목적 : 본 연구의 목적은 양쪽 눈을 반쪽씩 가린 안경착용 중재가 뇌졸중 환자의 좌측편측무시에 미치는 효과를 알아보는 것이다. 연구방법 : 연구 기간은 2014년 10월 뇌졸중 환자 1명을 대상으로 실시되었고 참가자는 자발적으로 연구에 동의하였다. 실험은 1일 1회씩 총 12회 실시하였다. A기간은 기초선 기간으로 안경을 착용하지 않고 3회의 자료를 수집하였다. B기간은 치료기간으로 안경을 세수할 때와 잠잘 때를 제외한 모든 시간 동안 착용하도록 하였다. A(withdrawal)는 안경을 제거하고 3회의 자료를 수집하였다. 모든 검사의 측정은 1일 1회씩 동일 시간과 동일 검사자에 의해서 이루어졌다. 결과 : 선나누기 검사의 오차 평균(mm) 앨버트 테스트의 오차평균은(개)는 크게 감소하지 않았다. 결론 : 이 치료가 편측무시를 감소시키는 방법이라고 할 수는 없었다. 앞으로의 연구에서는 좀더 여러 가지 편측무시 평가를 사용하여 치료의 효과를 알아보고 여러 사항을 고려한 연구가 필요하다고 생각된다.
The regain of independent ambulatory ability is a important goal in the rehabilitation program of hemiplegic patient. Not only the function of lower extremity muscles, but also trunk muscles which stabilize extremities and pelvis, are important factors in normal gait. Therefor, it is necessary to develop an effective program which can improve muscle strength and symmetric activity of trunk muscles. The purpose of this study was to evaluate the influence of trunk muscle strengthening by forceful respiratory exercise on the gait asymmetry ratio in hemiplegic patient. 45 Hemiplegic patients due to stroke was randomized in 3 groups, forceful expiratory training(FET), forceful inspiratory training(FIT) and control group. In the experimental groups, ordinary physical therapy with forceful expiratory training and forceful inspiratory training for 20 minutes duration 3 times per week for 6 weeks were respectively performed. In the control group, only ordinary physical therapy was done. Before and after experiments, temporal-spatial gait parameters was measured in all patients. The data of 28 patients who carried out the whole experimental course were statistically analysed. The results of these experiment are as follows : 1. In comparison of difference of single support time asymmetry ratio among 3 groups, the FET group was significantly decreased than the control group (p<.05). 2. In comparison of difference of step length asymmetry ratio among 3 groups, the FIT group was significantly decreased than the control group (p<.05). Based on these results, it is concluded that the forced respiratory exercise program for 6 weeks can be improve the gait asymmetry ratio in hemiplegic patients. Therefore, the forced respiratory exercise is useful to improve the walking ability in hemiplegic patients. Since this study dealt only with the patients who could walk more than 3 meters in distance on floor independently, the further study for evaluating the influence of the forceful respiratory exercise on patients with acute stage stroke and also the development in various methods of use are expected.
Purpose : The purpose of this study figures out how the biofeedback exercise combined with a Shaker exercise and a jaw-opening exercise affects the suprahyoid muscle activation of stroke with a swallowing disorder. Methods : The study period was from June, 2018 to September, 2018, to 45 patients who were suitable for selection criteria. Participants were divided into three groups: a visuoauditory biofeedback group (VABG), and a visual biofeedback group (VBG), and a self-exercise group (SG). The three groups were divided into the Shaker exercise and the jaw-opening exercise, and the biofeedback training by themselves. Three groups performed an intervention three times a day, five times a week, and four weeks long. Also, suprahyoid muscle activity was measured with a pre-test, a post-test and a follow-up test. Results : To know the suprahyoid muscle activity after the intervention, there were statistically significant differences between the pre-test and the post-intervention (p< .01). All three groups showed the improvement with the mean comparing followed by VABG, VBG and SG. Comparing between the post- test and the follow-up test, all three groups showed the reduction of suprahyoid muscle activity with the mean comparing, followed VABG, VBG and SG. Conclusion : It found that it was more effective when providing a double-sensory biofeedback than when training with a single-sensory biofeedback. Therefore, it is necessary to provide a multi-sensory input when applying the biofeedback in rehabilitation of the swallowing disorder.
The present study was aimed at investigating the effect of swimming training on brain function after focal cerebral ischemia in rats. Therefore, this study was examined on neurogenesis in dentate gyrus of hippocampus using 5-bromo-2'-deoxyuridine (BrdU) to label proliferating cells and assessed the neurological response following focal cerebral ischemia in rats using neurological motor behavioral test. In an observer-blinded fashion, twenty male Sprague-Dawley (280~310 g, 7 weeks old) rats were divided into four groups: MCAO plus swimming group (ME, $n_1$=5), MCAO plus control group (MC, $n_2$=5), SHAM plus swimming group (SE, $n_3$=5), SHAM plus control group (SC, $n_4$=5). The results of this study were as follows: 1) The limb placing time before and after swimming in the ME group were significantly longer than the MC group (p<.05), the SE group were significantly longer than the SC group (p<.01). 2) The balance beam scores before and after swimming in the ME group was higher than the SE group, the MC group was higher than the SC group but was not significantly different (p>.001). 3) The foot fault index before and after swimming training in ME group was significantly lower (i.e., improved) than the MC group (p<.001) and the SE group (p<.001), the SE group was significantly lower (i.e., improved) than the SC group (p<.001). 4) The mean number of BrdU-positive cells in the dentate gyrus in the ME group was significantly higher than the MC group (p<.001) and the SE group (p<.01). The MC group and the SE group was significantly higher than the SC group (p<.001). 5) There was significantly correlation between limb placing time and number of BrdU-positive cells on swimming training, there was positive correlation (r=.807, p<.0001) and between foot fault index and BrdU-positive cells number, there was negative correlation (r=-.503, p<.05). However, between balance beam scores and BrdU-positive cells number, there was no correlation. In conclusion, the present study demonstrates that the role of swimming training improves behavioral motor function probably by enhancing cell proliferation in that hippocampus. This study provides a model for investigating the stroke rehabilitation that underlies neurogenesis and functional ability.
In this paper, the power spectral analysis and the fractal analysis of heart rate variability(HRV) were performed to evaluate the effects of brain lesion on cardiovascular system and autonomic function for 24 normal subjects and 22 hemiplegic patients. The ECG and respiration signals were recorded at tilt angles of $0^{\circ}$ and $70^{\circ}$ for 5 and 6 minutes successively under the condition of frequency controlled respiration (0.25Hz). For normal subjects, HR, LF component, HF component and fractral dimension of HRV were distinctly changed after orthostatic stress, whereas, for hemiplegic patients, those were little changed. Complexity and variability of heart rate of patients were smaller than those of normal subjects. Sympathetic tone of patients was higher than that in normal subjects. All of these results support that autonomic disorder and cardiovascular disturbance accompanied by brain lesion could be assessed by the power spectral analysis and fractal analysis of HRV.
Purpose : This study examines the effects of pre-eccentric exercise and stretch ing to bicepsbrachii to prevent delayed onset muscle soreness and recovery of muscular function depending on the training intensity with 28 normal adults in their twenties. Methods : The subjects were divided into a control group, a group without any previous eccentric exercise, and a stretching group. Pre-eccentric exercise group conducted exercise with the intensity of 25% of maximal voluntary contraction. Pre-eccentric exercise and stretching was applied before to induce delayed onset muscle soreness and after, 24 hour post, 48 hour post, and 72 hour post. Measurements were conducted to examine pain and muscular function changes before, immediately after, and after inducing delayed onset muscle soreness. After inducing delayed onset muscle soreness, measurements were taken at the 24th hour, 48th hour, and 72nd hour. Results : The pre-eccentric exercise group and stretching group showed a significant difference from the control group by isometric contract ion power and mechanical pain threshold as a result of measuring delayed onset muscle soreness. Conclusion : From these results, electrical stimulation using presynaptic inhibition mechanism of transcutaneous electrical stimulation (TES) had positive effects for walking ability on inhibition of muscle tone in lower extremity. The motor level stimulation group experienced a more significant effect than the sensory level stimulation group. Therefore, the transcutaneous electrical stimulation (TES) is considered to be effective on walking ability increasing through inhibition of muscle tone in lower extremity for rehabilitation of post stroke hemiplegic patients.

In comparison of difference of the gait velocity among 3 groups, there was the significantly difference between the FIT group and the control group (p<.05). 2) In comparison of gait cadence before and after experiment, the gait cadence was significantly increased in FIT group (p<.05). In comparison of the difference of the gait cadence among 3 groups, there was no significant difference between the FIT group and the control group (p>.05). Based on these results, it is concluded that the forced respiratory exercise program for 6 weeks can be improve the FAP and temporal-spatial gait parameters in hemiplegic patients. Therefore, the forced respiratory exercise is useful to improve the walking ability in hemiplegic patients.
Objectives : The purpose of this study is to identify influence that PNF method have an effect in function restoration of the impaired patients of central nervous system. Methods : The data were collected by 15 adult stroke patients. The treatment was based on proprioceptive neuromuscular facilitation techniques. Temporal and spatial parameters of gait were analysed for using the computerized GAITRite system. Results : In the comparison of functional ambulation profile(FAP) before and after experiment, the FAP was significantly increased in the PNF method. The gait velocity, cadence and single support time asymmetry ratio was significantly increased in the PNF method. The Motor Assessment Scale was significantly increased in the PNF method. Conclusions : Based on these results, it is concluded that the forced PNF method for 6 weeks can be improve the temporal-spatial gait parameters including FAP in hemiplegic patients. Therefore, the forced PNF method is useful to improve the function restoration in hemiplegic patients. Further study should be done to analyze the effects of intervention duration of treatment, optimal time to apply the treatment in more long peried.
Purpose : In order to study the effectiveness of East-West pain treatment on central poststroke pain(CPSP), we evaluated its effect on alleviation of pain and rehabilitation of CPSP patients who were treated with eletroacupuncture and west pain treatment for four weeks. Methods : Twenty four patients diagnosed by their pain characteristics of central pain form stroke were treated with sympathetic nerve block, gabapentin, amitriptyline, and electroacupuncture for four weeks. Pain intensity through the visual analogue scale(VAS), and improvements of mobility and rehabilitation through the modified Barthel index(MBI) and Rankin scale(RS), respectively, before and after pain treatment were also assessed. Results : VAS pain scores were significantly improved from $7.7{\pm}1.7$ to $4.4{\pm}2.0$ with pain treatment(p<0.05). In accordance with improvement of ain scores, RS and MBI scores ere also improved from $2.88{\pm}0.95$ to $2.13{\pm}1.01$ and from $83.0{\pm}16.9$ to $94.7{\pm}9.5$(p<0.05), respectively, with pain treatment(p<0.05). Conclusions : It was suggested that the active pain treatment was contributed to the rehabilitation of CPSP patients, resulting in improvement of quality of life of CPSP patients. Futhermore, East pain treatment in combination with West pain treatment may be useful modality to alleviate CPSP.
뇌졸중은 대부분 편마비를 수반하기 때문에 이로 인한 신체적 증상 외에도 언어장애, 신경장애와 같은 갑작스러운 변화가 생기며 이로 인한 심리적인 부적응과 상실감 및 신체적인 기능의 저하는 일상생활에서의 실패나 좌절감을 주기 때문에 심각한 정서적인 문제를 야기 시킬 수 도 있다. 뇌졸중 환자라 하면 신체적인 재활만을 고려하게 되는데 실제로는 실어증이나 구음 장애 등의 심각한 언어 기능의 저하가 일어나며 이와 더불어 인지기능의 저하, 우울증과 같은 정신적인 장애, 심지어는 청각이나 시각 등의 장애도 나타난다. 따라서 언어치료와 그 밖의 치료를 통합시켜 주는 것이 중요하며 특히 심리적으로 위축되기 쉬운 언어장애를 가진 실어증 환자에게 있어서 풍요로운 감정을 만들어주는 음악 치료의 응용은 매우 효과적일 것이다. 본 연구는 브로카 실어증을 가진 뇌졸중 노인 2명을 대상으로 멜로디 억양치료 기법(MIT)을 사용하여 주 2회씩 5주 동안 총 10회의 세션을 실행하여 결과를 분석한 사례연구이다. 본 연구의 목적은 환자의 구어적인 언어에서의 단어말하기의 음절수의 변화를 보고자 함이었으며 또한 언어 재활치료 프로그램 내에서 자신의 능력의 가치를 확인하고 성공할 때에 가지게 되는 긍정적인 심리적 효과 즉, 자신감을 줄 수 있는 효과를 유도하고자 하였다. 총 10회의 음악치료 세션 단계에서는 MIT 기법을 사용해 Manual에 따라 점수를 기록하고 세션에 따르는 향상 정도를 측정하였는데 구체적인 음절수의 변화를 보기 위하여 각각의 수준에서의 발화 음절수의 성취도를 분석하였다. 2음절에서 4음절로 구성된 프로그램에서의 결과로 보여 지는 것은 다음과 같다. 대상자 A는 Level I의 예비적인 단계는 모두 수행하였고 2음절 단어의 경우 5회에는 Level III로 넘어갈 수 있었으며, 7회에는 Level IV로 넘어 갔다. 3음절의 단어들은 7회에서 Level III로 넘어갈 수 있었으며 9회에는 Level IV로 넘어 간 것을 알 수 있었다. 마지막 4음절의 단어에서는 첫 세션 시에 8%의 저조한 성공률을 보였으나 반복적인 시도의 효과로 6회 세션에서 성취도가 급격히 증가하였으며 8회 세션에서 Level III로, 10회 세션에서 Level IV로 향상된 것을 알 수 있었다. 대상자B는 대상자 A와 마찬가지로 Level I의 예비적인 단계는 모두 수행하였고 2음절 단어의 경우 4회에서 Level III로 넘어갈 수 있었으며, 6회에는 Level IV로 넘어 갔다. 3음절의 단어들은 5회에는 Level III로 넘어갈 수 있었고 7회에는 Level IV로 넘어 간 것을 알 수 있었다. 마지막 4음절의 단어에서는 첫 세션 시에 10%의 저조한 성공률을 보였으나 역시 반복적인 시도의 효과로 5회 세션에서 성취도가 급격히 증가하였으며 7회 세션에서 Level III로 넘어갈 수 있었으나 10회 세션이 되기 까지 유의미한 성취도는 보여지지 않았다. 결론적으로 뇌졸중 환자의 언어재활 치료에 있어서의 멜로디 억양치료기법은 통계적으로 유의미하지는 않았지만 브로카 실어증을 가진 환자 2명의 음절수의 발화 성공률의 증가나 과제에 있어서의 전체적인 성공률을 볼 때 향상된 것을 알 수 있었다. 그러므로 이것은 브로카 실어증 환자의 언어적인 능력과 그들의 언어적 재활에 있어서 긍정적인 영향을 주는 것으로서 치료적인 효과를 암시한다.
