말초신경자극이 동통반응에 미치는 영향 (Effect of Electrical Stimulation of Peripheral Nerve on Pain Reaction)

  • Experiments were conducted in ischemic decerebrate cats to study the effects of electroacupuncture and electrical stimulation of peripheral nerve on pain reaction. Flexion reflex was used as an index of pain. The reflex was elicited by stimulating the sural nerve(20 V, 0.5 msec duration) and recorded as a compound action potential from the nerve innervated to the semitendinosus muscle. Electroacupuncture was performed, using a 23-gauge hyperdermic needle, on the tsusanli point in the lateral upper tibia of the ipsilateral hindlimb. The common peroneal nerve was selected as a peripheral nerve which may be associated with electroacupuncture action, as it runs through the tissue portion under the tsusanli point. Both for electroacupuncture and the stimulation of common peroneal nerve a stimulus of 20 V-intensity, 2 msec-duration and 2 Hz-frequency was applied for 60 min. The results are summerized as follows: 1) The electroacupuncture markedly depressed the flexion reflex; this effect was eliminated by systemic application of naloxone $(0.02{\sim}0.12\;mg/kg)$, a specific narcotic antagonist. 2) Similarly, the electrical stimulation of the common peroneal nerve significantly depressed the flexion reflex, the effect being reversed by naloxone. 3) When most of the afferent nerves excluding sural nerve in the ipsilateral hindlimb were cut, the effect of electroacupuncture on the flexion reflex was not observed. Whereas direct stimulation of the common peroneal nerve at the proximal end from the cut resulted in a significant reduction of the flexion reflex, again the effect was reversible by naloxone application. 4) Transection of the spinal cord at the thoracic 12 did not eliminate the effect of peripheral nerve stimulation on the flexion reflex and its reversal by naloxone, although the effect was significantly less than that in the animal with spinal cord intact. These results suggest that: 1) the analgesic effect of an electroacupuncture is directly mediated by the nervous system and involves morphine-like substances in CNS, 2) the site of analgesic action of electroacupuncture resides mainly in the brainstem and in part in the spinal cord.

근전도 전자 망치를 이용한 깊은힘줄반사의 측정자 간 신뢰도 (Inter-Rater Reliability of the Deep Tendon Reflex by using EMG Electric Reflex Hammer)

  • Purpose : The purpose of this study was to evaluate the reliability analysis of the deep tendon reflex by using electromyography (EMG). Methods : The study was tested on 30 volunteers who are women in their 20s. Using an electronic reflective hammer of EMG, deep tendon reflex was measured on all subjects with the participation of three trained physical therapists as raters. First, the subjects were comfortably seated on a table with their knees bent at 90 °. The three raters tapped the electric hammer at intervals of 10 seconds to avoid habituation until a total of 10 compound muscle action potential records were collected. Intraclass correlation coefficients (ICCs) were calculated to assess the inter-rater reliability of the deep tendon reflex with the use of EMG. The items of analysis included amplitude (mV), latency (ms), duration (ms), and area (mV × ms) of the compound evoked potentials. Results : Based on the average records of 10 compound muscle action potential, excellent reliability (ICC: .912) was achieved in terms of area, and there was good reliability in terms of latency (ICC: .795) and duration (ICC: .800). In the shortest latency of the compound muscle action potential, good reliability was achieved in terms of amplitude (ICC: .865), duration (ICC: .781), and area (ICC: .832). In the amplitude of peak-to-peak of compound muscle action potential, excellent reliability was recorded in terms of amplitude (ICC: .924), and good reliability was recorded in terms of duration (ICC: .801) and area (ICC: .874). Conclusion : The findings in this study indicate that electromyography via an electric hammer is a reliable method of assessing and measuring deep tendon reflexes. Especially, it may be an excellent gauge in the area of average values of the compound muscle action potentials and the amplitude of peak-to-peak of compound muscle action potentials.

정상인에서 고빈도의 무통증성 경피적 신경자극이 RIII Nociceptive Flexion Reflex와 Temporal Summation에 미치는 영향 (The Effects of High-frequency, Non-noxious TENS on RIII Nociceptive Flexion Reflex and Temporal Summation in Human Subjects)

  • Background: Transcutaneous electrical nerve stimulation (TENS) has been used widely, but its effects are controversial. This is probably due to the varying intensity and type of pain. We designed a study to assess the effects of the TENS on the RIII nociceptive flexion reflex as the resting pain level and the temporal summation as a repeated, movement related pain in 7 normal volunteer subjects. Methods: High frequency (80 Hz), non-noxious TENS was applied over the left popliteal fossa for 20 minutes. Ipsilateral RIII reflexes induced by single electrical stimulus and temporal summation of pain responses to repeated stimuli (five stimuli at 2 Hz) were recorded before, during (just before stopping), and subsequently at 20 minutes after TENS. Results: R (III) nociceptive flexion reflex activity during and after TENS was more significantly decreased than before treatment. However, the temporal summation threshold was not changed. Conclusions: We conclude that high frequency, non-noxious TENS could be effective on resting pain relief in the same segment but not on the movement related pain.

다양한 진자운동을 재현가능한 경직의 동적 역치 모델 (Dynamic Threshold Model of Spasticity that Can Predict Various Pendulum Motions)

  • The objective of this work is to develop the knee joint model for representing various pendulum motions and quantifying the spasticity. Knee joint model included the extension and flexion muscles. The joint moment consists of both the active moment from the stretch reflex and the passive moment from the viscoelastic joint properties. The stretch reflex was modeled as nonlinear feedback of muscle length and the muscle lengthening velocity, which is Physiologically-feasible. Moreover, we modeled the spastic reflex as having dynamic threshold to account far the various pendulum trajectories of spastic patients. We determined the model parameters of three patients who showed different pendulum trajectories through minimization of error between experimental and simulated trajectories. The simulated joint trajectories closely matched with the experimental ones, which show the proposed model can predict pendulum motions of patients with different spastic severities. The predicted muscle force from spastic reflex appeared more frequently in the severe spastic patient, which indicates the dynamic threshold relaxes slowly in this patient as is manifested by the variation coefficient of dynamic threshold. The proposed method provides prediction of muscle force and intuitive and objective evaluation of spasticity and it is expected to be useful in quantitative assessment of spasticity.

정상 성인에 대한 정량적 무릎힘줄반사 검사의 측정자간 신뢰도 (Inter-Rater Reliability of Quantitative Knee Tendon Reflex Test for Healthy Subjects)

  • The purpose of this study was to evaluate inter-rater reliability of the amplitude and first knee extension angles in deep tendon reflex test by using an electrical hammer. Twenty-five healthy adults participated in the study. Compound muscle action potential is elicited by tapping the knee tendon with an electrical hammer in deep tendon reflex tests. The amplitude and knee extension angle were simultaneously measured. The mean value of the amplitude and the knee extension angles through three time trials for each tester, are used for determining the inter-rater and Intra-class Correlation Coefficients (ICCs) reliabilities. According to the results, the ICCs of the amplitude is .280 and that of the knee extension angle is .789. Pearson correlation coefficients of the amplitude of the action potential and the knee extension angles are .685, showing significant statistically moderate correlation. Inter-rater reliability in the amplitude was not significant. More objective and quantitative deep tendon reflex tests should be done to obtain higher reliability in further studies.

  • The effect of cervical proprioceptors on the control of eye movement and body posture was examined in unanesthetized labyrinthine intact and bilateral labyrinthectomized cats. Cervico-ocular reflex(COR) was elicited by stimulation of the cervical proprioceptors by means of sinusoidal rotation of head or body in the darkness. The following results were obtained: 1) In labyrinthine intact cats, sinusoidal rotation of the whole body elicited compensatory eye movement(vestibulo-ocular reflex: VOR); the direction of eye movement was opposite to the direction of head rotation. 2) Anticompensatory eye movement was observed by sinusoidal rotation of the body with head fixed in labyrinthine intact cats; the direction of eye movement was the same as the direction of head rotation. 3) Compensatory eye movement was observed by sinusoidal rotation of the head with body fixed or sinusoidal rotation of the body with head fixed in both acute and chronic bilateral labyrinthectomized cats. These results suggest that the cervical proprioceptors are important in the control of ocular movement and posture in the bilateral labyrintectomized cats, although they are questionable in labyrinthine intact cats.

안와관골 복합체 골절에서 술전 평가로서 눈깜박반사 검사의 유용성 (Usefulness of the Blink Reflex Study as a Preoperative Evaluation in the Orbitozygomatic Complex Fracture)

  • Purpose: As the evaluation of the preoperative sensibility in the orbitozygomatic complex fracture, used by most surgeons, depends on the patient's subjective judgements, it is difficult to make generalization and to use it as an objective evaluation method. We used the blink reflex study to objectively evaluate injury to the infraorbital nerve. Methods: From December 2008 to November 2009, a total of 16 patients underwent the patient's subjective report on sensory symptoms and the blink reflex study preoperatively. Among patients having orbitozygomatic complex fracture of type III or more according to Henderson's classification and simultaneously suspected as being injured along the infraorbital nerve pathway, patients who had difficulty in checking preoperative sensibility and said 'normal sensibility' were selected as candidates. Results: Fifteen patients showed abnormal R1 on the fracture side. These results suggested that most of patients had injury to the infraorbital nerve. Conclusion: Contrary to the existing tests, the blink reflex study is a useful diagnostic tool in reflecting injury to the infraorbital nerve objectively.

Reflex Action of the Semicircular Canals on Cervical Extensor and Flexor Muscles in Cats

  • In urethane anesthetized cats, each vestibular semicircular canal nerve was electrically stimulated, and reflex responses of the cervical extensor and flexor (the splenius capitis and sternomastoid muscles) were recorded by means of electromyography. Stimulation of a unilateral (anterior, horizontal or posterior) canal nerve elicited excitation of the contralateral cervical muscles and inhibition of the ipsilateral ones; during the canal nerve stimulation, the two muscles in one side of the neck revealed synergistic responses. Based on these experimental results, we formulated a diagram showing the functional connections between the vestibular semicircular canals and the cervical muscles in the vestibulocollic reflex.
