Decreased absorption of midazolam in the stomach due to low pH induced by co-administration of Banha-sasim-tang

  • Objectives Banha-sasim-tang (BST), which consists of seven different herbs, is one of the most popular herbal formulae for treating gastrointestinal disorders in Eastern Asia. The commonly used herbal medicine is often co-administered with other therapeutic drugs, which raises the possibility of herb-drug interactions and may modify the clinical safety profile of therapeutic drugs. Methods We investigated the potential herb-drug interactions between BST extract and midazolam (MDZ) in mice. The area under the plasma concentration-time curve (AUC) of MDZ and 1'-hydroxymidazolam (1'-OH-MDZ) was evaluated for both oral and intraperitoneal administration of MDZ, following oral administration of BST (0.5 and 1 g/kg). Results It was found that the AUC of MDZ and 1'-OH-MDZ was lower in case of oral administration of MDZ. Administration of BST extract was not associated with hepatic cytochrome P450 activity. BST extract induced a strong reduction in pH and it has been reported that oral mucosal absorption of MDZ is lower at low pH. The decreased absorption rate of MDZ might be caused by the ingredients of BST and may not be related to other factors such as increased excretion of MDZ by P-glycoprotein. Conclusions The altered pharmacokinetics of midazolam caused by co-administration with BST in vivo could be attributed to a decrease in pH and subsequent reduction of MDZ absorption rate.

직장인 대상 저염화 교육 프로그램의 효과 평가 (Evaluation of the Effectiveness of a Salt Reduction Program for Employees)

  • 본 연구는 대구시 8개 병원과 사업체에 근무하는 직장인 251명을 대상으로 교육집단 166명, 비교육집단 85명으로 나누어 5주간 저염화 영양교육 프로그램을 운영하고 그 효과를 평가하였으며, 그 결과를 요약하면 다음과 같다. 1) 저염화 교육프로그램 실시 결과 교육전 두 집단간의 짠맛에 대한 미각판정치에 차이가 없었으나, 교육 후 교육집단의 경우 미각판정치가 유의하게 감소하였다 (p < 0.001). 또 한 교육 후 영양지식, 짜게 먹는 식태도 및 식행동이 비교육 집단에서는 유의한 차이가 없었으나 교육집단에서는 모두 유의하게 개선되었다 (p < 0.001). 2) 교육집단에서 24시간 소변 수집을 통한 나트륨 섭취량은 교육 전에 비해 교육 후 감소하였으나, 통계적으로 유의하지는 않았다. 3) 외식 빈도가 잦을수록, 외식비가 높을수록 짜게 먹는 경향이 있는 것으로 나타났으며, 미각판정치와 짜게 먹는 식행동 및 식태도간 유의한 상관관계를 나타내었다. 또한 짜게 먹는 식태도와 나트륨 섭취량간에 유의한 상관관계를 나타내어 식태도가 나쁠수록 나트륨 섭취량이 높은 것으로 나타났다.

2-2-Methylene bis (3, 4, 6-trichlorophenoxy acetic acid) (MTPA)의 체내흡수(體內吸收) 분포(分布) 및 배설(排泄)에 관(關)한 연구(硏究) (Studies on Absorption Distribution and Excretion of 2-2-Methylene bis (3, 4, 6-trichlorophenoxy acetic acid) (MTPA) in Rabbits)

  • It has been reported previously that 2,2-methylene bis(3,4,6-trichlorophenoxy acetic acid) (MTPA) is effective treating for clonorchiasis and less toxic to the hosts. In this studies the absorption, distribution and excretion of MTPA were observed. For this purpose $^{14}C-MTPA$ was synthesised from bis(2-hydroxy-3,5,6-trichlorophenoxyl) methan $^{14}C$ and administerd to the normal rabbit in a single dose of 10mg/kg IV or 20 mg/kg P.O. or to the Clonorchis infected rabbit in dose of 20 mg/kg/day for 6 days. Radioactivity in blood, tissue, bile, urine, feces and tissue of the fluke was measured after the drug was given. The concentration of MTPA in these samples were calcurated from the radioactivity. The result obtained as followes. 1. The increase in concentration of MTPA in blood and urine after oral administration of MTPA was so slow that the absorption of MTPA from the gastrointestinal tract appears very slow. 2. It is presumed that the excretion of MTPA also is slow because the reduction of MTPA concentration in blood after IV injection was very slow. 3. Large amount of MTPA was excreted from the bile. 4. During repeat dose of 20mg/kg/day for 6 days the concentration of MTPA in blood and tissue gradually increased. 5. The highest concentration of MTPA in the kidney and liver, heart, lung, spleen and muscle in decreasing order and the lowest concentration in the brain was noted. 6. During daily dose of 20 mg/kg of MTPA for 6 days of administration the concentration of MTPA gradually increased in urine and feces and the concentration of MTPA in feces was higher than of in urine. It appeares that MTPA take place enterophepatic circulation. 7. It is assumed that accumulation in large amount of MTPA in the liver and tissue of clonorchis, excretion of large amount from the bile is a favorable property of MTPA as a chemotherapeutic agent for clonorchiasis.

다시마 첨가 배추김치의 체중 감량 및 지질 저하 효과 (Weight Reduction and Lipid Lowering Effects of Sea Tangle Added Korean Cabbage Kimchi)

  • 체중감량 효과가 있는 기능성 배추김치를 개발하기 위하여 기호도가 우수하고 섬유소가 풍부하여 지질저하 기능성 물질을 함유한 다시마를 첨가한 해조첨가 배추맛김치(이하 해조김치) 레시피를 개발하고, 잘 숙성된 해조김치를 고지방식이를 섭취한 쥐에게 공급하여 지질저하 및 비만억제 효과를 살펴보았다. 배추김치에 다시마를 20% 정도 첨가하였을 때(해조김치) 종합적인 관능평가가 가장 우수하였다. 해조김치의 숙성 특성은 일반 김치와 같이 전형적인 발효양상을 보였다. 해조김치의 비만억제 효과를 살펴보기 위하여 20% 고지방식이에 동결 건조한 김치 10%를 첨가하여 쥐를 8주간 사육하였다. 실험군으로는 고지방식이군, 대조김치군, 해조김치군 그리고 시판용 공장김치를 첨가한 J-김치군으로 하였다. 식이 섭취량, 식이효율은 각 군간에 유의적인 차이가 없었다. 김치첨가에 의한 효과를 고지방식이군과 비교하여 보았을 때 상대적인 내장 지방량은 해조김치군(-17%), J-김치군(-11%) 그리고 대조김치군(-9%) 순으로 감소하였다(p<0.05). 혈청 leptin 농도는 해조김치군(-59%)에서 감소하였는데(p<0.05), 이는 해조김치군의 지방세포의 수가 대조군에 비해 감소함을 의미한다. 혈청 빌리루빈 농도는 해조김치군(-24%)에서 감소하였다(p<0.05). 혈청 중성지방과 콜레스테롤 농도는 고지방군에 비해 각각 해조김치군(-51%와 -21%), J-김치군(-40%, -23%) 그리고 대조김치군(-39%, -13%) 유의적으로 낮았다(p<0.05). 분변의 중성지방과 콜레스테롤 농도는 각각 해조김치군(49%와 71%), 대조김치군(36%, 61%), 그리고 J-김치군(32%, 65%)에서 증가하였다. 김치 종류에 따른 비만억제 효과를 살펴보았을 때 해조김치의 상대적인 내장 지방량은 다른 김치군에 비해 유의적으로 낮았고(p<0.05), 렙틴, 혈청지질, 분변지질 및 빌리루빈의 농도는 김치군간 유의적인 차이는 없었으나, 해조김치군의 비만억제 효과가 가장 높았다. 이러한 결과를 살펴보면, 일반 김치에 비해 해조김치를 섭취하면 체중 감량의 효과가 있을 것으로 생각된다. 이러한 해조김치의 비만 억제 기전은 다시마 첨가에 의해 식이섬유소의 함량이 7% 증가함으로써 소장에서 식이지방 흡수를 방해하여 혈청 중성지방의 농도를 낮추고 이로 인하여 지방세포로 중성지질의 이동이 낮아져 복부지방 축적이 억제되었기 때문으로 생각된다.

Effect of corn gluten and its hydrolysate consumptions on weight reduction in rats fed a high-fat diet

  • This study examined the effects of com gluten (CG) and its hydrolysate consumptions on weight reduction in rats fed a high-fat diet. Eight-month-old male Sprague-Dawley rats (n=40) were fed a high-fat diet (40% calorie as fat) for 4 weeks. They were then randomly divided into four groups and fed the isocaloric diets with different protein sources for 8 weeks. The protein sources were casein (control group), intact CG (CG group), CG hydrolysate A (CGHA group, 30% of protein as peptides and 70% as free amino acids) and CG hydrolysate P (CGHP group, 93% of protein as peptides and 7% as free amino acids). Body weight gain, adipose tissue weights, nitrogen balance, absorptions of energy, protein and fat, lipid profiles in plasma, liver and feces and hepatic activities of camitine palmitoyl transferase (CPT), fatty acid synthase (FAS), malic enzyme (ME) and glucose-6-phosphate dehydrogenase (G6PDH) were assessed. The CGHA diet had the highest amount of BCAAs, especially leucine, and most of them existed as free amino acid forms. The CGHA group showed significant weight reduction and negative nitrogen balance. Protein absorption and apparent protein digestibility in the CGHA group were significantly lower than those in other groups. Adipose tissue weights were the lowest in the CGHA group. Activity of CPT tended to be higher in the CGHA group than in other groups and those of FAS, ME and G6PDH were significantly lower in the CGHA group than in other groups. In conclusion, the CGHA diet which had relatively high amounts of free amino acids and BCAAs, especially leucine, had a weight reduction effect by lowering adipose tissue weight and the activities of FAS, ME and G6PDH in experimental animals, but it seemed to be a negative result induced by lowering protein absorption, increasing urinary nitrogen excretion and protein catabolism.

황금약침액(黃芩藥鍼液)이 Cisplatin에 의해 유발된 급성신불전(急性腎不全)에 미치는 영향 (Beneficial Effect of Scutellaria Balicalensis Georgi. Extract(SBGE) on Cisplatin-Induced Acute Renal Failure in Rabbits)

  • The present study was carried out to determine if Scutellaria balicalensis Georgi. extract (SBGE) exerts beneficial effect against the renal failure in rabbits. Rabbits were pretreated with SBGE orally for 7 days, followed by cisplatin injection (5 mg/kg, i.p.), cisplatin injection caused an increase in serum creatinine level, which was accompanied by a reduction in GFR. The fractional excretion of Na+, K+, glucose, and phosphate increased in cisplatin-treated animals, which was prevented by SBGE pretreatment. The uptake of p-aminohippurate, an organic anion, in renal cortical slices was inhibited by the administration of cisplatin, and the effect was prevented by SBGE. cisplatin injection increased lipid peroxidation, which was prevented by SBGE pretreatment. These results indicate that lipid peroxidation plays a critical role in cisplatin-induced acute renal failure. SBGE exerts the protective effect against acute renal failure induced by cisplatin, and its effect may be attributed to an antioxidant action.

고지방식이를 섭취한 흰쥐에서 김치유산균분말의 비만 억제 및 지질 저하 효과 (Weight Reduction and Lipid Lowering Effects of Kimchi Lactic Acid Powder in Rats Fed High Fat Diets)

  • 김치유산균추출물의 비만억제 및 지질 저하 효과를 20%(w/w) 고지방식이를 섭취시킨 횐 쥐에서 살펴보았다. 제한식이를 하였음에도 불구하고 HFD의 체중은 ND에 비해 약 50%증가 하였고, HFK10 및 HFK20에서는 HFD에 비해 각각 13%그리고 15%(p<0.01) 감소하였다. HFD의 복부지방의 함량은 ND에비해 98% 증가하였고(p<0.01), HFK10 및 HFK20에 서 각각 42%, 48% 유의적으로 감소하여(p<0.01) 체중 저하 효과 보다 복부지방 축적을 억제하는 효과가 더 높은 것으로 나타났다. 혈장 중성지방의 농도는 HFD가 ND에 비해 약 55% 증가하였고(p<0.01), HFK10 및 HFK20은 HFD에 비해 26%, 및 35% 농도 의존적으로 감소하였다(p<0.01). 혈장 총 콜레스테롤 농도 역시 HFK10 및 HFK20에서 농도의존적으로 유의적인 감소를 보였고(p<0.01) LDL 농도는 HFD가 ND보다 61% 정도 증가하였고(p<0.01), HFK10 및 HFK20에서 약 8%, 33%감소하여(p<0.01)총 콜레스테롤 보다 LDL의 감소가 더 현저하였다. HDL은 증가하여 김치유산균추출물의 첨가에 의한 지질 개선 효과가 관찰되었다. 김치 유산균 추출물의 간 기능 개선 효과는 고지방 식이에 의해 유발된 지방간 현상으로 정상대조군에 비해 상승되었던 혈장 AST 및 ALT 활성이 감소됨으로서 관찰되었다. 분변의 지방농도를 HFD와 HFK10 및 HFK20간에 비교하였을 때 중성지방 농도는 각 62%와 111% 정도 현저하게 증가하였고, 콜레스테롤 농도는 HFK20에서 약 31% 정도 증가하였다(p<0.05). 이상의 결과에서 사균의 형태인 김치유산균 추출물은 비만을 억제하고 혈장 지질을 저하시키는 효과가 있음이 확인되었으며, 간 기능 보호효과도 확인되었다. 이러한 김치유산균 추출물의 효과는 장내에서 지방의 흡수를 억제하기 때문으로 생각된다.

Nutrient Digestibility and Metabolizable Energy Content of Mucuna pruriens Whole Pods Fed to Growing Pelibuey Lambs

  • The nutrient digestibility, nitrogen balance and in vivo metabolizable energy supply of Mucuna pruriens whole pods fed to growing Pelibuey lambs was investigated. Eight Pelibuey sheep housed in metabolic crates were fed increasing levels of Mucuna pruriens pods: 0 (control), 100 (Mucuna100), 200 (Mucuna200) and 300 (Mucuna300) g/kg dry matter. A quadratic (p<0.002) effect was observed for dry matter (DM), neutral detergent fibre (aNDF), nitrogen (N) and gross energy (GE) intakes with higher intakes in the Mucuna100 and Mucuna200 treatments. Increasing M. pruriens in the diets had no effect (p>0.05) on DM and GE apparent digestibility (p<0.05). A linear reduction in N digestibility and N retention was observed with increasing mucuna pod level. This effect was accompanied by a quadratic effect (p<0.05) on fecal-N and N-balance which were higher in the Mucuna100 and Mucuna200 treatments. Urine-N excretion, GE retention and dietary estimated nutrient supply (metabolizable protein and metabolizable energy) were not affected (p>0.05). DM, N and GE apparent digestibility coefficient of M. pruriens whole pods obtained through multiple regression equations were 0.692, 0.457, 0.654 respectively. In vivo DE and ME content of mucuna whole pod were estimated in 11.0 and 9.7 MJ/kg DM. It was concluded that whole pods from M. pruriens did not affect nutrient utilization when included in an mixed diet up to 200 g/kg DM. This is the first in vivo estimation of mucuna whole pod ME value for ruminants.

  • A series of organosulfur compounds were synthesized with the aim of developing chemopreventive compounds active against hepatotoxicity and chemical carcinogesis. 2-(Allylthio) prazine (2-AP) was effective in inhibiting cytochrome P450 2E1-mediated catalytic activities and protein expression, and in inducing microsomal epoxide hydrolase and major glutathione S-transferases. 2-AP reduced the hepatotoxicity caused by toxicant sand elevated cellular GSH content. Development of skin tumors, pulmonary adenoma and aberrant crypt foci in colon by various chemical carcinogens was inhibited by 2-AP pretreatment. Anticarcinogenic effects of 2-AP at the stage of initiation of tumors were also observed in the aflatoxin B1 ($AFB_1$)-induced three-step medium-term hepatocarcinogenesis model. Reduction of $AFB_1$-DNA adduct by 2-AP appeared to result from the decreased formation of $AFB_1$-8,9-epoxide via suppression of cytochrome P450, while induction of GST 2-AP increases the excretion of glutathione-conjugated $AFB_1$ . 2-AP was a radioprotective agent effective against the lethal dose of total body irradiation and reduced radiation-induced injury in association with the elevation of detoxifying gene expression. 2-AP produces reactive oxygen species in vivo, which is not mediated with the thiol-dependent production of oxidants and that NF-KB activation is not involved in the induction of the detoxifying enzymes. the mechanism of chemoprotection by 2-AP may involve inhibition of the P450-mediated metabolic activation of chemical carcinogens and enhancement of electrophilic detoxification through induction of phase II detoxification enzymes which would facilitate the clearance of activated metabolites through conjugation reaction.

$^{99m}Tc-DISIDA$ 스캔에 의한 간기능의 정량적 평가 (Quantitative Evaluation of Liver Function Using $^{99m}Tc-DISIDA$ Cholescintigraphy)

  • Since hepatocyte clearance, leading edge parencymal transit time and biliary excretion can be evaluated separately with hepatobiliary scan using $^{99m}Tc-DISIDA$, hepatobiliary scan may be useful in differentiating intrahepatic cholestasis from extrahepatic cholestasis. Excretory liver function was analysed in 13 healthy subjects and 11 patients with clinically suspected hepatocellular disease and 9 patients with extrahepatic biliary obstruction confirmed by surgery, radiological and clinical evidence. Indices of total liver activity (% TLA), liver parechymal uptake (% LPU), heart pool clearance (% HPC) and liver-heart rate (% LHR) were calculated from time activity curve over heart and liver. Compared with healthy subjects, significant reduction (p<0.05) in total liver activity (% TLA) and liver-heart rate (% LHR) was observed in all patients group. But no useful indices was demonstrated in differentiating hepatocellular disease from extrahepatic biliary obstruction.
