The Effect of Catastrophic Health Expenditure on the Transition to and Persistence of Poverty in South Korea: Analysis of the Korea Welfare Panel Study Data, 2007-2012 (재난적 의료비 지출이 빈곤화 및 빈곤 지속에 미치는 영향: 복지패널 2007-2012년 자료 분석)

  • Background: The low benefit coverage rate of South Korea's health security system causes catastrophic health expenditure. And catastrophic health expenditure can be the cause of the transition to and persistence of poverty. This study was conducted to ascertain the effect of catastrophic health expenditure on the transition to and persistence of poverty, using 6 years of the Korea Welfare Panel Study Data. Methods: This study was conducted among the 22,528 households that participated in the Korea Welfare Panel Study, 2007-2012. Catastrophic health expenditure was defined as equal to or exceeds thresholds (10%, 20%, 30%, and 40%) of household's capacity to pay. The effect of catastrophic health expenditure on the transition to and persistence of poverty was ascertained via multivariate logistic regression. Results: Four-point-seven percent to 20.6% of the households are facing catastrophic health expenditure. Rates of the transition to (relative risk [RR], 18.6 to 30.2) and persistence of (RR, 74.8 to 76.0) poverty of households facing catastrophic health expenditure was higher than households not facing catastrophic health expenditure. Even after adjusting the characteristics of the household and the household head, catastrophic health expenditure was found to affect transition to (odds ratio [OR], 2.11 to 3.04) and persistence of (OR, 1.53 to 1.70) poverty. Conclusion: To prevent catastrophic health expenditure and transition to and persistence of poverty resulting from catastrophic health expenditure, the reinforcement of South Korea's health security system including the benefit coverage enhancement is required.

  • Studies aimed at reducing the occurrence of cracks by the shrinkage of concrete are in demand because the repair and reinforcement for cracks caused by declining concrete durability costs the user to maintain the concrete structure. In particular, in underground power facilities for power transmission, the cost is a heavy burden to repair and reinforce. For this reason, underground power facilities demanded effective methods for crack reduction at the engineering design step. This study, as a part of the development of shrinkage reducing agent for low shrinkage concrete on underground power facilities, investigated TEA to complement the shrinkage reducing agent to improve the early strength of concrete. In the case of TEA 3% as a shrinkage reducing agent, the early strength was improved significantly, and the shrinkage reducing effect was excellent. In addition, TEA 3.0 % and the shrinkage reducing agent 2.0 % showed excellent shrinkage property and compressive strength. On the other hand, more study of shrinkage reducing materials, including performance reviews on the shrinkage reducing materials with variable factors and type of materials, will be needed to generalize these results.

  • Steel structures have been allowed to have fatigue damage tolerance in fact. If it would be assessed whether fatigue crack is growing or not and How fast fatigue crack is propagating, we should make a rational decision on methods and a period of reinforcement in the maintenance. In this study, fatigue crack growth tests on two kinds of through-thickness cracked steel plates and a out-of-plane gusset welded joint were conducted to evaluate fatigue crack growth rate using the COD(Crack Opening Displacement), and COD measurement using strain gauges was examined to offer a practical method. As a result, we proposed a reasonable assessing method for fatigue crack growth rate using the COD and it was experimentally proved practical to estimate the COD through measuring strains.

  • The methods to improve the protection and explosion-proof performance of concrete structures include the backside reinforcement or concrete material property improvement and the addition of structural members or supports to increase the resistance performance, but they are inefficient in terms of economics and structural characteristics. This study is about the basic study on the fiber composite panel cover, and the nano-composite material and adhesive as the filler, to maximize the specific performance of each layer and the protection and explosion-proof performance as the composite panel component by improving the tensile strength, light weight, adhesion and fire-proof performances. The fiber composite panel cover (aramid-polyester ratios of 6:4 and 6.5:3.5) had a 2,348 MPa maximum tensile strength and a 1.8% maximum elongation. The filler that contained the nano-composite material and adhesive had a 4 MPa maximum tensile shear adhesive strength. In addition, the nano-composite filler was 30% lighter than the normal portland cement

Evaluation of Reinforcement Effect of Deteriorated PSC Beam through Cutting Its External Tendons (외부강선 파단실험을 통한 노후 PSC 교량의 보강효과 평가)

  • In this study, the PSC girder bridge retrofitted with external tendons is tested to verify the strengthening effects. We measure the variations of the displacement and strain at mid-span of each beam as external tendons are removed in sequence. The structural behavior of the bridge are examined using controlled truck load tests for the systems before and after all external tendons were removed. From the test results, the characteristics of structural behavior of the bridge do not change significantly, but the natural frequency is decreased after the external tendons are removed. The strengthening effects of bridges can be exactly estimated by analytical methods some extent. As a result of this study, when a PSC girder bridge is deteriorated, the bridge can be retrofitted effectively by External Prestressing Strengthening Method, and the strengthening effects can be predicted through accurate structural analysis.

A Comparison of Lower Limb Muscle Activity Affecting Sit-to-Stand Movement of Stroke Patients (뇌졸중 환자의 앉은 자세에서 일어서기 동작 수행에 영향을 주는 다리 근 활성 비교)

  • Purpose: This study aimed (1) to identify asymmetry observed in electromyography (EMG) activity patterns of lower limb muscles between affected and unaffected sides of stroke patients during sit to stand (STS) movement and (2) to identify the muscles of the paretic lower limb that affected STS performance in stroke patients. Methods: This study consisted of 27 patients (15 males and 12 females) with poststroke hemiparesis. The activation of the rectus femoris (RF), tibialis anterior (TA), and gastrocnemius (GCM) of the affected side and unaffected side during STS movement were recorded using surface EMG. In addition, EMG lower limb muscle activation patterns were measured in the all section, pre-thigh off (pre-TO), and post-thigh off (post-TO) section. All measurements were obtained during five repetitions of the STS (FTSTS) movement. An independent t-test was performed to compare the affected side and unaffected side of the lower limb during the STS movement. A stepwise linear regression analysis was conducted to identify the muscles that affected STS performance. Results: Activation of both the RF and TA significantly affected the lower limb during the STS movement (p<0.05). Two muscles showed a difference on the post-TO section. The results of the stepwise multiple regression analysis showed that the RF and TA muscles of the affected lower limb were affected in FTSTS. Conclusion: These results provide a basis for reinforcement training, focusing on the RF and TA muscles of the affected side of stroke patients to improve asymmetric movement and performance in STS movement.

Redesign of the Records Management Process at the Production Stage for Long-Term Electronic Records Preservation: A Focus on Cloud On-Nara System (전자기록의 장기보존을 위한 생산단계의 기록관리 프로세스 재설계: 클라우드 온-나라 시스템을 중심으로)

  • Deficiencies are present in records management during the production stage of long-term electronic records preservation. As such, this study seeks to address these issues by introducing Cloud On-Nara System (On-Nara Document 2.0) at records management sites and providing various systems and linked services. In particular, the factors that degrade the continuity of long-term preservation in the records management process, from the production to the utilization of electronic records, in On-Nara Document 2.0 were identified, and the causes were analyzed in compliance with ISO 15486-1:2016 and the Public Records Act. As a result, in redesigning the records management procedure, the organization of the records management hub between systems and the conversion of the document preservation format for long-term preservation were proposed. Moreover, the application of consistent production format selection criteria and the reinforcement of the production and management of granular metadata were recorded. These measures were proposed in an effort to reestablish management methods.

Simplifed Method for Estimating Energy-Dissipation Capacity of Flexure-Dominant RC Members (휨지배 철근콘크리트 부재의 에너지소산성능 평가 방법)

  • As advanced earthquake analysis/design methods such as the nonlinear static analysis are developed, it is required to estimate precisely the cyclic behavior of reinforced concrete members that is characterized by strength, deformability, and capacity of energy dissipation. However, currently, estimation of energy dissipation depends on empirical equations that are not sufficiently accurate, or experiment and sophisticated numerical analysis which are difficult to use in practice.0 the present study, nonlinear finite element analysis was performed to investigate the behavioral characteristics of flexure-dominant RC members under cyclic load. The effects of axial force, arrangement of reinforcing bars, and reinforcement ratio on the cyclic behavior were studied. Based on the investigation, a simplified method to estimate the capacity of energy dissipation was proposed, and it was verified by the comparison with the finite element analyses and experiments. The proposed method can estimate the energy dissipation of RC members more precisely than currently used empirical equations, and it is easily applicable in practice.

Fire Resistance Performance of High Strength Concrete Columns with Fireproof Gypsum Board (방화석고보드를 부착한 고강도 콘크리트 기둥의 내화성능)

  • In this study, fire resistance performance of high strength concrete specimen with fireproof gypsum board was investigated for possible use in upgrading fire-resistant performance of the existing building and repair of fire damaged structures. Fire test of eight identical high strength concrete columns were carried out for 180 minutes in accordance with ISO-834. The temperature distributions in longitudinal reinforcement and concrete temperature at various depths were recorded. The fireproof performance of gypsum board and explosive spalling of concrete were observed. The specimens with 15 mm thick twoply fireproof gypsum board spalled after gypsum board crumbled regardless of fastening methods. However, when the thickness of fireproof gypsum board was more than 30 mm, it was possible to prevent the explosive spalling and control the rebar temperature. Although the effect of cover thickness could not be compared because the explosive spalling occurred, there seemed to be no difference in insulation efficiency.

Accuracy evaluation of resin complete denture made with glass fiber mesh reinforcement before and after curing (유리섬유 보강재로 제작한 레진의치의 온성 전·후에 따른 정확성 평가)

  • Purpose: The aim of this study was to evaluate accuracy of glass fiber mesh complete denture of before and after curing. Methods: Edentulous model was selected as the master model. Ten study models were made by Type IV stone. Wax complete dentures were produced by the denture base and artificial teeth. CD and GD groups were measured six measurement distance before curing. The wax complete denture was investment after measurement is completed. Using a heat polymerization resin was injected resin. After injecting the resin it was curing. A complete denture was re-measured after curing. The measured data was verified by paired t-test. Results: Overall CD group was larger the value of the measured length. In the CD group, A-D point was larger. The smallest point was the B-D point. However, there was no statistically significant difference only C-D point(p>0.05). In the GD group, A-B point was larger. but B-D point was the smallest. A-D and B-C statistically points showed significant differences(p<0.05). Conclusion: Glass fiber mesh resin complete denture can be clinically applied to the edentulous patient.
