Risk Factors for Poorer Breast Cancer Outcomes in Residents of Remote Areas of Australia

  • To investigate patient, cancer and treatment characteristics in females with breast cancer from more remote areas of Australia, to better understand reasons for their poorer outcomes, bi-variable and multivariable analyses were undertaken using the National Breast Cancer Audit database of the Society of Breast Surgeons of Australia and New Zealand. Results indicated that patients from more remote areas were more likely to be of lower socio-economic status and be treated in earlier diagnostic epochs and at inner regional and remote rather than major city centres. They were also more likely to be treated by low case load surgeons, although this finding was only of marginal statistical significance in multivariable analysis (p=0.074). Patients from more remote areas were less likely than those from major cities to be treated by breast conserving surgery, as opposed to mastectomy, and less likely to have adjuvant radiotherapy when having breast conserving surgery. They had a higher rate of adjuvant chemotherapy. Further monitoring will be important to determine whether breast conserving surgery and adjuvant radiotherapy utilization increase in rural patients following the introduction of regional cancer centres recently funded to improve service access in these areas.

  • In emergency situations, first aid workers are the main task of trauma evaluation and care. However, the scope of first-aid treatment is small and they are mainly carried out to the emergency room of the hospital. If a specialist who is in charge of an emergency situation is connected and emergency medical treatment through remote medical direction is performed, accurate diagnosis and appropriate care can be helped. This paper has developed an smart application(app) to support remote medical direction for emergency medical services. The developed smart app allows emergency rescuers to call a doctor at a remote location and transmit real-time status of emergency patients to vital sign and video. This will help to diagnose the patient's condition accurately. In addition, emergency care can be instructed and response in the emergency room can be made quickly.

  • Medical technology is gradually being developed by applying information technologies. Especially, RFID technology is being used for precise disease history information of patients [4]. And in case the patient is far away, the patient can be treated using network communication of the internet [5][6]. The internet makes us to treat or operate the patient without being restricted to time or space. If the above technologies are made as a system, the patient can be treated or operated without being restricted to time or space. In this paper, we present a patient treatment system has been implemented with a system using RFID and network communication of the internet [1][2][3][4]. The system is driven as follows. First, the information of patient can be checked from a remote PC, if the tag that a patient has been read through a reader. And a remote treatment is performed by controlling robot's arm with a joystick using internet network [19][20][21]. The RFID system was implemented in frequency of 125 KHz [1]. The information of patient can be checked with PDA, PC and C-LCD using Bluetooth and WLAN [7][8][9][10]. For the treatment and operation of the patient, the robot's arm has been formed using AX-12 motor, joystick and two buttons [11][12][13][14][15] [17][18].

E-mail Survey for Developing Clinical Trial Protocol on Individualized Acupuncture Treatment for Knee Osteoarthritis (무릎 관절염의 맞춤형 침구 임상시험 프로토콜 개발을 위한 전자우편 설문 조사)

  • Objectives : This survey was accomplished to find out how Korean medical doctors take acupuncture prescriptions for knee osteoarthritis in real clinical practice. Methods : The survey questions were developed by the consensus from 4 professors and 10 residents who major in acupuncture & moxibustion for developing clinical trial protocol on individualized acupuncture treatment for knee osteoarthritis. The questionnaires were distributed via e-mail to 3,306 members of Korea Oriental Medical Association from March 15th to March 23rd in 2005.84 members completed answers, and the computerized data were analyzed by ISP statistical program. Results : 1. 68 out of 84 Korean medical doctors used pattern diagnosis. 2. 61 out of 84 Korean medical doctors used both local and remote points, 20 doctors remote points only, and 3 doctors local acupuncture points only. 3. In case of doctors who use remote acupuncture points only, the acupuncture prescription principle was Saam or five element acupuncture (66%), along the meridian pathway (14%), Eight constitutional acupuncture (11%), Taegeuk acupuncture (2%), and miscellaneous (18%). Conclusion : In our e-mail survey, Korean medical doctors who experienced more than 10 year practice answered that they use five element acupuncture or Saam acupuncture according to meridian pathway theory as the most common principle of their acupuncture treatment prescription.

The Comparative Study on the Effect of Acupuncture Treatment with or without Indirect Moxibustion Treatment for Acute Ankle Sprain (급성 족관절 염좌에 대한 침구병행 치료와 침 단독 치료 효과의 비교 연구)

  • Objectives : The purpose of this study is to compare with the effect of acupuncture treatment with or without indirect moxibustion treatment for acute ankle sprain. Methods : This study was carried out on the 48 patients who had been treated for acute ankle sprain from June 1st to November 30th 2009 in the Dept. of Oriental Rehabilitation, Pohang Oriental Medical Hospital, Daegu Hanny University. The patients were divided into 3 groups ; Group A took near acupuncture point_needling, Group B took remote acupuncture point needling, Group C took remote acupuncture point needling with indirect moxibustion. 3 Groups had been co-treated with rest, ice, compression and elevation(RICE) therapy, physical treatment and administered Ojeok-san(Wuji-san) extract granule. To evaluating the efficiency of each treatment, Ankle-Hindfoot Scale(AHS) and Visual Analogue Scale(VAS) were applied before treatment and after 3rd treatment. Results : In results, the AHS scores were increased and the VAS scores were decreased after 3rd treatment in all group. Also, AHS scores and the VAS scores of intergroup were showed no significant change. Conclusions : In this study, there is no significant effect between only acupuncture treatment and moxibustion co-treatment for acute ankle sprain.

  • Wastewater treatment processes are usually located in the outskirts of cities. But these processes should be dealt with continuous maintenance by expert operators. Therefore, in this paper, unmaned and automated control system is designed for the SBR(Sequencing Batch Reactor) plant. This plant is constructed in Gimhae city. Networks and wireless modules are employed for the data transmission. A local controller is in the SBR plant as a client and a monitoring system is located in the other place as a server. Remote control and monitoring system are constructed at the laboratory of ours. Measured data from plant sensors are translated to the remote site using communication modules, and then the data could be displayed and analyzed by means of remote monitoring and control systems.

  • This paper introduces unmaned fully automation systems, which are applied for the CSTR(Continuously Stirred Tank Reactor) and SBR (Sequencing Batch Reactor) wastewater treatment system. The pilot plant is constructed in the country side which is little far from a main city. So networks and wireless modules are employed for the data transmission. The SBR plant has a local control and the remote monitoring system which is contained communication parts which consist of ADSL (Asymmetric Digital Subscriber Line) network and CDMA (Code Division Multiple Access) Wireless module. Remote control and monitoring systems are constructed at laboratory in a metropolis.

Remote Cerebral and Cerebellar Hemorrhage after Massive Cerebrospinal Fluid Leakage

  • Dural tears can occur during spinal surgery and may lead to cerebrospinal fluid (CSF) leakage which is rarely involved in remote cerebellar hemorrhage. Only a few of cases of simultaneous cerebral and cerebellar hemorrhage have been reported in the English literature. We experienced a case of multiple remote cerebral and cerebellar hemorrhages in a 63-year-old man who exhibited no significant neurologic deficits after spinal surgery. Magnetic resonance imaging (MRI) performed 4 days after the surgery showed a large amount of CSF leakage in the lumbosacral space. The patient underwent the second surgery for primary repair of the dural defect, but complained of headache after dural repair surgery. Brain MRI taken 6 days after the dural repair surgery revealed multifocal remote intracerebral and cerebellar hemorrhages in the right temporal lobe and both cerebellar hemispheres. We recommend diagnostic imaging to secure early identification and treatment of this complication in order to prevent serious neurologic deficits.

A Clinical Trial of Acupuncture Treatment for Frozen Shoulder (Frozen Shoulder의 침치료에 대한 임상연구)

  • Objectives : 1. Clinical trial for the efficacy evaluation of Korean acupuncture techniques in treating frozen shoulder. 2. Development of the standard clinical guidelines of the acupuncture treatment for the frozen shoulder. 3. Development of the new clinical protocol for the acupuncture treatments. 4. Verification of the hypothesis that treating at both the remote and the nearby acupoints according to the meridian theory is more effective than treating at only the nearby acupoints. Methods : 1. Research designed as Single blind, Randomized, Sham acupuncture controlled clinical Trial. 2. Assignment of 86 patients to one of three groups treated at nearby acupoints(group A), remote & nearby acupoints(group B), and sham points(group C) respectively. 3. Trial conducted at KIOM CRC of Dunsan oriental medical hospital, Daejeon Univ. 4. Estimation of the recovery rate of the frozen shoulder in subjects aged over 40. 5. Efficacy evaluation using VAS, SPADI, ROM and Improvement rate. Results : 1. There was no significant difference in VAS among the three groups. 2. Pain related scores in SPADI of the group B were significantly lower than those of the group A. 3. There was no significant difference in disability related scores of SPADI among the three groups. 4. External rotation of upper arm in the group B was significantly improved in comparison with that in the group C. 5. Abduction of upper arm in group A was improved with weak statistical significance in comparison with that in the group C. Conclusion : Acupuncture at both the remote and the nearby acupoints according to the meridian theory is effective to improve external rotation of frozen shoulder, and acupuncture at the nearby acupoints is effective to improve adduction of frozen shoulder. However it is not clear that acupuncture treatment at both the remote and the nearby acupoints according to the meridian theory is more effective than treating at only the nearby acupoints in the treatment of frozen shoulder. Since our study was a short term trial, a long term trial for a more precise evaluation of acupuncture treatment for frozen shoulder will be needed in the future.
