Background: Cigarette smokers have their own motivation and justification to smoke. For example, smoking reduces their stress or enhances their pleasure. This study aimed to identify the (a) prevalence of cigarette smokers among undergraduates in Malaysia, (b) gender differences in nicotine dependence among current smokers, (c) differences in psychological problems (depression, anxiety and stress) based on the status of smoking cigarettes (current, former and non-smokers) and (d) extent to which precipitating factors (tension reduction, addiction, automatism, handling, social interaction, pleasure, and stimulation) predict the smoking behavior among current smokers. Materials and Methods: In this study 780 undergraduate students participated from a private university in Kuala Lumpur and Selangor state in Malaysia. The Depression, Stress and Anxiety Scale, Modified Reason for Smoking Scale and Fagerstrom Nicotine Dependence Test were used to measure psychological problems, predictors of smoking behavior and nicotine dependency among current smokers. Results: The results showed that 14.7%(n=106) of the students were smokers. Current smokers exhibited more psychological problems (depression, anxiety and stress) compared to former and non-smokers. Addiction, tension reduction, pleasure and automatism were predictors of smoking behavior among the current smoking students. Step wise regression analysis showed that smoking behavior was highly predicted by nicotine dependency or addiction. Smoking students were motivated to smoke cigarettes as they believed that it reduced their tension and enhance pleasure. Conclusions: Hence, there is a need for health promotion and anti-tobacco prevention as cigarette smokers experience more psychological problems. Nicotine dependency or addition was one of the major causes for smoking behavior among the student population in Malaysia.
Background: Lack of proper control of acute postoperative pain often leads to lingering or chronic pain. Several studies have emphasized the role of beta-blockers in reducing postoperative pain. Esmolol is a selective short-acting beta-blocker that produces few side effects. The purpose of this study was to examine the effect of intravenous intraoperative esmolol on postoperative pain reduction following orthopedic leg fracture surgery. Methods: In a clinical trial, 82 patients between 20-65 years of age with tibia fractures and American Society of Anesthesiologists (ASA) physical status I & II who underwent surgery were divided into two groups. Group A received esmolol and group B received normal saline. Postoperative pain was measured at three time points: entering the recovery unit, and at 3 h and 6 h following surgery, using the Visual Analogue Scale (VAS). A P value of < 0.05 was considered significant. Results: Mean VAS scores at all three time points were significantly different between the two test groups (P = 0.02, P = 0.0001, and P = 0.0001, respectively). The consumption of pethidine was lower in group A than in group B (P = 0.004) and the duration of its effect was significantly longer in time (P = 0.026). Conclusions: Intravenous intraoperative esmolol is effective in the reduction of postoperative pain following leg fracture surgery. It reduced opioid consumption following surgery and delayed patient requests for analgesics.
Purpose: This study compared the different sling and resistance exercises on pelvic rotation during active straight leg raises (ASLR) and on pain in patients with chronic low back pain (CLBP). Methods: Twenty subjects were divided randomly into a sling group (SG) and a resistance exercise group (REG). Internal oblique (IO), external oblique (EO), rectus abdominis (RA), and rectus femoris (RF) muscle activity; pelvic rotation angle during ASLR; and visual analogue scale, pressure pain threshold were measured. Sling and resistance exercises were then performed for 30 minutes and the measurements taken again. Results: Both groups showed significantly lower RF muscle activity and significantly higher EO and IO muscle activity (p<0.05). The RA muscle activity decreased significantly in the SG, but increased significantly in the REG (p<0.05). The pelvic rotation angle was significantly lower in the SG (p<0.05). The pain press threshold increased significantly in both groups (p<0.05). The visual analogue scale decreased significantly in the SG (p<0.05). Conclusion: Both exercises appear to be beneficial for modifying the muscle activity and pain control in the intervention of CLBP. On the other hand, the sling was more effective in increasing the pressure threshold than resistance exercise, and the pelvic rotation angle was reduced. Therefore, both exercises can help patients with CLBP change their muscle activity and control pain. CLBP patients should use a sling for short periods of time to learn to reduce the pain and control pelvic rotation.
Purpose: We sought to examine whether using the rhythmic stabilization (RS) technique before proprioceptive neuromuscular facilitation (PNF) wrist taping affected pain and grip strength in patients with wrist pain to provide a basis for the application of PNF taping. Methods: The study consisted of 41 badminton enthusiasts (aged 20-40 years) who reported discomfort and pain due to overuse of their wrists. The subjects were randomly assigned to an experimental group (n=20) or a control group (n=21). In the experimental group, PNF wrist taping was applied after application of the PNF RS technique, and PNF wrist taping was applied after stretching in the control group. The tape was applied five times a week for 3 weeks. Pain was measured using the visual analog scale (VAS). Grip strength was measured using a dynamometer. The paired t-test was performed to compare grip strength and pain within the groups before and after the intervention. Covariance analysis was conducted to compare differences between the experimental group and control group. The level of significance was set as ${\alpha}=0.05$. Results: Within-group changes in grip strength and VAS were significantly different in the control group and experimental group (p<0.01). Grip strength and VAS showed more improvement in the experimental group than in the control group (p<0.01). Conclusion: PNF wrist taping after stretching and the PNF RS technique both significantly reduced pain and improved grip strength in club badminton players with wrist pain. These improvements were significantly greater in the experimental group in which the PNF RS technique was applied. The results suggest that PNF may be considered useful to improve grip strength and reduce wrist pain.

Objectives: This study is to report the clinical effect of oriental medicine on a patient suffering from postabortal syndrome with somatic pain disorder, Qi deficiency and depression. Methods: The patient had a miscarriage by cervical incompetence in 24 weeks' gestation even though she had operated cervical cerclage in 20 weeks' gestation. The patient received herbal medication, acupuncture, moxibustion during 20 days of outpatient treatment. The clinical effects were evaluated through VAS (Visual Analogue Scale) and EPDS (Edinburgh Postnatal Depression Scale Test). Results: The clinical symptoms of somatic pain and Qi deficiency were reduced during the treatment. The EPDS score also decreased from 19 points (high risk level) to 8 points (low risk level). Conclusions: This case study showed that herbal medicine, acupuncture and moxibustion treatment appeared to effectively reduce postabortal syndrome. Since patients previously experienced cervical incompetence tend to have habitual abortion for next pregnancy, further long term observation and preventive treatment are needed in this case for next safe pregnancy and childbirth.
An MP3 encoder chip has been designed and fabricated with the hardware and software co-design concepts. In the aspect of the software. the calculation cycles of the distortion control loop. which requires most of the calculation cycles in MP3 encoding procedure. have been reduced to $67\%$ of the original algorithm through the 'scale factor Pre-calculation'. By using a floating Point 32 bit DSP core and designing the FFT block with the hardware. we can get the additional reduction of the calculation cycles in addition to the software optimization. The designed chip has been verified using HW emulation and fabricated via 0.25um CMOS technology The fabricated chip has the size of $6.2{\time}6.2mm^2$ and operates normally on the test board in the qualitative and quantitative aspect.

Background: Although many studies have been conducted on worker fatigue and sickness absence, the association between fatigue and sickness absence is unclear in Korean workers. This study was conducted to investigate the effect of worker fatigue on future sickness absence. Methods: The study was conducted on workers who received medical check-ups at a university hospital for two consecutive years (2014-2015). During check-ups in the first year, the Fatigue Severity Scale (FSS) was used to assess fatigue levels, and during check-ups in the second year, sickness absence was surveyed to determine whether they had been absent from work due to physical or mental illness during previous 12 months. The χ2 test was used to analyze relationships between sociodemographic and occupational characteristics, fatigue levels, and sickness absence. Odds ratios (ORs) were calculated by logistic regression analysis controlled for confounding factors. Results: A total of 12,250 workers were included in the study, and 396 (3.2%) workers experienced more than one day of sickness absence during the study period. Adjusted ORs for sickness absence were 3.35 (95% confidence interval [CI]: 2.64-4.28) in the moderate-fatigue group and 6.87 (95% CI: 4.93-9.57) in the high-fatigue group versus the low-fatigue group. For men in the moderate- and high-fatigue groups, adjusted ORs for sickness absence were 3.40 (95% CI: 2.58-4.48) and 8.94 (95% CI: 6.12-13.07), and for women in the moderate- and high-fatigue groups, adjusted ORs for sickness absence were 2.93 (95% CI: 1.68-5.10) and 3.71 (95% CI: 1.84-7.49), respectively. Conclusions: Worker fatigue is associated with sickness absence during the following 12 months, and this association appears to be stronger for men than women. These results support the notion that sickness absence can be reduced by evaluating and managing work-related fatigue.

Objective: The purpose of this study is to evaluate the effectiveness of unilateral decompression and pedicle screw fixation for the unilateral symptomatic foraminal stenosis. Methods: The study group comprises consecutive 16 patients who underwent unilateral decompression and bone cement augmented pedicle screw fixation from May 2003 to January 2006. The patients were evaluated by visual analog scale (VAS) for pain and the scoring system of the Japanese Orthopedic Association (JOA) for low back pain. The result of surgery was also evaluated with McNab's classification. Excellent or good outcome was considered as successful. The patients were followed at postoperative 1 month, 3 month, 6 month, and 1 year with standing AP and lateral films. Results: The average VAS and JOA score of the 16 patients were 7.8(range, 6-9) and 5.8(range, 3 - 10) before surgery and 2.2(range, 0 - 5)and 12.3(range, 9 - 15) at the time of last follow up. Both VAS and JOA score improved significantly after the surgery (p<0.05, t-test). All patients improved after the operation and no revision surgery was required. No metal failure or pseudoarthrosis was observed during the follow-up. The success rate was 87.5%. Conclusion: Our data suggest that unilateral decompression and pedicle screw fixation for the unilateral symptomatic foraminal stenosis is an effect method for obtaining satisfactory clinical outcome. Its possible advantage is shorter operation time and reduced surgical extent. We believe that the reduced stiffness of unilateral fixation was compensated by pedicle screw augmentation and interbody fusion.
Recently, the remodeling projects of old low-rise buildings were launched in Korea. However, most of them were not satisfied with the value set forth by the KBC2005. Even though there are some research studies on how to improve the seismic performance of such buildings as newly constructed buildings, there is little research in measuring the actual vibrations on low old buildings to prove the effect of retrofit. There also has not been any in-depth research on the dynamic characteristics of full-scale structures using vibration measurements of the building that was damaged to failure. Using an actuator, the dynamic characteristics of reinforced three-storey concrete buildings were evaluated before and after they were damaged. After an 80-mm horizontal displacement by the actuator, frequency in the long and short directions were reduced to 20.85% and 5.77% respectively ; damping ratio was also reduced to 53.9% and 23.15% respectively.

Infiltration and the rising level of groundwater caused by rainfall are a major cause of the landslide and sliding. In order to secure the safety factor of slope, the slope stabilization and reinforcement works are used to enhance consistency. Nailing, Slope Drainage method and the surface vegetation measures can be simultaneously applied in the Mass Nail method, which is also environmental friendly reinforcement method. To demonstrate the reinforcement effect of a strengthened slope by Mass Nail, the changes in water contents by rainfall were considered while performing Scale Model Test. As a result, safety factor of reinforced slope was about 1.4~2.3 times increased on the unstrength slope. In the case of increasing water content 10% to 22%, The maximum stress was reduced to 12%~24% at the average rate of 18% on the unstrength slope and the reinforced slope by the Mass Nail Method was reduced to 4%~23% at the average rate of 14%.
