Endoscopic Findings and Treatment of Gastric Neoplasms in Familial Adenomatous Polyposis

  • Purpose: Gastric neoplasia is a common manifestation of familial adenomatous polyposis (FAP). This study aimed to elucidate the clinical characteristics, endoscopic features including fundic gland polyposis (FGPsis), and treatment outcomes of gastric neoplasms (GNs) in patients with FAP. Materials and Methods: A total of 35 patients diagnosed with FAP, including nine patients from four pedigrees who underwent esophagogastroduodenoscopy (EGD), were investigated regarding patient characteristics, GN morphology, and treatment outcomes. Results: Twenty-one patients (60.0%) had 38 GNs; 33 (86.8%) and 5 (13.2%) were histologically diagnosed with adenocarcinoma and adenoma, respectively. There were no specific patient characteristics related to GNs.Nodule-type GNs were more prevalent in patients with FGP than without (52.2% vs. 0.0%, P=0.002) in the upper body of the stomach. Conversely, depressed-type GNs were fewer in patients with FGPsis than in those without (13.0% vs. 73.3%, P<0.001). Slightly elevated-type GNs were observed in both groups (34.8% vs. 20.0%, P=0.538). Even within pedigrees, the background gastric mucosa and types of GNs varied. In total, 24 GNs were treated with endoscopic submucosal dissection (ESD) and eight with endoscopic mucosal resection (EMR). EMR was selected for GNs with FGPsis because of the technical difficulty of ESD, resulting in a lower en bloc resection rate (62.5% vs. 100%, P=0.014). Conclusions: Our study indicates the necessity of routine EGD surveillance in patients diagnosed with FAP. Notably, the morphology and location of GNs differed between patients with and without FGPsis. Endoscopic treatment and outcomes require more attention in cases of FGPsis.

Structural Equation Modeling for Public Hospital Quality of Care, Image, Role Performance, Satisfaction, Intent to (Re)visit, and Intent to Recommend Hospital as Perceived by Community Residents (지역주민이 인지하는 공공병원 의료의 질, 이미지, 역할수행, 만족도, (재)이용 의향, 타인추천 의향 구조모형)

  • Purpose: The study purposes were to construct and test structural equation modeling on the causal relationship of community residents' perceived quality of care, image, and role performance with satisfaction, intention to (re)visit and intention to recommend hospital. Methods: A cross-sectional survey was conducted with 3,900 community residents from 39 district public hospitals. The questionnaire was designed to collected information on personal characteristics and community awareness of public hospitals. Community awareness consisted of 6 factors and 18 items. The data were collected utilizing call-interview by a survey company. Research data were collected via questionnaires and analyzed using SPSS version 20.0 and AMOS version 20.0. Results: Model fit indices for the hypothetical model were suitable for the recommended level: ${\chi}^2=796.40$ (df=79, p<.001), GFI=.93, AGFI=.90, RMSR=.08, NFI=.94. Quality of care, image, and role performance explained 68.1% of variance in community awareness. Total effect of quality of care process factors on satisfaction (path coefficients=3.67), intention to (re)visit (path coefficients=2.67) and intention to recommend hospital (coefficients=2.45) were higher than other factors. Conclusion: Findings show that public hospitals have to make an effort to improve community image through the provision of quality care, and excellent role performance. Support for these activities is available from both Central and Local Governments.

Grounded Theoretical Analysis on the Hospital Accreditation Experience of Head Nurses in General Hospitals (종합병원 수간호사의 의료기관평가 경험에 대한 근거이론적 접근)

  • Purpose: The purpose of this study was to conduct a grounded theoretical analysis on the hospital accreditation experience of head nurses in order to understand their behavior on the adaption of this new system. Methods: The participants were 8 head nurses with more than 3 years of experience. The data were collected through in-depth interviews using audiotape recording analyzed by the constant comparative method described in Strauss and Corbin's methodology. Results: There were 113 concepts, 26 subcategories and 12 categories identified through the open coding process. In the axial coding, the following paradigm model was proposed: 1) the causal conditions were 'hardware problem' and 'software problem', 2) the contextual conditions were 'vertical relationship', 'individual preference', and 'family support', 3) the intervening conditions were 'passive conditions' and 'active conditions', 4) the action/interaction strategies were 'leading role' and 'dependent role', 5) the consequences were 'positive acceptance' and 'negative acceptance', 6) the central phenomenon was 'difficult situation' and 7) the core category was 'leading in harmony'. Conclusion: The new system led head nurses having difficulties as the middleman between the hospital administration and general nurses, but they made a continuous effort to overcome and adapt to it through a number of strategies.

Patients on Anticoagulants after a Head Trauma : Is a Negative Initial CT Scan Enough? Report of a Case of Delayed Subdural Haematoma and Review of the Literature

  • Mild traumatic brain injury is common in elderly patients, many of whom are on anticoagulant. The common practice is to discharge these patients from the emergency room if the computed tomography (CT) of the brain is normal. However, a very small proportion of these patients may develop a life threatening intracranial haematoma in the following days. We present here a case of a 66-year-old male on anticoagulant therapy that developed a subdural haematoma 48 hours after a mild head injury, with a normal initial CT scan of the brain. The patient underwent a craniotomy with evacuation of a large subdural clot. Postoperatively he had progressively improved and six months later has a Glasgow Outcome Score of three. This case is characterized by the delayed onset of a subdural haematoma in a patient on anticoagulation and we discuss here the possible pathogenesis related to this phenomenon. We also briefly review the pertinent literature and the current guidelines for the management of this type of head injuries.

Histological Changes in Biceps Muscle after Tenotomizing the Biceps Long Head in a Rat Model

  • Background: Popeye deformity is common after rupture of the biceps muscle's long head tendon. Herein, we report on histological changes in biceps brachii muscles following tenotomy of the long head biceps tendon. Methods: Twelve Sprague-Dawley rats (12-week-old) underwent tenotomy of the long head biceps tendon in the right shoulder. At postoperative weeks 4, 7, and 10, the operative shoulders were removed by detaching the biceps brachii muscle from the glenoid scapula and humerus; the opposite shoulders were removed as controls. H&E staining was performed to elucidate histological changes in myocytes. Oil-red O staining was performed to determine fatty infiltration. Myostatin antibody immunohistochemistry staining was performed as myostatin is expressed by skeletal muscle cells during myogenesis. Results: H&E staining results revealed no changes in muscle cell nuclei. There were no adipocytes detected. Compared with that of the control biceps, the cross-sectional area of the long head biceps was significantly smaller (p=0.00). Statistical changes in the total extent of the 100 muscle cells were significant (p=0.00). Oil-red O staining revealed no fatty infiltration. Myostatin antibody immunohistochemical staining revealed no significant difference between the two sides. Conclusions: Muscular changes after tenotomy of the long head biceps included a decrease in the size of the individual muscle cells and in relative muscle mass. There were no changes observed in muscle cell nuclei and no fatty infiltration. Moreover, there were no changes detected by myostatin antibody immunohistochemistry assay.

  • For the purpose to clarify the effects of the mothers health during pregnancy on the weight and length of the newborn infants, this study was carried out on 500 new born infants delivered at St, Mary's Hospital, Ewha Woman's University Hospital and Red Cross Hospital in Seoul during the eight-month period from March, 1969 to October, 1969. In this study, the relationship between the T weight and length of the new born infants, and in each case, the weight, length, age, duration of labour, duration of pregnancy, weight gain during pregnancy and diets level during pregnancy of the mother were observed. The relationship between the weight and length of the new born infants and the parity n-as also observed. The results obtained in this study were as follows. 1. The weight of new born infants of both sexes was significantly correlated with the parity, the weight of the mother, and the duration of labour of the primipara. The weight of the mother and the duration of pregnancy were significantly correlated with the weight of the male new born infants only, and the weight of the new born infants and the weight gain of the mother during pregnancy were significantly correlated in the female new born infants. 2. In general, the weight of the new born infants of well or excellently nourished mothers were heavier than those of poorly nourished ones. 3. The length of the new born infants was significantly correlated mother the duration of labour in the primipara.

Influence of Family and Medical Staff Support and Hope on Organ Transplant Recipients' Psychosocial Adjustment (장기이식 수혜자의 가족 및 의료인 지지와 희망이 사회 심리적 적응에 미치는 영향)

  • Purpose: This study was conducted to investigate the factors that influence the psychosocial adjustment of organ transplant recipients. Methods: Participants were 132 organ transplant recipients who received follow-up more than 3 months after the organ transplantation at one general hospital in Seoul. A questionnaire survey was done from November 3 to December 3, 2015. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient, and hierarchical multiple regression. Results: Family support, medical staff support and hope were significantly correlated with psychosocial adjustment. In the hierarchical multiple regression analysis, hope appeared to be the most important factor influencing psychosocial adjustment (${\beta}=.39$, p<.001). Conclusion: The findings show that hope had an influence on the psychosocial adjustment of organ transplant recipients. Thus, to improve psychosocial adjustment it is important for nurses to develop interventions to increase hope in organ transplant recipients.

Effect of Static Recovery and Dynamic Recovery on the Cardiopulmonary Variables, Lower Extremity Muscle Activity after Progressive Resistance Exercise to Maximal Point

  • This study was to examine on the respiratory variables, heart rate and muscle activity between the static recovery and dynamic recovery after progressive resistance exercise to maximal point. Subjects were 15 students enrolled in N University. All were tested two times (static recovery and dynamic recovery) and were requested to perform a walking on a treadmill after progressive resistance exercise to maximal point. Electromyography(EMG) was used to monitor the muscle activity(TA: Tibialis Anterior, MG: Medial Gastrocnemius) during gait. CPEX-1 was used to measure the respiratory variables and heart rate. The dynamic recovery group was shown the significant lower heart rate than that of static recovery group at during gait. Respiratory rate showed statistically a significant difference. Electromyography(RMS, root mean square) showed a non-significant difference. But the dynamic recovery group of muscle activity was found highly in TA and MG. This study indicated that the dynamic recovery method evidenced more faster than the static recovery method. And this type of dynamic rest by walking can be a help of recovery after exercise.

Hepatitis B Virus Gene C1653T Polymorphism Mutation and Hepatocellular Carcinoma Risk: an Updated Meta-analysis

  • Although there have been many studies investigating possible associations between the C1653T mutation and risk of HCC, the results have been inconsistent. We conducted searches of the published literature in Pubmed and Embase databases up to January 2013. Seventeen studies with a total of 1,085 HCC cases and 1,365 healthy controls were retrieved. We found a significant association between the C1653T mutation and HCC risk (OR = 2.01, 95%CI= 1.49-2.70). In the subgroup analysis by ethnicity, a significant association was also found in Asians (OR = 2.07, 95%CI= 1.71-2.51). In subgroup analysis by HBV genotype, B and C were linked with development of HCC (B:OR = 2.21, 95%CI= 1.13-4.34; C:OR = 2.26, 95%CI= 1.61-3.16). However, no significant association was found between the C1653T mutation and HCC risk in HBeAg positive cases. In conclusion, this meta-analysis suggests that the C1653T mutation may be associated with susceptibility to HCC.

Roles of Empowerment and Emotional Intelligence in the Relationship between Job Embeddedness and Turnover Intension among General Hospital Nurses (종합병원 간호사의 직무배태성과 이직의도와의 관계에서 임파워먼트와 감성지능의 역할)

  • Purpose: The purpose of this study was to identify the roles of empowerment and emotional intelligence in the relationship between job embeddedness and turnover intension among general hospital nurses. Methods: The sample for this study consisted of 224 nurses from four general hospitals of less than 500 beds located in Seoul and Gyunggi Province. Data were analyzed using frequency, percentage, mean, standard deviation, t-test, ANOVA, Scheff$\acute{e}$ test, Pearson Correlation and Hierarchical Multiple Regression. Results: It was found that: (a) Job embeddedness and emotional intelligence had negative effects on turnover intension; (b) Emotional intelligence moderated the relationship between job embeddedness and turnover intension, but empowerment did not. Conclusion: Findings indicate a need to reduce the degree of turnover by enhancing job embeddedness among general hospital nurses. Further research needs to be done to refine this study.
