Ultrasound-guided intraoral botulinum toxin injection into the lateral pterygoid muscle for chronic temporomandibular joint dislocation

  • Objectives: Botulinum toxin type A (BTX), a powerful neurotoxin, can be an effective treatment choice for diverse muscular disorders and can reduce abnormal muscle activities. Abnormal movements of the mandible can be caused by involuntary and uncontrolled contractions of the lateral pterygoid muscle (LP) in various pathological situations. Previous reports have shown that BTX can reduce abnormal contractions of the LP. However, needle placement into the LP for BTX injection requires skill, experience, and sufficient anatomical knowledge. To place the needle precisely into the LP, ultrasonography (USG) can be used as an effective needle-guidance modality. USG is a non-invasive imaging modality able to create real-time images without any potential risks, including radiation exposure. Patients and Methods: The patients who had been performed USG-guided BTX injection into the LP using an intraoral approach were included in this study with a literature review and case presentations. Using the USG, four patients received BTX injections to treat recurrent temporomandibular dislocation and oromandibular dystonia resulting from involuntary LP activity. Result: Involuntary movements of the mandible were improved successfully in all patients, and showed satisfactory results without significant complication. Conclusion: The intraoral approach could prevent potential complications during needle placement. USG-guided BTX injection is an effective, convenient, and safe method that provides real-time imaging without unnecessary pain to the patient.

Development of a Laser-Generated Ultrasonic Inspection System by Using Adaptive Error Correction and Dynamic Stabilizer (적응적 에러 보정과 다이나믹 안정기를 이용한 레이저 유도 초음파 검사 시스템 개발)

  • Laser-generated ultrasonic inspection system is a non-contact scanning inspection device with high spatial resolution and wide bandwidth. The amplitude of laser-generated ultrasound is varied according to the energy of pulse laser and the surface conditions of an object where the CW measuring laser beam is pointing. In this paper, we correct the generating errors by measuring the energy of pulse laser beam and correct the measuring errors by extracting the gain information of laser interferometer at each time. h dynamic stabilizer is developed to stably scan on the surface of an object for an laser-generated ultrasonic inspection system. The developed system generates ultrasound after adaptively finding the maximum gain time of an laser interferometer and processes the signal in real time after digitization with high speed. In this paper, we describe hardware configuration and control algorithm to build a stable laser-generated ultrasonic inspection system. Also, we confirmed through experiments that the proposed correction method for the generating errors and measuring errors is effective to improve the performance of a system.

A Comparative Study on Heat Transfer Characteristics in Tissue Model with Application of Heating or Cooling Therapeutic Modalities (조직모델에서 냉.온치료기의 열전달 특성 비교 연구)

  • Objectives : This study aimed to investigate temperature characteristics by heat transfer type of therapeutic modalities. Methods : We selected heating and cooling modalities that are frequently used in clinical by heat transfer type: conduction, convection, radiation, and conversion. We used ham as tissue model, and applied the modalities for 30 minutes. We measured real-time changed temperature($^{\circ}C$) of the surface, 2, 4, 6, 8, 10, 12, and 14 mm depth. Results : 1. In conduction-using hot pack, ice pack, and CryoStamp heating-cooling combination therapy unit($40^{\circ}C/{\sim}15^{\circ}C$), the surface temperature sharply rose close to equilibrium in first 5 minutes. 2. In convection-using smokeless moxa, temperature slowly rose to the maximum at 25-minute elapsed time. But in another convection-using CRAiS cryotherapy device, result was similar to that of conduction. 3. In radiation-using infrared lamp, result was similar to that of conduction, but not reached equilibrium during applying time. 4. In conversion-using ultrasound device, temperature was the highest at 6 mm depth, and not reached equilibrium during applying time. Conclusions : We could comprehend temperature characteristics and proper use of modalities by heat transfer type. It would be necessary to consider in vivo physical conditions in further studies.

New Circular Wave Generation Method for Synthetic Focusing in Ultrasonic Imaging Systems (초음파 영상 시스템에서 합성 집속을 위한 새로운 구면파 발생 방법)

  • The synthetic focusing in the ultrasonic imaging systems has been formed in the way that one element transmits a circular wave and receives an echo signal. The amplitude of the signal transmitted from one element is too small to propagate a long distance so that the SNR(Signal to Noise Ratio) is very low in an image obtained by the synthetic focusing. To solve this problem, a defocusing method which uses several elements has been proposed. In this method, the SNR is improved due to using several elements to transmit the circular wave. But if the number of transmitting elements increases, the phase distortion is severe in the defocusing method. In this paper, we propose a new method that can generate a circular wave using a lot of elements without phase distortion. At first, we generate limited plane waves with different propagation angles and then superpose them to make a circular wave. We show that the circular wave can be used to improve SNR in the real-time 3D ultrasonic imaging as well as the synthetic focusing through computer simulation and experiments.

Estimates of Genetic Correlations between Production and Semen Traits in Boar

  • Currently, boars selected for commercial use as AI sires are evaluated on grow-finish performance and carcass characteristics. If AI sires were also evaluated and selected on semen production, it may be possible to reduce the number of boars required to service sows, thereby improving the productivity and profitability of the boar stud. The objective of this study was to estimate genetic correlations between production and semen traits in the boar: average daily gain (ADG), backfat thickness (BF) and muscle depth (MD) as production traits, and total sperm cells (TSC), total concentration (TC), volume collected (SV), number of extended doses (ND), and acceptance rate of ejaculates (AR) as semen traits. Semen collection records and performance data for 843 boars and two generations of pedigree data were provided by Smithfield Premium Genetics. Backfat thickness and MD were measured by real-time ultrasound. Genetic parameters were estimated from five four-trait and one five-trait animal models using MTDFREML. Average heritability estimates were 0.39 for ADG, 0.32 for BF, 0.15 for MD, and repeatability estimates were 0.38 for SV, 0.37 for TSC, 0.09 for TC, 0.39 for ND, and 0.16 for AR. Semen traits showed a strong negative genetic correlation with MD and positive genetic correlation with BF. Genetic correlations between semen traits and ADG were low. Therefore, current AI boar selection practices may be having a detrimental effect on semen production.

  • 초음파 영상 기법은 장기, 연조직, 혈류를 검사하는데 쓰이는 영상 진단법이다. 초음파 장비를 통해 얻어진 초음파 볼륨 데이터는 장비 특성상 많은 잡음(speckle noise)을 포함하기 때문에, 깨끗한 영상을 얻기 위해서는 잡음 필터링(noise filtering)이 필요하다. 그런데, 볼륨 데이터 모든 영역에 대해 잡음 필터링을 적용할 경우 전처리 시간이 길어져 실시간으로 초음파 볼륨 데이터를 렌더링하기 어렵다. 본 논문에서는 실시간으로 입력되는 초음파 볼륨 데이터를 가시화 하기위하여 전처리 시간 없이 잡음을 제거하는 방법을 제안한다. 전처리 시간에 전체 볼륨 데이터에 대해 잡음 필터링을 적용하지 않고, 영상을 생성하는 동안 참조되는 복셀(voxel)에 대해서만 잡음 필터를 적용하여 얻은 값을 사용한다. 이때 필터링에 소요되는 시간을 최소화하기 위해 가장 단순한 평균화 필터를 사용한다. 그리고 복셀에 적용되는 3차원 필터를 3단계의 1차원 필터 연산 단계로 분할 한 후, 각 단계별 연산을 거친 복셀들에 대해서는 다시 연산을 하지 않도록 하여 중복을 피한다. 이를 통해 전처리 시간 없이 기존 방법과 동일한 화질을 유지하는 최종 영상을 만들어 낸다.

Effect on Activation of Abdominal Local Muscles During Modified Bridge Exercise in Healthy Individuals (변형된 교각운동이 복부 국소근육의 활성화에 미치는 효과)

  • The purpose of this study was to investigate abdominal local muscle activity during modified bridge exercise. 17 subjects participated in this study. Abdominal muscles measured. External oblique abdominis (EO), internal oblique abdominis (IO), transvers abdominis (TrA), and rectus abdominis (RA) during general bridge exercise and modified bridge exercise, respectively. Electromyogram (EMG) and real-time ultrasound were used to verify alteration of muscles. Activation of RA and EO muscles of non-dominant foot was significantly difference in general bridge exercise group, not modifiedl bridge exercise group. In the modified bridge exercise group, thickness of IO and TrA muscle of non-dominant foot was significantly difference in modified bridge exercise group than general bridge exercise group. Therefore, modified bridge exercise may be apply as more effective exercise for local muscle activity than global muscle.

A Study on Ultrasound Pulsed Doppler Systems for Sending the Blood Flow (혈류 진단을 위한 초음파 펄스 도플러 시스템에 관한 연구)

  • In the conventional pulsed doppler system, gated CW is used to excite a ultrasonic transducer so that a group of linear RF amplifiers are required to excite a ultrasonic array transducer in the scanning pulsed doppler system. A pulsed doppler system without linear high voltage RF amplifiers, which excites the transducer impulsively, is studied theoretically and experimentally. In this paper, an experimental 8-channel pulsed doppler system is implemented, which uses quadrature defection to detect the direction of motion and can compensate the attenuation effect. The designed pulsed doppler system shows the possibility of real time multichannel doppler flow meter.

  • A running high voltage equipments produce ultrasonic wave that has unique sound by the specific characteristics of the electricity. The generation of the ultrasonic wave is made by the electric transform like arcing, corona, and tracking so on. The mechanical losses and fatal human damages are happened by the electric failure of high voltage equipments. To prevent and diagnose the obstacle factors of the high voltage equipments, the measurement of the ultrasonic wave became to be prominent. However standardized data have been a deficient situation by now. This paper measures the ultrasonic wave coming from the real running transformer equipments and transforms it as an audio frequency. Measured data represents as frequency and time domain through the FFT(Fast Fourier Transform) transform. In conclusion, the purpose of this paper is to standardize the analyzed data.

Comparisons of Abdominal Muscles Thickness During Single Leg Holding Exercise on Stable Surface and on a Foam Roller Using Ultrasound Imaging (안정된 면과 폼 롤러 위에서 한 발 들기 자세 시 초음파를 이용한 복근들의 두께 양상 비교)

  • The purpose of this study was to compare the thickness of the abdominal muscles during single leg holding exercise (SLH) in a hooked lying position on stable surface and on a foam roller. Healthy twenty subjects who had no medical history of lower extremity or lower back pain were recruited for this study. Muscle thickness of transverse abdominis (TrA), internal oblique (IO), and external oblique (EO) was recorded using real-time ultrasonography during SLH. Paired t-test with Bonferroni adjustment was used to compare the muscles thickness during SLH on stable surface and on a foam roller. The result showed that TrA and IO muscle demonstrated greater thickness during SLH on foam roller than those on the stable surface. This finding suggests that SLH on an unstable foam roller is more effective to increase thicknesses of TrA and IO muscles than stable surface.
