Challenges in neuro-machine interaction based active robotic rehabilitation of stroke patients

  • Study results in the last decades show that amount and quality of physical exercises, then the active participation, and now the cognitive involvement of patient in rehabilitation training are known of crux to enhance recovery outcome of motor dysfunction patients after stroke. Rehabilitation robots mainly have been developing along this direction to satisfy requirements of recovery therapy, or focusing on one or more of the above three points. Therefore, neuro-machine interaction based active rehabilitation robot has been proposed for assisting paralyzed limb performing designed tasks, which utilizes motor related EEG, UCSDI (Ultrasound Current Source Density Imaging), EMG for rehabilitation robot control and feeds back the multi-sensory interaction information such as visual, auditory, force, haptic sensation to the patient simultaneously. This neuro-controlled and perceptual rehabilitation robot will bring great benefits to post-stroke patients. In order to develop such kind of robot, some key technologies such as noninvasive precise detection of neural signal and realistic sensation feedback need to be solved. There are still some grand challenges in solving the fundamental questions to develop and optimize such kind of neuro-machine interaction based active rehabilitation robot.

암 생존자의 암 재활에 대한 개념분석 (Concept Analysis of Cancer Rehabilitation for Cancer Survivors)

  • Purpose: The purpose of this study was to identify and define the concept of cancer rehabilitation in the lives of cancer survivors. Methods: A hybrid model of concept development was used to interface theoretical analaysis and empirical observation with a focus on definition. A comprehensive literature review and semi-structured interviews with five cancer survivors were completed. Results: The concept of cancer rehabilitation was found to be a route toward a positive experience having meaning in two dimensions: recovery of the activities of daily living and intrapersonal growth. Four attributes and ten indicators were defined. Conclusion: Cancer rehabilitation was defined as a positive process of change through recovery of the activities of daily living and intrapersonal growth. Therefore, oncology nurses can be encouraged to design, implement, and evaluate cancer rehabilitation nursing interventions with the results of this study. Further research needs to develop cancer rehabilitation measurements.

  • Denervated skeletal muscle produces muscle atrophy as well as changes at the neuromuscular junction which leads to terminal axonal sprouting and an ultrastructural remodeling. NT-3 is expressed in adult muscle and motoneurons. Normally NT-3 has a potential role in regulating adult neuromuscular jungtion and recovering following muscle atrophy. Also, it could influence synaptic neurotransmission between motoneurons and skeletal muscle cells. The purpose of this study was to investigate the effect of electrical stimulation therapy(EST) on NT-3 expression in neuromuscular junction following sciatic nerve transsection in rats. 원주시 지역사회중심 재활사업 평가 (Evaluation of a Community-Based Rehabilitation Program in Wonju City)

  • This study evaluated the community-based rehabilitation services provided by the Wonju Public Health Center from Jan. 2000 to Dec. 2002. Ninety-four persons with disabilities dwelling in the community participated and the surveys were completed in an interview during home visits. The respondents' demographic, socio-economic, and medical characteristics, rehabilitation service received, willingness to receive home-visit rehabilitation services, and satisfaction with the rehabilitation services were analyzed by frequency and percentage. A Likert scoring system consisting of five agreement-disagreement categories was applied to each item, consisting of Very Satisfied, Satisfied, So-So, Poorly Satisfied, and Very Poorly Satisfied. The major findings were as follows: 1) The rehabilitation services used included medical rehabilitation (26.9%), followed by social assistance (23.5%), diagnosis by a physician at home (17.3%), medical examination (12.3%), housekeeping services (6.2%), and vocational and educational rehabilitation (3.5%). 2) Of the medical services, the respondents desired physical therapy at home and free rental of rehabilitation equipment, such as wheelchairs, canes, walkers, the most, followed by home visit occupational therapy, nursing services, and oriental medicine service in descending order. 3) Some of the respondents expressed so-so satisfaction (50.0%) or dissatisfaction (16.9%) with the rehabilitation services provided by the Wonju Public Health Center. These findings should prove useful when planning or extending community-based rehabilitation programs for the homebound disabled in the community.

Dual task interference while walking in chronic stroke survivors

  • Objective: Dual-task interference is defined as decrements in performance observed when people attempt to perform two tasks concurrently, such as a verbal task and walking. The purpose of this study was to investigate the changes of gait ability according to the dual task interference in chronic stroke survivors. Design: Cross-sectional study. Methods: Ten chronic stroke survivors (9 male, 1 female; mean age, 55.30 years; mini mental state examination, 19.60; onset duration, 56.90 months) recruited from the local community participated in this study. Gait ability (velocity, paretic side step, and stride time and length) under the single- and dual-task conditions at a self-selected comfortable walking speed was measured using the motion analysis system. In the dual task conditions, subjects performed three types of cognitive tasks (controlled oral word association test, auditory clock test, and counting backwards) while walking on the track. Results: For velocity, step and stride length, there was a significant decrease in the dual-task walking condition compared to the single walking condition (p<0.05). In particular, higher reduction of walking ability was observed when applying the counting backward task. Conclusions: Our results revealed that the addition of cognitive tasks while walking may lead to decrements of gait ability in stroke survivors. In particular, the difficulty level was the highest for the calculating task. We believe that these results provide basic information for improvements in gait ability and may be useful in gait training to prevent falls after a stroke incident.

  • Purpose: Lifestyle intervention (LSI) provides basic recommendations that improve the quality of life and health of patients with minor disabilities. The LSI intervention strategies are associated with active living, healthy weight, healthy eating, and emotional stability. These intervention strategies can change an unhealthy lifestyle to a healthy lifestyle and provide important health care information. Main issue: This study focused on a new LSI-based knee rehabilitation protocol and proved the effect of exercise prescription on the knee. The clinical significance of this study demonstrated that continuous rehabilitation, effective rehabilitation, and recurrence prevention can be achieved by prescribing the appropriate exercise for patients after discharge. Therefore, practical lifestyle medicine knowledge and information are provided by the home-based rehabilitation self-exercise program with the new LSI-based knee rehabilitation protocol. Conclusions: The LSI-based protocol can improve and maintain health conditions and knee function. With the aim of improving self-care abilities, this program is expected to make significant contributions recurrence prevention, reduced mortality, and improved quality of life, physical function, and fitness.
