The Clinical Effects of Heating-Conduction Acupuncture Therapy for Lumbago Induced by Iliolumbar Ligament Sprain (전열침이 장요인대 염좌로 인한 요통에 미치는 임상적 효과)

  • Objectives : The purpose of this study is to investigate the clinical effects of heating-conduction acupuncture therapy on lumbago induced by iliolumbar ligament sprain. Methods : From September 25 to December 30, 2009, the 8 outpatients who had admitted to department of oriental rehabilitation medicine, Dae-Jeon oriental medical hospital, Dae-Jeon university, with acute lumbago were performed heating-conduction acupuncture therapy on iliolumbar ligament sprain. To evaluate the efficiency of heating-conduction acupuncture therapy, visual analogue scale(VAS) and short-form McGill pain questionnaire(SF-MPQ) were applied before treatment and after 1st, 2nd treatment. Results : 1.The VAS and SF-MPQ scores showed statistically significant improvement after 1st treatment. 2. The VAS and SF-MPQ scores after 2nd treatment showed better improvement than those of before and 1st treatment, but had no statistical significance compared with those of before and 1st treatment. Conclusions : Heating-conduction acupuncture therapy has clinical effects of pain reduction on patient with acute lumbago induced by iliolumbar ligament sprain.

Effects of Robot Rehabilitation for Range of Motion and Balance in Ankle Sprain Patient: A Single Case Study

  • Purpose: Recently, many studies on robotic rehabilitation have been conducted, but such studies on patients with ankle sprains are lacking. This study aimed to investigate the effects of robot-assisted rehabilitation on the range of motion of the ankle and balance in patients with ankle sprain. METHODS: This study used the A-B-A' design and was conducted for a total of fifteen days. The subjects performed general physical therapy for five days each, during the baseline A and A' periods. In period B, robot rehabilitation was performed for five days, along with general physical therapy. The subjects were evaluated based on weight-bearing lunge test (WBLT), single leg stance (SLS), and functional reach test (FRT). RESULTS: The WBLT, STS, and FRT showed significant improvement in periods B and A' compared to period A, but there was no significant improvement in period A' compared to period B. Conclusion: This study confirmed that robot-assisted rehabilitation was an effective intervention for improving the function of patients with ankle sprain. In the future, a study with a control group comparison should be performed.

Effects of Chuna Manual Therapy after Hip Arthroplasty: A Systematic Review and Meta-Analysis of Randomized Controlled Trials (고관절 치환술 후 추나치료 효과: 체계적 문헌고찰 및 메타분석)

  • Objectives This study was conducted to assess the effect of chuna manual therapy after hip arthroplasty. Methods We searched across 9 electronic databases (PubMed, Cochrane Library, Wangfang data, China National Knowledge Infrastructure [CNKI], Oriental Medicine Advanced Searching Integrated System [OASIS], National Digital Science Library [NDSL], Korean Medical Database [KMBASE], Koreanstudies Information Service System [KISS], Research Information Sharing Service [RISS]) to find randomized controlled clinical trials for chuna manual therapy after hip arthroplasty. Results On inclusion criteria, 11 appropriate studies were included and analyzed. The deep vein thrombosis incidence of the chuna manual therapy group was statistically lower than the conventional treatment group (p=0.0002). Chuna manual therapy significantly improved the Harris hip score compared with conventional treatment (p<0.00001). Also, chuna manual therapy combined with herbal fumigation therapy significantly elevated the hip joint function score of Harris hip score compared with the conventional therapy group (p<0.00001). Conclusions The systematic review showed that chuna manual therapy had significant effects on hip arthroplasty. Nonetheless, considering the high risk of bias and geographic bias, further research with well-designed studies is required to support the effectiveness of chuna manual therapy.

  • Purpose : The objective of this study was conducted to find out the facilitation therapy. Therapeutic exercise concepts are changed from classical therapeutic exercise and neurophysiolosical approach to facilitation therapy. Methods : This is literature study with books and PNF international course books. Results : Facilitation concepts are changed. Complex movements are the result of spinal reflex-mechanism. It was changed the to reflex-reponses are variable and organization of complex movements are determined by the necessity to move. Therefore therapy goals and concepts of spasticity have to change. Conclusion : Facilitation therapy approach by use input systems, which are needed to interact with environmental and task demands. The systems are visual, tactile, propriocepsis, vestibular, acoustical and olfactory. Facilitation therapy need these system all together with shaping.

  • Objectives : This study was aimed to find out the physiological mechanism of craniosacral therapy, especially in CV4 technique and cranial rhythmic impulses. Methods : Recent studies were reviewed for cranial rhythmic impulses, and the ancient and present text were reviewed for the therapy of sub-occiput and cranial-suture part. Results and Conclusions : 1. Suboccipital muscles releasing may have made CV4 technique to effect. 2. The mechanism of the meridians of acupuncture and craniosacral system can be cooperative together in many parts of the clinical treatments. 3. Arterial vasomotor waves have a frequency similar to reports of cranial rhythmic impulses. The methods which have relation to heart rate variability, will be useful for the study of craniosacral therapy.

  • Purpose: For patients with CNS injury who are restricted in the use of public transportation, car driving means more than simple movements and is essential for their independent lives, such as participation in society and returning to jobs. Therefore, in order to enhance the quality of life of disabled persons, their high perception and necessity of driving rehabilitation are required. The purpose of this study is to determine the perception and necessity of driver rehabilitation in patients with CNS injury. Methods: In order to survey the perception of patients with CNS injury and necessity of driving rehabilitation, questionnaires were distributed to patients with CNS injury. Questionnaires were composed of demographic characteristics, disability related characteristics, and driver's license related characteristics. Results: Our results showed that the number of driving participants with a driver's license for the disabled was significantly higher than that for non- driving participants with a previous general driver's license in the perception of driving rehabilitation. Conclusion: We suggest that driving rehabilitation for patients with CNS injury should be supported in terms of evaluation and treatment.

Related Factors of the Motivation for Rehabilitation in Stroke Patients (뇌졸중 환자의 재활동기 관련 요인 연구)

  • The purpose of this study was to investigate the relations between financial stress, depression, and conjugal affection and the motivation for rehabilitation. One hundred eighty-six stroke patients were included in the study and a survey with 65 questions was used. The survey includes questions to evaluate personal factors, characteristics related with physical therapy, characteristics related with disease, depression, conjugal affection, and motivation for rehabilitation. Education, employment, left hemiplegia and right hemiplegia, depression, financial stress, conjugal affection were statistically significant factors (p<.05). These variables have significant effects on motivation for rehabilitation. This study indicates that financial stress and depression need to be decreased to improve motivation for rehabilitation of stroke patients. It also indicates that the factors facilitating conjugal affection, education, and occupation need to be considered for rehabilitation programs.

Effect of East-West Integrative Rehabilitation on Activities of Daily Living and Cognitive Functional Recovery in Stroke Patients: A Retrospective Study (뇌졸중 환자의 일상생활 및 인지기능 회복에 대한 의·한의 협진 재활치료의 효과)

  • Objectives This study was conducted to verify the effectiveness of east-west integrative rehabilitation therapy on activity of daily living and cognitive functional recovery in stroke patients by comparing with integrative rehabilitation therapy group and conventional rehabilitation therapy group in a single institution. Methods The medical records of 106 stroke patients hospitalized in Department of Rehabilitation Medicine, Dongguk University Bundang Oriental Hospital from January 1, 2017 to February 28, 2019 were reviewed. After screening and dividing it into conventional rehabilitation (CR) group and integrative rehabilitation (IR) group, Korean version of Modified Barthel Index (K-MBI), functional independence measure (FIM), clinical dementia rating-sum of boxes (CDR-SB) were statistically analyzed. Results IR group showed significant improvement in K-MBI, FIM, and CDR-SB after treatment (p<0.001) and there was a statistically significant difference in K-MBI and CDR-SB score changes than CR group (p<0.05). And chronic patient of IR group showed significant improvement in K-MBI, FIM, and CDR-SB after treatment (p<0.01) and there was a statistically significant difference in CDR-SB score changes than CR group (p<0.05). In particular, the earlier the treatment initiation time, the more the improvement in function and when the treatment started within 2 years from the onset and patients took acupuncture and pulsed electromagnetic therapy, all scales significantly improved (p<0.001). Conclusions IR showed more improvement on activities of daily life and cognitive functional recovery than CR in this study.

The Effects of Aroma Therapy on Sleep Disorder Patients with Musculoskeletal Pain (향기요법이 통증을 주소증으로 입원한 환자의 수면장애에 미치는 영향)

  • Objectives : The purpose of this study is to investigate the effects of aroma therapy on sleep disorder patients with musculoskeletal pain. Methods : The 44 patients were divided into 2 groups: Experimental group(Exp. group) was treated with aroma therapy and control group was treated except aroma therapy. To evaluate the efficiency of aroma therapy, the Korean sleep scale A(Jin-Joo Oh, Mi-Soon Song, Sin-Mi Kim, 1998) and visual analogue scale(VAS) were applied before treatment and after 5 days treatment. Results : 1. The Experimental group has improved statistical significance in sleep disorder score as compare to the control group. 2. The improvement of sleep disorder score of Exp.group has difference according to impression. 3. The improvement of sleep disorder has an effect on pain decrease. Conclusions : Aroma therapy has effects of improvement on sleep disorder with musculoskeletal pain.

The Comparative Study on the Satisfaction of the Physical Therapy facilities in the front Military hospitals and Divisions

  • Objective: This study is about the physical therapy facilities and environmental and service satisfaction of patients visiting in the front military hospitals and divisions. Design: Survey study Methods: Prior to this survey, 28 users of the front division and military hospital were surveyed in advance to verify the survey through reliability analysis. For this survey, 1,300 questionnaires were distributed to seven frontline divisions and three military hospitals, of which 578 military hospitals and 479 divisions were collected. Among them, 1,042 pieces of data were used for the analysis, excluding 15 questionnaires that exceeded the research period of the division. Analysis methods used Cronbach-α and multiple regression for mean comparison analysis, factor analysis for validation, and reliability verification to verify differences between groups. Results: As a result of the satisfaction analysis, the division's stage satisfaction was higher than that of military hospitals, and as a result of analyzing the satisfaction items by sector, accessibility, re-visit doctors, and professionalism accounted for a large portion in the field of physical therapy services. As a result of a regression analysis, the factors that affect the satisfaction of physical therapy had the greatest impact on facility and environmental satisfaction, and expertise in physical therapy services. Conclusions: In the future, it will be necessary to improve the comfort of the physical therapy room and the expertise of physical therapy in order to improve the satisfaction level of physical therapy at military medical facilities.
