clematis apiifolia, and had changed into plants such as Artemisia princeps var. orientalis, Artemisia keiskeana, Miscanthus sinensis, Pinus densiflora, Rhododendron mucronulatum, Rubus crataegifolius, and Zanthoxylum schinifolium according to elapsed year of forest roads. Leguma such as Lespedeza sp., Pueraria thunbergiana, Amorpha fruticosa, etc., herbs such as Artemisia keiskeana, Artemisia princeps var. orientalis, Chrysanthemum zawadskii var. latilobum, Miscanthus sinensis, etc., and woody plants such as Pinus densiflora, Rubus crataegifolius, Rhododendron mucronulatum, etc., with foreign herbs were considered as proper species for replantation in granite forest road cut-slope.

Effect of fibroblast growth factor on injured periodontal ligament and cementum after tooth replantation in dogs

  • Purpose: The purpose of this animal study was to perform a histological and histomorphometric analysis in order to elucidate the effect of fibroblast growth factor-2 (FGF-2) on injured periodontal ligament (PDL) and cementum after tooth replantation in dogs. Methods: The roots of 36 mandibular premolars from six mongrel dogs were used in this study. The roots were randomly divided into three groups: (1) a positive control group (n=12), in which the PDL was retained; (2) a negative control group (n=12), in which the PDL and the cementum between the notches were removed; and (3) an experimental group (n=12), in which the PDL and the cementum between the notches were removed and the roots were soaked in an FGF-2 solution ($30{\mu}g/0.1mL$). After treating the root surfaces, the extracted roots were replanted into extraction sockets. The animals were sacrificed four and eight weeks after surgery for histologic and histomorphometric evaluation. Results: At four and eight weeks, normal PDLs covered the roots in the positive control group. In the negative control group, most replanted roots showed signs of replacement resorption. In the experimental group, new PDL-like tissue and cementum-like tissue were observed to partially occupy the region between the root surfaces and the newly formed bone. Histomorphometric analysis showed that the mean length of the newly formed cementum-like tissue on the roots treated with FGF-2 was significantly greater than that of the tissue on the roots in the negative control group (four weeks, P=0.008; eight weeks, P=0.042). However, no significant differences were observed between the roots treated with FGF-2 and the negative control roots with respect to newly formed PDL-like tissue. Conclusions: The results of this study suggest that use of FGF-2 on injured root surfaces promotes cementogenesis after tooth replacement in dogs.

REVASCULARIZATION AND REINNERVATION OF CALCITONIN GENE-RELATED PEPTIDE IMMUNOREACTIVE NERVES IN REPLANTED RAT MOLARS (재식한 흰쥐 구치 치수조직의 재혈관화와 CGRP 면역반응 신경섬유의 변화)

  • The purpose of this study was to investigate the revascularization and reinnervation of calcitonin gene-related peptide immunoreactive nerves in immediately replanted rat molars. First maxillary right molars in 56 rats(35days old) were extracted and immediately replanted. The rats were killed 1, 2, 3, 7, 14, 28 and 42 days after replantation and revascularization of pulpal blood vessels were examined microangiogram with korean traditional ink and reinnervation of pulpal nerve were examined immunohistochemical method using calcitonin gene-related peptide(CGRP) antiserum. The results were as follows; 1. Revascularization and reinnervation of CGRP immunoreactive nerve fibers were observed mesial side whole pulp tissue of replanted teeth. Revascularization and reinnervation of CGRP immunoreactive fiber were made at 2days after replantation in entire pulp of replanted teeth and the distribution density of blood vessels were gradually increased according time elapsed, but did not achieve the density of control. 2. Postoperative dentin formation in replanted teeth revealed at 1week after replantaton and gradually increased according to time elapsed. 3. Revascularization and Reinnervation of CGRP immunoreactive nerve fibers were established at the same time and it seems to be closed relatationship between revascularization and reinnervation.

THE EFFECT OF PERIODONTAL REGENERATION AND ANTI-RESTORATION OF DEXAMETHASONE AND OP-1 FOLLOWING DELAYED REPLANTATION IN RAT MODEL (흰 쥐 모델에서 지연재식 시 dexamethasone과 OP-1의 표면처리가 치주조직 재생 및 항흡수 작용에 미치는 효과)

  • The purpose of this study was to observe the effect of dexamethasone and osteogenic protein-1(BMP-7) on bone, cementum and periodontal tissue regeneration. A total of 60 Sprague-Dawley white female mice were selected and beta-APN was used for five days to extract the maxillary first molar a traumatically. After the extraction of the teeth, the mesiobuccal root canal was filled with Caviton$^{\circledR}$. The teeth were etched with citric acid for 1 min and coated with one of four different experimental solutions : DEX(500nM/ml), DEX(1000nM/ml), OP-1(100$\mu\textrm{g}$/ml) and OP-1(500$\mu\textrm{g}$/ml) for three minutes depending on the group. All teeth were then replanted under microscope. All replantation procedures were done within 30 minutes. Teeth that were replanted after 30 minutes of bench dry only was used as positive control. All animals were sacrificed at 3 weeks following replantation and histologic observtion was done. The results were as follows ; 1. Active root resorption rate was decreased by the order of OP-1(500$\mu\textrm{g}$/ml), DEX(1000nM/ml), OP-1(100$\mu\textrm{g}$/ml), and DEX(500nM/ml). There was statistically less root resorption in OP-1 (500$\mu\textrm{g}$/ml) and DEX(1000nM/ml) group(P<0.05). 2. The group with higher concentration of dexamethasone(1000nM/ml) had statistically more bone union compared to positive control group(P<0.05),but there were no significant differences among four experimental groups. 3. OP-1(500$\mu\textrm{g}$/ml) and DEX(1000nM/ml) groups showed less degree of inflammation compared to the OP-1(100$\mu\textrm{g}$/ml). DEX(500nM/ml), and positive control group (P<0.05). In conclusion, the group with higher concentration of OP-1 had the best results on root resorption, bone ankylosis and anti-inflammatory effects compared to the other experimental groups, but a long-term study is also necessary to evaluate the exact pharmacological effects of the drugs in the future.

Native Plants Selection for Ecological Replantation and Vascular Plants in Forest Road Slope - In Case Study on Forest Road of Gyeongsangnam-do and Jeollanam-do - (임도사면의 생태적 녹화를 위한 자생식물 선정 및 관속식물상 - 경상남도와 전라남도 임도를 중심으로 -)

  • This study was carried out to select proper species fur early stage replantation in forest road cut-slope. From one to fifteen years elapsed forest roads in four regional areas of Gyeongsangnam-do and Jeollanam-do, sample plots were selected, and their vegetations and environmental factors were investigated. Invaded species and plant coverage have increased with elapsed years. The occurrence rate of herbs have decreased, while the occurrence of trees have increased with elapsed years. The ordination analysis showed that distribution of species was influenced by the elapsed year, the slope angle of the cut-slope, elevation, and the direction angle of the cut-slope. Dactylis glomerata, Poa pratensis, Amorpha fruticosa, Lespedeza sp., Artemisia princeps var. orientalis, Duchesnea chrysantha, Miscanthus sinensis, Pueraria thunbergiana, Zanthoxylum schinifolium, Rhus chinensis, and Weigela subsessilis were considered as proper species for replantation in forest road cut-slope, besides the species used already. Seventy seven family, 190 genus, 233 species, 38 variety, and 271 species in total were counted as invaded plant species in the survey zone. Naturalized plants were 18 species. Invaded species are, with high frequency first, Rubus crataegifolius, Artemisia princeps var. orientalis, Zanthoxylum schinifolium, Pueraria thunbergiana, Pinus densiflora, Lespedeza maximowiczii, Lysimachia clethroides, Lespedeza bicolor, Youngia denticulata, Miscanthus sinensis, and Erigeron annuus.

A retrospective study of the intentionally replanted mandibular second molars with C-shaped root canal configurations (C-형 근관계를 가진 하악 제2대구치의 의도적 재식술 결과에 대한 후향적 연구)

  • Objectives: The purpose of this retrospective study was to evaluate the success rate of intentionally replanted mandibular second molar with C-shaped canal configurations and to access the impact of preoperative periapical lesion on the success of intentional replantation procedure. Materials and Methods: This retrospective chart review study evaluated 52 intentionally replanted mandibular second molar teeth treated at Seoul National University Dental Hospital Department of Conservative Dentistry from January 2005 to December 2007. Seventeen teeth were lost for the follow-up, and another 6 teeth did not meet inclusion criteria of C-shaped root canal configurations. Healing outcome such as success, uncertain healing, and failure after follow-up was evaluated by clinical criteria and radiographs. Results: The overall success rate was 72.4% for the 29 intentionally replanted C-shaped mandibular second molars. The success rate of replanted teeth with preoperative periapical lesions was similar to that of replanted teeth which have no periapical lesions. Conclusions: Therefore, root canal treatment failure on C-shaped mandibular second molar can be predictably treated by intentional replantation regardless of the presence of periapical lesion.
