전방십자인대 재건술 환자에 대한 비손상측과 손상측 간의 하지 근수축 특성 비교 (Comparison of the Muscle Contractile Properties on Lower Extremity Between Non-injury and Injury in Subjects with Anterior Cruciate Ligament Reconstruction)

  • Background: The purpose of this study was to identify the comparison of the muscle contractile properties on lower extremity between non-injury and injury in subjects with anterior cruciate ligament reconstruction (ACLR). Methods: Twenty-four subjects on the post-operative ACLR participated in this study. Measurement method were using tensiomygraphy to analyze the displacement maximum(Dm) and contraction time(Tc) on the both quadriceps. Results: Compared difference of the Dm between non-injury and injury, the rectus femoris (RF) and vastus lateralis (VL) increased significantly (p<.01). However, vastus medialis (VM) no significantly difference between non-injury and injury. Compared difference of the Tc between non-injury and injury, the RF increased significantly (p<.01) but VL and VM no significantly difference between non-injury and injury. Conclusions: These findings suggest that occurred muscle atrophy of the RF and VL and change properties of muscle fibers on the RF from fast muscle fibers to slow muscle fibers on the injury side of post-operative ACLR. Therefore, when apply to rehabilitation of lower extremity for post-operative ACLR, we should consider the enhance of RF and VL on injury side.

  • Purpose : The purpose of this study was to confirm changes in the muscle activity of the quadriceps femoris with changes in the ankle and hip joint angles during the transition from sitting to standing. Method : Twenty-five healthy 20-30-year-old women participated in the study. The subjects performed standing-up movements in four positions: standard posture (hip = $90^{\circ}$/ankle = $0^{\circ}$); posture on a decline board (hip > $90^{\circ}$/ankle < $0^{\circ}$), posture on a footboard (hip > $90^{\circ}$/ankle = $0^{\circ}$); and posture on a decline board with a higher chair (hip = $90^{\circ}$/ankle < $0^{\circ}$). Then, the muscle activities of the rectus femoris, vastus medialis and vastus lateralis were measured using surface electromyography. Result : The muscle activities of the rectus femoris, vastus medialis and vastus lateralis in the footboard application were statistically significantly higher than in the application of the decline board with a higher chair. Conclusion : This study confirms that the flexion of the hip joint has a greater effect than the plantar flexion of the ankle joint on the muscle activity of the quadriceps during a sit-to-stand movement.

하회별신굿탈놀이가 보행기능 및 근 활성도에 미치는 영향 (The Effects of the Hahoe Mask Dance on the Gait and Muscle Activity in the Elderly)

  • Purpose : The purpose of this study was to evaluate the effects of the Hahoe mask dance on the gait and muscle activity in the elderly who lived at a elderly welfare facility. Methods : Dancing group was consisted of 20 subjects(over 70 years). The time of measurement was assigned in pre measurement, after 1 month, after 2 months. Subjects was received the Hahoe mask dance for three times a week during 8 weeks. The control group was consisted of 20 the elderly (similar age) who didn't received dancing. Assessment of gait function included the gait velocity, step time, stride, step length, it was analyzed in activity for 4 muscles (rectus femoris, biceps femoris, tibialis anterior, and lateral gastrocnemius). Results : Activities for muscles of control group was decreased in rectus femoris, bieps femoris, however, those of experimental group was increased significantly(p<.05). While control group showed decrease in the gait velocity per second (p<.05), experimental group showed statistically significant increase(p<.05). Control group had a significant long time than that of control group in gait time of right and left foot(p<.05). Control group was a little longer than control group in stride time of right and left foot(p<.05). Step length for control group was shorter than that for experimental group both foots(p<.05). Conclusion : In conclusion, this study confirmed that activities for 4 muscles by EMG have showed significant increases, so we will use a programme of muscle improvement in elderly community welfare faculty. We recommend that further research should explore the degree of muscle activity by larger sample sizes and longer follow-up periods.

전방보행과 후방보행 시 속도변화에 따른 근전도 비교 분석 (The Comparative Analysis of EMG Depending on Variations of Speed in Forward Walking and Backward Walking)

  • The purpose of this study was to examine the differences of lower limbs muscle activities depending on three walking speeds of 2.5km/h, 5.0km/h and 7.5km/h during forward walking and backward walking making 14 students the subjects of this study. To achieve this aim, surface electrodes for factor analysis of EMG were adhered to rectus femoris, biceps femoris, tibialis anterior and gastrocnemius medial head of right lower limbs. The conclusions through this study are as follows. 1) The muscle activity of rectus femoris was higher in backward walking group than in forward walking group and it was the highest at 7.5km/h walking speed. 2) The muscle activity of biceps femoris was higher in forward walking group than in backward walking group. It was the lowest at 5.0km/h walking speed and the highest at 7.5km/h walking speed. 3) The muscle activity of tibialis anterior was higher in backward walking group than in forward walking group. It was the lowest at 5.0km/h walking speed and the highest at 7.5km/h walking speed. 4) The muscle activity of gastrocnemius medial head was higher in backward walking group than in forward walking group except P2. It was the lowest at 5.0km/h walking speed and the highest at 7.5km/h walking speed.

Effects of Aquatic and Ground Obstacle Training on Balance and Muscle Activity in Patients With Chronic Stroke

  • Background: Obstacle training affects lower limb muscle activity, balance, reducing the risk of falls, and making gait more stable. Objects: This study aimed to investigate the effects of aquatic and ground obstacle training on balance and muscle activity in patients with chronic stroke. Methods: The study subjects included 30 patients with stroke, who were divided into aquatic ($n_1=15$) and ground ($n_2=15$) groups. Groups underwent obstacle training three times per week, 30 min per session, for six weeks that went as follows: walking over sites with the paralyzed leg, stepping onto and down from a box step, and walking over obstacles with the non-paralyzed leg. Results: The experimental results were obtained by comparing muscle activity. Activity of the rectus femoris, biceps femoris, tibialis anterior, and gastrocnemius were significantly increased in the aquatic group (p<.05). Activity of the biceps femoris and tibialis anterior were significantly increased in the ground group (p<.05); however, the rectus femoris and gastrocnemius were not significantly different. In the comparison of maximal distance regarding the limits of stability, it was significantly increased on the non-affected side, affected side, and anterior and posterior distance in the aquatic group (p<.05). It was significantly increased in the non-affected side and anterior and posterior distance the ground group (p<.05); however, maximal distance on the affected side distance was not significantly different. Conclusion: Gait training with aquatic and ground obstacles is effective for improving balance and gait ability of patients with stroke. However, it was more effective for the aquatic group than for the ground group.

Effect of Graston Technique on Fatigue Recovery after High-Intensity Exercise of the Quadriceps Femoris

  • PURPOSE: This study was conducted to compare the effects of the Graston-based massage method and microcurrent application on muscle fatigue after causing muscle fatigue of the quadriceps femoris by applying high-intensity exercise. METHODS: Study subjects (56 healthy subjects in their 20s) were randomized into a microcurrent group (MG), Graston group (GG), and control group (CG). To induce fatigue of the quadriceps femoris, the subjects performed squats 100 times without a break within about 5 minutes. Muscle fatigue was measured immediately after performing squats and 15 minutes after intervention. To measure muscle fatigue, surface electromyogram (EMG) was used to obtain and compare the median frequency. Microcurrent or Graston massage was applied to their quadriceps muscles 15 minutes after performing the squats. RESULTS: Muscle fatigue increased significantly in the vastus lateralis before and after intervention in the Graston group (p<.05), while no significant difference was observed in the rectus femoris and vastus medialis (p>.05). There were also no significant differences in the microcurrent group or the control group. CONCLUSION: Both the microcurrent group and Graston group showed a tendency for increasing median frequency values compared to the control group, but there was no significant difference except in the vastus lateralis treated with the Graston technique. Although there was no significant difference, the Graston technique could be utilized to reduce the occurrence of DOMS by preventing muscle fatigue in clinical practice or sports medicine.

EMS를 결합한 다리 근력 강화훈련이 대학 축구선수의 넙다리네갈래근 근활성도에 미치는 영향 (The Effect Of EMS Combined Lower Limb Strengthening Training on the Quadriceps Femoris Muscle Activity of the Soccer Player)

  • Background: This study aims to investigate the effect of lower limb strengthening training combined with electro muscle stimulation on the quadriceps femoris muscle activity of soccer players. Methods: Thirty university soccer players were selected as study subjects and divided into a lower limb strengthening training group combined with EMS (Group I) and a general lower limb strengthening training group (Group II), and 15 subjects were randomly assigned. After receiving general soccer training, subjects in this study additionally mediated lower limb strengthening training combined with EMS and general lower limb strengthening training for 26 minutes, 3 times a week for 8 weeks. Quadriceps femoris muscle activity was analyzed before mediation. Vastus medialis, vastus lateralis, and rectus femoris were measured with maximum isometric contraction in the manual muscle test position in order to analyze leg muscle activity. The same items as above were re-measured and a between-group analysis was conducted after 8 weeks of mediation. Results: As a result of comparative analysis of lower extremity muscle activity between groups, the lower limb strengthening training group combined with EMS showed a statistically significant difference in lower extremity muscle activity compared to the general lower limb strengthening training group. Conclusion: As a result, it was found that lower limb strengthening training combined with EMS was more effective in improving quadriceps femoris muscle activity. Based on this study, we are going to provide basic data on the possibility of using EMS in the field of sports rehabilitation for soccer players.

가슴압박시 구급대원의 체간 각도와 근활성도 분석 (Analysis of trunk angle and muscle activation during chest compression in 119 EMTs)

  • Purpose: We aimed to investigate trunk angle and muscle activation of the extremity and back to evaluate the effect of chest compression on work-related musculoskeletal disorders in 119 emergency medical technicians (EMTs). Methods: Eighteen 119 EMTs performed 2-minute chest compression without interruption on a cardiopulmonary resuscitation manikin, during which we measured changes in the trunk and shoulder joint angles, muscle activation (triceps brachii, biceps brachii, erector spinae, gluteus maximus, pectoralis major, rectus abdominis, and rectus femoris) and chest compression accuracy. Results: The decrease in trunk angle by trunk muscle activation was the highest in event 2, the major direction of chest compression. Both shoulder joint angles had no significant difference. Muscle activation of the triceps brachii (p < .01), biceps brachii (p < .05), rectus abdominis (p < .05) and rectus femoris (p < .01) significantly increased during the compression phase compared with the decompression phase, with the rectus femoris showing an increase of 19%. Muscle activation of the erector spinae significantly increased in the decompression phase compared with the compression phase (p < .01). Conclusion: 119 EMTs mainly use the triceps brachii, biceps brachii and pectoralis major muscles during chest compression.

한센노인의 체간운동이 슬관절 굽힘·폄 동작의 최대수축 시 근피로도에 미치는 영향 (Effects of Muscle Fatigue through Maximum Contraction during Flexion and Extension of Knee Before and After Isometric Trunk Exercise on Elderly Hansen's Disease)

  • This study was to investigate the change of muscle fatigue through maximum contraction during flexion and extension of knee joint before and after the isometric trunk exercise was implemented on elderly Hansen's disease. 18 subjects exercised for 60 minutes twice a week for 12 weeks. The subjects were divided into normal sensory group, group with sensory loss in left sole, group with sensory loss in right sole, and group with sensory loss in both soles, according to the sensory condition on their soles. We obtained following results. Muscle fatigue in normal sensory group, there were significant differences in left right hamstring, left rectus femoris(p<.05), in group with sensory loss in left sole, there were significant differences in left right hamstring(p<.05), in group with sensory loss in right sole, there were significant differences in left right hamstring, right rectus femoris and group with sensory loss in both sole, there were significant differences in right hamstring(p<.05). The differences of muscle fatigue according to the sensory condition on their soles, there were significant differences in left hamstring between normal sensory group and group with sensory loss in both sole(p<.05).

정상인의 팔걸이 사용에 따른 보행 시 하지 근 활성도의 변화 (Change of lower limb muscle activation according to the use of arm sling in normal subjects)

  • Background: The purpose of this study was to investigate the change of lower limb muscle activation according to the use of arm sling in normal subjects. Design: Cross-sectional Study Methods: Seven healthy subjects (6 males and 1 female, 25.42 years, 173.57 cm, 71.71 kg) were recruited on a voluntary basis. To measure the lower limb muscle activation during walking with and without arm sling, we used a wireless surface electromyography (sEMG) (FreeEMG1000, BTS Bioengineering, Milano, Italy). Six wireless sEMG electrodes were attached to the following three major muscle groups of the both side lower limb: rectus femoris, biceps femoris, medial gastrocnemius. All subjects wore arm sling on their right side during measurement. Results: In the stance phase, there was a significant increase in right side rectus femoris muscle activation in walking without arm sling compared to the walking with arm sling (p<.05). Additionally, In the stance phase, there was a significant increase in left side tibialis anterior muscle activation in walking without arm sling compared to the walking with arm sling (p<.05). Conclusion: The results of this study suggest that there is a significant association between the arm swing restriction and lower limb muscle activation. Therefore, it seems that it can be applied as basic data for gait training with an arm slings.
