Central serous chorioretinopathy associated with low dose systemic corticosteroid treatment of Behcet's disease (베체트병 환자에서 저용량 스테로이드 사용과 관련하여 발생한 중심성장액맥락망막병증)

  • Central serous chorioretinopathy may induce poor eyesight and serous retinal detachment. However, its exact cause has not been well established thus far. It can be associated with systemic high-dose corticosteroid treatment mainly for young and middle-aged men and may spontaneously regress or recur after withdrawal from corticosteroid. After corticosteroid administration for Behcet's disease, it is necessary to identify any ocular symptoms. Behcet's disease can lead to the development of ocular complications, such as uveitis, hypopyon, retinal vasculitis, optic neuritis, angiogenesis, secondary cataract, and glaucoma. It is possible to diagnose any of these complications via optical coherence tomography and digital indocyanine green angiography. It is easy to neglect an ocular symptom that may appear after a low-dose corticosteroid treatment as an ocular complication in patients with Behcet's disease. Thus, we report on a case concerning high-dose corticosteroid treatment with a literature review.

  • Background: Low back pain is a frequent condition that results in substantial disability and causes admission of patients to neurosurgery clinics. To evaluate and present the therapeutic outcomes in lumbar disc hernia (LDH) patients treated by means of a conservative approach, consisting of bed rest and medical therapy. Methods: This retrospective cohort was carried out in the neurosurgery departments of hospitals in KahramanmaraŞ city and 23 patients diagnosed with LDH at the levels of L3-L4, L4-L5 or L5-S1 were enrolled. Results: The average age was $38.4{\pm}8.0$ and the chief complaint was low back pain and sciatica radiating to one or both lower extremities. Conservative treatment was administered. Neurological examination findings, durations of treatment and intervals until symptomatic recovery were recorded. $Las{\grave{e}}gue$ tests and neurosensory examination revealed that mild neurological deficits existed in 16 of our patients. Previously, 5 patients had received physiotherapy and 7 patients had been on medical treatment. The number of patients with LDH at the level of L3-L4, L4-L5, and L5-S1 were 1, 13, and 9, respectively. All patients reported that they had benefit from medical treatment and bed rest, and radiologic improvement was observed simultaneously on MRI scans. The average duration until symptomatic recovery and/or regression of LDH symptoms was $13.6{\pm}5.4$ months (range: 5-22). Conclusions: It should be kept in mind that lumbar disc hernias could regress with medical treatment and rest without surgery, and there should be an awareness that these patients could recover radiologically. This condition must be taken into account during decision making for surgical intervention in LDH patients devoid of indications for emergent surgery.

  • Purpose: Although Bednar's aphthae are common and regress spontaneously, these lesions may lead to feeding intolerance and are often misdiagnosed, rendering examinations useless. This study sheds new light on the clinical features of Bednar's aphthae. Methods: Sixteen neonates and infants were newly diagnosed with Bednar's aphthae via routine health check-ups in an outpatient clinic. Medical records were retrospectively reviewed, and the following parameters were analyzed; sex, gestational age, birth weight, mode of delivery, and perinatal problems. A physical examination was carried out during the next outpatient visit to examine the healing process and check for the existence of scars or complications. Results: Initial presentation included changes in feeding habits (n=10), longer feeding time, reduced intake, and increased irritability. In 6 patients, Bednar's aphthae were discovered incidentally, without prior symptoms. Feeding posture and method of feeding are important causes of Bednar's aphthae. Eleven patients were fed in a horizontal position, whereas 5 patients were fed in a semiseated position. Fifteen patients were bottle-fed, whereas 1 patient was exclusively breastfed. After correcting the feeding position, the ulcerative lesions disappeared within 1 month of diagnosis. During the follow-up period, lesions did not recur in any of the patients. Conclusion: This study suggests that Bednar's aphthae are caused by mechanical pressure. A diagnosis of Bednar's aphthae should be considered when lesions are found on the palate of infants and when symptoms seem to be feeding related. Proper education of parents can both treat Bednar's aphthae and easily prevent its recurrence.

  • The normal intracranial structures are relatively resistant to therapeutic radiation, but may react adversely in a variety of ways, and the damage to nerve tissue may be slow in making its appearance, and once damage has occured the patient recovers slowly and incompletly. Therefore, it is important to consider the possibility of either recurrent tumor or late adverse effect in any patient who has had radiotherapy. The determination o( rnorphological/pathological correlation is very important to the therapeutic radiologist who uses CT scans to define a treatment volume, as well as to the clinician who wishes to explain the patient's clinical state in terms of regress, progression, persistence, or recurrence of tumor or radiation-induced edema or necrosis, The authors are obtained as following results ; 1. The field size(whole CNS, large, intermediate, small field) was variable according to the location and extension of tumor and histopathologic diagnosis, and the tatal tumor dose was 4,000 to 6,000 rads except one of recurred case of 9,100 rads. The duration of follow up CT scan was from 3 months to 5 year 10 months. 2, The histopathologic diagnosis of 9cases were glioblastoma multiforme(3 cases), pineal tumor (3), oligodendroglioma (1), cystic astrocytoma (1), pituitary adenoma (1) and their adverse effects after radiation therapy were brain atrophy (4 cases) , radiation necrosis(2), tumor recurrence with or without calcification (2), radiation·induced infarction (1). 3. The recurrent symptoms after radiation therapy of brain tumor were not always the results of regrowth of neoplasm, but may represent late change of irradiated brain. 4. It must be need that we always consider the accurate treatment planning and proper treatment method to reduce undesirable late adverse effects in treatment of brain tumors.

Application of Bioinformatics for the Functional Genomics Analysis of Prostate Cancer Therapy

  • Prostate cancer initially responds and regresses in response to androgen depletion therapy, but most human prostate cancers will eventually recur, and re-grow as an androgen independent tumor. Once these tumors become hormone refractory, they usually are incurable leading to death for the patient. Little is known about the molecular details of how prostate cancer cells regress following androgen ablation and which genes are involved in the androgen independent growth following the development of resistance to therapy. Such knowledge would reveal putative drug targets useful in the rational therapeutic design to prevent therapy resistance and control androgen independent growth. The application of genome scale technologies have permitted new insights into the molecular mechanisms associated with these processes. Specifically, we have applied functional genomics using high density cDNA microarray analysis for parallel gene expression analysis of prostate cancer in an experimental xenograft system during androgen withdrawal therapy, and following therapy resistance, The large amount of expression data generated posed a formidable bioinformatics challenge. A novel template based gene clustering algorithm was developed and applied to the data to discover the genes that respond to androgen ablation. The data show restoration of expression of androgen dependent genes in the recurrent tumors and other signaling genes. Together, the discovered genes appear to be involved in prostate cancer cell growth and therapy resistance in this system. We have also developed and applied tissue microarray (TMA) technology for high throughput molecular analysis of hundreds to thousands of clinical specimens simultaneously. TMA analysis was used for rapid clinical translation of candidate genes discovered by cDNA microarray analysis to determine their clinical utility as diagnostic, prognostic, and therapeutic targets. Finally, we have developed a bioinformatic approach to combine pharmacogenomic data on the efficacy and specificity of various drugs to target the discovered prostate cancer growth associated candidate genes in an attempt to improve current therapeutics.

Rapid Regression of White Matter Changes in Hypoglycemic Encephalopathy (저혈당성 뇌병증에서 뇌백질 변화의 빠른 퇴행)

  • Purpose : In a previous report, it took several days for white matter lesions to regress in hypoglycemic encephalopathy. We present a case of rapid diffusion-weighted image (DWI) changes in hypoglycemic encephalopathy. Case Report: A 58-year-old male patient was found semi-comatous with the only abnormality in his laboratory tests showing hypoglycemia (44 mg/dL). After rapid correction of glucose level, immediate brain DWI showed bilateral subcortical white matter lesions. After about 5 hours, follow-up DWI showed resolved subcortical white matter lesions, with newly-appeared bilateral fronto-temporo-parietal cortical lesions. Conclusion: Both white matter and cortex involvement in hypoglycemic encephalopathy has been shown in several reports, but rapid regression of white matter changes in hypoglycemic encephalopathy has been rarely reported. It is important to know that MR imaging changes in hypoglycemic encephalopathy can be made as quick as just a few-hour-long.

Flow cytometry analysis of DNA ploidy of transmissible venereal tumors in the Jindo dogs (유식세포 분석법에 의한 진도개 전파성 성기육종의 DNA Ploidy 유형분석)

  • Transmissible venereal tumor(TVT) is a naturally occurring contagious neoplasm which can be transmitted by mechanical contact during mating in dogs and transplanted as intact viable cells to dogs and other members of canine family such as coyotes, jackals, wolves, and foxes. The incidence of this tumors tends to increase in Korean native Jindo dogs. This is probably due to the high density and unrestrained management system. With time, TVT reaches the maximum size and then tends to regress spontaneously unless individuals are immunologically compromised. It consists of different types of cells depending on the stage. In this study, 10 tumors were selected from Jindo dogs. These were histologically calssified into three stages; progressive, steady-state, and regressive. Mitotic figures were counted, and their histological appearance at each stage is compared with their DNA ploidy. Histologically, 5 tumor cases were calssed as the progressors, 3 cases as the steady-state tumors, and 2 cases as regressors. Progressors were composed of round cells with large nuclei containing conspicuous nucleoli and frequent mitotic figures. A few spindle-shaped cells and inflammatory cells including mainly lymphocytes, a few neutrophils and macrophages were also seen. In the steady-state tumors, there was an increased number of spindle shaped cells and mitotic figures were rare. Six tumors were diploid and four were aneuploid with the variation coefficient of 7.02. Two of five progressive tumors were aneuploid. Two of three steady-state tumors were aneuploid while both tumors at the regressive stage were diploid. Progressive and steady-state tumors had a much larger S/G2M fraction and a higher mitotic index than regressive tumors. Two tumors which persisted for more than one year were aneuploid. These results suggest that the progressive and steady-state tumors had more active cell division than the regressive neoplasms.

Long Term Impact of Distribution Information Technology Investment on Firm Value (무선인식 유통정보기술 투자가 장기 주가수익률에 미치는 영향에 관한 연구)

  • Purpose - This paper investigates the long term impact of RFID investment on firm value in Korea. We wand to find out why the long term performance of some firm's RFID investment is better than others. To understand the dynamics of the long term returns from RFID investment announcements, we divide our events into groups for each of the independent firm characteristic variable such as investment time period, kind of markets, industries, solvency and growth potential. We composed portfolios based on the RFID investment announcement date for each group and evaluate the monthly abnormal excess returns. Research design, data, and methodology - Based on these calendar-time portfolios, we measure the long term returns from 86 RFID investment announcements of 46 firms from 2003 to 2017. We construct the calendar-time portfolio for 3, 6, 9, 12 months of holding periods. Using the weighted least squares method, we regress the raw monthly returns of the portfolios on the Fama-French model and Carhart(1997) model. As a result, we can get the estimated risk adjusted mean monthly abnormal excess return αP for each of the calendar-time portfolio. Results - We found that early adopters, large firms, non-manufacturing firms have very significant excess returns. We also found modestly significant excess returns for financially stable firms and slow growing firms. Put together, top managers of the firms which plan to invest RFID should understand the strategic role of RFID adoption and the generalized business process of distribution information technology investment in Korea. Moreover, the findings of this paper provide useful trading strategies to the managers of large funds who are considering on investing in RFID adopting firms. Conclusions - Put together, the results of this paper give us a new insight into how the RFID and IT technology in general and other characteristic factors' interactions affect the long term performance of firms. Using the unbiased estimates of long term returns of the calendar-time portfolios, this paper extends the understandings on short term impact of RFID adoption of existing studies. This paper also extends the current understandings of firm characteristics that affect the long term performance of RFID adopting firms.

Performance Improvement of General Regression Neural Network Using Principal Component Analysis (주요성분분석에 의한 일반회귀 신경망의 성능개선)

  • This paper proposes an efficient method for improving the performance of a general regression neural network by using the feature to the independent variables as the center for partern-layer neurons. The adaptive principal component analysis is applied for extracting, efficiently the fcarures by reducing the dimension of given independent variables. In can acluevc a supertor property of the principal component analysis that converts input data into set of statistically independent features and the general regression neuralnetwork, espedtively. The proposed general regression neural network has been applied to regress the Solow's economy(2-independent variable set) and the wie elephone(1-independent vanable set). The simulation results show that the proposed meural networks have better performances of the regressionfor the lest data, in comparison with those using the means or the weighted means of independent variables. Also,it is affected less by the number of neurons and the scope of the smoothing factor.

Positivism And Relativism In Consumer Research: Any Possibility of Eclecticism? (소비자 연구방법론으로서의 Positivism과 Relativism의 절충주의 모색에 관한 소고)

  • According to Ferber (1988), philosophy of science considerations are particularly important for an area like consumer research that seeks to be interdisciplinary. The fact that the topic is studied by so many disciplines with different research traditions immediately raises the question of how one evaluates the (often conflicting) knowledge claims of its various practitioners. Closely related to this is the issue of whether it is even possible to study the topic in a truly interdisciplinary manner or whether a multidisciplinary approach is worth trying. In fact, this issue has triggered heated debates on positivism and relativism in consumer research and a significant number of consumer researchers, though majority of them still resorts to the traditional positivist paradigm, has rejected the tenets of positivism and has turned instead to relativism as the emergent paradigm for knowledge generation. The purpose of this paper is to elaborate on the possibility of eclecticism between positivism and relativism in consumer research. However, self-justifying system of each research camp let any comparison or attempt to reconcile regress to the questioning of philosophical assumption, "Objective reality does exist?", which can not be resolved. As Kuhn (1970) noted, philosophy of science may be chosen for reasons that are ultimately personal and subjective. Therefore, the possibility of eclecticism may be reserved. Rather, complementarity of each research practice can be suggested to make consumer research achieve holistic disciplinary status. Both camps can get nowhere by challenging respective philosophical underpinnings but can advance knowledge on substantive and methodological domains by complementing each other.
