비소세포 폐암에서 단클론항체 M30를 이용한 세포자멸사 측정 (Detection of Apoptosis by M30 Monoclonal Antibody in Non-small Cell Lung Carcinomas)

  • 배경: 종양 발생기전에 있어서 세포자멸사가 중요한 역할을 한다. 조직 내의 세포자멸사 측정은 TUNEL (terminal deoxyribonucleotidyl transferase mediated nick end lagbelling), ISEL (in situ nick end labelling) 등의 방법을 쓰지만, 반응 세포의 비특이성 및 결과 판독의 주관성 등이 논란의 대상이 되어왔다. 최근 이러한 단점을 보완할 수 있는 단클론항체 M30이 소개되어, 인체 조직의 대장암, 가슴 샘종, 유방암, 자궁내막암 등에서 M30 면역염색을 이용한 세포자멸사 연구가 있었으나 폐암에 대한 연구는 없었다. 저자들은 비소세포 폐암에서 M30 면역염색에 의해 발현되는 세포자멸사 양상이 세포주기 핵심조절자인 p53 면역염색 발현양상 및 임상양상과 갖는 연관성을 살펴보고자 하였다. 대상 및 방법: 비소세포 폐암으로 근치적 절제수술을 받은 환자 45명을 대상으로 하였다. M30과 P53 면역조직화학염색에 실시하여 각 항체의 발현양상과 임상 병리학적 특성을 비교 분석하였다. 술 후 생존기간과 무병 생존기간을 구하였고, 단변량 분석을 실시하여 생존기간에 영향을 미치는 인자를 알아보았다. 다변량 분석을 실시하여 M30과 p53 발현양상이 술 후 사망위험도 및 재발위험도에 미치는 영향을 알아보았다. 결과: M30 양성세포 수는 p53 양성군이 p53 음성군보다 유의하게 많았고(p53 양성군 $61.7{\pm}26.8$개 vs. p53 음성군 $45.6{\pm}29.6$개, p=0.005), 세포사멸사 지수가 1 이상(Apoptosis Index, $Al{\ge}1$)인 환자 수도 p53 양성군에서 유의하게 많았다(p53 양성군 52.4% (l1/21) vs. p53 음성군 16.7% (4/24), p=0.025). 단변량 분석에서는 흡연량, 활동도(Performance Status, PS), 그리고 AI가 술 후 생존기간에 유의한 차이를 보였다. 다변량 분석에서는 AI가 높은 군에서 수술 후 암사망 위험도(Relative risk, R.R 7.482; 95% Confidence Interval, CI $1.886{\sim}29.678$; p=0.004)와 재발 위험도(R.R 3.795; 95% CI $1.184{\sim}12.158$; p=0.025)가 유의하게 증가하였다. p53 발현양상은 수술 후 생존기간과 암사망 위험도 및 재발 위험도에 영향을 미치지 않았다. 결론: 이상의 연구에서 단클론 항체 M30을 이용한 면역조직화학염색이 비소세포 폐암의 세포자멸사를 관찰하는 데 매우 유용한 방법임을 확인하였으며, 환자의 예후를 예측하는 데에도 도움이 될 수 있음을 알 수 있었다.

Prognostic Role of Hypoxic Inducible Factor Expression in Non-small Cell Lung Cancer: A Meta-analysis

  • Introduction: Reported prognostic roles of hypoxic inducible factor (HIF) expression in non-small cell lung cancer (NSCLC) have varied. This meta-analysis aimed to examine the relationship between HIF expression and clinical outcome in NSCLC patients. Methods: PubMed were used to identify relevant literature with the last report up to December $20^{th}$, 2012. After careful review, survival data were collected from eligible studies. We completed the meta-analysis using Stata statistical software (Version 11) and combined hazard ratio (HR) for overall survival (OS). Subgroup specificity, heterogeneity and publication bias were also assessed. All of the results were verified by two persons to ensure accuracy. Results: Eight studies were finally stepped into this meta-analysis in which seven had available data for HIF-$1{\alpha}$ and three for HIF-$2{\alpha}$. Combined HRs suggested that higher expression of $HIF1{\alpha}$ had a negative impact on NSCLC patient survival (HR=1.50; 95%CI=1.07-2.10; p=0.019). The expression of HIF-$2{\alpha}$ was also relative to a poorer survival (HR=2.02; 95%CI=1.47-2.77; p=0.000). No bias existed in either of the two groups. Conclusion: This study suggests that elevations of HIF-$1{\alpha}$ and HIF-$2{\alpha}$ expression are both associated with poor outcome for patients with NSCLC. The data support further and high quality investigation of HIF expression for predicting poor outcome in patients with NSCLC.

Populus alba × P. grandidentata 조직배양묘(組織培養苗)의 토양(土壤)에서의 활착(活着)과 생장(生長) (Survival and Early Growth of Populus alba × P. grandidentata In Vitro Culture Plantlets in Soil)

  • 무균상태(無菌狀態)와 최적(最適)의 환경조건(環境條件)에서 배양(培養)된 Populus alba ${\times}$ P. grandidentata 조직배양묘(組織培養苗)를 일반(一般) 온실(溫室)의 환경조건(環境條件)에 경화(硬化)시키고, 높은 활착율(活着率)과 빠른 조기생장(早期生長)을 위해서 3가지 배양토(培養土)에 이식(移植)하여 3주간(週間) misting bench에서 경화(硬化)시킨 후(後) 생육상태(生育狀態)를 비교(比較) 관찰하였다. Terralite, Jiffy mix, Jiffy-7-pellet의 3가지 배양토중(培養土中) Terralite가 조직배양묘(組織培養苗)의 활착(活着)과 조기생장(早期生長)에 가장 좋았으며, 온실이식(溫室移植) 3주후(週後)의 활착율(活着率)은 각각 평균(平均) 90%, 86.7%, 86.7%였다. 액아(腋芽)에서 기원(起源)한 조직배양묘(組織培養苗)와 multiple shoot에서 기원(起源)한 조직배양묘(組織培養苗)에 대(對)한 외부형태(外部形態)의 비교(比較)에는 차이(差異)가 없었으나 생장(生長)에 대(對)한 비교(比較)에는 유의성(有意性)이 인정되었다. 온실내(溫室內)에서 경화기간(硬化期間)동안 작고 좁으며 가는 잎과 줄기, 짧은 internode와 같은 유성(幼性)이 조직배양묘(組織培養苗)에 나타났으나, 이식(移植) 5~6주후(週後)에는 정당적(正當的)인 식물체(植物體)의 형태(形態)로 생장(生長)이 진작되었다.

Effects of Differential Distribution of Microvessel Density, Possibly Regulated by miR-374a, on Breast Cancer Prognosis

  • Background: The discovery that microRNAs (miRNAs) regulate proliferation, invasion and metastasis provides a principal molecular basis of tumor heterogeneity. Microvessel distribution is an important characteristic of solid tumors, with significant hypoxia occurring in the center of tumors with low blood flow. The distribution of miR-374a in breast tumors was examined as a factor likely to be important in breast cancer progression. Methods: Breast tissue samples from 40 patients with breast cancer were classified into two groups: a highly invasive and metastatic group (HIMG) and a low-invasive and metastatic Group (LIMG). Samples were collected from the center and edge of each tumor. In each group, six specimens were examined by microRNA array, and the remaining 14 specimens were used for real-time RT-qPCR, Western blot and immunohistochemical analyses. Correlation analysis was performed for the miRNAs and target proteins. Follow-up was carried out during 28 months to 68 months after surgery, and survival data were analyzed. Results: In the LIMG, the relative content of miR-374a was lower in the center of the tumor than at its edge; in the HIMG, it was lower at the edge of the tumor, and miR-374a levels were lower in breast cancer tissues than in normal tissues. There was no difference between VEGF-A and VCAM-1 mRNA levels at the edge and center of the tumor; however, we observed a significant difference between VEGF-A and VCAM-1 protein expression levels in these two regions. There was a negative correlation between miR-374a and target protein levels. The microvessel density (MVD) was lower in the center of the tumor than at its edge in HIMG, but the LIMG vessels were uniformly distributed. There was a significant positive correlation between MVD and the number of lymph node metastases (Pearson correlation, r=0.912, P<0.01). The median follow-up time was 48.5 months. LIMG had higher rate of disease-free survival (100%, P=0.013) and longer median survival time (66 months) than HIMG, which had a lower rate of 75% and shorter median survival time (54 months). Conclusions: Our data demonstrated miR-374a to be differentially distributed in breast cancer; VEGF-A and VCAM-1 mRNA had coincident distribution, and the distribution of teh respective proteins was uneven and opposite to that for the miR-374a. These data might explain the differences in the distribution of MVD in breast cancer and variation in breast cancer prognosis.

Hematopoietic stem cell transplantation in children with acute leukemia: similar outcomes in recipients of umbilical cord blood versus marrow or peripheral blood stem cells from related or unrelated donors

  • Purpose: This study compared outcomes in children with acute leukemia who underwent transplantations with umbilical cord blood (UCB), bone marrow, or peripheral blood stem cells from a human leukocyte antigen (HLA)-matched related donor (MRD) or an unrelated donor (URD). Methods: This retrospective study included consecutive acute leukemia patients who underwent their first allogeneic hematopoietic stem cell transplantation (HSCT) at Samsung Medical Center between 2005 and 2010. Patients received stem cells from MRD (n=33), URD (n=46), or UCB (n=41). Results: Neutrophil and platelet recovery were significantly longer after HSCT with UCB than with MRD or URD ($p$ <0.01 for both). In multivariate analysis using the MRD group as a reference, the URD group had a significantly higher risk of grade III to IV acute graft-versus-host disease (GVHD; relative risk [RR], 15.2; 95% confidence interval [CI], 1.2 to 186.2; $p$=0.03) and extensive chronic GVHD (RR, 6.9; 95% CI, 1.9 to 25.2; $p$ <0.01). For all 3 donor types, 5-year event-free survival (EFS) and overall survival were similar. Extensive chronic GVHD was associated with fewer relapses (RR, 0.1; 95% CI, 0.04 to 0.6; $p$ <0.01). Multivariate analysis showed that lower EFS was associated with advanced disease at transplantation (RR, 3.2; 95% CI, 1.3 to 7.8; $p$ <0.01) and total body irradiation (RR, 2.1; 95% CI, 1.0 to 4.3; $p$=0.04). Conclusion: Survival after UCB transplantation was similar to survival after MRD and URD transplantation. For patients lacking an HLA matched donor, the use of UCB is a suitable alternative.

CCNG2 Suppressor Biological Effects on Thyroid Cancer Cell through Promotion of CDK2 Degradation

  • This study aimed to analyze the expression and clinical significance of cyclin G2 (CCNG2) in thyroid carcinoma and the biological effects of CCNG2 overexpression in a cell line. Immunohistochemistry and Western blotting were used to analyze CCNG2 protein expression in 63 cases of thyroid cancer and normal tissues to allow the relationship with clinical factors to be assessed. CCNG2 lentiviral and empty vectors were transfected into the thyroid cancer K1 cell line. Reverse transcription-polymerase chain reaction (RT-PCR) and Western blotting were applied to detect the mRNA and protein levels of CCNG2. MTT assay and cell cycle were also conducted to assess the influence of up-regulated expression of CCNG2 on K1 cell biology. The level of CCNG2 protein expression was found to be significantly lower in thyroid cancer tissue than normal tissues (P<0.05). Western blot: The relative amount of CCNG2 protein in thyroid cancer tissue was respectively found to be significantly lower than in normal tissues (P<0.05), correlating with lymph node metastasis, clinic stage and histological grade (P<0.05), but not gender, age or tumor size (P>0.05). Loss of CCNG2 expression correlated significantly with poor overall survival time on Kaplan-Meier analysis (P<0.05). The results for biological functions showed that K1 cell transfected CCNG2 had a lower survival fraction, a greater percentage in the G0/G1 phases, and lower cyclin-dependent kinase 2 (CDK2) protein expression compared with K1 cells non-transfected with CCNG2 (P<0.05). CCNG2 expression decreased in thyroid cancer and correlated significantly lymph node metastasis, clinic stage, histological grade and poor overall survival, suggesting that CCNG2 may play important roles as a negative regulator in thyroid cancer K1 cells by promoting degradation of CDK2.

  • 본 연구는 외래종 쌍별귀뚜라미 (Gryllus bimaculatus, GB)의 도입에 따른 토착종 왕귀뚜라미 (Teleogryllus emma, TE)의 개체군 크기 감소 위협이 예상됨에 따라, 왕귀뚜라미 (TE)와 쌍별귀뚜라미 (GB)의 체장과 개체수 비율에 따른 생존 경쟁을 조사한 실험이다. 실험에 사용된 쌍별귀뚜라미 (GB)와 왕귀뚜라미 (TE) 공시충은 경북대학교 농업생명과학대학 곤충생태학연구실 내의 온실에서 대량 사육한 개체들을 사용하였다. 사육온도는 $28^{\circ}C{\sim}30^{\circ}C$이며, 장일 조건 (16L : 8D), 상대습도는 50%~60%로 유지하였다. 매일 두 종의 개체수를 계측하여 개체수 변화를 조사하였다. 본 실험에서는 더듬이를 제외한 체장 0.5 cm, 1 cm의 왕귀뚜라미 (TE)와 쌍별귀뚜라미 (GB)를 이용하여 왕귀뚜라미 (TE) : 쌍별귀뚜라미 (GB)의 개체수 비율을 1 : 2, 1 : 1, 2 : 1로 나눈 총 3가지의 조건으로 실험하였다. 그 결과, 개체가 작을 때에는 쌍별귀뚜라미 (GB)의 세력이 약하고, 시간이 지날수록 쌍별귀뚜라미(GB)의 세력이 왕귀뚜라미(TE)보다 강해지는 것을 알 수 있었다. 최종 생존종은 대부분 쌍별귀뚜라미 (GB)였으며, 쌍별귀뚜라미 (GB)의 노화가 예상됨에도 불구하고 생존력이 강한 경향을 보였다. 쌍별귀뚜라미 (GB)의 생존율은 큰 차이를 보이지 않았던 반면, 왕귀뚜라미 (TE)의 생존율이 큰 폭으로 감소하는 현상이 나타났다.

Performances of Prognostic Models in Stratifying Patients with Advanced Gastric Cancer Receiving First-line Chemotherapy: a Validation Study in a Chinese Cohort

  • Purpose: While several prognostic models for the stratification of death risk have been developed for patients with advanced gastric cancer receiving first-line chemotherapy, they have seldom been tested in the Chinese population. This study investigated the performance of these models and identified the optimal tools for Chinese patients. Materials and Methods: Patients diagnosed with metastatic or recurrent gastric adenocarcinoma who received first-line chemotherapy were eligible for inclusion in the validation cohort. Their clinical data and survival outcomes were retrieved and documented. Time-dependent receiver operating characteristic (ROC) and calibration curves were used to evaluate the predictive ability of the models. Kaplan-Meier curves were plotted for patients in different risk groups divided by 7 published stratification tools. Log-rank tests with pairwise comparisons were used to compare survival differences. Results: The analysis included a total of 346 patients with metastatic or recurrent disease. The median overall survival time was 11.9 months. The patients were different into different risk groups according to the prognostic stratification models, which showed variability in distinguishing mortality risk in these patients. The model proposed by Kim et al. showed relative higher predicting abilities compared to the other models, with the highest χ2 (25.8) value in log-rank tests across subgroups, and areas under the curve values at 6, 12, and 24 months of 0.65 (95% confidence interval [CI]: 0.59-0.72), 0.60 (0.54-0.65), and 0.63 (0.56-0.69), respectively. Conclusions: Among existing prognostic tools, the models constructed by Kim et al., which incorporated performance status score, neutrophil-to-lymphocyte ratio, alkaline phosphatase, albumin, and tumor differentiation, were more effective in stratifying Chinese patients with gastric cancer receiving first-line chemotherapy.

  • Although logistics service quality is considered an essential strategy for success and survival in today's internet environment, there are still a lot of question: what variables determine the extent of logistics service effect, how difficultly do the determinants affecting logistics service effect and how accomplish the decision making in the implementation of inter-organization information system. The purpose of this study is to examine a model to utilize the application and effect of logistics service through e-marketplaces. The results of empirical analysis are mentioned as follows; First, among the interorganization information system factors, four variables(top management support in organization characters, competitive degree in environment characters, relative advantage and information efficiency in innovation characters) have been shown to be primary determinant of the effect of logistics service quality. Second, the type of independent variable used results in a significant difference in the effect of logistics service quality. Third, Korean firms have to pursue the corporate strategy focused on logistics service quality.

Partially Parametric Estimation of Lifetime Distribution from a Record of Failures and Follow-Ups

  • In some observational studies, we have often random censoring model. However, the data available may be partially observable censored data consisting of the observed failure times and only those nonfailure times which are subject to follow up. In this paper, we present an extension of the problem of partially parametric estimation of the survival function to such partially observable censored data. The proposed estimator treats the observed failure times nonparametrically and uses a parametric model only for those nonfailure times which are subject to follow-up. We discuss the motivation and construction of the proposed estimator and investigate the limiting properties of the proposed estimator such as asymptotic normality. Also, when the assumed parametric model is exponential, the asymptotic variance of the estimator is obtained. Furthermore, an example is given to compare the proposed estimator with the modified Kaplan Meier(MKM) estimator. From the results, it is shown that the relative efficiency of the proposed estimator is higher than that of the MKM estimator in the follow-up study with increasing time.
