Prognostic Impact of Left Atrial Strain After Mitral Valve Repair Surgery in Patients With Severe Mitral Regurgitation

  • Background and Objectives: The prognostic value of left atrial (LA) function in terms of long-term clinical outcomes after mitral regurgitation (MR) surgery remains unclear. Therefore, we investigated the impact of preoperative LA global longitudinal strain (LAGLS) on the long-term postoperative clinical outcomes in chronic severe MR patients who underwent mitral valve (MV) repair surgery. Methods: From January 2012 to December 2017, we analyzed 338 patients (mean age, 51.9±12.5 years; 218 males [64.5%]) treated with MV repair surgery for severe MR. The primary outcome was cardiovascular events, defined as the composite of all-cause death, newly developed atrial fibrillation (AF), and re-hospitalization for cardiovascular causes. Results: During a median follow-up of 45 months (interquartile range, 26-65), 30 (8.9%) cardiovascular events, 5 (1.5%) all-cause death, 8 (2.4%) newly developed AF, and 26 (7.7%) re-hospitalizations occurred. On multivariable analysis, baseline LAGLS was an independent predictor of cardiovascular events (adjusted hazard ratio [HR], 0.91; 95% confidential interval [CI], 0.85-0.97; p=0.004) and re-hospitalization (adjusted HR, 0.93; 95% CI, 0.86-1.00; p=0.037). According to the optimal cutoff value of LAGLS, patients with low LAGLS (<23.6%) had a significantly higher risk of cardiovascular events (adjusted HR, 2.70; 95% CI, 1.04-7.00; p=0.041) than those with high LAGLS (≥23.6%). In a subgroup analysis, patients with high LAGLS had better clinical outcomes regardless of whether the patient had a LA volume index <60 mL/m2. Conclusions: In patients with chronic severe MR who received successful MV repair surgery, preoperative LAGLS is an independent predictor of long-term postoperative outcomes.

전기 유압식 심실보조장치의 동물실험 연구 (An Animal Study on Electrohydraulic Type Ventricular Assist Device)

  • 신개발한 전기 유압식 심실 보조장치의 생 체 내 성능 테스트와 상기 심실보조장치를 이용한 좌심실 보 조가 자연심장 좌심실의 기능에 미치는 영향을 평가하기 위한 동물실험을 고안하였다. 총 성숙 면양 8마리가 실험에 사용되었으며 이중 7마리로부터 자료 수집이 가능하였다. 심실 보조시 간은69분부터 7일 사이로 심실보조장치는 자연심장의 박동수범위 내에서 동기식 및 비동기식 방식 모 두 만족스럽게 작동하였으며, 좌심방 내의 음압의 발생 없이 주어진 정상 좌심방압 내에서 분당 4리터 이상의 보조 혈류량을 얻을 수 있었다. 심실보조 개시 3일 이후부터 혈중 유리혈색소의 급격한 증가를 보여 상당량의 용혈이 진행되고 있음 을 시사하였다. 또한 심실보조 후 혈액 남의 부동형 고분자판막(floating type polymer valve)에 소량의 혈 전이 발견되어 혈액적 합성 및 혈전저항성 면에 있어 보다 많은 연구 및 개선의 여지가 있음을시사하였 다. 심실보조시작후의 혈역학의 변화는혈류보조량분당2.0∼2.5리터 사이에서 자연심장의 박동수및 박출량과 좌심실 분당작업량(left ventricular minute work), 심내막 생육력비 (endocardial viability ratio), 흔합 정맥혈 및 관상동맥 정맥혈 산소량은 증가하는 소견을 보였으며,반대로 좌심실압 및 좌심방압과 좌심실압 변화율(left ventricular dp/dt)은 감소하는 소견을 보였다. 이중 심박출량의 변화 외에는 모두 통계적으로 유의한 차이를 보여 효과적 인 좌심실의 탈부하가 이루어지고 있음을 입증하였다. 또한 동기식 심실보조시 심내막 생육력비 및 관상정맥혈의 산소량은 비동기식 심실보조시와 비교할 때 역시 통계적으로 유의한 차이를 보여 동기식 심실보조에 의한 반박동(counterpulsation)이 심근의 관 류량을 증가시키며 따라서 손상된 심근의 회복에 유리 함을 시사하였다.

스파터링 조건이 FeMn계 top 스핀 밸브의 exchange bias 및 자기적 특성에 미치는 영향 (Effect of sputtering conditions on the exchange bias and giant magnetoresistance in Si/Ta/NiFe/CoFe/Cu/CoFe/FeMn/Ta spin valves)

  • 6개의 타겟을 가진 직류 마그네트론 방식을 이용하여 스파터링 전력 및 압력을 변화시켜 Si/Ta(50 $\AA$)NiFe(60 $\AA$)/CoFe(20 $\AA$)/Cu(26 $\AA$)/CoFe(40 $\AA$)/FeMn(150 $\AA$)Ta(50 $\AA$) 스핀 밸브 박막을 제조하여 교환자기이방성 및 자기적 특성을 조사하였다. FeMn 층의 증착시 스파터링 전력을 증가시킴으로써 교환이방성을 증가시킬 수 있었으며, X-선 회절 실험결과 스파터링 전력 증가에 따른 교환이방성의 증가는 FeMn (111)면의 우선성장 발달에 기인하는 것으로 판단되었다. 강자성상을 사이에 두고 있는 Cu의 스파터링 압력을 1-5 mTorr 증가시 교환이방성이 급격히 감소하며, 자기저항비 및 자장민감도도 감소하였다. Si/Ta/NiFe/CoFe/Cu(t), 30 W/CoFe, 100 W/FeMn, 100 W/Ta 스핀 밸브에서 Cu 두께를 22-38 $\AA$까지 변화시켜 자기저항비를 조사한 결과 Cu의 두께가 22 $\AA$일 때 자기저항비 6.5%까지 얻을 수 있었으며, Cu 두께를 감소시켜 교환이방성을 증가시킬 수 있었다. 이와 같은 Cu 두께 감소에 따른 교환이방성의 증가는 FM-AFM 스핀-스핀 상호작용에 의하여 설명하였다.

NiO 스핀밸브 박막의 Specular Effect에 의한 자기저항비의 향상에 대한 연구 (Study on the Specular Effect in NiO spin-valve Thin Films)

  • Si/SiO$_2$/NiO(60nm)/Co(2.5nm)/Cu(1.95nm)/Co(4.5nm)/NOL(t nm) 구조와 Si/SiO$_2$/NOL(t nm)/Co(4.5nm)/Cu(1.95nm)/Co(2.5nm)/NiO(60nm)의 구조를 갖는 바닥층 스핀밸브와 꼭대기층 스핀밸브를 제작하고, NOL의 두께변화에 따른 비저항($\rho$) 값과 비저항의 변화량( $\rho$), 교환결합력(H$_{ex}$), 보자력(H$_{c}$)의 자기적 특성을 연구하였다 NOL로 NiO 03nm의 두께로 삽입한 결과, 최대 자기저항비(magnetoresistance ratio)는 바닥층 스핀밸브에서 12.51%의 얻었으며, 자기저항비의 향상률은 꼭대기층 스핀밸브에서 더 높은 결과를 얻었다. 또한, 두 형태 모두 비저항의 변화량($\rho$)은 거의 일정하였고, 비저항($\rho$)값은 감소하였다 이러한 결과는 NOL의 삽입하였을 때 NOL/강자성층(free ferromagnetic layer) 계면에서 유도 전자의 specular 산란 효과를 가져왔고, 이로 인하여 전자의 평균 자유이동경로(mean free path; MFP)가 확장되어 전류의 전도도를 증가시켰다 이러한 specular효과에 의해 비저항의 변화량은 일정하게 유지되는 동안에 비저항 값은 감소하게 되어 결과적으로 자기저항비의 향상을 가져왔다.

특성곡선법을 이용한 디젤엔진 가스유동 1차원 수치해석의 타당성 평가 (Validation of diesel engine gas flow one-dimensional numerical analysis using the method of characteristics)

  • In order to design a diesel engine system and predict its performance, it is necessary to analyze the gas flow of the intake and exhaust system. A gas flow analysis in three-dimensional (3D) format needs a high-resolution workstation and enormous time for analysis. Therefore, the method of characteristics (MOC) was used for a gas flow analysis with a fast calculation time and a low-resolution workstation. An experiment was conducted on a single cylinder diesel engine to measure pressure in cylinder, intake pipe and exhaust pipe. The one-dimensional (1D) gas flow was analyzed under the same conditions as the experiment. The engine speed, valve timing and compression ratio were the same conditions and the intake pressure was inputted as the experimental results. Bent pipe such as an exhaust port that cannot be realized in 1D was omitted. As results of validation, the cylinder pressure showed accuracy, but the exhaust pipe pressure exhibited inaccuracy. This is considered as an error caused by the failure to implement a bent pipe such as an exhaust port. When analyzed in 3D, calculation time required 61 hours more based on a model of this study. In the future, we intend to implement a bent pipe that cannot be realized in 1D using 3D and prepare a method to supplement reliability by using 1D-3D coupling.

디지털 스크롤 압축기를 적용한 시스템 에어컨의 냉난방특성에 대한 실험적 연구 (Experimental Study on the Cooling and Heating Characteristics of System A/C Applying the Digital Scroll Compressor)

  • In order to investigate the cooling and heating characteristics of a variable-capacity system A/C applying a digital scroll compressor, the cooling and heating capacities and COP are measured by the psychrometric calorimeter. The capacity of the system is controlled by the digital scroll compressor, which is operated by controling PWM valve and the loading vs. unloading time. In the case of unloading compared that of loading, the consumption power of the compressor is about 11% and the capacity variation of the system A/C is within about 1%. When the system A/C is operated under the cooling and heating standard conditions, COP is nearly uniform but cooling capacity and heating capacity increase at minimum, rated and maximum modes. The system A/C applying the digital scroll compressor is effective for the range with high load or the width of large load variation. When the auxiliary heater is on, at the cold region, the system A/C produces the excellent heating capacity.

지열 발전을 위한 HFC-245fa 유기 랭킨 사이클의 성능해석 (Analysis of HFC-245fa organic Rankine cycle for geothermal power generation)

  • In this study, an ORC (Organic Rankine Cycle) is investigated for a low-temperature geothermal power generation by a simulation method. A steady-state simulation model is developed to analyze cycle's performance. The model contains a turbine, a pump, an expansion valve and heat exchangers. The turbine and pump are modelled by an isentropic efficiency. Simulations were carried out for the given heat source and sink inlet temperatures, and given flow rate that is based on the typical power plant thermal-capacitance-rate ratio. HFC-245fa is considered as a working fluid of the cycle. Simulation results, at the given secondary working fluids conditions, show that even though the power can be presented by both the evaporating temperature and the turbine inlet superheat, it depends on the evaporating temperature primarily.

심실중격결손증의 임상적 고찰 (Clinical Analysis of Ventricular Septal Defect)

  • A clinical analysis of 138 cases of ventricular septal defect operated on from 1983 to June 1988 at the department of Thoracic and Cardiovascular Surgery, School of Medicine, Chung-nam National University was done. The following results were obtained. 1. The VSD were 27.6 % and 41.0 % respectively of 500 open heart surgery cases and 334 congenital heart disease. 2. The range of age when the VSD was repaired was 3 months to 45 years, the mean age was 10.4 years and the 53.6 % of patients were under 7 years. The sex ratio was 6:4 in males favor. 3. The two common symptoms were frequent upper respiratory infection and dyspnea whose frequency was 62.5 % and 58.7% respectively. 4. 52% of the patients was below 25 percentile of the standard body weight. 5. The most common chest PA findings were cardiomegaly and increased pulmonary vascularity. 6. On anatomical classification, perimembranous type was most common [65.5%], subpulmonary arterial type was 27.6 %, perimembranous plus subpulmonary arterial type was 3.4 % multiple VSD was 0.8 % and LV-RA defect was 2.6 %. 7. Associated cardiac anomalies were founded in 39 cases [35.5 %] and PDA and aortic valve prolapse were most common associated anomalies and extracardiac anomalies were founded in 6.7 % of patients. 8. Mean extracorporeal circulation time was 68 min. and mean aortic cross-clamping time was 42 min. 9. The postoperative complications developed in 26 % of patients and the most common one was minor wound infection. 10. Right bundle branch block developed in 54.8% of the patients who had right ventriculotomy, 40.0% of the patients who had right atriotomy and 10.5% of the patients who had pulmonary arteriotomy. 11. The overall operative mortality was 5.1 % and the operative mortality of the patients who body weigh under 10 Kg was 23.8%.

터보 팽창기를 활용한 NGL 회수공정 최적화에 대한 연구 (An Optimization Study on the NGL Recovery Process Using Turbo-expander)

  • 본 연구에서는 전처리 공정을 거친 천연가스로부터 에탄 이상의 성분을 회수하기 위한 탈메탄탑에 대한 전산모사와 공정 최적화를 수행하였다. 전처리된 천연가스는 탈메탄탑 상부의 차가운 기상류와의 열교환 및 프로판 냉동 사이클이 포함된 예냉공정을 거친 후에 기상과 액상이 분리된다. 기상은 터보 팽창기를 거치면서 생산되는 동력을 residue gas의 압력을 높이기 위한 압축기에 전달한 후에 부분적으로 응축되어 탈메탄탑 상부로 주입된다. 액상류는 줄-톰슨 팽창 밸브를 거친 후 더욱 냉각되어 탈메탄탑의 중간부로 주입된다. 원료 대비 에탄의 회수율은 75% 이상으로 정하였으며, 탈메탄탑의 탑저에서 에탄에 대한 메탄의 몰비는 0.015로 정하였다. 한편 프로판 냉동 사이클의 heat duty를 최소화시키기 위해서 원료를 분리하여 side reboiler와 열교환시킴으로써 냉열의 일부 회수할 수 있었다.

타이어 시뮬레이터를 이용한 능동형 타이어 공기압 제어 시스템 개발 (Development of an Active Tire Pressure Control System Using a Tire Simulator)

  • This study was performed to develop an active tire pressure control system that can adjust tire pressure to the optimum level according to traveling and working condition of agricultural tractor. For the development of active tire pressure control system, pneumatic supplier, solenoid valve block including pneumatic supply line, infinite rotation type pneumatic supplier with rotary joint unit, tire pressure transceiver module and control algorithm were developed. Also, tire simulator was developed. Using this tire simulator, the feasibility of each part constructing actual system was tested by checking the performance. The average communication success ratio was 98.3% between tire pressure transmitter and receiver module according to the various tire rotational speed and data receipt position of receiver module. The communication performance of the developed transmitter and receiver module was very stable in any condition. The tire pressure control system was accomplished by using the proportional control algorithm in this study. Also tire pressure control performance of developed control system was analyzed by using the tire simulator. As a result of control performance analysis to the developed system, the developed control system took 307 seconds to inflate agricultural tractor's tire from 50 kPa to 180 kPa. In opposite case, it took 210 seconds. Also it was able to control the tire pressure accurately under ${\pm}0.9%$ (FS) in any condition.
