Re-pull-through Operation in Hirschsprung's Disease (Hirschsprung 병에서의 재 교정 수술의 성적)

  • A re-pull-through operation for Hirschsprung's disease is performed when the primary operation has failed because of the remnant or acquired aganglionosis, anastomotic stricture and/or fistula. The purpose of this study is to review our experience of the re-pull-through procedure for Hirschsprung's disease. From May 1978 to July 2003, 26 patients who underwent re-pull-through operations at the Department of Pediatric Surgery, Seoul National University Children's Hospital, were studied retrospectively by means of chart review as well as telephone interview. The mean age at primary operation and re-operation were llmonths (2 months - 10 years) and 43 months (1 year - 23 years 3 months), respectively. Initial operation for Hirschsprung's disease was Duhamel's procedure in 17, Swenson's in 6 and Soave's 3. Causes of failure of primary operation were remains of secondary aganglionic segment (n=23), vascular arcade injury (n= 1), rectoperineal fistula (n=2, due to mesenteric torsion and poor blood supply), Mean interval between the primary operation and the re-operation was 34 months (6 months-22 years). Reoperation methods were Soave's in 12, Duhamel's in 8, APSP (abdomino-posterosagittal pull-through procedure) in 5, and Swenson's in 1 case. In 2 cases of repeated rectoperineal fistula or rectourethral fistula, re-APSP were performed 3 times, respectively. A total of 29 re-pull-through operations were performed. Postoperative complications were wound infection (n=1), adhesive ileus (n=1), rectoperineal fistula (n=3), rectourethral fistula (n=2), and death due to pulmonary embolism (n=1). Mean follow up period of reoperation was 78 months (1 month-23years). In current state, 2 patients have an ileostomy because of repeated rectoperineal fistula and rectourethral fistula. Of the remaining 23 patients, 21(91 %) are totally continent with or without minimal soiling. Reoperation for Hirschsprung's disease was effective and safe for the patients complicated to the initial pull-through operation.

Resistive Switching Characteristics of Amorphous GeSe ReRAM without Metalic Filaments Conduction

  • We proposed amorphous GeSe-based ReRAM device of metal-insulator-metal (M-I-M) structure. The operation characteristics of memory device occured unipolar switching characteristics. By introducing the concepts of valance-alternation-pairs (VAPs) and chalcogen vacancies, the unipolar resistive switching operation had been explained. In addition, the current transport behavior were analyzed with space charge effect of VAPs, Schottky emission in metal/GeSe interface and P-F emission by GeSe bulk trap in mind. The GeSe ReRAM device of M-I-M structure indicated the stable memory switching characteristics. Furthermore, excellent stability, endurance and retention characteristics were also verified.

Inter-bay Re-marshalling Planning in the Automated Container Terminal (자동화 컨테이너 터미널의 베이간 컨테이너 재배치작업 계획)

  • The container terminal operators established a re-marshalling plan to reduce the loading operation time and the release operation time. Re-marshalling is to rearrange the containers in the container yard to the advantageous position to shorten the working time using the spare time of the automated yard crane. This study assumed the automated container terminal with a perpendicular layout and deals with the inter-bay re-marshalling planning problem in a yard block. The inter-bay re-marshalling plan determines the container to be moved, the location to be relocated, and the sequence of relocation operations. This study presents a mixed integer programming model that simultaneously determines the storage location and the operation sequence while satisfying the spatial availability during the re-marshalling. Numerical experiments are conducted to understand re-marshalling operation using a beam search method.

The Cause of Cervical Lymph Node Recurrence after the Initial Surgery of Papillary Thyroid Carcinoma (갑상선 유두암의 일차 수술 후 경부 림프절 재발의 원인)

  • Background/Objectives: Papillary thyroid carcinoma (PTC) has generally an indolent character with a good prognosis. However, recurrence remains a major concern for the patients during their lifetime. Despite the slowly progressing character of PTC, recurrence can occur within a short period after initial surgery. This study aimed to determine the clinical findings and cause of recurrence in patients who underwent re-operative surgery due to neck node recurrence by reviewing the CT (computed tomographic) scan imaging of the recurrence of PTC retrospectively. Materials & Methods: We reviewed the medical records of patients referred to Ajou University Hospital from January 2002 to January 2018. All patients had re-operative surgery due to neck node recurrence and CT scan results of preoperative evaluation and postoperative follow up. Over this period, 110 patients who underwent re-operation due to neck node recurrence with a CT scan were included in our cohort, resulting in a total of 220 re-operations. Results: The time from initial operation to first re-operation was examined in 110 patients. The median time to re-operation was 28 months, with a range of 4 months to 186 months. Most re-operations (82.7%) occurred within the first five years, 43.6% were in the first two years from the initial surgery. The result of the retrospective CT review showed newly developed cases (21,19.1%), missed diagnosis cases (42,38.2%), real recur cases after surgery (33,30.0%), and remnant lymph nodes (LNs) cases (14,12.7%). We further sub-analyzed 14 cases with remnant LNs. Reasons for remnant LNs included insufficient operation (N=5) and beyond general surgical extent. (N=9). Conclusion: Re-operation due to cervical lymph node recurrence is mostly a persistent disease. They included a missed diagnosis and incomplete operation. These finding may reduce the reoperation of cervical lymph node recurrence by accurate preoperative evaluation and complete surgical resection at the initial surgery.

Effects of Core Competence and Resource Sharing on the Relational Bond among Franchisees and on Re-contract Intention in the Franchising Parent Companies (프랜차이즈 모기업 핵심역량 자원이 가맹점 관계결속과 재계약의도에 미치는 영향)

  • Purpose - The domestic franchise industry has made significant contributions to industry such as investment, employment, and community economy development, facilitating growth potential. The franchise industry provides management knowhow transfer between parent companies and franchisees as per contracts addressing use of business signs, franchisees' independent position, franchisor support, and royalties to be paid to the franchisors. However, many franchisors lack management knowhow and provide insufficient support because of poor control of franchisees and not having a systematic approach to support. This results in dissatisfied franchisees and failure to establish long-term relationships. Few studies have examined relational commitment and/or re-contract intention by support resources between franchisors and franchisees, despite a considerable output of theories and studies of the growth of the franchise industry. The purpose of this study, therefore, was to examine the effects of the ability to provide resources on relational commitments and re-contract intention, and to suggest causal relationships and implications. Research design, data, and methodology - The subject was domestic franchisors registered with the Association of Franchise with more than 50 franchisees. Franchisees under contract for 2 years and considering re-contract of their franchise 2 years later were used. The subjects totaled 300 franchisees in Seoul. A questionnaire survey was used to investigate the subject of franchisees' concessions during the 10 days from November 21, 2013 to November 30, 2013. After excluding 16 surveys with poor answers, 284 responses were finally used. Four areas and 44 questions were used. A nominal scale was used for four common characteristics questions including gender, ages, educational background, and franchise managing time. Questions regarding ability, relational commitment, and re-contract intention made use of a Likert 5-point scale. Data coding and data cleaning were used. SPSS 18.0 was used as follows. First, frequency analysis was done to investigate demographic characteristics. Second, exploratory factor analysis was done to verify validity of testing tools, and Cronbach's α coefficient was used to verify reliability. Third, correlation analysis and multiple regression analysis were done. An exploratory factor analysis was done to verify validity of concepts. A correlation analysis was done to test relationships between the variables, and multiple regression analysis was done to verify franchisor's ability, franchisees' relational commitment, and re-contract intention. Results - The following were the outcomes. First, store operation management, finance operation management, and human resource management affected the calculated bond. Second, store operation management and finance operation management affected the emotional bond. Third, store operation management, finance operation management, human resource management, and marketing management affected the prescriptive bond. Fourth, calculated bond and prescriptive bond had an effect on re-contract intention. Conclusions - As stated above, in franchise management, parent companies' offer and instruction of core competence to their franchisees as an information resource could improve the relational bond by helping them grow together through the resource sharing. Consequently, core competence factors were promoting factors that could improve franchisees' re-contract intention for a long time.

  • Ship owners had pursued higher benefits by demanding the new design and construction of ships with higher operational efficiency. There was a necessity for shipyards to suggest a more economical design and advanced operation concept in order to meet the demands. Especially, since BOG combustion and activation of the re-liquefaction unit had to be taken into account in ship design in addition to fuel oil and gas consumption, the evaluation of the operating efficiency considering the technological trends was necessary. In this paper, it was aimed to study the design philosophy and operation strategy by considering the effect of fuel oil and gas consumption, BOG combustion, and activation of the re-liquefaction unit on the operating cost for laden voyage according to ship speed, BOR, and activation of the re-liquefaction unit. For this purpose, the costs were acquired by conducting the sailing simulation of an LNGC based on a mathematical model including the maneuvering equations of motion. The design philosophy and operation strategy was reviewed in terms of the operating cost.

Estimating the Expected Number of Re-handles for Empty Containers during Loading Operation (적하작업을 위한 공 컨테이너 기대 재취급 횟수 추정)

  • This paper assessed the impacts of the specification of the yard, handling equipment, and operation approaches on the expected number of re-handles during the loading operation for empty containers. When the various types of empty containers from multiple shipping liners are placed in separate spaces from each other, then the storage space cannot be fully utilized. So as to increase the utilization of the storage space, empty containers from multiple vessel liners are stored together incurring additional re-handles during the loading operation. Several formulas are derived for the estimation of the expected number of handles, including re-handles, for empty container retrieval from a bay. Transfer cranes and top handlers are utilized as handling equipment and various retrieval strategies are examined. Numerical analysis was conducted to assess the effects of various designs and operational parameters of the container stacking yard on the expected number of handles.

A Study on the trajectory prediction of the satellite re-entry in Korea (국내 위성추락 예측 연구)

  • As we, human expand its everyday life boundary to the geosynchronous orbit, we have experienced frequent chance of the atmospheric re-entry and surface impact of space objects(satellite and space debris). Recently a satellite re-entry monitoring room in Korea has been operated to predict the time and the location of the re-entry of space objects. However, we do not have a domestic version of a numerical re-entry model for normal operation using TLE (Two line Element) information from the United States Strategic Command yet. The space information from the several space operation centers has been used to analyse the re-entry situations. In this paper, the re-entry time is calculated with TLE based on the several atmosphere models, the result is comprehensively analyzed, a new re-entry case model fitted from the result of the predicted satellite re-entry times by a new Rubber Sheet Shift Method used by the domestic satellite re-entry room is suggested.

Retrospective Analysis of Re-operated Patients after Chronic Subdural Hematoma Surgery

  • Objective : The aim of this study is to analyze the clinical symptoms, radiological changes, interval from first operation to symptom recurrence and to propose the proper treatment method for re-operated patients following chronic subdural hematoma surgery. Methods : Between January 1992 and April 2003, 18 of 138patients of chronic subdural hematoma repeatedly underwent surgical treatment. The symptoms, mental status by Bender grade, radiological hematoma size and midline shifting, interval from symptom onset to diagnosis, surgical method and prognosis by Glasgow outcome scale[GOS] between the first attack and the recurrence were compared. Results : The symptoms at the time of recurrence were nearly the same as with the first attack, but two patients[2/18, 11.1%] showed a more declined mentality. In addition, the recurred hematoma sizes were the same or large than those previously found. Many patients were recurred within two weeks[13/18, 72.2%]. Most patients were operated on using the previous burr hole, with the exception of one patient who recurred at a different site. All patients had a good prognosis more than GOS 4 [GOS 4:4, GOS 5:12], but two died due to extracranial complication and infection. Conclusion : These results suggest that the early diagnosis and treatment are important, mostly recurred same symptoms within two weeks. Re-operation using the previous burr hole site is a good method.

A Thermal Performance Analysis on the Operation Modes of Radiant Floor Heating System Using SERI-RES (SERI-RES를 이용한 Radiant Floor Heating System의 열 성능 분석)

  • The present paper deals with two types of heating methods widely used for the Radiant Floor heating systems in Korea. Of these, one method circulates hot water according to the predefined schedule ("intermittent heating") while the other runs the system with the aid of thermostats ("continuous heating"). The standard version of SERI-RES has been modified for the numerical simulation of the problem. Preliminary results show relatively large temperature swings in the case of intermittent heating with solar availability. On the other hand, the case of continuous heating would avoid such undesirable temperature fluctuations. These results are also verified by experimental evaluations.
