비디오 영상에서 2차원 자세 추정과 LSTM 기반의 행동 패턴 예측 알고리즘 (Behavior Pattern Prediction Algorithm Based on 2D Pose Estimation and LSTM from Videos)

  • This study proposes an image-based Pose Intention Network (PIN) algorithm for rehabilitation via patients' intentions. The purpose of the PIN algorithm is for enabling an active rehabilitation exercise, which is implemented by estimating the patient's motion and classifying the intention. Existing rehabilitation involves the inconvenience of attaching a sensor directly to the patient's skin. In addition, the rehabilitation device moves the patient, which is a passive rehabilitation method. Our algorithm consists of two steps. First, we estimate the user's joint position through the OpenPose algorithm, which is efficient in estimating 2D human pose in an image. Second, an intention classifier is constructed for classifying the motions into three categories, and a sequence of images including joint information is used as input. The intention network also learns correlations between joints and changes in joints over a short period of time, which can be easily used to determine the intention of the motion. To implement the proposed algorithm and conduct real-world experiments, we collected our own dataset, which is composed of videos of three classes. The network is trained using short segment clips of the video. Experimental results demonstrate that the proposed algorithm is effective for classifying intentions based on a short video clip.

운동재활프로그램 참여자의 건강인지, 주관적 삶의 질, 운동지속의사의 관계 (Relationship between health cognition, subjective quality of life, and intention to continue exercise among exercise rehabilitation program participants)

  • 본 연구는 운동재활프로그램 참여자의 건강인지, 주관적 삶의 질, 운동지속의사의 관계를 검증하기 위하여 수도권 소재 병원 및 스포츠센터 등에 재활의 목적으로 운동프로그램에 참여하는 사람을 대상으로 설문조사를 실시하였다. 조사는 센터의 협조를 통하여 연구자가 목적 및 설문에 대한 정보를 상세히 설명하고, 동의 후 총 228부 중 신뢰성이 없다고 판단한 24부를 제외한 204부를 대상으로 하였다. 수집된 자료는 빈도분석, 기술통계, 요인분석과 신뢰도 분석을 실시하였으며, 잠재변인 간 관계 파악을 위해 상관분석과 회귀분석을 적용하였다. 분석한 결과 운동재활프로그램 참여자의 건강인지의 정도와 주관적 삶의 질, 운동지속의 관계에는 유의미한 영향을 주는 것으로 나타났다. 향후 건강인지의 수준을 높일 수 있는 방안이 필요하며, 개인의 물리적 환경 및 사회적 환경을 고려한 후속연구가 필요하다고 사료된다.

일상생활수행 제한 노인들의 재활운동행위의 실천에 따른 영향요인 분석(계획된 행위이론을 적용하여) (Influential Factors on Rehabilitation Exercise Practice in Elderly Lmited Activities of Daily-Living: An Analysis based on the Theory of Planned Behavior)

  • 이 연구는 계획된 행동이론(TPB)을 적용하여 일상생활수행에 제한이 있는 노인 환자들의 자발적 재활운동 행위의 실천에 영향을 주는 예측인자를 분석하고자 부산 경남지역 노인전문병원에 입원한 노인 중일상생활수행에 제한이 있는 350명을 대상으로 1차 설문조사를 실시하였다. 1차 설문자 중 316명에 대해 4주간의 재활운동교육을 시행하고 2주후 재활운동 실천 행위를 확인하기 위해 2차 설문조사를 하였다. 연구결과 재활운동행위에 대한 의도에 미치는 영향력은 태도(.61), 지각된 행위통제(-.56), 주관적 규범 (.27) 순이었으며, 운동행위 실천에 대해 의도는 유의하였으나, 지각된 행위는 유의하지 않아 선행연구결과와 차이가 있었다. 본 연구에서 태도가 지각된 행위통제에 비하여 의도를 더 잘 예측하는 것으로 나타났는데, 이는 노인일수록 주관이 강하고 자신의 생각을 쉽게 바꾸지 않는 성향에 따라 태도가 의도로 표출되는 것이므로 재활운동행위의 실천을 유도하기 위해서는 다양한 교육을 통한 긍정적인 건강신념을 가지게하는 것이 우선되어야 최대 효과를 기대할 수 있을 것으로 생각된다.

운동의도 검출을 위한 근육반력과 근전도의 비교 (Comparison of EMG and Muscle Reaction Force to Detect Exercise Intention)

  • Activeness of exercise is critical for stroke rehabilitation so that application of stimulation in response to patient's intention would be effective in FES cycling. The purpose of this study was to investigate the relationship between muscle reaction force (MRF) and electromyogram (EMG) during cycling exercise, for the future usage of MRF as patients' intention signal. Seven young men ($24{\pm}1.63$ yrs) participated in this study. Cycling speed was set to 20 RPM and 60 RPM. MRF and EMG were measured in the vastus lateralis muscle of right leg. Active cycling was performed at the maximal load (16 Nm) of an ergometer. Angle dependent artifact in MRF was measured from passive cycling and was subtracted from the MRF of active cycling. The delay of MRF with respect to EMG envelope and their correlation coefficients were derived from the best of cross correlation. MRF was significantly correlated with EMG amplitude in all subjects (p<0.01). Their mean correlations were 0.84 and 0.91 for 20 RPM and 60 RPM, respectively. Mean delay in MRF was 59.14 ms and 53.14 ms for 20 RPM and 60 RPM, respectively. The result suggests that MRF can be used to assess patient's intention for exercise as a substitute to EMG. The method may be applied to FES cycling to encourage patient's effort which is critical for stroke rehabilitation.

Effects of Group Rehabilitation Gymnastics for Stroke Patients

  • Objective: This study is an experimental study to investigate the effect of group rehabilitation gymnastics on stroke patients. Design: A randomized controlled trial. Methods: From August 1, 2017, to February 30, 2018, 49 subjects hospitalized after being diagnosed with a stroke at K Rehabilitation Hospital had at least 8 weeks (more than 3 times a week) in the group rehabilitation exercise experimental group (25 patients), and the control group (24 patients) did not attend. Data analysis was performed using paired t-test for dependent variables before and after the experiment using SPSS 25. Results: In the experimental group, daily activities were statistically significantly increased from 59.04±25.19 points before treatment to 66.96±24.35 points after treatment (t=2.24, p=0.035). Lung capacity also significantly increased from 280.00±86.99 points to 334.40±93.23 points (t=4.21, p<0.001), and hospital reuse intention also significantly increased from 8.04±1.57 points to 8.88±1.16 points (t=2.67, p=0.013). In the case of the control group, although the MBI, lung capacity, stress, hospital reuse intention, and hospital use satisfaction increased after the experiment compared to before the experiment, there was no significant difference. Conclusions: In conclusion, group rehabilitation gymnastics for stroke patients is effective for daily living movements, lung capacity, and hospital re-use intention. Therefore, if it is continuously applied to stroke patients, it can be used as an intervention to improve the quality of nursing by enhancing physical function.

운동 의도에 따른 뇌파-근전도 신호 간 연결성 분석 (Connectivity Analysis Between EEG and EMG Signals by the Status of Movement Intention)

  • 뇌와 근육은 상의 하달식 구조로 상지 운동 수행 과정에서 뇌파와 근전도 신호의 변화와 함께 기능적 연결성이 발생한다. 본 논문에서는 사용자가 상지 운동을 수행하였을 때의 뇌파와 근전도 신호에 대해 코히어런스 방법을 적용하여 운동 의도 여부에 따른 뇌와 근육간의 연결성 차이를 규명하고자 한다. 상지 운동을 수행하는 과정에서 운동 피질 영역의 뇌파는 뮤 리듬(mu rhythm, 8~14 Hz)과 베타 리듬(beta rhythm, 15~30 Hz)에서 활성화 되며, 근전도 신호는 베타 리듬과 파이퍼 리듬(piper rhythm, 30~60 Hz)에서 활성화 된다. 뇌파와 근전도 신호간의 코히어런스 분석 결과 운동 의도를 포함한 능동 운동 수행 시 수동 운동을 수행하였을 때 보다 유의미한 차이로 높은 코히어런스 계수가 확인되었다. 이는 인지적 반응과 근육의 움직임 사이의 코히어런스 관계로 운동 의도가 포함된 상지 운동 수행 과정에서의 뇌와 근육간의 연결성을 해석할 수 있었다. 운동 의도에 따른 뇌-근육간의 코히어런스 특징을 이용한다면 재활기기 사용자의 운동 의도에 따라 피드백이 필요한 재활 훈련 시스템 설계에 도움이 될 수 있을 것으로 사료된다.

편마비 환자를 위한 보행 재활기구 개발 (Development of the Gait Rehabilitation Equipment for Hemiplegic Patients after Stroke)

  • The aim of this study is to design and develop the gait rehabilitation equipment that judge patient's movement of his/her center of gravity using pressure sensors, and to aid hemiplegic patients to balance themselves using an automatic stepper that changes the patient's center of gravity. It is hard to bear the weight on the affected side for hemiplegic patients. The gait rehabilitation equipment detects the footing phase of hemiplegic patient during training and moves the unaffected footing side of the stepper up and moves the affected footing side down simultaneously so that the patient's center of gravity can shift from unaffected side to affected side. The gait rehabilitation system was developed and applied for hemiplegic patients during exercise. Eight hemiplegic patients and one normal adult were studied. The developed gait rehabilitation system could judge not only the normal adult's intention but also the patient's intention to move his/her center of gravity. Even though the most of hemiplegic patients exercised in automatic mode and a few hemiplegic patients exercised in manual mode, the developed gait rehabilitation system can aid the hemiplegic patients to train more easily.

The Effect of Physical Therapist's Expertise and Interactivity on Revisit Intention Based on Trust

  • Purpose: This study aimed to examine how the expertise and interactivity of a physical therapist impact a patient's intention to revisit a hospital based on trust. Methods: We surveyed 274 patients who received physical therapy in Seoul and Gyeonggi Province to assess their reliability and revisit intentions based on their expertise and interactivity. SPSS 22.0 was used for frequency analysis and reliability verification, while AMOS 18.0 was used for confirmatory factor analysis and model verification. Results: Physical therapist interactivity significantly impacted patients' intentions to revisit based on trust. The physical therapist's expertise had a significant effect on trust but did not demonstrate a significant effect on the intention to revisit. Conclusion: The interactivity of physical therapists has an important effect on patients' intentions to revisit a hospital based on trust. Although therapist-centered expertise can generate trust in patients, it positively affects the intention to revisit the hospital. Therefore, it is suggested that physical therapists' patient-centered expertise and interactivity build patients' trust and are important for revisiting intention.

장애인의 운동실태와 영향요인 (State and factors influencing on exercise in the disabled)

  • 본 연구는 장애인 운동건강증진 프로그램을 위한 기초자료로서 활용하기 위하여 장애인의 운동실태를 확인하고 운동에 영향을 미치는 요인을 파악하고자 하는 서술적 연구로서 한국에 거주하는 장애인 369명을 대상으로 연구가 이루어졌다. 연구결과 지역사회에 거주하는 장애인의 84.5%가 운동을 규칙적으로 하고 있는 것으로 나타났다. 대상자의 운동은 장애원인(p<.001), 장애유형(p<.001), 장애등급(p<.01), 교육정도(p<.01), 경제상태(p<.05), 재활체육 의도(p<.001)에 따라 통계적으로 유의한 차이가 있었다. 장애인의 운동에 영향을 미치는 요인은 재활체육 의도로 나타났으며, 재활체육에 대한 의도를 가지고 있는 대상자가 의도를 갖고 있지 않은 대상자에 비해 운동을 실천할 승산은 16.18배(95% CI = 7.45~37.14) 높은 것으로 나타났다. 이상과 같은 연구결과 장애인들이 꾸준히 규칙적으로 운동을 유지할 수 있도록 병원 퇴원 후 지역사회에서 지속적인 간호중재와 장애인 대상 운동 유지 증진을 위한 간호전략 및 프로그램의 개발이 필요하다.

Variations in lateral abdominal muscle thickness during abdominal drawing-in maneuver in three positions in a young healthy population

  • Objective: To investigate the appropriate position for abdominal drawing-in maneuver (ADIM) exercise by rehabilitative ultrasound image. Design: Cross-sectional study. Methods: Twenty-eight young adults with no history of low back pain participated in the study. Three positions compared were crook lying position with hip $60^{\circ}$ flexion, standing position with the feet hip width apart and knees straight, and saddle standing positionunsupported with the knees $20^{\circ}$ flexed. Once in the appropriate position, the subjects were verbally cued to draw in their abdominal wall, with the intention of pulling their navel inward toward their lower back. The thickness of each transversus abdominis (TrA), internal oblique (IO), and external oblique (EO) muscles were measured via ultrasound and recorded at the end of inspiration. Results: When compared to the TrA thickness of rest, the TrA thickness was significantly increased in all three positions (crook lying, standing, and saddle standing) during the ADIM (p<0.05). IO thickness was significantly greater in standing and saddle standing than in crook lying (p<0.05). EO thickness was constant in all the three positions. Conclusions: The present study suggests that standing and saddle standing positions could be recommended for the ADIM to maximize recruitment of the TrA and IO activation. Specifically, the saddle standing position with knees flexed to $20^{\circ}$ was observed to increase the TrA activation more than the standing position. These findings should be considered when core stability exercises such as the ADIM are conducted.
