Background: While the use of bioprosthetic valves for mitral valve replacement (MVR) is increasing, very few studies have compared bovine pericardial and porcine valves in the mitral position to help guide bioprosthetic selection. Methods: In the present study, patients who underwent MVR using bovine pericardial valves were compared with those who underwent MVR with porcine bioprostheses between January 1996 and July 2018. Those with prior MVR, infective endocarditis, congenital mitral valve disease, or ischemic mitral regurgitation were excluded. The primary outcomes were structural valve deterioration (SVD) and mitral valve reoperation from any cause, and death was regarded as a competing risk. Competing risk analysis and propensity score-matching were used for comparisons. Results: Among the 388 patients enrolled, pericardial and porcine bioprostheses were implanted in 217 (55.9%) and 171 (44.1%), respectively. Propensity score-matching yielded 122 pairs of patients that were well-balanced for all baseline covariates. No significant differences were observed between the groups in unadjusted (p=0.09) and adjusted overall survival (hazard ratio [HR], 1.13; 95% confidence interval [CI], 0.72-1.76; p=0.60). Competing risk analysis revealed no significant differences in the risks of mitral reoperation (HR, 1.07; 95% CI, 0.50-2.27; p=0.86) and development of SVD (HR, 1.57; 95% CI, 0.56-4.36; p=0.39) between the groups. Matched population analysis confirmed similar results regarding reoperation (HR, 0.99; 95% CI, 0.40-3.22; p=0.98) and SVD (HR, 1.39; 95% CI, 0.41-4.73; p=0.60). Conclusion: No significant differences in survival or valve durability were observed between bovine pericardial and porcine bioprosthetic MVR. These findings require further validation through studies with larger sample sizes.
Acid deposition is one of the most serious environmental problems in ecosystems. The present study surveyed the effects of simulated acid rain on leaf litter mass loss and microbial community in the decomposing leaf litter of Sorbus anifolia in a microcosm at $23^{\circ}C$ and 40% humidity. Microbial biomass was measured by substrate-induced respiration (SIR) and phospholipid fatty acids (PLFAs), and the microbial community structures were determined by composition of PLFAs at each interval of decomposition in litter sample and at each pH treatment. The microbial biomass showed peaks at mid-stage of decomposition, decreasing at the late stage. The leaf litter mass loss of S. anifolia decreased with decreasing pH during early and mid-decomposition stages; however the mass loss becomes similar between pH treatments at late-decomposition stage. The acidification remarkably lowers the microbial biomass of bacteria and fungi; however, microbial diversity was unchanged between pH treatments at each stage of litter decomposition. With changes of decomposition stage and pH treatment there were considerable differences in replacement and compensation of microbial species. Fungi/bacteria ratio was considerably changed by pH treatment. The PLFA profile showed significantly larger fungi/bacteria ratio at pH 5 than pH 3 at the early stage of decomposition, and the difference becomes smaller at the later decomposition stage. At low pH, pH 3 and pH 4, the fungi/bacteria ratios were stable according to the litter decomposition stages. Simulated acid rain caused decreases of 10Me17:0, 16:1${\omega}$7c, 18:1${\omega}$7, 15:0, but increase of 24:0. In addition, litter mass loss showed significant positive correlation with microbial biomass measured by SIR and PLFA on the decomposing leaf litter.
Objective : Traditionally, staged surgery has been preferred in the treatment of compound comminuted depressed fracture (FCCD) after traumatic brain injury (TBI) and involves the removal of primarily damaged bone and subsequent cranioplasty. The main reason for delayed cranioplasty was to reduce the risk of infection-related complications. Here, the author performed immediate reconstruction using a titanium mesh in consecutive patients with FCCD after TBI, reported the surgical results, and reviewed previous studies. Methods : Nineteen consecutive patients who underwent single-stage reconstruction with titanium mesh for FCCD of the skull from April 2014 to June 2018 were retrospectively analyzed. The demographic and radiological characteristics of the patients with FCCD were investigated. The characteristics associated with surgery and outcome were also evaluated. Results : The frequency of TBI in men (94.7%) was significantly higher than that in women. Most FCCDs (73.7%) occurred during work, the rest were caused by traffic accidents. The mean interval between TBI and surgery was 7.0±3.9 hours. The median Glasgow coma scale score was 15 (range, 8-15) at admission and 15 (range, 10-15) at discharge. FCCD was frequently located in the frontal (57.9%) and parietal (31.6%) bones than in other regions. Of the patients with FCCDs in the frontal bone, 62.5% had paranasal sinus injury. There were five patients with fractures of orbital bone, and they were easily reconstructed using titanium mesh. These patients were cosmetically satisfied. Postoperatively, antibiotics were used for an average of 12.6 days. The mean hospital stay was 17.6±7.5 days (range, 8-33). There was no postoperative seizure or complications, such as infection. Conclusion : Immediate bony fragments replacement and reconstruction with reconstruction titanium mesh for FCCD did not increase infectious sequelae, even though FCCD involved sinus. This suggests that immediate single-stage reconstruction with titanium mesh for FCCD is a suitable surgical option with potential benefits in terms of cost-effectiveness, safety, and cosmetic and psychological outcomes.

This paper proposes a faulty node detection scheme that performs collective monitoring of a distributed networked control systems using interval weighting factor. The algorithm is designed to observe every node's behavior collectively based on the pseudo-random Bose-Chaudhuri-Hocquenghem (BCH) code. Each node sends a single BCH bit simultaneously as a replacement for the cyclic redundancy check (CRC) code. The fault judgement is performed by performing sequential check of observed detected error to guarantee detection accuracy. This scheme can be used for detecting and preventing serious damage caused by node failure. Simulation results show that the fault judgement based on decision pattern gives comprehensive summary of suspected faulty node.
The emissions of several air pollutants ($NO_x$, CO, VOCs, etc.) for the replacement of all diesel buses by Compressed Natural Gas (CNG) buses were estimated in the Busan Metropolitan Area (BMA). These emissions were calculated from emission factors considering the different driving speeds with bus routes, distance traveled, and deterioration factors. For the purpose of this study, three categories of fuel type were selected: (1) the combination of diesel (65%) and CNG buses (35%) (DSL+CNG case), (2) all diesel buses (DSL case), and (3) all CNG buses (CNG case). The emissions of $NO_x$ and CO in the CNG case were estimated to be significant decreases (by about 50% and 98%, respectively) relative to the DSL case. Conversely, the emission of VOCs (980.7 ton/year) in the CNG case were a factor of 3.3 higher than that (299.8 ton/year) in the DSL case. In addition, the diurnal variations of emissions between two city buses (e.g. diesel and CNG) and all other vehicles were distinctly different because the two city buses operate at a regular time interval. Our overall results suggest the possibility that the pollutant emissions from the CNG buses can exert less influence on air quality in the target area than those from the diesel buses.

Objectives: Smoking cessation decreases morbidity and mortality due to chronic obstructive pulmonary disease (COPD). Pharmacotherapy for smoking cessation is highly effective. However, the optimal prescription rate of smoking cessation medications among smokers with COPD has not been systemically studied. The purpose of this study was to estimate the national prescription rates of smoking cessation medications among smokers with COPD and to examine any disparities therein. Methods: We conducted a retrospective study using National Ambulatory Medical Care Survey data from 2007 to 2012. We estimated the national prescription rate for any smoking cessation medication (varenicline, bupropion, and nicotine replacement therapy) each year. Multiple survey logistic regression was performed to characterize the effects of demographic variables and comorbidities on prescriptions. Results: The average prescription rate of any smoking cessation medication over 5 years was 3.64%. The prescription rate declined each year, except for a slight increase in 2012: 9.91% in 2007, 4.47% in 2008, 2.42% in 2009, 1.88% in 2010, 1.46% in 2011, and 3.67% in 2012. Hispanic race and depression were associated with higher prescription rates (odds ratio [OR], 5.15; 95% confidence interval [CI], 1.59 to 16.67 and OR, 2.64; 95% CI, 1.26 to 5.51, respectively). There were no significant differences according to insurance, location of the physician, or other comorbidities. The high OR among Hispanic population and those with depression was driven by the high prescription rate of bupropion. Conclusions: The prescription rate of smoking cessation medications among smokers with COPD remained low throughout the study period. Further studies are necessary to identify barriers and to develop strategies to overcome them.
Twenty-four new born crossbred (Bos indicus$\times$Bos taurus) calves were distributed in two equal groups and assigned to two different pre-starter diets with (Group 1) and without (Group 2) fish meal to study the effect of replacement of animal protein by vegetable protein in the diet and the age of animals on ruminal metabolic development. All calves were fed colostrum for 24 h and whole milk until weaning at 8 weeks of age. Rumen fluid samples were collected on 4 d, 1 wk, and then weekly interval up to 8 wk of age. Rumen fluid samples were analysed for pH, TVFA, lactic acid and N fractions (total N, total soluble N, trichloro acetic acid (TCA) soluble N, TCA precipitable N and ammonia N). Weekly feed intake and live weight gain pattern showed an increasing trend with the advancement of age, but were similar in both groups. The pH fell steadily during 0-4 wk of age and then stabilized in later period. A close relationship (r=0.80) between starter intake and TVFA concentration was observed in both the groups. Lactic acid (meq/l) and ammonia N (mg/dl) concentration showed initial rise (0.55 and 14.97 on day 4 to 3.38 (7 wk) and 32.85 (4 wk), respectively) to fall (2.74 and 17.60) again during 8 wk of age in response to increase in dry feed consumption (10% initially to 83% of diet dry matter at 8 wk of age). The TCA precipitable fraction of N did not show any change during 0-8 wk of age. Data indicate that the metabolic changes responded rapidly to dry feed intake which did not differ in fish meal and non-fish meal groups, and a poor voluntary consumption of oat hay retards the progressive changes in live weight and rumen microbial development.

Objective : The potassium disturbance associated with thiopental continuous infusion in neurosurgical patients is well known. However, the effect of propofol continuous infusion on serum potassium levels has not been investigated extensively. Methods : We reviewed the medical records of 60 consecutive patients who received coma therapy or deep sedation for intracranial pressure control using either thiopental or propofol between January 2010 and January 2012. Results : The overall incidence of hypokalemia (K<3.5 mmol/L) was comparable between thiopental and propofol groups (89.2% vs. 82.6%). But, the incidence of moderate to severe hypokalemia (K<3.0 mmol/L) was significantly higher in thiopental group (51.4% vs. 13.0%, p=0.003). The lowest potassium level (2.9 mmol/L vs. 3.2 mmol/L, p=0.020) was lower in thiopental group. The patients in the thiopental group required greater potassium replacement than the propofol group patients (0.08 mmol/kg/h vs. 0.02 mmol/kg/h, p<0.001). On multivariate analysis, thiopental [odds ratio, 95% confidence interval, 7.31 (1.78-27.81); p=0.005] was associated with moderate to severe hypokalemia during continuous infusion. The incidence of rebound hyperkalemia (K>5.0 mmol/L, 32.4% vs. 4.3%, p=0.010) and the peak potassium concentration (4.8 mmol/L vs. 4.2 mmol/L, p=0.037) after the cessation of therapy were higher in thiopental group. On multivariate analysis, thiopental [8.82 (1.00-77.81); p=0.049] and duration of continuous infusion [1.02 (1.00-1.04); p=0.016] were associated with rebound hyperkalemia once therapy was discontinued. Conclusion : Propofol was less frequently associated with moderate to severe hypokalemia after induction and rebound hyperkalemia following the cessation of continuous infusion than thiopental.

In recent, there have been several developments in lamp technology that promise savings in electrical power consumption and improved quality of the lighting space. Above all, the advantage of ring-shaped electrodeless fluorescent lamp is the removal of internal electrodes and heating filaments that are a light-limiting factor of conventional fluorescent lamps. The ring-shaped electrodeless lamp is intended as a high efficiency replacement for the incandescent reflector lamp in many applications. Therefore, the life time of ring-shaped electrodeless fluorescent lamps is substantially higher than that of conventional fluorescent lamps and last up to 60,000 hours. In this paper, maxwell 3D finite element analysis program(Ansoft) was used to obtain electromagnetic properties associated with the coil and nearby structures. The electromagnetic emitting properties were presented by 3D simulation software operated at 250 KHz and some specific conditions. The optical characteristics were measured luminance and a temperature and an optical spectrum distribution for 10 min in a one minute interval at the same time. With a goal of finding alternative materials, we show measurement results of electrical characteristics of a ring-shaped electrodeless fluorescent lamp as a function of frequency and the number of coil turns using a highly permeable($\mu$$_{r}$(equation omitted) 2,000) Mn-Zn ferrite. These results are compared with those of conventional ring-shaped electrodeless fluorescent lamp. It is found that the resistance, inductance and impedance are increased while the quality factor decreases as frequency increases.s.
Conventional methods for assessing remaining life of critical high temperature components in fossil power plants rely on nondestructive inspection practices and accompanying life analysis based on fracture mechanics By using these conventional methods. It has been difficult to perform uninterrupted in-service inspection for life prediction. Thus, efforts have been made for developing on-line remaining life monitoring systems employing information on the shape of structures, operating variables and material properties. In thus study, a software for on-line life monitoring system which performs real-time life evaluation of a high temperature system headers was developed. The software is capable of evaluating creep and fatigue life usage from the real-time stress data calculated by using temperatures/stress transfer Green functions derived in advance for the specific headers. The major benefits of the developed software life in determining future operating schedule, inspection interval, and replacement plan by monitoring real-time life usage based on prior operating history.
