Strategy for Clinical Progress Note of Physical Therapy (물리치료 임상기록 전략)

  • Documentation of patient care has much emphasis during the past few years. As reviewing the orthopedic, neurosurgery, rehabilitation medicine of progress note, SOAP note, modified SOAP note and have been integrated a model of Korean clinical progress note of physical therapy. Academically, educationally and clinically development of physical therapy depend upon writing clinical progress note. Based on the research results, the following considerations and guideline are presented. 1. Include writing a clinical progress note of physical therapy in the school curriculums. 2. Physical therapist write the clinical progress note their procedure to the patients. 3. Korean Physical Therapy Association can provide a model of clinical progress note of physical therapy.

Rehabilitation Therapy Selection Process in Parents of Children with Cerebral Palsy: Grounded Theory (뇌성마비아동 보호자들의 재활치료 선택 과정: 근거이론 중심)

  • The purpose of this study was to identify factors that affect the optimal selection of rehabilitation therapy and explore a processing model of rehabilitation decision-making for parents of children with cerebral palsy. A ground theory of Strauss and Corbin(1988)was used. Thus, collecting data was classified into open coding 140 concepts, 29 sub-categories and 8 categories. As a result of analysis, we identified that the main phenomenon was 'finding optimal therapy in trial and error' and parents experienced 6 stages including 'embarrassment', 'confusion', 'maximization', 'wandering', 'strategy', and 'balance'. Furthermore, 'rehabilitation-daily life balance' was a key category for continuation of treatment. This qualitative study discribed factors influencing determination of treatment and its continuation, and suggested a processing model of parents' decision- making for rehabilitation of their children.

Development and Usability Evaluation of A Virtual Reality-Based Vestibular Rehabilitation System for Balance Enhancement (균형감각 증진용 가상현실 기반 전정재활 시스템 개발 및 사용성 평가 )

  • PURPOSE: The primary objective of this study was to develop a virtual reality-based vestibular rehabilitation system to enhance balance perception, target rehabilitation specialists, and evaluate its usability. A key goal was establishing a system refinement strategy based on the collected data. METHODS: We conducted a study involving ten adults aged 10 to 29 in Gwangju Metropolitan City to evaluate the usability of a virtual reality-based vestibular rehabilitation system to enhance balance perception. After introducing the product and explaining its use to the participants, balance assessments and training were conducted using computerized dynamic posturography (CDP) (also called the test of balance [TOB]). Subsequently, participants were given a questionnaire to evaluate subjective stability, operability, and satisfaction. Frequency analysis was utilized to determine the frequency of the variable values of the measurement items in the survey for descriptive statistics. RESULTS: We found that the average usability score was 2.587. When broken down by category, stability received an average rating of 2.725, operability scored an average of 2.783, and satisfaction averaged 2.454. These findings suggest that most participants experienced positive sentiments and considerable satisfaction. CONCLUSION: The study successfully developed a virtual reality-based vestibular rehabilitation system, which was an improvement over the previous model and addressed its shortcomings. The results show that users with vestibular impairments are satisfied and more engaged with this system, indicating that additional studies are warranted.

The Effects of Ankle Strategy Exercise on Balance of Patients with Hemiplegia (편마비 환자의 발목관절 전략 운동이 균형 조절 능력에 미치는 영향)

  • PURPOSE: PURPOSE: The purpose of this study was to analyze the effect and examine the feasibility of an intervention of ankle strategy exercise on balance of patients with hemiplegia. METHODS: The subject were randomly allocated to three groups: ankle strategy exercise (group A), balance exercise (group B) and control (group C). Group A was received the conventional physical therapy plus ankle strategy exercises for 20 minutes in one session. Group B was received the same conventional physical therapy plus balance exercises for 20 minutes in one session. Two active groups were performed in the session 3 times a week, for a total of 6 weeks. and Group C was only received the same conventional physical therapy. Balance test was assessed using center of pressure (COP) in the anteroposterior (A-P), mediolateral (M-L) direction, Berg balance scale (BBS) and Timed Up and Go Test (TUG). RESULTS: All groups showed improvements in balance parameters. In especial, the Group A was statistically significant differences in almost part evaluation items and showed more improvements in BBS and TUG parameters than Group B and Group C. and moving distance of M-L COP was more improvements than Group B. CONCLUSION: Ankle strategy exercises had more influence on balance than balance exercises and neurodevelopmental treatment.

Design of a Novel Gait Rehabilitation Robot with Upper and Lower Limbs Connections (상하지 연동된 새로운 보행재활 로봇의 설계)

  • This paper proposes a new rehabilitation robot with upper and lower limb connections for gait training. As humans change a walking speed, their nervous systems adapt muscle activation patterns to modify arm swing for the appropriate frequency. By analyzing this property, we can find a relation between arm swinging and lower limb motions. Thus, the lower limb motion can be controlled by the arm swing for walking speed adaptation according to a patent's intension. This paper deals with the design aspects of the suggested gait rehabilitation robot, including a trajectory planning and a control strategy. The suggested robot is mainly composed of upper limb and lower limb devices, a body support system. The lower limb device consists of a slider device and two 2-dof footpads to allow walking training at uneven and various terrains. The upper limb device consists of an arm swing handle and switches to use as a user input device for walking. The body support system will partially support a patient's weight to allow the upper limb motions. Finally, we showed simulation results for the designed trajectory and controller using a dynamic simulation tool.

Biopuncture, A Multitarget Therapy in the Treatment of Individuals with Knee Osteoarthritis: state of the art

  • Background: Biopuncture is a described technique that consists in subcutaneous injection of antihomotoxic drugs (homeopathic compounds) mixed with other substances such as local anesthetics or dextrose; This has been proposed as a treatment option to reduce musculoskeletal pain in various locations, including knee osteoarthritis. Objectives: The objective of this manuscript was to carry out a comprehensive review of the published information on the use of Biopuncture in patients with knee osteoarthritis. Methods: A scientific search was performed using online databases following the terms (Biopuncture) and (Knee Osteoarthritis) to identify scientific manuscripts that were related to the use of Biopuncture in the treatment of individuals with knee osteoarthritis. Results: With the information found, a theoretical framework was integrated that describes the components of Biopuncture, its mechanism of action and practical topics for the application of the technique. Conclusion: Biopuncture appears to be a potential, simple and low-risk therapeutic strategy in the treatment of knee osteoarthritis, which is applied through periarticular subcutaneous injections, with multitarget mechanisms of action at various physiopathological levels such as the modulation of the inflammatory process, decreased peripheral sensitization, and stimulation of antidegenerative and trophic mechanisms. Perhaps it can be part of the integrative treatments for knee osteoarthritis.

A Relationship between Depression and The metamemory and Memory Performance in Elderly Women (여성노인의 우울유무에 따른 메타기억 및 기억수행의 차이)

  • Purpose: This study tries to analyze the differences of memory performance and the metamemory of the elderly women according to degree of depression. And also it attempts to find the correlations among the sub-concepts of metamemory which have close relationships to the memory performance followed by the depression. Methods: The subjects of this study are 60 the elderly women who are older than sixty years in Busan city, Korea. We use the MIA(Dixon, et al., 1988) to measure metamemory and measure the memory performances such as the immeadiate word recall, the delayed word recall, the word recognition task, and face recognition. Results: 1. The average point of deprssed elderly womens' metamemory was significantly lower than non-depressed womens' point(t=10.86 p<.0017). Looking into subconcept of metamemory, depressed elderly womens' strategy, capacity, change, achievement point were significantly lower than non-depressed women. 2. In terms of immediate word recall and delayed word recall performances, depressed elderly women are significantly lower than non-depressed elderly women. 3. The degree of depressed elderly womens' metamemory(strategy, achievement, change, capacity) has significant correlations with immediate word recall performances. Conclusion: Metamemory has close relationship with the memory performance of elderly women. And also depressed elderly's sub-concepts of metamemory which have influences on their memory performance are different from non-depressed elderly's sub-concepts. Therefore, when we try to develop some programs to prevent memory decrease of elderly women, we should take these point into consideration.

Association with safety consciousness and cognitive-behavioral factors among university students: Focusing on the health and safety survey for university students living in dormitories on campus (대학생의 안전의식과 인지행동요인 간의 관련성: 대학생활관 입사생 건강안전기초조사결과를 중심으로)

  • Objectives: The purpose of this study was to investigate safety consciousness (SC), cognitive-behavioral strategy (CBS), and safety behavior (SB) among university students living in dormitories on campus. Methods: The data was analyzed to compare the safety behavior by sex, safety related factors, and safety consciousness index in the SPSS 23 program using Chi-square test, t-test, Pearson correlation coefficients, and multiple regression. Results: The participants in the health and safety behaviors survey were 2,721 students who have been in dormitories on campus. On the mean of items, the Public Safety Consciousness Index (PSCI) was 93.1, SC was 26.9, CBS was 15.7, and SB was 50.4. It were significant correlations between SC, CBS, and SB among university students (r=0.74-0.78), and CBS was identified as the mediator. Finally, it was significant increase the SC, CBS, and SB by sex and participation on safety education, but decrease by age and experience of accident and disaster. Conclusions: It is necessary to develop safety education program enhancing safety consciousness and safety behavior in order to prevent accidents on campus. And it suggests that safety education should be included in regular curriculum of university to improve health status and achieve academic goals.

The Communication Repair Strategy Characteristics According to Communication Breakdown of Elderly Man With Alzheimer's Dementia (알츠하이머 치매 노인의 의사소통 단절에 따른 의사소통 회복전략 특성)

  • Objective : Many communication recovery strategies should be used when communication breakdowns occur for successful communication, however, communication problems increase due to inadequate use of such strategies in older people with dementia. The purpose of this study was to investigate the difference of recovery strategy between dementia and the elderly in conversational discourse. Method : The subjects were eight of Alzheimer's dementia and 10 general elderly. Conversation discourse tasks were conducted face-to-face with the subjects. Communication breakdown and communication recovery strategies were analyzed based on 200 utterances collected in the conversation discourse. Result : First, the AD group had more communication breakdown than the control group, but the recovery rate did not differ between the groups. Second, in the AD group, the nonspecific recovery strategy and the clarification demand strategy were used as the expression strategy. The recovery rate after using expressive strategy was more than 90% in explanation strategy, combined strategy, nonspecific repair strategy, and repetition confirmation strategy. The response strategy used a lot of paraphrase strategy and combined strategies, and the recovery rate after using the response strategy was 100% for the simplification strategy, repeat strategy and gesture strategy. Conclusion : The AD group showed more breakdown of research subjects and breakdown of researchers than control group, and it showed ability to use various expression strategy and response strategy though there was difference in repair rate between communication repair strategy. AD group used nonspecific repair strategy in expression strategy the most and paraphrase strategy in response strategy the most. This shows different characteristic from ordinary elderly people. Therefore, it is necessary to utilize this repair strategy for rehabilitation of AD elderly.
