Risk factors for cytological progression in HPV 16 infected women with ASC-US or LSIL: The Korean HPV cohort

  • Objective This study was to identify the risk factors for cytological progression in women with atypical squamous cells of undetermined significance (ASC-US) or low-grade squamous intraepithelial lesions (LSIL). Methods We analyzed data from women infected with the human papillomavirus (HPV) who participated in the Korean HPV cohort study. The cohort recruited women aged 20-60 years with abnormal cervical cytology (ASC-US or LSIL) from April 2010. All women were followed-up at every 6-month intervals with cervical cytology and HPV DNA testing. Results Of the 1,158 women included, 654 (56.5%) and 504 (43.5%) women showed ASC-US and LSIL, respectively. At the time of enrollment, 143 women tested positive for HPV 16 (85 single and 58 multiple infections). Cervical cytology performed in the HPV 16-positive women showed progression in 27%, no change in 23%, and regression in 50% of the women at the six-month follow-up. The progression rate associated with HPV 16 infection was higher than that with infection caused by other HPV types (relative risk [RR], 1.75; 95% confidence interval [CI], 1.08-2.84; P=0.028). The cytological progression rate in women with persistent HPV 16 infection was higher than that in women with incidental or cleared infections (P<0.001). Logistic regression analysis showed a significant relationship between cigarette smoking and cytological progression (RR, 4.15; 95% CI, 1.01-17.00). Conclusion The cytological progression rate in HPV 16-positive women with ASC-US or LSIL is higher than that in women infected with other HPV types. Additionally, cigarette smoking may play a role in cytological progression.

Prognostic Accuracy of the Quick Sequential Organ Failure Assessment for Outcomes Among Patients with Trauma in the Emergency Department: A Comparison with the Modified Early Warning Score, Revised Trauma Score, and Injury Severity Score

  • Purpose: To evaluate the severity of trauma, many scoring systems and predictive models have been presented. The quick Sequential Organ Failure Assessment (qSOFA) is a simple scoring system based on vital signs, and we expect it to be easier to apply to trauma patients than other trauma assessment tools. Methods: This study was a cross-sectional study of trauma patients who visited the emergency department of Jeju National University Hospital. We excluded patients under the age of 18 years and unknown outcomes. We calculated the qSOFA, the Modified Early Warning Score (mEWS), Revised Trauma Score (RTS), and Injury Severity Score (ISS) based on patients' initial vital signs and assessments performed in the emergency department (ED). The primary outcome was mortality within 14 days of trauma. We analyzed qSOFA scores using multivariate logistic regression analysis and compared the predictive accuracy of these scoring systems using the area under the receiver operating characteristic curve (AUROC). Results: In total, 27,764 patients were analyzed. In the multivariate logistic regression analysis of the qSOFA, the adjusted odds ratios with 95% confidence interval (CI) for mortality relative to a qSOFA score of 0 were 27.82 (13.63-56.79) for a qSOFA score of 1, 373.31 (183.47-759.57) for a qSOFA score of 2, and 494.07 (143.75-1698.15) for a qSOFA score of 3. In the receiver operating characteristic (ROC) curve analysis for the qSOFA, mEWS, ISS, and RTS in predicting the outcomes, for mortality, the AUROC for the qSOFA (AUROC [95% CI]; 0.912 [0.871-0.952]) was significantly greater than those for the ISS (0.700 [0.608-0.793]) and RTS (0.160 [0.108-0.211]). Conclusions: The qSOFA was useful for predicting the prognosis of trauma patients evaluated in the ED.

Dietary Behaviors Associated with Health Perception of Korean Adolescents from Multicultural Families: based on data from the 2017 ~ 2019 Korea Youth Risk Behavior Surveys (한국 다문화가족 청소년의 건강인식과 관련한 식행동 분석: 2017 ~ 2019 청소년건강행태조사 자료를 바탕으로)

  • Objectives: This study examined the association between dietary behaviors and perceived health status among Korean adolescents from multicultural families. Methods: This cross-sectional study included 2,459 Korean adolescents from multicultural families (aged 13 ~ 18 years) who participated in the 2017 ~ 2019 Korea Youth Risk Behavior Surveys. Information on the sociodemographic variables, dietary behaviors, and lifestyle variables was self-reported using a web-based questionnaire. The dietary behaviors analyzed in this study were the breakfast and food intake frequencies, including fruit, vegetable, milk, fast food, carbonated drink, sweet drink, and high caffeine/energy drinks. The adolescents' health perception was self-rated as healthy, average, or unhealthy. The dietary behaviors associated with health perception were examined using a multiple logistic regression after adjusting for the confounding variables. Results: In this study population, 7.6% of adolescents perceived their health status as unhealthy, and 25.4% perceived it as average. The adolescents who were girls, middle school students, and in households with a low economic status showed significantly higher percentages of poor health perception (P-values < 0.001). Skipping breakfast was significantly associated with a negative health perception. Compared to the adolescents who consumed fruits every day, those who did not consume fruits during the previous week showed a higher odd ratio (OR) for a negative health perception [OR = 2.29, 95% confidence interval (CI) = 1.32-3.97]. The adolescents who frequently consumed carbonated drinks (≥ 5 times/week) perceived their health status as unhealthy relative to those who did not consume carbonated drinks (OR = 2.15, 95% CI = 1.25-3.71). Skipping breakfast was significantly associated with an increased OR for a negative health perception in girls but not in boys. Compared to adolescents with a normal weight, those with overweight/obesity (OR = 1.75, 95% CI = 1.21-2.52) and underweight (OR = 2.19, 95% CI = 1.25-3.82) showed higher ORs for negative health perception. Positive associations of overweight/obesity and underweight with negative health perception were observed in boys but not in girls. Conclusions: Dietary behaviors and weight status were associated with the health perception in Korean adolescents from multicultural families. These findings suggest that nutrition interventions on breakfast intake and healthy food choices for this population might effectively improve their weight and perceived health status.

Development of Thickness Measurement Method From Concrete Slab Using Ground Penetrating Radar (GPR 기반 콘크리트 슬래브 시공 두께 검측 기법 개발)

  • In this paper, we proposed a thickness measurement method of concrete slab using GPR, and the verification of the suggested algorithm was carried out through real-scale experiment. The thickness measurement algorithm developed in this study is to set the relative dielectric constant based on the unique shape of parabola, and time series data can be converted to thickness information. GPR scanning were conducted in four types of slab structure for noise reduction, including finishing mortar, autoclaved lightweight concrete, and noise damping layer. The thickness obtained by GPR was compared with Boring data, and the average error was 1.95 mm. In order to investigate the effect of finishing materials on the slab, additional three types of finishing materials were placed, and the following average error was 1.70 mm. In addition, sampling interval from device, the effect of radius on the shape of parabola, and Boring error were comprehensively discussed. Based on the experimental verification, GPR scanning and the suggested algorithm have a great potential that they can be applied to the thickness measurement of finishing mortar from concrete slab with high accuracy.

Yin-Yang and Five-Element Characteristics of Day Master on Four Time Pillars of Birth in Korean Population with Schizophrenia: A Consilience-Based Holistic Approach (조현병 환자군과 일반 인구군간 출생일간(出生日干)의 음양오행적 특성 비교: 통섭(統攝)적 측면에서의 접근)

  • Objectives: The existing reductionist approach has not reached complete understanding of the cause of schizophrenia. The objective of this study was to investigate yin-yang and five-element characteristics reflected on four time pillars of birth of patients with schizophrenia through comparison with the general population in the perspective of consilience-based holistic approach. Methods: This study was conducted using a random sequential recruitment method for the general population and individuals with schizophrenia aged 18 to 64 based on the exact date and time of birth using structured questionnaires. Relative positional relations of yin-yang and five-element with day master were primarily examined. In addition, the strength of day master with a score range of 0~100 points was assessed through operational score allocation. Results: Of 591 participants, 576 (346: general population, 230: individuals with schizophrenia) were analyzed. Between-group analyses showed no significant difference in the distribution of types of day master (χ2=10.41, df=9, p=0.318). However, significant between-group differences were shown in the distribution of the strength of day master (t=2.14, p=0.032) and frequency of restraining month branch (χ2=5.23, df=1, p=0.022). In logistic regression analysis, 10-point increase on the strength of day master decreased the probability of onset of schizophrenia over the age of 30 by 29.6% (p=0.002; 95% confidence interval, 0.566~0.876). Conclusions: Findings in this study suggest that four time pillars of birth might be associated with schizophrenia through yin-yang and five-element theory and synchronicity principle, implicating the plausibility of consilience-based holistic approach in the determination of risk factors or cause of schizophrenia.

  • Weather radar images can be used in a variety of ways because of their high visibility in terms of visuals. In other words it has the advantage of being able to grasp the flow of weather phenomena using not only the raw data of the weather radar, but also the change characteristics between consecutive images. In particular image processing techniques are gradually expanding in the field of meteorological research, and in the case of image data having high resolution such as weather radar images it is expected to produce useful information through a new approach called image processing techniques. In this study the weather phenomena flow was calculated as a vector from the change of the weather radar image according to time interval with the optical flow method, one of the image processing techniques. The characteristics of the weather phenomena to be analyzed were derived through vector analysis resolution suitable for the scale of weather, vector interpolation in regions where no radar echo exists, and the removal of relative flow vectors to distinguish the flow of specific weather and the entire atmosphere. Through this study, it is expected that not only the use of raw data of weather radar, but also the widening of the application area of weather radar, such as the use of unique characteristics of image data, and the active use of image processing techniques in the field of meteorology in the future.

Distribution and Origin of Carbonate Sediments near Dok Island: Preliminary Study (독도주변 탄산염퇴적물의 분포와 성인: 예비조사 결과)

  • Based on the constituent analysis of sediments near Dok Island, the origin and sedimentary facies were Investigated. The sediments are mainly from originated from volcanic and volcaniclastic rock fragments derived from Dok Island and carbonate sediments formed by a variety of shallow-dwelling organisms that secreted calcareous skeletons. Carbonate producers include mollusks (bivalves and gastropods), encrusting & branching bryozoans, encrusting & segmented red algae, worm tubes, barnacles, diatoms, sponge spicules and echinoderm fragments. The distribution and relative amount of these constituents are basically dependent upon water depth and grain size even though local variations can be observed within the same depth interval. Five sedimentary facies can be divided: nearshore facies (<20 m), neritic facies ($20{\sim}100m$), upper transitional facies ($100{\sim}200m$), lower transitional facies ($200{\sim}700m$), and hemipelagic facies (>700 m). The sediments that were sampled below the water depth of 2,000 m still contain a significant amount of carbonates (ca. $10{\sim}20%$), implying that the carbonate compensation depth in the East Sea may well exceed this water depth.

CT Quantitative Analysis and Its Relationship with Clinical Features for Assessing the Severity of Patients with COVID-19

  • Objective: To investigate the value of initial CT quantitative analysis of ground-glass opacity (GGO), consolidation, and total lesion volume and its relationship with clinical features for assessing the severity of coronavirus disease 2019 (COVID-19). Materials and Methods: A total of 84 patients with COVID-19 were retrospectively reviewed from January 23, 2020 to February 19, 2020. Patients were divided into two groups: severe group (n = 23) and non-severe group (n = 61). Clinical symptoms, laboratory data, and CT findings on admission were analyzed. CT quantitative parameters, including GGO, consolidation, total lesion score, percentage GGO, and percentage consolidation (both relative to total lesion volume) were calculated. Relationships between the CT findings and laboratory data were estimated. Finally, a discrimination model was established to assess the severity of COVID-19. Results: Patients in the severe group had higher baseline neutrophil percentage, increased high-sensitivity C-reactive protein (hs-CRP) and procalcitonin levels, and lower baseline lymphocyte count and lymphocyte percentage (p < 0.001). The severe group also had higher GGO score (p < 0.001), consolidation score (p < 0.001), total lesion score (p < 0.001), and percentage consolidation (p = 0.002), but had a lower percentage GGO (p = 0.008). These CT quantitative parameters were significantly correlated with laboratory inflammatory marker levels, including neutrophil percentage, lymphocyte count, lymphocyte percentage, hs-CRP level, and procalcitonin level (p < 0.05). The total lesion score demonstrated the best performance when the data cut-off was 8.2%. Furthermore, the area under the curve, sensitivity, and specificity were 93.8% (confidence interval [CI]: 86.8-100%), 91.3% (CI: 69.6-100%), and 91.8% (CI: 23.0-98.4%), respectively. Conclusion: CT quantitative parameters showed strong correlations with laboratory inflammatory markers, suggesting that CT quantitative analysis might be an effective and important method for assessing the severity of COVID-19, and may provide additional guidance for planning clinical treatment strategies.

Neoadjuvant therapy impact in early pancreatic cancer: "bioborderline" vs. "non-bioborderline"

  • Backgrounds/Aims: To analyze the results of the neoadjuvant treatment of patients in our center with early pancreatic cancer. Methods: Eighty-four patients with early pancreatic cancer (I-II) were included, of which 59 were considered "bioborderline" (carbohydrate antigen [CA] 19-9 > 37 U/L), and 25 were considered "non-bioborderline" (CA19-9 < 37 U/L). The R0 resection rate, presence of negative nodes, survival, and recurrence rates were analyzed in two groups, the NEO group (neoadjuvant + surgery) and the nonNEO group (upfront surgery). Results: A 28.6% pathologic complete response was observed in the NEO group of the whole sample. The residual R0 was 85.7%, and nodes were negative in 78.6% of the patients in the NEO group of bioborderline patients. All non-bioborderline patients treated with neoadjuvant were R0, and no affected nodes were observed in any of them. The median overall survival (OS) in patients with elevated CA19-9 levels in the NEO group was 31.4 months vs. 13.1 months in the non-NEO (log-rank test p = 0.006), with a 62% relative reduction in the mortality rate (hazard ratio = 0.38, 95% confidence interval: 0.20-0.79; p= 0.008). The median OS in patients with normal CA19-9 levels in the NEO group was 65.9 months vs. 16.2 months in the non-NEO group, without statistically significant differences between the two but with a trend toward significance (log-rank test p = 0.08). Conclusions: A neoadjuvant strategy seemed to improve local control and the survival of patients with early pancreatic cancer, both those with elevated CA19-9 and normal marker levels.

The Effect of Temperature on the Colony Development of Bombus ignitus Smith (호박벌(Bombus ignitus Smith)의 봉군발육에 미치는 영향)

  • The queens of Korean native bumblebee species, Bombus ignitus were collected from the field in the spring of 1997 and reared under the various temperatures as 15$^{\circ}C$$\pm$1$^{\circ}C$, 2$0^{\circ}C$$\pm$1$^{\circ}C$, $25^{\circ}C$ $\pm$1$^{\circ}C$, 3$0^{\circ}C$$\pm$1$^{\circ}C$, 60%$\pm$5% of relative humidity and 14L : 10D, to investigate their ecological characteristics, as comparing with control(29$^{\circ}C$$\pm$1$^{\circ}C$, 60%$\pm$5% of relative humidity and red light). As a result, eggs were laid by foundation queen at 15$^{\circ}C$, but they didn't hatched out while at 2$0^{\circ}C$ eggs were laid by them and they hatched out. When a foundation queen laid eggs at $25^{\circ}C$ and they all were hatched out and normally grown. Finally they were developed to the normal colony. B. ignitus worker survived for 77days at 2$0^{\circ}C$, about 69days at $25^{\circ}C$, about 68days at 3$0^{\circ}C$ and about 63days at 29$^{\circ}C$(control). The first brood workers emerged in 25-27days from the egg which a foundation queen laid. At the various temperatures, the captive queens stand to lay eggs in about 18days at 15$^{\circ}C$, 15days at 2$0^{\circ}C$, 11days at $25^{\circ}C$, 11days at 3$0^{\circ}C$ and 4days in control. The first worker and the first drone from the egg cell of the collected queens appeared in about 27days and 72days at 2$0^{\circ}C$, 26days and 88days in $25^{\circ}C$, 24days and 65days at 3$0^{\circ}C$, 25days and 71days in control, respectively. In colony foundation, 33% of the collected queen at 2$0^{\circ}C$ and $25^{\circ}C$, 100% of the disposed queen at 3$0^{\circ}C$ and in control, and 67% of them at 3$0^{\circ}C$ and in control both produced new queens in 66days and 88days, respectively. The life span of the colony founded covered fro about 3 months at $25^{\circ}C$, 3$0^{\circ}C$ and in control. At lower temperatures, the life span of queen is shorter; 2 months at 15$^{\circ}C$ and 3 months at 2$0^{\circ}C$, respectively. A colony which normally developed, varied in size with rearing temperatures; about 20heads at 2$0^{\circ}C$, 482heads at $25^{\circ}C$, 330heads at 3$0^{\circ}C$ and 452heads in control. A foundation queen monthly oviposited 1.5egg cells at 15$^{\circ}C$, 3.0egg cells at 2 $0^{\circ}C$, 21.7egg cells at $25^{\circ}C$, 42.3egg cells at 3$0^{\circ}C$ and 47.0egg cells in control. As a colony developed in June and July, egg cells as well as daily average cells increased in number, as compared to those in May and August. Also, in June and July, interval(days) of egg laying is shorter than in May and August. Number of nectar pots during the colony development varied with rearing temperatures; 31pots at $25^{\circ}C$, 39pots at 3$0^{\circ}C$, 23pots in control. The emergence of new queen showed a distinctively different two patterns; early emerging type and late emerging type. Workers are rapidly increased from early in July to early in August at $25^{\circ}C$, but at 3$0^{\circ}C$ and in control, emergence of workers are remarkably increased from the middle of June and last until July. No new queen emerged at 15$^{\circ}C$, 2$0^{\circ}C$ and $25^{\circ}C$. New queens at 3$0^{\circ}C$ and in control emerged between late in June and early in July. Thus emergence of new queen was temperature dependent.
