Objectives : This study is to report the effect of Scolopendrid Pharmacopuncture on two patients suffering from the pain of popliteal part. Methods : We treated the patients with the pain of popliteal part by acupuncture according to meridian(Taeyang and Soyang-Meridian), and we injected 0.1${\sim}$0.2cc of Scolopendrid Pharmacopuncture to his pain site. And then we evaluated their pain by VAS(Visual Analog Scale). Results : 1. A combined treatment-Scolopendrid Pharmacopuncture and Acupucture- had a very excellent effect on relieving pain of popliteal part originated from trigger point and referred pain of Popliteus or Biceps Femoris. 2. It appears that Scolopendrid Pharmacopuncture is very effective against trigger point of muscle and pain site fixed.
In Incheon Severance Hospital, a secondary delivery hospital, anesthesiologists have treated cancer pain in the operation room when referred from other department. Intrathecal neurolytic block is a valuable means of producing high quality pain relief in any hospital. It is simple to carry out, requires brief hospitalization, can be used in elderly or severely ill patients, can be repeated with the block wears off and its duration is sufficient for the terminal cancer patients. We reviewed the clinical charateristics of the intrathecal alcohol and phenol-glycerine used in two cases of cancer with pain.
Despite recent methodological advancement of the practical pain medicine, many cases of the chronic anorectal pain have been intractable. A 54-year-old female patient who had a month history of a constant severe anorectal pain was referred to our clinic for further management. No organic or functional pathology was found. In spite of several modalities of management, such as medications and nerve blocks had been applied, the efficacy of such treatments was not long-lasting. Eventually, she underwent temporary then subsequent permanent sacral nerve stimulation. Her sequential numerical rating scale for pain and pain disability index were markedly improved. We report a successful management of the chronic intractable anorectal pain via permanent sacral nerve stimulation. But further controlled studies may be needed.
Central neuropathic pain may occur in 10~20% of the patients after spinal cord injury. The central pain syndrome include spontaneous continuing and intermittent pain as well as evoked pain. The pain is evoked by non-noxious stimulation of the region (allodynia) and repeated stimulation (wind-up phenomenon). Four patients were referred suffering from severe pain, allodynia and hyperaesthesia after spinal cord injury. They had received conventional treatment with non-steroidal anti-inflammatory drugs, steroid, anticonvulsant, antidepressant and rehabilitation which failed to provide pain relief. We administered combination of low doses of morphine and ketamine (10 mg) through the epidural catheter with other conventional therapy. Satisfactory pain relief was achieved in each patient. The reduction of pain was not associated with severe side effects. The most bothersome side effect of ketamine was dizziness in one patient, only caused by bolus injection (ketamine 10 mg with normal saline 10 ml). This suggests synergy from this combination that provides an alternative treatment for central pain.
A 25-year-old epileptic female patient scheduled for vaginal delivery, was referred to the pain clinic for the relief of labor pain. She had been taking anticonvulsant drugs, but suffered from seizure attacks three or four times a month. We had induced continuous lumbar epidural analgesia successfully and she subsequently gave birth to a healthy infant. So we report that continuous lumbar epidural analgesia should be considered as a safe method for reduction of labor pain and prevention of seizure attacks in epileptic pregnant women.
Splanchnic nerve block is effective in relief of upper abdominal cancer pain. But we often get confused at the unexpected complaints of pain accompanying a metastatic of referred origin in spite of the precise block under image intensifier. We could found aut a thermographic change from the diffuse hyperemission state of abdomen before the block to the localized state after the block. It may be helpful in differentiating objectively the accuracy of the block at the unexpected complaints of pain by using thermography before and after the block.
In order to review the studies related to the facial pain by acupuncture, we have referred to a PubMed site by using MeSH searching word of "facial pain and acupuncture". 12 journals with 20 papers were reported on the facial pain and acupuncture. Among them, 4 papers were appeared in Am J Chin Med, 3 in Acupunct Electrother Res, 2 in Br. Dent J, J Orofac Pain and Swed Dent J respectively, and 1 in 7 journals respectively. According to the classification of the patterns, 15 papers were done by clinical studies, 2 by review and meta-analysis respectively, and 1 by experimental study. TMD was the most disease that related to facial pain and CMD, neurogenic facial pain (trigeminal neuralgia, postherpetic pain), dental pain and MPS were also appeared. Most of the studies didn't mention the acupuncture points, L14 Habkok in 4 papers and ST36 Chogsamni in a paper were only mentioned. Various modalities were applied in the trials such as classical acupuncture, electroacupuncture, TENS, SSP and auricular acupuncture. By the above results, we concluded that various acupuncture techniques are used with effectiveness on the facial pain. It would be needed further research on the acupuncture point specific and therapeutic techniques as well.

Trigeminal neuralgia (TN) is characterized by recurrent paroxysms of unilateral facial pain that typically is severe, lancinating, and activated with cutaneous stimulation. There are two types of TN, classical TN and atypical TN. The pain nature of classical TN are the same as those described above, whereas atypical TN is characterized by constant, burning pain. We describe the case of a 49-year-old male presenting with right-sided facial pain. The patient was diagnosed with temporomandibular joint disorder at a dental clinic and was on medical treatment, but his symptoms worsened gradually. He was referred to our pain clinic for further evaluation. Radiologic evaluation, including MRI, showed a parapharyngeal tumor. For the relief of TN, a right mandibular nerve (V3) root block was performed at our pain clinic, and then he was scheduled for radiation and chemotherapy.
The pain is common among individuals with physical disabilities. It can interfere with therapy since patients with pain can become uncooperative and reluctant to move. This paper reviews the natural physiological mechanisms that can reduce pain perception, and considers physiological mechanisms which contribute to clinical pain by describing how the pain system changes its sensitivity depending upon the body's needs. The peripheral and central mechanisms contributing to sensitised nociceptive system are described with reference to the symptoms of clinical pain such as hyperalgesia, allodynia sopntaneous 'on-going'-projected and referred pain. It is suggested that in some chronic pain the nociceptive system maintains a state of sensitivity despite the absence of on-going tissue damage and under such circumstances the nociceptive system itself may have become dysfunctional. Such situations are often initiated by damage to nervous tissue which results in changes in the activity and organization of neuronal circuits within the central nervous system. The ability of the nociceptive system to operate in a suppressed state is also discussed with reference to pain modulation. The physical therapist can help facilitate the activation of these mechanisms through a combination of noninvasive modalities, functional activities, and the therapeutic use of self.
Until several years ago we didn't think seriously about cervical problems as a cause of headaches, but since the publication of articles by Bogduk et al they have gotten more attention. Cervical headaches are associated with movement abnormalities of the structures of the neck such as cervical nerve roots, discs, joints and soft tissues. Considering this, we thought that the atlanto-axial joint could be one of the causes of these headaches. Headaches originating from this joint can be recognized by the fact that the pain worsens with rotation of the head in the horizontal plane. Pain can also be referred to the frontal area or around the orbit. We did atlanto-axial joint blocks using a posterior approach on 10 patients who suffered from this type of headache. The results were promising with 9 out of 10 patients showing more than 50% improvement on the numeric rating scale. There were no serious complications observed. We concluded that the atlanto-axial joint block can be an effective procedure in treating this specific type of headache.
