The Effects of Treatment of Korean Medicine for Superior Labrum Anterior to Posterior Lesions: A Retrospective Chart Review

  • Background: This study aimed to investigate the clinical effectiveness of treatment of Korean medicine on superior labrum anterior to posterior (SLAP) lesions. Methods: A total of 55 inpatients diagnosed with SLAP lesions by magnetic resonance imaging, were investigated from May 1st, 2014 to May 31st, 2019 at Haeundae Jaseng Hospital of Korean Medicine. The patients were sorted by gender, age, causing factor, illness duration, period of hospitalization, SLAP lesion type, complications, and treatments. Treatments included acupuncture, pharmacopuncture, Chuna therapy, herbal treatment, and physiotherapy. After treatment, the Numeric Rating Scale, Shoulder Pain and Disability Index, and European Quality of Life 5-Dimension questionnaire were used to evaluate treatment effect. Results: There were more males than females in this study (1:0.83). Patients were more likely to be in their 50s (38.18%), have an unknown etiology (70.91%), and be in the subacute disease stage (41.82%). According to the SLAP lesion type, most of the inpatients had Type 2 lesions (69.09%). For inpatients diagnosed with SLAP lesions, the mean shoulder numeric rating scale score decreased from 5.55 ± 0.90, to 4.07 ± 1.18 (p < 0.001), the mean Shoulder Pain and Disability Index score decreased from 50.35 ± 18.36, to 39.90 ± 19.34 (p < 0.001), and the mean European quality of life 5-dimension index increased from 0.70 ± 0.16, to 0.75 ± 0.13 (p < 0.01) after treatment. Conclusion: Treatment of Korean medicine effectively decreased pain and increased the quality of life of the patients with SLAP lesions in this study.

초음파와 방사선 투시장치를 이용한 경추 신경근 차단술의 임상결과 비교 (Comparison of the Clinical Outcomes of an Ultrasound-Guided and C-Arm Guided Cervical Nerve Root Block)

  • 목적: 초음파 유도하 경추 신경근 차단술을 시행한 환자들의 임상결과를 방사선 유도하 경추 신경근 차단술을 시행한 환자들과 비교하고 합병증 및 수술 비율 등에 대해 비교하고자 하였다. 대상 및 방법: 경추 추간판 탈출증으로 진단된 환자들에 대해 2015년 5월 1일부터 2018년 2월 8일까지 초음파 유도하 경추 신경근 차단술을 시행한 97예와 2010년 6월 1일부터 2017년 8월17일까지 방사선 유도하에 시행한 94예 환자들의 경추통과 방사통을 시술 전과, 3주, 6주 후 verbal numeric rating scale (VNRS)을 이용하여 조사하였고 시술과 연관된 합병증 유무도 의무기록을 통해 조사하였다. 결과: 초음파 유도하 경추 신경근 차단술을 시행한 97예 중 66예가 연구에 포함되었고 이들의 평균 연령은 57세였다. 남성이 41명 여성이 25명이었고 목표 신경근은 경추 5번이 8예, 6번이 37예, 7번이 21예였다. 방사선 유도하 경추 신경근 차단술을 시행한 94예 중 77예가 연구에 포함되었고 이들의 평균 연령은 55세였다. 남성이 40명, 여성이 37명이었고 목표 신경근은 5번이 6예, 6번이 42예, 7번이 26예, 8번이 3예였다. 시술 후 경추통의 numeric rating pain scale은 초음파 유도하의 경우 중간값 5.4점에서 3주, 6주에 2.7점, 1.4점으로 감소하였고(p=0.0023, p<0.001), 방사선 유도하의 경우 시술 전 중간값 6.2점에서 3주에 3.1점, 6주에 1.5점이었다(p<0.001, p<0.001). 방사통의 경우 초음파 유도하 차단술의 경우 시술 전 중간값 6.3점에서 3주에 2.8점, 6주에 1.5점으로 호전되었고(p<0.001,p<0.001), 방사선 유도하의 경우 시술 전 7.4점, 3주, 6주에 3.3점, 1.9점으로 감소하였다(p<0.001, p<0.001). 합병증의 경우 방사선 유도하 차단술의 경우 호너 증후군이 1예, 간대성 근경련이 1예였으며 초음파 유도하차단술의 경우 일시적인 수부 부종을 호소하는 1예가 있었다. 결론: 초음파 유도하 경추 신경근 차단술을 시행한 환자들의 임상결과는 방사선 유도하 경추 신경근 차단술을 시행한 환자들과 통계적 차이를 보이지 않았다.

  • 인터넷의 발전으로 온라인 쇼핑이 활성화 되면서 소비자들의 구매 형태가 기존의 대면 구매방식에서 온라인 구매방식으로 변하고 있다. 이에 수많은 판매자가 쇼핑몰로 유입되었고, 판매자들 간의 경쟁이 매우 치열한 실정이다. 따라서 쇼핑몰 내 판매자들은 소비자의 구매 패턴 및 제품 판매동향을 분석하여 합리적인 마케팅 전략을 세울 필요가 있다. 본 논문에서는 오픈 쇼핑몰에서 판매업자가 동일 제품을 판매하는 경쟁사의 제품 가격, 평점, 판매수량을 시간대별로 분석하여 소비자의 구매 패턴을 파악했다. 또한 수집된 정보들을 차트로 가시화하여 자사와 경쟁사의 판매동향을 쉽게 비교할 수 있도록 하였다. 위 시스템을 사용하면 분석된 구매패턴을 통해 판매수량을 예측할 수 있고 판매동향을 파악하여 상품의 합리적인 가격 선정이 가능하다.

A Retrospective Chart Review of 122 Inpatients with Knee Osteoarthritis Treated with Korean Medicine: An Analysis of the Effects of Treatment

  • Background: Korean medicine treatment was assessed in patients with knee osteoarthritis (OA) according to subgroups of: sex, age, cause of knee OA, body mass index, hospitalization period, history, OA compartment, phenotype, and comorbidity. Methods: A retrospective review was performed of 122 inpatients who were admitted to the Hospital of Korean Medicine for Korean medicine treatment of knee pain, and were diagnosed with knee OA based on magnetic resonance imaging findings. Analysis of patient subgroups (sex, age, cause of knee OA, body mass index, hospitalization period, history, OA compartment, phenotype, and comorbidity) was carried out and treatments including acupuncture, cupping, pharmacopuncture, herbal medicine, chuna therapy, medicinal steaming therapy, manual therapy, and extracorporeal shock wave therapy were listed. The numeric rating scale (NRS), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and 5-level EuroQol- 5 Dimension (EQ-5D-5L) scores were measured before and after treatment to assess the effects of treatment on pain and quality of life. Results: Seventeen males and 105 females were included in this study. Most patients were in their 60s. In the total study population, NRS, WOMAC, and EQ-5D-5L scores were improved statistically significant when comparing before and after treatment. The NRS and WOMAC scores improved statistically significant in the medial, patellofemoral, medial + patellofemoral, medial + lateral + patellofemoral compartment. Conclusion: Korean medicine treatment significantly reduced pain, stiffness, and physical dysfunction, and improved the quality of life of patients with knee OA, suggesting that it may be an effective alternative to the current conservative treatments.

  • Purpose: This study was conducted to collect and analyze real world data to evaluate the effectiveness and safety of comprehensive traditional Korean Medicine treatment on quality of life, anorexia and cachexia of cancer patients. Methods: We analyzed medical records of 62 cancer patients admitted to O-I Dang Korean Medicine Hospital from February 2018 to February 2019. The primary outcome was a change score in the Anorexia/Cachexia Subscale of Functional Assessment of Anorexia/Cachexia Therapy (FAACT). The secondary outcomes were Functional Assessment of Cancer Therapy-General (FACT-G), Trial Outcome Index(TOI) of FAACT, 11 point Pain Intensity Numeric Rating Score (11 PI-NRS) and Patient Global Impression of Change (PGIC) and adverse event. Results: Cachexia and quality of life in cancer patients assessed by FAACT, increased by $5.59{\pm}14.83$ (p=0.004) after treatment. PI-NRS was reduced by $2.10{\pm}1.81$ (p<0.001) and TOI and FACT-G total scores were increased by $5.17{\pm}11.70$ (p=0.001) and $3.59{\pm}10.94$ (p=0.012), respectively. These results were also clinically meaningful assessed via minimal clinically important difference (MCID). There was no severe adverse event. Conclusion: These findings suggest that comprehensive traditional Korean Medicine treatment might be effective and safe strategy for improving quality of life, anorexia, cachexia and pain of cancer patients. Further advanced studies with controlled group and more participants with rigorous design are needed to ensure these findings.

산업재해로 발생한 외상성 허리손상에 대한 새로운 재활치료프로그램의 효과 비교 (Comparison of Rehabilitation Programs in Traumatic Low Back Injuries with Industrial Accident)

  • Objectives: The purpose of this study was to investigate the effect of intensive rehabilitation programs on pain, range of motion (ROM), lumbar muscle strength, core muscle endurance, disability, and depression in patients with traumatic low back injuries and to compare the efficacy of this therapy with that of conventional rehabilitation therapy. Methods: The study was performed with a retrospective medical chart review of patients with traumatic low back injury referred to the rehabilitation center at the Daegu Hospital of the Korean Workers Compensation and Welfare Service. Forty-four patients were allocated to either the conventional rehabilitation group (CRG; n = 22) or the intensive rehabilitation group (IRG; n = 22). The CRG group patients, who received 30-min therapist-supervised physical therapy and modality therapy five times per week for four weeks, were compared with the IRG group patients, who received 60-min therapist-supervised physical therapy, 30-min therapist-patient 1:1 matching rehabilitation therapy, and modality therapy five times per week for four weeks. Outcome measures were a numerical rating scale, ROM, lumbar muscle strength, lumbar core muscle endurance, thickness of lumbar deep focal core muscle (transverse abdominis and lumbar multifidus), Oswestry disability index (ODI), and depression (Korean version patient health questionnaire-9). Results: There were statistically significant improvements after treatment in all outcome measures in both groups (p < 0.05). In the intergroup comparison, NRS scores on the activity and thickness of lumbar deep focal core muscles increased significantly more in the IRG than in the CRG (p < 0.05). There were no statistically significant intergroup differences in NRS scores on resting, ROM except left lateral bending, lumbar muscle strength, core muscle endurance, ODI, and depression. Conclusions: We could confirm the superior effectiveness of an intensive rehabilitation program compared to conventional rehabilitation therapy in patients with traumatic low back injuries.

일개 의료기관에 입원한 교통사고 후 환자의 경항통 및 특성에 대한 관찰 : 후향적 챠트 리뷰 (Observation of Factors on Post-traffic accident Neck Pain in a Medical Center : Retrospective Chart Review)

  • Many prior studies on neck pain after a traffic accident (TA), but there is a lack of research on risk factors for post-TA neck pain in Korea. The purpose of this study was to examine the relationship between post-traffic neck pain and the demographic characteristics of TA patients and to find any factors affecting the neck pain after TA. In this study, 120 TA patients in a Korean medicine hosipital were analysized. The Korean version of the Neck disability Index (NDI) and Numeral Rating Scale (NRS) were used. Data were summarized by frequency(%) and mean(standard deviation). Pearson correlation test, Independent sample t-test, chi-squre test, one-way ANOVA and two-way ANOVA were performed. The IBM SPSS Advanced Statistics for window, version 20.0 was used for statistical processing. All p-values less than 0.05 were considered statistically significant. NDI and NRS were highly correlated. NRS and NDI showed higher scores for women, those in 30s, BMI≥25, and side collisions, but there were no statistically significant differences. For women, the direction of collision was observed to affect NDI. In this study, it was confirmed that the NDI and NRS had a high correlation. However, it was confirmed that sex, degree of obesity, direction of traffic accident collision are not factors that significantly affect the intensity of neck pain and the functional disorder by neck. It is necessary to conduct an additional study by larger scale.

Analysis of the Effects on the Level of Pain and Functional Improvement After Integrated Korean Medicine in Patients with Shoulder Impingement Syndrome: A Retrospective Chart Review

  • Background: This study investigated the clinical effectiveness of Korean medicine (KM) treatment for shoulder impingement syndrome (SIS). Methods: There were 61 patients who were diagnosed with SIS in the Jaseng hospital network of KM (7 hospitals located in Korea: Gangnam, Daejeon, Bucheon, Haeundae, Bundang, Ulsan, and Gwangju) between January 1st, 2015 and December 31st, 2020 who were retrospectively reviewed. The patients were grouped according to complications, intake of analgesics, duration of illness preadmission, and treatment. Treatments consisted of herbal medicine, acupuncture, cupping, Chuna, pharmacopuncture, bee venom pharmacopuncture, medicinal steaming, Daoyin exercises, and physical therapy. By comparing the Numeric Rating Scale (NRS), Shoulder Pain and Disability Index, and European Quality of Life 5-Dimensions questionnaire scores, the effectiveness of integrated KM treatment was evaluated. Results: There were 14 males and 47 females. For inpatients diagnosed with SIS, the mean NRS score decreased from 5.78 ± 1.33 to 3.40 ± 1.43 (p < 0.001). The mean Shoulder Pain and Disability Index score decreased from 53.87 ± 14.76 to 38.56 ± 18.87 (p < 0.001), and the mean European Quality of Life 5-Dimensions questionnaire increased from 0.67 ± 0.13 to 0.76 ± 0.09 (p < 0.001) after KM treatment. Medicinal steaming (0.398; p < 0.001), acupuncture (0.290), cupping (0.288), bee venom pharmacopuncture (0.282), and Daoyin exercises (0.262; p < 0.05) had a positive correlation with improved changes in the NRS score. Conclusion: Conclusion: Treatment with integrated KM treatment improved the pain, range of motion, shoulder function, and quality of life of patients with SIS.

The Effects of Korean Medicine Treatment for Meniscus Tears: A Retrospective Chart Review

  • Changes in symptoms and dysfunction related to meniscus tears following the use of Korean medicine for ≥ 4 days were studied. The medical charts of 53 cases of diagnosed meniscus tears (magnetic resonance imaging) with an admission Numeric Rating Scale (NRS) score ≥ 4, between 2017 and 2022 were retrospectively reviewed. Treatments included acupuncture, pharmacopuncture, herbal treatment, Chuna therapy, and physiotherapy. The NRS, Western Ontario and McMaster Universities Osteoarthritis Index, and European Quality of Life 5 Dimensions were performed at admission and discharge. There were 42 females and 11 males in this study. Patients were more likely to be in their 60s (38.18%), have an unknown etiology (81.13%), and have complex tears (50.94%). After receiving a combination of alternative Korean medicine during hospitalization, the mean NRS score improved from 6.82 ± 1.19 to 3.66 ± 1.83 (p < 0.001), the Western Ontario and McMaster Universities Osteoarthritis Index score improved from 46.47 ± 20.99 to 37.98 ± 19.23 (p < 0.001), and the mean European Quality of Life Five Dimensions score improved from 0.61 ± 0.18 to 0.68 ± 0.14 (p < 0.001) after treatment. These results suggest that Korean medicine treatment of meniscus tears alleviated pain and improved physical function.

Effects of Spinal Mobilization with Leg Movement and Neural Mobilization on Pain, Mobility, and Psychosocial Functioning of Patients with Lumbar Disc Herniation: A Randomized Controlled Study

  • Objective: The purpose of this study was to investigate the effect of spinal mobilization with leg movement (SMWLM) and neural mobilization (NM) in patients with lumbar disc herniation (LDH) accompanied by radiating pain. Design: Three-group pre-test-post-test control group design. Methods: We enrolled 48 participants, whom we randomly assigned to three groups. The SMWLM group (n=16) underwent 20 min of conventional physical therapy (CT) and 20 min of SMWLM. The NM group (n=16) underwent 20 min of CT and 20 min of NM. The control group (n=16) underwent 20 min of CT. These interventions in all the groups were performed three times a week for 4 weeks. Numeric pain rating score (NPRS), body grid chart score (BGCS), passive straight leg raise (PSLR), active lumbar flexion range of motion (ALFROM), korean version oswestry disability index (KODI), and korean version fear avoidance beliefs questionnaire (KFABQ) were measured pre- and post-intervention. Results: In all three groups, the NPRS, PSLR, KODI, and KFABQ scores were significantly different pre- and post-intervention (p<0.05). Significant differences were observed in BGCS and ALFROM in the SMWLM and NM groups pre- and post-intervention (p<0.05). The SMWLM group showed more improvement in the NPRS of leg pain, ALFROM, and KFABQ score than that exhibited by the NM and control groups (p<0.05). Conclusions: Both SMWLM and NM were effective for improving back and leg pain, centralization of symptoms, mechanical sensitivity, lumbar mobility, lumbar functional disability, and psychosocial functioning in patients with LDH with radiating pain.
