Acute Effects of 2-Bromopropane and 1,2-Dibromopropane on Hepatotoxic and Immunotoxic Parameters in Female BALB/c Mice

  • In the present studies, the acute toxic effects of 2-bromopropane (2-BP) and its analog, 1,2-dibromopropane (1,2-DBP), were investigated in female BALB/c mice. The mice were treated orally with either 2-BP at 2000 and 4000 mg/kg or 1,2-DBP at 300 and 600 mg/kg. Four days before necropsy, the mice were immunized intraperitoneally with sheep red blood cells (SRBCs). 1,2-DBP reduced the weights of the spleen and thymus weights and decreased the number of splenic cells. In addition, treatment with 1,2-DBP suppressed the antibody response to SRBCs. Meanwhile, only the antibody response was significantly suppressed by treatment with 2-BP. In the subsequent studies, the time course effects of 2-BP and 1 ,2-DBP on the hepatotoxic parameters were compared in female BALB/c mice. When mice were treated orally with either one of these chemicals for 6, 12, 24 and 48 h, the activities of serum alanine aminotransferase and aspartate aminotransferase elevated significantly only with 1,2-DBP 24 h after the treatment. The hepatic content of glutathione was reduced by 1,2-DBP. Meanwhile, these parameters were increased by 2-BP. The present results suggest that 1,2-DBP in the Solvent 5200 also contributes to the immnunotoxicity, although 2-BP is a major component.

Oryung-san Ameliorates Pioglitazone Side Effects (오령산의 피오글리타존 부작용 경감 효과)

  • Thiazolidinediones (TZDs) induce insulin sensitization through the activation of PPAR${\gamma}$. However, the undesirable effect such as weight gain was observed. The purpose of this study was to find out an herbal drug that could reduce the side effects of pioglitazone. Among herbal formula that we have searched, oryung-san (OR) inhibited the differentiation of preadipocytes and did not affect on glucose uptake in 3T3-L1 adipocytes. In vitro, glucose uptake assay and Oil Red-O staining in 3T3-L1 adipocytes were conducted. In vivo, pioglitazone (PIO, 30 mg/kg), oryung-san (OR, 300 mg/kg), or pioglitazone co-administered with oryung-san (PIO+OR) were administered orally for 7 weeks in high fat diet (HFD) fed ICR mice and measured the body weight and blood glucose level every week. PIO+OR group significantly reduced body weight gain, triglyceride, and total cholesterol compared to PIO group. In addition, PIO+OR group showed a significant reduction of plasma glucose level (72%) compared to HFD control group. Insulin levels in PIO+OR group was also markedly decreased by 85% and 41% compared to HFD control and PIO group, respectively. Diameter of white adipocytes was decreased in the PIO+OR group compared to that in PIO group. Moreover, PIO+OR group reduced expression of PPAR${\gamma}$ and SREBP1a compared to PIO group. Taken together, oryung-san can improve side effects of pioglitazone, such as weight gain and edema, and shows a synergistic effect in plasma insulin levels.

The Effects of Sopungsan and Gamisopungsan on Immune Response and the Anti-Allergic Reaction to Rats and Mice (消風散과 加味消風散이 免疫反應 및 抗 알레르기에 미치는 影響)

  • The object of this research is to find out the clinical effects of Sopungsan and Gamisopungsan on Immune response and the An1i-allergic reaction to rats and mice. The results obtained are as follows: 1. Both sopungsan and Gamisopungsan have a tendency to decrease on the delayed type hypersensitivity response in methotrexate treated mice, but are not recognized as having significance. 2. Both Sopungsan and Gamisopungsan reveal the increasing effects with significance on the hemagglution titer in mice. 3. Gamisopungsan reveals the increasing effect with significance on the hemolysin titer in mice. 4. Both Sopungsan and Gamisopungsan have a tendency to increase on the appearance of Rosette forming cells in mice, but are not recognized as having significance. 5. Both Sopungsan and Gamisopungsan reveal the increasing effects with significance on phagocytic index K and a in mice 6. Sopungsan reveals the decreasing effect, on the homologous passive cutaneous anaphylaxis in rats provoked by the IgE-like antibody aganist egg white albumin. 7. Gamisopungsan reveals the decreasing effect with significance on vascular permeabi1ity response to intradermal histamin in rats. 8. Sopungsan reveals the decreasing effect with significance, on vascular permeability response to intradermal serotonin in rats. 9. Both Sopungsan and Gamisopungsan reveal the decreasing effects with significance on the delayed type hypersensitivity response to picryl chloride in mice. 10. Both Sopungsan and Gamisopungsan reveal the decreasing effects with significance on the delayed type hypersensitivity response to sheep red blood cell in mice. According to the above results, Sopungsan and Gamisopungsan are concluded to have the increasing effect of immunity and anti-allergic reaction.

Toxicity of Methylcyclohexane and Its Effect on the Reproductive System in SD Rats

  • Objectives: There is limited data regarding the toxicity of methylcyclohexane, despite its wide use in rubber adhesives, paint diluents, and cleansing agents. This study aimed to verify the toxicity and influence on the reproductive system of methylcyclohexane after its repeated injection in Sprague Dawley (SD) rats. Methods: Methylcyclohexane was injected subcutaneously into male and female SD rats once a day, five times a week, for 13 weeks at different doses (0, 10, 100, and 1,000 mg/kg/day) for each group. The toxicity of testing material was verified by observing the change in body and organ weight, hematological change, pathological findings, and effect on the reproductive system at each different concentration. Results: In the 1,000 mg/kg/day group, there were cases of animal deaths. In animals that survived, hematological changes, including a decrease in the red blood cell count, were observed. A considerable weight gain or loss and pathological abnormalities in the liver, kidney, and other organs were found. However, the 10 and 100 mg/kg/day groups did not cause deaths or other specific abnormalities. In terms of reproductive toxicity, there were changes in hormone levels, including a significant decrease in hormones such as estradiol and progesterone (p < 0.001) in male animals. Menstrual cycle change for female animals did not show concentration dependency. Conclusion: When injected repeatedly for 13 weeks, methylcyclohexane proved to be toxic for the liver, heart, and kidney at a high dose. The absolute toxic dose was 1,000 mg/kg/day, while the no observed adverse effect level was less than 100 mg/kg/day. The substance exerted little influence on the reproductive system.

Role of Arbitrary Intensity Profile Laser Beam in Trapping of RBC for Phase-imaging

  • Red blood cells (RBCs) are customarily adhered to a bio-functionalised substrate to make them stationary in interferometric phase-imaging modalities. This can make them susceptible to receive alterations in innate morphology due to their own weight. Optical tweezers (OTs) often driven by Gaussian profile of a laser beam is an alternative modality to overcome contact-induced perturbation but at the same time a steeply focused laser beam might cause photo-damage. In order to address both the photo-damage and substrate adherence induced perturbations, we were motivated to stabilize the RBC in OTs by utilizing a laser beam of ‘arbitrary intensity profile’ generated by a source having cavity imperfections per se. Thus the immobilized RBC was investigated for phase-imaging with sinusoidal interferograms generated by a compact and robust Michelson interferometer which was designed from a cubic beam splitter having one surface coated with reflective material and another adjacent coplanar surface aligned against a mirror. Reflected interferograms from bilayers membrane of a trapped RBC were recorded and analyzed. Our phase-imaging set-up is limited to work in reflection configuration only because of the availability of an upright microscope. Due to RBC’s membrane being poorly reflective for visible wavelengths, quantitative information in the signal is weak and therefore, the quality of experimental results is limited in comparison to results obtained in transmission mode by various holographic techniques reported elsewhere.

A Case of Hemolytic Disease of a Newborn by an Anti-$Di^a$ Antibody Treated with Intravenous Immunoglobulin (정맥용 면역글로불린 투여로 호전된 항-$Di^a$ 항체에 의한 신생아 용혈성 질환 1예)

  • Hemolytic disease in a newborn that causes early jaundice is common. It is often due to the Rh (D) and ABO incompatibility, but rarely due to unexpected antibodies. Among these unexpected antibodies, the anti-$Di^a$Dia antibody rarely occurs. The anti-$Di^a$ antibody was observed in the serum and red-cell eluate of an infant, and in the serum of his mother. The frequency of the appearance of the $Di^a$ antigen in the Korean population is estimated to be 6.4-14.5%. This paper reports a case of hemolytic disease in a newborn associated with the anti-$Di^a$ antibody. A full-term male infant was transferred to the authors' hospital due to hyperbilirubinemia the day after his birth. The laboratory data indicated a hemoglobin value of 11.6 g/dL, a reticulocyte count of 10.6%, a total bilirubin count of 14.4 mg/dL, a direct bilirubin count of 0.6 mg/dL, and a positive result in the direct Coombs' test. Due to the identification of an irregular antibody from the maternal serum, an anti-$Di^a$ antibody was detected, which was also found in the eluate made from the infant's blood. The infant had been treated with phototherapy and intravenous immunoglobulin since the second day after his birth and was discharged due to an improved condition without exchange transfusion. Therefore, in cases of iso-immune hemolytic disease in a newborn within 24 hours from birth who had a negative result in an antibody screening test, the conduct of an anti-$Di^a$ antibody identification test is recommended due to the suspicion of an anti-$Di^a$ antigen, followed by early administration of intravenous immunoglobulin.

Two Cases of Severe Pancytopenia Associated with Low-Dose Methotrexate Therapy in Patients with Chronic Kidney Disease and Rheumatoid Arthritis (류마티스 관절염을 가진 만성신질환 환자에서 저용량 methotrexate 투여 후 발생한 중증 범혈구 감소증 2예)

  • Due to its efficacy and tolerability, low dose oral methotrexate (MTX) therapy has been widely used for treatment of rheumatoid arthritis (RA). However, it can rarely cause serious, life-threatening hematologic toxicities, such as pancytopenia. We report here on two patients with chronic kidney disease (CKD), who developed severe pancytopenia after 5 years (cumulative dose 1,240 mg) and 4 years (cumulative dose 1,320 mg) of low dose MTX therapy for treatment of RA, respectively. Both patients presented with renal insufficiency, hypoalbuminemia, concurrent use of nonsteroidal anti-inflammatory drugs, and elevated mean corpuscular volume of red blood cells (RECs), all of which are known as risk factors of MTX-induced pancytopenia. Despite receiving treatment, which included REC and platelet transfusions, antibiotic therapy, granulocyte colony stimulating factor, and leucovorin rescue, one patient died of sepsis. Based on our case study, prompt investigation of risk factors associated with MTX toxicity is required for all patients receiving MTX therapy. MTX treatment, even at a low dose, should be discontinued in patients with advanced CKD.

Hematuria Screening Test for Urinary Bladder Mucosal Infiltration in Cervical Cancer

  • Objective: To determine the diagnostic performance of hematuria as a screening test for urinary bladder infiltration in cervical cancer patients with a prospective study design. Materials and Methods: Newly diagnosed cervical cancer patients at Srinagarind hospital from 14 June 2011 to 30 April 2012 were enrolled in this study. We collected midstream urine samples for urinalysis from every patient before routine cystoscopic exam for clinical staging. The presence of 3 or more red blood cells (RBCs) per high power field was defined as positive for hematuria. A two-by-two table was used to determine the diagnostic performance of hematuria to detect urinary bladder mucosal infiltration using cystoscopy and biopsy as the gold standard. Result: A total of 130 were patients included, 54 of which (41.5%) had hematuria. Of these, four patients (3.08%) had pathological report from cystoscopic biopsy confirmed metastatic squamous cell carcinoma. The sensitivity, specificity, PPV, NPV, and accuracy of hematuria as a screening test to detect urinary bladder mucosal infiltration of cervical cancer were 100%, 60.3%, 7.4%, 100%, and 61.5%, respectively. There was no single case of urinary bladder mucosal infiltration in patients initially staged less than stage III. Conclusions: Hematuria can be used as a screening test to detect urinary bladder mucosal infiltration of cervical cancer. This can reduce the number of cervical cancer patients who really need to undergo cystoscopy as a staging procedure to less than half and to less than 20% if stage III or more were included without missing a single case of urinary bladder mucosal infiltration.

Effects of Dietary Supplementation with Immunogen® on Growth, Hematology and Gut Microbiota of Fingerling Common Carp Cyprinus carpio

  • We studied the effects of the proprietary prebiotic Immunogen$^{(R)}$ on the growth, hematology and gut microbiota of common carp fingerlings. A basal diet was formulated using common feed ingredients and supplemented with Immunogen$^{(R)}$ at concentrations of 0, 5, 10, 20 and $40g\;kg^{-1}$, each of which was tested experimentally on replicated groups of fish. The trials ran for 8 weeks. Common carp fingerlings with an initial weight of $4.82{\pm}0.05g$ were randomly distributed among the experimental tanks at a stocking density of 25 fish per tank. The experimental diets were provided thrice per day; on each occasion the fingerlings were given a weight of feed that amounted to 4% of fish biomass. At the end of the experimental period, we determined the growth performance, feed conversion ratio, hematological parameters, body composition and gut micro-flora parameters of the test fish. Inclusion of $5g\;kg^{-1}$Immunogen$^{(R)}$ in the diet significantly improved growth performance and feed utilization in comparison with controls. However, the whole-body composition of the fish was not significantly influenced by prebiotic inclusion. Inclusion of $5g\;kg^{-1}$ Immunogen$^{(R)}$ significantly increased the total bacterial and Lactobacillus counts in fish intestines, but these bacterial parameters were significantly negatively impacted by higher concentrations of the prebiotic. Red blood cells counts were increased by prebiotic dietary supplementation at concentrations of 5 and $10g\;kg^{-1}$ prebiotic. Glucose and cholesterol levels were elevated by administration of Immunogen$^{(R)}$. Thus, dietary supplementation with $5g\;kg^{-1}$ Immunogen$^{(R)}$ improved fingerling common carp growth performance and feed utilization, and beneficially influenced the gut microflora

Massive pulmonary hemorrhage in enterovirus 71-infected hand, foot, and mouth disease

  • Hand, foot, and mouth disease (HFMD) is an acute, mostly self-limiting infection. Patients usually recover without any sequelae. However, a few cases are life threatening, especially those caused by enterovirus 71 (EV71). A 12-month-old boy was admitted to a primary hospital with high fever and vesicular lesions of the mouth, hands, and feet. After 3 days, he experienced 3 seizure episodes and was referred to our hospital. On admission, he was conscious and his chest radiograph was normal. However, 6 hours later, he suddenly lost consciousness and had developed a massive pulmonary hemorrhage that continued until his death. He experienced several more intermittent seizures, and diffuse infiltration of both lung fields was observed on chest radiography. Intravenous immunoglobulin, dexamethasone, cefotaxime, leukocyte-depleted red blood cells, fresh frozen plasma, inotropics, vitamin K, and endotracheal epinephrine were administered. The patient died 9 hours after intubation, within 3 days from fever onset. EV71 subgenotype C4a was isolated retrospectively from serum and nasopharyngeal swab by real-time reverse transcription-polymerase chain reaction. Here, we report a fatal case of EV71-associated HFMD with sudden-onset massive pulmonary hemorrhage and suspected encephalitis.
