양.한방 뇌졸중 환자 주간호자의 삶의 질, 발병 전 환자와의 관계 및 가족지지 비교 (Comparison in Quality of Life, Relationship with Patients and Family Support, between Caregiver of Patients with Cerebro-Vascular Accident(CVA) in Western and Oriental Medicine)

  • Purpose: This study was a descriptive study to compare of quality of life, relationship with CVA patients, and family support between caregiver of patient with CVA in western and oriental medicine. Method: Total 270 caregivers for CVA patients were selected from western medicine department (114) and oriental medicine department (156) at K hospital in Seoul. The instrument tools utilized in this study were quality of life, Family support, Caregiver's relationship with patients, daily activity. Collected data were analyzed by $X^2$-test, t-test using SPSS Windows 12.0 Program. Result: The result are as follows: 1) There was no significant difference in quality of life between caregivers of patients with CVA in western and oriental medicine(t=.26, p=.792). 2) There was significant difference in relationship with patients(t=2.64, p=.009) and family support(t=2.12, p=.035) before onset of disease between caregivers of patients with CVA in western and oriental medicine. Conclusion: As a result, caregivers of oriental medicine with CVA patients showed closer relationship with patients before the onset of disease and higher family support compare to caregivers of western medicine with CVA patients.

뇌졸중 환자의 편마비 부위와 독립적인 좌위 균형과의 관계 연구 (A Study of Relationship Between Independent Sitting Balance and Side of Hemiplegia)

  • The purpose of this study is to conduct a retrospective chart audit of initial physical therapy evaluation to determine the incidence of sitting imbalance and its relationship to the side of weakness in hemiparetic patients. A review of the records of 36 patients revealed that the left side was predominantly affected in 17 patients and the right side in 19 patients. Ages, time since onset, and proportion of men and women did not difer between the left and right hemiparetic patients. About half patients(52.8%) could sit independently, but 64.7% of those with left-sided weak-ness and 31.6% of those with right-sided weakness could not. A chi-sequare analysis revealed a significant relationship between the side of weakness and independent sitting balance(p<0.1). Patients with left hemiplegia are more likely to have difficulty with independent sitting balance(p<0.1). Patient with left hemiplegia are more likely to have difficulty with independent sitting than patients with right hemiplegia, which may affect their progress in rehabilitation.

강직성 척추염 환자의 자기 효능과 운동지속과의 관계 (The Relationship between Self-efficacy and Exercise Adherence in Patients with Ankylosing spondylitis)

  • The purpose of this study was to determine the relationship between self-efficacy and adherence of exercise in patients with ankylosing spondylitis. The subjects for this study were the 50 patients with ankylosing spondylitis who had been diagnosed as out patients in the Rheumatism Center of one university hospital in Seoul and participated in the 8 weeks exercise program. The data were collected by a questionnaire the period from April 1 to May 2, 2000. Data were analyzed by t-test, ANOVA, Pearson's correlation coefficient, Frequencies, using the SAS program. The results were as follows: The mean duration for exercise adherence of ankylosing spondylitis was 12.2 months. The mean self-efficacy score was 74.3 in a possible range of 10 to 100. A significant difference of self-efficacy was found between adherers and non-adherers and self-efficacy of adherer was higher than that of non-adheres(t=4.25, p=.001). Self-efficacy was significantly associated with the total duration of an exercise adherence(r=.42, p=.001). These findings may indicate that the relationship between self-efficacy and exercise adherence in patients with ankylosing spondylitis was supported. From these results, it can be suggested that the relationship between self-efficacy and exercise adherence in patients with ankylosing spondylitis. Therefore, intervention is needed to promote self-efficacy of ankylosing spondylitis and futher studies should develop self-efficacy promoting programs for patients with ankylosing spondylitis.

입원환자의 프라이버시와 스트레스가 병실 만족도에 미치는 영향 (The Effects of the Inpatients' Stress and Privacy for the Satisfaction with Patients' Room)

  • This study proposes the structural model of inpatient's satisfaction with their room. Relationship among patients' privacy, patients' stress, and patients' satisfaction were evaluated and were discussed. Survey research followed the literature review, in which the subjects answered questions of the 12 items for the patients' privacy requirements, 10 items selected from the HSRS, and an item of patients' satisfaction. A total of 120 copies were analyzed through the statistical process using the SPSS Win Program 20.0 and SEM by the AMOS 20.0. Results and conclusions are as follows: (1) the inpatients' privacy requirements was subdivided into 'the reserve factor', 'the territoriality factor', and 'the solitude factor'. (2) The inpatients' stress level was subdivided into 'the relationship factor', 'the unfamiliarity factor', and 'the control factor'. (3) The model of relationship among the subjects' privacy requirements, stress level and their satisfaction with the patients' room showed that the subjects' stress level affected their satisfaction with the patients' room directly and the subjects' privacy requirements gave an direct and indirect effect on their' stress level and an indirect effect on their satisfaction with patients' room.

암환자 가족간호자의 가족기능 영향요인 (Factors Influencing Family Functioning in Family Caregivers of Patients with Cancer)

  • Purpose: The purpose of this study was to identify the factors related to the functioning of family caregivers of patients with cancer. Method: Data were collected by questionnaires from 124 patient-family caregiver dyads at a hospital in Seoul. Data collection was done between August, 2004 and January, 2005. Data were analyzed using Pearson correlation coefficients and stepwise multiple regression. Results: The mean score for family functioning was 68.73. Family functioning showed a significant negative correlation with burden of family caregiver and performance status of patients with cancer, and a significant positive correlation with previous relationship between the patient with cancer and caregiver. The most powerful predictor of family functioning was the relationship between the patient and caregiver. The relationship between the patient with cancer and caregiver, and performance status of the patient accounted for 25.4% of the variance of family functioning. Conclusion: The results showed that the relationship between patients with cancer and caregivers and performance status of patients with cancer were significant factors influencing family functioning in family caregiver of patients with cancer.

Multilevel Analysis of the Relationship Between Prescribing Institutions and Medication Adherence Among Patients With Hypertension and Diabetes in Korea

  • Objectives: This study investigated the relationship between prescribing institutions and medication adherence among patients newly diagnosed with hypertension and diabetes. Methods: This study investigated patients with new prescriptions for hypertension and diabetes in Korea in 2019 with using data collected from general health screenings. A multilevel logistic regression model was applied to explore the relationship between patients' first prescribing institution and their medication adherence, defined as a medication possession ratio (MPR) over 80%. Results: The overall adherence rates were 53.7% and 56.0% among patients with hypertension and diabetes, respectively. The intra-class correlation coefficients were 13.2% for hypertension and 13.8% for diabetes (p<0.001), implying that the first prescribing institution had a significant role in medication adherence. With clinics as the reference group, all other types of hospitals showed an odds ratio (OR) less than 1.00, with the lowest for tertiary hospitals (OR, 0.30 for hypertension; 0.45 for diabetes), and the next lowest in health screening specialized clinics (OR, 0.51 for hypertension; 0.46 for diabetes). Among individual-level variables, female sex, older age, higher insurance premium level, and residing in cities were positively associated with adherence in both the hypertension and diabetes samples. Conclusions: This study showed that the prescribing institution had a significant relationship with medication adherence. When the first prescribing institution was a clinic, newly diagnosed patients were more likely to adhere to their medication. These results highlight the important role played by primary care institutions in managing mild chronic diseases.

주간호자가 인지하는 알츠하이머병 환자와 주간호자의 삶의 질 (Quality of Life in the Patients with Alzheimer's Disease and their Caregivers: Caregivers' Point of View)

  • Purpose: The purpose of this study was to explore the quality of life (QoL) in patients with Alzheimer's disease (AD) and their and caregivers and its influencing factors. Methods: A descriptive research design was used. 211 pairs of AD patients and their caregivers were recruited from a neurology outpatient clinic of S medical center in Seoul, Korea. Caregivers completed several structured questionnaires regarding AD patients' QoL as well as their QoL, patients' behavioral and psychological symptoms, patients' activities of daily living (ADL), social support, and quality of relationship between AD patients and caregivers. Cognitive status of AD patients was assessed by the researchers using Korean Mini-Mental State Examination. Results: The scores of QoL were 26.91 for AD patients and 85.62 for their caregivers. In the multiple regression, factors associated with AD patients' QoL were the quality of relationship (${\beta}$=.44), ADL (${\beta}$=-.33), patient's age (${\beta}$=.20), caregivers' QoL (${\beta}$=.20), and patient's gender (${\beta}$=.11). Factors associated with caregivers' QoL were the quality of relationship (${\beta}$=.34), relationship (${\beta}$=.32), patient's QoL (${\beta}$=.26), caregivers' gender (${\beta}$=.18), social support (${\beta}$=.12), and the level of caregivers' education (${\beta}$=.12). Conclusion: To improve the QoL in AD patients and their caregivers, strategies to promote their quality of relationship are needed.

우울 및 불안장애 환자에서 아동기 학대와 정서증상 및 리질리언스와의 관계 (Relationship of Affective Symptoms and Resilience with Childhood Abuse in Patients with Depressive or Anxiety Disorders)

  • Objective : The aim of this study is to identify the relationship between childhood abuse and affective symptom including resilience in patients with depression or anxiety. Methods : A total 256 outpatients diagnosed with depressive disorder or anxiety disorder according to DSM IV-TR, were evaluated with Beck Depression Inventory (BDI), State-Trait Anxiety Inventory (STAI), Parent-Child Conflict Tactics Scale (PCCTS), Connor-Davidson Resilience Scale (CD-RISC). Independent t-test, Chi-square analysis and analysis of covariance (ANCOVA) were performed to identify the demographics of patients and the relationship between affective symptoms including resilience and childhood abuse. Results : Among demographic and clinical variables, patients with childhood abuse history were significantly higher rate in patients who were living alone and unemployed. In affective symptoms, patients with childhood abuse history were significant more severe in depressive symptoms, and state anxiety score than patients without history of childhood abuse. Patients with childhood abuse history had higher score for trait anxiety and lower score for resilience than patients who had no history of childhood abuse. Conclusion : These finding suggest that history of childhood abuse might be risk factor on depressive and anxiety symptoms severity. And this might be a predictable factor of poor treatment outcome.

만성질환아 어머니의 삶의 질에 관한 연구 (The Study on the Quality of Life for the Mothers of Hospitalized Chronic Pediatric Patients)

  • The purpose of this study were to identify the quality of life for the mothers of hospitalized chronic pediatric patients, and to explore the factors affecting the QOL of those mothers. The subjects were 201 mothers whose children had hospitalized at one University hospital with chronic diseases. Data were collected from the December, 1997 to December, 1998. We used a revised QOL instrument consisting of 34 items, 5 point likert scale based on the Noh's QOL instrument. The revised QOL consists of six subscales, those are physical status and function, self esteem, emotional status, economic status, relationship with family members, and relationship with neighborhood. Data were analyzed by t-test, ANOVA, Pearson's correlation using SPSS-PC. The results were as follows: 1. The mean score of quality of life for the mothers of chronic pediatric patients was 100.31, and item mean was 2.95. In subscale analysis, item mean of economic status was the lowest, and that of relationship with family members was the highest. 2. Correlations between characteristics of chronic pediatric patients, their mothers and QOL of mothers were as follows; 1) Total QOL of mothers had a significant positive relationship with progressing time relapse after diagnosing and age of mothers. There was a significant negative relationship between the total QOL of mothers and number of hospitalization of their children. 2) QOL on self esteem and economic status had a significant relationship with age of pediatric patients, the time relapse after diagnosis, and age of mothers. Total number of family members and QOL on economic status showed a significant positive relationship. QOL on emotional status, economic status, and relationship with family members of mothers showed negative correlations with the number of hospitalization of their children. 3. Followings were the result of difference in QOL among different demographic cha- racteristics of the subjects. 1) QOL on economic status of mothers was significantly higher when fathers of pediatric patients had jobs. 2) Total QOL score, QOL on emotional status, and QOL on relationship with neighborhood were significantly higher when mothers of pediatric patients had spouses. 3) QOL on self esteem of mothers was significantly higher when mothers had religion. 4. Followings were the result of difference in QOL among different diagnosis of the children. 1) Total QOL score of mothers whose children had congenital heart disease was higher than that of mothers whose children had leukemia and cancer. 2) QOL on emotional status, economic status, and relationship with family members of mothers whose children had congenital heart disease were higher than those of mothers whose children had leukemia, cancer, and epilepsy.

Relationship between liver iron concentration determined by R2-MRI, serum ferritin, and liver enzymes in patients with thalassemia intermedia

  • Background Iron overload is a risk factor affecting all patients with thalassemia intermedia (TI). We aimed to determine whether there is a relationship of serum ferritin (SF) and alanine aminotransferase (ALT) with liver iron concentration (LIC) determined by R2 magnetic resonance imaging (R2-MRI), to estimate the most relevant degree of iron overload and best time to chelate in patients with TI. Methods In this cross-sectional study, 119 patients with TI (mean age years) were randomly selected and compared with 120 patients who had a diagnosis of thalassemia major (TM). Correlations of LIC, as determined by R2-MRI, with SF and ALT levels, were assessed in all participants. A P-value <0.05 was considered statistically significant. Results SF and LIC levels were lower in patients with TI than in those with TM; only ferritin values were significant. We found a statistically significant relationship between SF and LIC, with cut-off estimates of SF in patients with TI who had splenectomy and those who entered puberty spontaneously (916 and 940 ng/mL, respectively) with LIC >5 mg Fe/g dry weight (P<0.0001). A significant relationship was also found for patients with TI who had elevated ALT level (63.5 U/L), of 3.15 times the upper normal laboratory limit, using a cut-off for LIC ${\geq}5mg\;Fe/g\;dry\;weight$. Conclusion We determined the cut-off values for ALT and SF indicating the best time to start iron chelation therapy in patients with TI, and found significant correlations among iron overload, SF, and ALT.
