독시라민 과다복용에 의한 횡문근 용해 및 급성 신부전 2례 (Two Cases of Acute Renal Failure due to Rhabdomyolysis Complicating Doxylamine Succinate Intoxication)

  • Doxylamine succinate is an antihistamine used primarily as a sleep-induction. It can be gotten without a doctor's prescription in Korea, so it' s overdoses were frequently encountered. There were several reports that the overdoses of doxylamine might cause rhabdomyolysis, but few cases have been reported that it is related to acute renal failure (ARF). In cases that ARF occur, most of them are not severe enough to require hemodialysis. We experienced two cases of severe rhabdomyolysis complicating ARF after doxylamine overdose and treated with hemodialysis. Clinicians should be aware of the potentially lethal complications of rhabdomyolysis in patients who ingest doxylamine succinate and the needs for prompt intervention and careful assessment of renal function.

소아에서의 급성 신성신부전의 임상적 고찰 (Clinical Analysis of Acute Intrinsic Renal Failure in Neonates and Children)

  • 목적 : 소아 연령에서 입원 당시 혹은 다른 질환으로 입원하여 치료를 받던 중에 발생한 신성 급성 신부전의 임상적 특징과 경과 그리고 예후에 대해 분석하였다. 방법 : 2000년 1월부터 2006년 6월까지 부산백병원 소아청소년과에 입원한 환자 중 신성 급성 신부전으로 진단된 59명을 대상으로 하였다. 진단 기준은 혈청 크레아티닌치가 1.2 mg/dL 이상이거나 기저치에 비해 2배 이상 증가한 경우로 하였다. 핍뇨는 소변량이 0.5 mL/kg/hr 이하이며 무뇨는 <50 mL/day로 정의하였다. 결과 : 전체 환자의 성별 비는 2.2:1로 남아에게 많았으며 연령별 분포는 신생아 7명, 2개월-2세 10명, 3-6세 12명, 7-12세 21명, 13-16세 9명이었다. 신부전의 진단까지 평균 기간은 3.1${\pm}$2.8일이었다. 소변량에 따른 분류는 핍뇨군 21명, 비핍뇨군 36명 및 무뇨군 2명이었다. 원인 질환별 분류에서는 원발성 신질환군 30명, 감염군 14명, 종양군 9명 및 기타군 6명이였다. 연령별 분류에서는 신생아는 감염군, 2개월-2세는 신질환과 감염군과 3세 이상에서는 신질환군이 많았다. 계절별 발생 빈도는 차이가 없었으나 여름철에 용혈성 요독증후군의 발생이 많았다. 투석치료는 4명에서 복막투석을 시행하였다. 호전까지의 평균기간은 10.0${\pm}$6.7일이었다. 경과 중 18명이 사망하였는데 비핍뇨군의 사망률이 낮았다. 연령별로는 신생아가 원인별로는 종양군의 사망률이 각각 높았다. 결론 : 소아에서의 신성 급성 신부전은 조기진단과 기저질환의 적절한 관리로 대부분의 환자에서 정상적인 신기능의 회복을 기대할 수 있으며 성인에 비해 양호한 임상적 경과를 보였다.

만성신부전증 환자의 자가 구취인식이 구강건강관련 삶의 질에 미치는 영향 (Effect of self awareness of halitosis on the quality of life related to oral health in patients with chronic renal failure)

  • 본 연구는 혈액투석 중인 만성신부전증 환자들의 자가 구취인식이 구강건강관련 삶의 질에 미치는 영향을 파악하여 국내에서는 매우 드문 만성신부전 환자의 구강관련 기초자료로 사용하고자 실시하였다. 연구대상은 2013년 6월부터 7월까지 전주시에 위치한 혈액투석 전문병원에서 혈액투석 중인 만성신부전증 환자에게 설문조사를 실시하였다. 자가 구취 인식에 따른 구강 증상에서는 지난 한 달 동안 치아 아픔 증상, 잇몸에 피가 남, 혀 또는 뺨 아픔 증상, 입안 건조 증상에 유의한 차이가 나타났고, 구강기능 상태에서는 음식 씹는데 불편, 발음의 어려움, 상실된 치아 유무에 유의한 차이가 나타났다. 구강건강관련 삶의 질에서는 기능적 제한, 신체적 동통, 심리적 불편, 신체적 능력저하, 심리적 능력저하, 사회적 능력저하, 사회적 불리에 유의한 차이가 나타났다. 만성신부전 환자에서 인식한 구취가 구강건강관련 삶의 질에 미치는 영향요인 분석한 결과, 심리적 불편(B=-2.028, p<.01), 사회적 능력저하(B=2.596, p<.01), 사회적 불리(B=-2.173, p<.01)에 영향을 미치는 것으로 나타났다. 따라서 자가 인식한 구취는 심리적, 사회적으로 부정적인 영향을 미치므로 급격히 증가하고 있는 신부전 환자들에게 중요하게 고려할 요인이다.

만성 신부전 환자에서 혈액투석 전후의 기관지 과민반응검사 (Bronchial Hyperresponsiveness in Chronic Renal Failure Undergoing Hemodialysis)

  • 연구배경: 심인성 폐부종은 임상실험이나 동물실험에서 비특이적 기도과민 반응을 증가시킨다는 많은 보고가 있다. 또한 만성 신부전 환자는 혈관밖 폐수분 증가에 의하여 무증상에서 명확한 폐부종까지 다양한 임상 양상을 보이고 있다. 여러 보고에 의하면 만성 산부전 환자에서 혈관 밖 폐수분 증가로 폐활량이 감소하며 혈액투석 후는 폐 간질성 부종의 감소 혹은 소설로 폐활량이 호전 된다고 한다. 그런데, 이 폐 간질성 부종이 비특이적 기관지 과민 반응을 야기하는 한 기전으로 생각하고 있다. 그래서 연구자들은 만성신부전 환자에서 폐간질성 부종에 의하여 기관지 반응이 증가되는지 여부를 알기 위하여 본 실험을 시행하였다. 방법: 다른 동반질환이 없는, 1주에 3번씩 혈액투석을 하고 있는 18명의 만성신부전 환자를 대상으로 하였다. 이 환자들은 혈액투석전 폐기능 검사와 메타코린 기관지 유발검사를 하였고, 기관지 과민반응을 나타낸 환자는 혈액투석후 폐기능검사와 메타코린 기관지 유발 검사를 반복 시행하였다. 결과: 1) 18명중 12명에서 혈액투석전 메타코린 기관지 수축 유발검사에서 양성 반응을 하였고(66.7%), 이 기관지 과민반응은 혈액투석후 감소되었다. 2) 폐기능은 혈액투석후 호전되었고, 혈액투석 전후 $FEV_1$ 변화와 체중 변화는 유의한 상관 관계가 있었다(r=-0.62, p<0.01). 3) $PD_{20}$$FEF_{25}$ 사이에도 유의한 상관관계가 있었다(r=0.59, p<0.05). 결론: 만성 신부전 환자에서 관찰된 폐기능 저하와, 기관지 과민성 반응은 폐 간질성이 부종에 의할 것으로 사료된다.

만성신부전에 수반된 각종 내분비기능 변화에 관한 연구 (A Study on the Abnormalities of the Various Endocrine Functions Associated with Chronic Renal Failure)

  • In an attempt to evaluate the various serum hormonal changes in terminal renal failure, the fasting $T_4,\;T_3,\;rT_3$, gastrin, secretin prolactin, and aldosterone were measured by radioimmunoassay in 12 patients with chronic renal failure, who were admitted to Chosun University Hospital from January to June, 1981. From the analysis the following results were obtained. 1. Mean values of serum hormonal concentration in 10 normal control were as follows: $T_4,\;12.93{\pm}2.00ug/ml$; $T_3,\;113.0{\pm}28.7ng/ml$; $rT_3,\;0.11{\pm}0.10ng/ml$; gastrin, $100.0{\pm}47.1pg/ml$; secretin, $32.46{\pm}11.45pg/ml$; prolactin, $11.0{\pm}3.6ng/ml$; aldosterone, $137.0{\pm}58.5pg/ml$, 2. Mean values of serum hormonal concentration in 12 chronic renal failure were as follows: $T_4,\;7.34{\pm}2.43ug/ml$; $T_3,\;71.0{\pm}19.1ng/ml$; reverse $T_3,\;0.38{\pm}0.19ng/ml$; gastrin, $162.5{\pm}40.2pg/ml$; secretin, $107.50{\pm}20.48pg/ml$; prolactin, $34.0{\pm}17.2ng/ml$; Aldosterone, $86.5{\pm}19.8pg/ml$. 3. In chronic renal failure group, serum T 4, T 3 and adosterone level were significantly lower than those of the control group, but serum $rT_3$, gastrin, secretin and prolactin were significantly higher than those of the control group. 4. In the view of the correlation between serum hormonal concentions and serum creatinine levels in patients with chronic renal failure, $rT_3$, gastrin, secretin and prolactin showed increasing tendency(positive correlations), whereas $T_4,\;T_3$ and aldosterone showed decreasing tendency (negative correlations) with increment of serum cteatinine levels. And so, we observed the negative correlation between $T_3\;and\;rT_3$.

심근 경색 유발 심부전 모델에서 강리 추출물의 심장 보호 가능성 (Cardioprotective Potential of Gracilaria Verrucosa Extract in Myocardial Infarction-Induced Heart Failure Model)

  • Gracilaria Verrucosa (GV), a seaweed used in traditional Korean medicine, was studied for its effects on MI-induced heart failure in rats. MI is caused by a blocked coronary artery, leading to severe cardiac dysfunction. The study used a rat model to assess cardiac changes over time and evaluate the impact of GV on heart failure. Ischemia was induced through LAD ligation surgery, and the extent of ischemic area was measured as a prognostic factor. GV extract administration significantly improved cardiac morphology and reduced cardiac weight compared to the MI group. GV treatment also improved cardiac function, as evidenced by positive effects on chamber dilation during MI-induced heart failure. Parameters such as ejection fraction (EF) and fractional shortening (FS) were measured. The MI group showed decreased EF and FS compared to the sham group, while these parameters improved in the GV group. GV treatment also reduced levels of LDH, CPK, and CK-MB in the serum, indicating reduced myocardial damage. Histological analysis revealed that GV treatment attenuated cardiac hypertrophy and fibrosis, with reduced collagen deposition in the myocardium. Immunohistochemistry analysis showed suppressed expression of TGF-β1 and collagen 1, involved in fibrosis. In conclusion, GV showed potential in improving cardiac function in a rat model of MI-induced heart failure. It alleviated myocardial damage, attenuated cardiac hypertrophy and fibrosis, and suppressed fibrotic markers. Further studies are needed to explore its clinical efficacy and underlying mechanisms in cardiac diseases beyond animal models.

만성신부전 모델쥐에서 인의 함량은 같고 단백질 수준이 다른 식이가 신기능과 골격 대사에 미치는 영향 (Effect of Dietary Protein Level on Renal Function and Bone Metabolism in Rats with Reduced Renal Mass When Dietary P Level is Equal)

  • Studies were carried out to explore the influence of dietary protein level on bone metabolism in uninephrectomized rat (experimental renal failure model) when dietary Ca and P contents were equal. Male rats were uninephrectomized or sham operated and fed 8%, 15% and 40% casein diets for 24 weeks. Ca and P contents of the all diet were 0.4% and 0.6% respectively. The results are summarized as follows. We did not found any significant difference in PTH and Ca level of the serum, Ca intake and Ca excretion among the experimental groups. There was significant positive correlation between the PTH and phosphate level. There was significant inverse correlation between serum Ca and creatinine level. The effect of the dietary protein level and renal mass loss on density and Ca contents of the bone were small and different according to the kinds of the bone. Low protein diet was associated with a significant enhancement of scapular density. Femur and vertebra density, however, were not influenced by dietary protein level and uninephrectomy. Light microscopic examination showed several calcified foci in the kidney in all experimental groups. Low protein diets have been used for a long time in the conservative management of chronic renal failure as they have a beneficial effect in preventing the appearance of symptoms. This study elucidated that part of beneficial effects of the low protein diet related to the suppression of the hyperphosphatemia. And these results, even though uninephrectomized rats fed high protein diet, the secondary hyperparathyroidism is supressed by the regulation of the P level. Therefore this study emphasized the need to pay more attention to the regulation of dietary P level as well as dietary protein content in chronic renal failure. Prognostic Factors of Renal Scarring on Follow-up DMSA Scan in Children with Acute Pyelonephritis

  • Purpose: Early diagnosis and treatment of urinary tract infection have been emphasized to prevent renal scarring. If untreated, acute pyelonephritis could cause renal injury, which leads to renal scarring, hypertension, proteinuria, and chronic renal failure. The purpose of this study was to assess risk factors of renal scarring after treatment of acute pyelonephritis (APN). Methods: The medical records of 59 patients admitted at Daegu Fatima Hospital because of APN between March 2008 and April 2015 whose renal cortical defects were confirmed by using initial technetium-99m dimercaptosuccinic acid (DMSA) scans were reviewed retrospectively. We divided 59 patients into 2 groups according to the presence of renal scar and assessed risk factors of renal scar, including sex, age at diagnosis, feeding method, hydronephrosis, bacterial species, vesicoureteral reflux, and vesicoureteral reflux grade. Results: Of 59 patients (41%), 24 showed renal scar on follow-up DMSA scan. No significant differences in sex, hydronephrosis, bacterial species, and fever duration were found between the renal-scarred and non-scarred groups. As for age at diagnosis, age of >12 months had 5.8 times higher incidence rate of renal scarring. Vesicoureteral reflux (VUR) affected renal scar formation. VUR grade III or IV had 14.7 times greater influence on renal scar formation than VUR grade I or II. Conclusion: Our data suggest that the presence of VUR and its grade and age at diagnosis are risk factors of renal scar on follow-up DMSA scan after APN.

혈액투석을 위한 동정맥루의 장기관찰 성적 (Long-Term Results of Arteriovenous Fistula for Hemodialysis in Chronic Renal Failure)

  • From november, 1985 to May, 1993, 222 arteriovenous fistulae were made in 201 patients with chronic renal failure. Among them, a total of 183 arteriovenous fistulae in 173 patients were reviewed to evaluate the factors influencing patency rate of the vascular access. The results were revealed as follows: There were 102 men and 71 women,aged 10 to 76 years [mean = 45.7 years]. Sixteen patients of them had previous shunts. The procedures included establishment of 214 radiocephalic or brachioocephalic fistulae, 203 side to end, 9 side to side, 2 end to end, 2 autologous saphenous vein grafts, 6 Gore-Tex grafts. There were 28 early shunt failures[12%] due to use of 23 inadequate veins and 5 thrombosis.There were 32 late complications[14%]; 19 thrombosis, 4 aneurysm, 4 venous hypertension, 3 steal syndrome, 2 infections. There were 32 diabetic patients [17 %]. No significant differences in graft patency were noted between diabetic and nondiabetic individuals. There were no significant difference in graft patency between male and female. Overall shunt patency in 183 cases with chronic renal failure was 96% at I month, 95% at 3 months, 93% at I year, 91% at 2 years, 84% at 3 years, 56% at 5years.This Study showed that early postoperative thrombosis and diabetic vasculopathy were most causes of the vascular access failure and suggested that prevention of thrombi and well control of diabetes mellitus were most important to enhance patency rates of the vascular access.

원인 불명의 급성 신부전으로 발현된 급성 파라콰트중독 1례 (A case of Diagnosing Paraquat Intoxication on Transferred Patient with Acute Renal Failure)

  • Paraquat, a widely used herbicide, is extremely toxic, causing multiple organ failure in human. Many treatment modality has been used, but now paraquat is very fatal drug. Elimination rate of plasma paraquat seems to be a factor for the survival rate. So early diagnosis and early treatment are very important. Plasma paraquat concentration could be measured by radioimmunoassay. But it is impossible that the test was done at Emergency room and the result was checked immediately. There was relation between plasma paraquat concentrations and urine paraquat concentration. Because of its simplicity and low cost, urine paraquat concentration test is complementary to the plasma concentration measurement. If the patient has psychotic problem or unconscious mental state, and is observed unexplained dyspnea and oral ulcer, urine paraquat test is very important to rule out acute paraquat intoxication. We experienced a patient who was presented as unexplained acute renal failure initially and was diagnosed as paraquat intoxication later.
