A 6-year-old castrated male domestic short hair cat with the clinical signs of anorexia and vomiting was admitted to the local animal hospital. Abdominal radiography and ultrasonography revealed renomegaly and severe hydronephrosis in the right kidney. Surgically excised right kidney was submitted for diagnosis. On the cut surface, two milky white masses and severe dilation of renal pelvis were observed. Most of the neoplastic masses were composed of uniform well differentiated tubules lined by a single layer of cuboidal to columnar cells and projected papillae into the lumen. The neoplastic cells were strong positive for cytokeratin (CK) MNF116, but negative for CK 7. Based on the clinical and gross findings, histopathology and immunohistochemistry, this case was diagnosed as papillary renal adenoma with hydronephrosis.
Objective : $^{99m}Tc$-dimercaptosuccinic acid(DMSA) scan is considered to be the most sensitive examination for detection of renal scars. However, because of its high radiation exposure to the kidney and its limited usefulness for patients with low grade vesicoureteral reflux(VUR), some authors have suggested that DMSA scans should be reserved primarily for children with VUR grade 3 and above. The aim of this study was to reevaluate the necessity of DMSA scans as a screening test in infants without reflux or with low grade reflux. Methods : In this retrospective study, 189 infants(mean age : 6.2 months) diagnosed as UTI were enrolled. Voiding cystourethrogram(VCUG), DMSA scan and renal ultrasonography were performed within 1 month of UTI. VUR grade was classified into three subgroups; low grade(grade 1-2), moderate grade(grade 3), and high grade(grade 4-5), respectively. Results : Renal defects were present in 67 of 189 infants, and 82 of the 378 renal units. The incidence of renal defects was significantly correlated with VUR grade(P<0.01); 28 percent without reflux, 38 percent with low grade, 53 percent with moderate grade, 100 percent with high grade, respectively. However, there was no significant differences in incidence of renal defects between the low grade and moderate grade group. Conclusion : In this study, renal defects were found in quite high percentages; 28 percent patients without reflux and 38 percent patients with low grade VUR, respectively. Moreover, there was no significant difference in the incidence of renal defects between the low grade and moderate grade groups. Therefore, DMSA scan should be performed for infants with UTI as a screening test regardless of the presence of VUR.
Purpose : Neonatal hydronephrosis has been detected with increasing frequency with the widespread use of prenatal ultrasonography, but the consensus about its postnatal management has not yet been reached, especially about surgical intervention. We attempted to determine the guideline of follow-up study and surgical intervention of hydronephrosis by analyzing clinical outcomes of neonates with hydronephrosis. Materials and Methods : Between 1994 and 2000, 128 hydronephrotic kidneys were postnatally confirmed. Cases associated with other urologic anomalies were excluded and 90 unilateral hydronephrotic kidneys with a minimum follow-up of 12 months were enrolled in this study. We classified the patients into 6 groups according to the anterior posterior pelvic diameter(APPD) at initial ultrasonography(USG) within 1 month after birth. Renal USG and $Tc^{99m}-mercaptoacetyl$ triglycerine(MAG3) scan were done according to a set protocol, and pyeloplasty was performed when indicated according to our protocol. Results : Most cases whose APPD were below 10 mm improved or resolved. Only few cases with APPD above 20 mm showed spontaneous improvement and most(88%) had undergone operation. Those with initial APPD within 10-19 mm showed variable outcomes. When the risk factors for irreversible renal functional deterioration were analyzed, the age at pyeloplasty and pre-operative functional deficit were significant. Conclusion : We concluded that in infants with initial APPD below 10 mm, consideration of surgery is not needed, and in those with initial APPD above 20 mm, early operation is recommended. Our set protocol based on initial USG is useful, but the cut-off value of relative renal function(RRF) for operation might be increased to 40% to improve post operative RRF.
Purpose : Nutcracker syndrome must be considered when hematuria or proteinuria occurs in a healthy child. The purpose of this study is to investigate the prevalence of nutcracker syndrome among children with asymptomatic hematuria or proteinuria, and to obtain the ratios of the peak velocity of the left renal vein between the aortomesenteric portion and the hilar portion in children with asymptomatic hematuna or protelnuria in which nutcracker syndrome was excluded and to observe whether the ratios are affected by sex, age or urinalysis findings. Methods : Using Doppler ultrasonography, we measured the flow velocity and obtained the peak velocity ratios of the left renal vein at the aortornesenteric portion and at the hilar Portion of the left kidney in children with asymptomatic hematuria or proteinuria who visited the Division of Pediatric Nephrology, Severance Hospital from May 2001 to March 2004. Results : Of 304 children with asymptomatic hematuna or proteinuria, 107 children(35.2%) were diagnosed with nutcracker syndrome. For 197 children with asymptomatic hematuria or proteinuria excluding nutcracker syndrome, the mean ratio of the peak velocity was 2.54 $\pm$0.73, which was not affected by sex, age or urinalysis findings. Conclusion : Nutcracker syndrome was the major cause of asymptomatic hematuria or proteinurla In children, comprising 35% of all cases. Doppler ultrasonography was helpful in the screening of nutcracker syndrome and prevention of its complications. For children with asymptomatic hematuria or proteinuria excluding nutcracker syndrome, the peak velocity ratio of the left renal vein did not differ from that of normal children and was not affected by sex, age or urinalysis findings. Hasāh al-kuliya (Nephrolithiasis) is a common disease with a worldwide prevalence of about 12%. A 64 years old male patient, who visited the outpatient department of National Research Institute of Unani Medicine for Skin Disorder (NRIUMSD), Hyderabad, presented with complaints of mild dull pain in right flank region and burning micturition for one month. The ultrasonography of whole abdomen confirmed that there was one stone of 5 mm in calyceal region of each kidney. The patient was treated with the Unani regimen comprising of three formulations Majoon, Hajrul Yahood (5 gm), Jawarish Zarooni Sada (5 gm) and Habb-i-Mudir (1000 mg) for 6 months. The patient did not report any acute pain during the therapy. After 6 months of treatment the ultrasonography of whole abdomen showed absence of the stone from each kidney, ureters and bladder. There was no recurrence of the disease during next 6 months of post treatment follow up. This case report documented the successful medical treatment and prevention of recurrence of bilateral nephrolithiasis through Unani medicines. It concluded that the Unani regimen was effective and safe in the management of bilateral nephrolithiasis.
A two-year-old, intact female Golden Retriever had previously been diagnosed with a portosystemic shunt (PSS) during an ultrasonographic examination at a local animal hospital. The serum biochemistry revealed elevated liver enzymes and bile acid levels. The abdominal radiographic examination revealed mild serosal detail loss and microhepatica, while abdominal ultrasonography revealed mild ascites and high-velocity flow to the caudal vena cava (CVC) suspected as a PSS. The gallbladder was not observed within the hepatic parenchyma during ultrasonography. Computed tomography (CT) revealed an absent gallbladder and dilation of the common bile duct (CBD). Dilations of the gastroduodenal, splenic, colic and renal veins were also observed. A dilated left phrenico-abdominal vein that entered the CVC was previously misinterpreted as a PSS on the ultrasound examination. Based on the imaging examinations, the dog was diagnosed with congenital gallbladder agenesis associated CBD dilation.
Renal size(length, width and height) of rabbits was measured by radiographs and nephrosonograms and compared with actual size. After measuring on the radiographs and nephrograms, both kidneys were removed from the body and actual size was also measured. On radiographs, right kidney was observed at the T13-L2 vertebrae and left kidney was at L2-L4 vertebrae. On nephrosonograms, the renal cortex was visible as small, homogenous echoes that were hypoechoic relative to the surrounding tissues, whereas the renal medulla was anechoic to slightly hypoechoic. The actual length, width and height of the left kidney were $35.84{\pm}3.12(mean{\pm}SD)$, $23.52{\pm}3.21$, $15.11{\pm}2.58cm$, respectively, whereas those of the right kidney were $36.02{\pm}3.42$, $23.69{\pm}3.50$ and $14.13{\pm}3.55cm$, respectively. On radiographs, the length and width of both kidneys were a little magnified(102-104%) when compared to actual size. On nephrosonograms, the length, width and height of bothkidneys were lessened(70-96 %) when compared to actual size. The length and width of kidney were 1.85 and 1.25 times the length of the second lumbar vertebrae on the ventrodorsal view. In correlation and correlation coefficient of body weight with the renal size, the body weight and renal size were significantly correlated with each other other(p<0.01) and the correlation coefficents of body weight with left, right and both Kindneys were 0.748, 0.794 and 0.859, respectively.

Autosomal-dominant polycystic kidney disease (AD-PKD) is common in Persian and Persian-related breeds, and is sporadically reported in Scottish Fold cats. A 5-year-old male Scottish Fold cat was diagnosed with polycystic kidney disease based on screening tests and abdominal ultrasonography and died 3.5 months after diagnosis. The cat had 14 kittens with three queens, including his female sibling, with an age range of 3 months to 8 years. Genetic testing to confirm the genetic transmission of AD-PKD which detects the mutated PKD1 gene was performed. Abdominal ultrasonography confirmed the presence of renal cysts. Nineteen cats were screened in the present study (13 males and 6 females), with an age range of 3 months to 8 years. The results of renal ultrasonography agreed with the genetic test results in the 19 cats in which both tests were performed and 8 cats were diagnosed as ADPKD based on these tests. AD-PKD has not been investigated in cats in South Korea. Moreover, this is the first report of AD-PKD in a family unit of Scottish Fold cats.
Glomerulocystic kidney disease(GCKD) is a rare form of renal cystic disease defined histopathologically by containing dilated Bowman's space with variable atrophy of glomerular tufts, which may occur as sporadically or as familial cases and can be presented as a major component of heritable syndromes. It has not been recognized in Korean children but only one report of adult case has been reported having GCKD. We experienced a case of GCKD in a 10-year-10-month-old boy, who was admitted for hypertension. Abdominal ultrasonography and computed tomography revealed clustered numerous small cysts in left kidney and renal biopsy findings was consistent with the GCKD showing cystic dilatation of Bowman's space with intact glomerular structure.

An 11-year-old, 4.3 kg, mixed breed, intact female dog exhibiting vomiting and a reduced appetite was presented. On physical examination, palpation of the right kidney elicited discomfort. Blood analysis revealed leukocytosis with neutrophilia, and elevation of alkaline phosphatase and blood urea nitrogen. Escherichia coli was detected in urine culture, whereas urine analysis showed absence of glucose. Radiography and ultrasonography suggested that there was gas in the right renal parenchyma, and computed tomography was performed while the dog was awake to investigate the suspected gas in the right kidney. After unilateral ureteronephrectomy, a histological diagnosis of right emphysematous pyelonephritis (EPN) was made. EPN refers to a severe necrotizing infection of the renal parenchyma with gas accumulation in the tissue. It is rare in both human and veterinary medicine, and almost all EPN patients have uncontrolled diabetes mellitus. This case report describes the imaging-based diagnosis of a rare occurrence of unilateral EPN in a non-diabetic dog.
