In emergency dispatching at 119 Command & Dispatch Center, some inconsistencies between the 'standard emergency aid system' and 'dispatch protocol,' which are both mandatory to follow, cause inefficiency in the dispatcher's performance. If an emergency dispatch system uses automatic speech recognition (ASR) to process the dispatcher's protocol speech during the case registration, it instantly extracts and provides the required information specified in the 'standard emergency aid system,' making the rescue command more efficient. For this purpose, we have developed a Korean large vocabulary continuous speech recognition system for 400,000 words to be used for the emergency dispatch system. The 400,000 words include vocabulary from news, SNS, blogs and emergency rescue domains. Acoustic model is constructed by using 1,300 hours of telephone call (8 kHz) speech, whereas language model is constructed by using 13 GB text corpus. From the transcribed corpus of 6,600 real telephone calls, call logs with emergency rescue command class and identified major symptom are extracted in connection with the rescue activity log and National Emergency Department Information System (NEDIS). ASR is applied to emergency dispatcher's repetition utterances about the patient information. Based on the Levenshtein distance between the ASR result and the template information, the emergency patient information is extracted. Experimental results show that 9.15% Word Error Rate of the speech recognition performance and 95.8% of emergency response detection performance are obtained for the emergency dispatch system.
Background: The effect of mobilization on lumbar back pain has been fully described in several clinical aspects, but evidence for muscle strength would be still less clear. Objective: To assess the effect of lumbar mobilization on lower limb strength in healthy individuals. Methods and Analysis: Healthy people aged 18-65 will be included regardless of race or sex. Original peer-reviewed primary reporting randomized controlled trials (RCTs) will be included. Electronic databases, such as MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, Web of Science, Pedro, CINAHL, ClinicalTrials.gov will be searched from inception until July 30. Only studies published in English will be included in this review. Two reviewers will complete the screening for eligibility independently, and the other two reviewers will also complete the risks of data extraction and bias assessment independently. Lower Limb strength will be assessed as primary outcome, and particular intervention or participant characteristics will be assessed as the secondary outcomes. Meta-analysis will be conducted using Review Manager 5.3.3, and evidence level will be assessed using the method for Grading of Recommendations Assessment, Development and Evaluation. Outcomes will be presented as the weighted mean difference or standardized mean difference with 95% CI. If I2 ≤ 50%, P>.1, the fixed effect model will be used, otherwise, random-effects model will be used. Ethics and dissemination: This review might not be necessary ethical approval because it does not require individual patient's data; these findings will be published in conference presentations or peer-reviewed journal articles. PROSPERO registration number: CRD42020150144.

Objectives: The purpose of this overview was to summarize the evidence regarding the effectiveness of Cognitive Behavioral Therapy (CBT) for sleep disorders through systematic reviews (SRs) and meta-analyses (MAs). Methods: An overview of systematic review was conducted according to the study protocol (reviewregistry1320). A comprehensive literature search was performed using three databases (Pubmed, Cochrane Central Register of Controlled Trials, and Web of Science) and three Korean databases (KoreaMed, KMbase, and ScienceON). Final studies were selected by three authors according to inclusion and exclusion criteria, and data needed for analysis were extracted by a pre-planned extraction framework. Methodological quality of systematic review was assessed using the 'Assessment of multiple systematic reviews 2 (AMSTAR2)'. Results: Fourteen SRs and MAs were included, of which eleven SRs were performed MAs. Twelve studies studied insomnia among sleep disorders, and the rest are nightmares and sleep disturbances with PTSD. Ten studies reported the effect of CBT on sleep disorders measured by insomnia severity index (ISI) and sleep onset latency (SOL), and all reported a significant improvement effect. Eight studies reported the effect of CBT on sleep disorders measured by wake time after sleep onset (WASO), and seven studies reported a significant improvement effect. The methodological quality of the studies evaluated with AMSTAR 2 was mainly low or very low because of omission of protocol registration and excluded study list. Conclusions: Practical guidelines and studies show that CBT is effective for sleep disorders, but access to CBT needs to be improved.
Objectives: This study investigated the pattern identification (PI) and clinical index of Parkinson's disease (PD) for personalized diagnosis and treatment. Methods: This prospective observational multi-center study recruited 100 patients diagnosed with PD from two Korean medicine hospitals. To cluster new subtypes of PD, items on a PI questionnaire (heat and cold, deficiency and excess, visceral PI) were evaluated along with pulse and tongue analysis. Gait analysis was performed and blood and feces molecular signature changes were assessed to explore biomarkers for new subtypes. In addition, unified PD rating scale II and III scores and the European quality of life 5-dimension questionnaire were assessed. Results: The clinical index obtained in this study analyzed the frequency statistics and hierarchical clustering analysis to classify new subtypes based on PI. Moreover, the biomarkers and current status of herbal medicine treatment were analyzed using the new subtypes. The results provide comprehensive data to investigate new subtypes and subtype-based biomarkers for the personalized diagnosis and treatment of PD patients. Ethical approval was obtained from the medical ethics committees of the two Korean medicine hospitals. All amendments to the research protocol were submitted and approved. Conclusions: An objective and standardized diagnostic tool is needed for the personalized treatment of PD by traditional Korean medicine. Therefore, we developed a clinical index as the basis for the PI clinical evaluation of PD. Trial Registration: This trial is registered with the Clinical Research Information Service (CRIS) (KCT0008677)
Objectives The purpose of this study is to assess the reporting quality and methodological quality of systematic reviews from the Journal of Pediatrics of Korean Medicine. Methods Systematic reviews were selected from the Journal of Pediatrics of Korean Medicine (JPKM) by utilizing Oriental Medicine Advanced Searching Integrated System (OASIS) and JPKM homepage. Two independent researchers assessed the reporting quality through Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guideline checklist, and assessed the methodological quality of systematic review through Assessment of Multiple Systematic Reviews (AMSTAR) 2 tool checklist. Results Four systematic reviews were finally selected for the assessment. When assessed by PRISMA, three literatures were little insufficient, and one literature was sufficient. When assessed by AMSTAR 2, three literatures were moderate quality, and one literature was critically low quality. Also, all of the reviews had no information about 'Protocol and registration', 'publication bias', and 'conflicts of interest'. Conclusions Systematic review is important for Journal of Pediatrics of Korean Medicine and Korean Medicine Society. Efforts are needed to improve the reporting and methodological quality of the systematic reviews through PRISMA and AMSTAR 2.

In this paper, we propose an Internet of Things (IoT) platform for ocean observation buoys. The proposed system consists of various sensor modules, a gateway, and a remote monitoring site. In order to integrate sensor modules with various communications interfaces, we propose a controller area network (CAN)-based sensor data packet and a protocol for the gateway. The proposed scheme supports the registration and management of sensor modules so as to make it easier for the buoy system to manage various sensor modules. Also, in order to extend communication coverage between ocean observation buoys and the monitoring site, we implement a multi-hop relay network based on a mesh network that can provide greater communication coverage than conventional buoy systems. In addition, we verify the operation of the implemented multi-hop relay network by measuring the received signal strength indication between buoy nodes and by observing the collected data from the deployed buoy systems via our monitoring site.
Two location algorithm (TLA) considers two location areas (LAs) to track the location of a mobile station (MS). The TLA significantly outperforms the single location algorithm when the call-to-mobility ratio (CMR) is low (i.e., the user moves more frequently than to receive calls). However, because of the paging cost for the users moving back to the previously visited LA, the performance of the TLA degrades when the CMR is high. To reduce those paging cost, we propose the new algorithm called TLA with call information (TLAC). In TLAC, we use the location information of the MS within the setup message when the MS originates a call in the previously visited LA. Through the setup message, the system can know the exact location of the MS without the additional location registration operation, and the paging cost for the arriving calls in the visited LA can be reduced. An analytical model is proposed to compare the TLAC and previous TLA protocol. Our study indicates that the TLAC always guarantees better performance than the TLA. We also observe that the TLAC is much better than the TLA when the CMR is high.
In recent years, there has been increased to access multimedia service through the Internet due to rise of the access speed. We suppose the co-existence of the various access networks in adjacent area in the future, such as wireless LAN, Bluetooth, and 3G Cellular So, the size of the cell will be smaller and the number of access point will be increased. In other words, the potable devices need the efficient mobility scheme due to move between cells frequently Currently, the several of the scheme, which is Cellular IP, HAWAII, Mobile IPv4 regional registration, and hierarchical Mobile IPv6, is suggested to support micro-mobility but Mobile IPv4 is standard scheme to support the mobility of the mobile node. MPLS, which forwards the packet through switching instead of routing using IP address, has begun to deploy in the internet backbone to increase the performance of mobility protocol. The integration of both MPLS and Mobile IP improves the scalability of the Mobile IP data forwarding process by leveraging on the features MPLS which are fast switching and high scalability. Moreover it abbreviates IP-in-IP tunneling between HA and FA. In this paper, we suggest efficient mobility scheme through integration of both MPLS and hierarchical Mobile IPv6.
