  • The temperature distribution in double-concentric diffusion flames have been investigated experimentally by rapid insertion technique. Using a fine thermocouple and rapid insertion mechanism, the temperature has been measured before soot particles attach the thermocouple junction which can affect the temperature signal by changing the radiation heat loss. For double-concentric diffusion flames, the temperature at the axis is higher than that of normal coflow diffusion flames because of the inverse diffusion flame at the center of the flame. However, it is almost same at the periphery on which the inverse flame does not have an effect.

  • The effect of temperature distributions on soot volume fraction in double-concentric diffusion flames have been investigated experimentally. Using fine thermocouple wires and a rapid insertion mechanism, we have measured temperature without the effect of soot particles attached to the thermocouple junction, which can lower the temperature signal about 100 K by increasing the heat loss from the junction by radiation. The temperature at the flame axis is higher in the double-concentric diffusion flames than in normal co-flow diffusion flames because of the inverse diffusion flame. However, it is almost the same as that at the periphery of normal flames, on which the inverse flame does not have an effect. Thus, the lower soot concentration found in the double-concentric diffusion flame can be explained by the effect of nitrogen diffusion from the central air jet.

기관 협착에서 Long T-tube의 삽입 방법 (A technique for insertion of a long T-tube in tracheal stenosis)

  • A technique for insertion of a long silicone T-tube in patient with critical stenosis and high-risk resection and primary anastomosis of long segment of the distal trachea is presented. It was not easy to insert a long T-tube by existing methods because of flexibility of a T-tube and tightness of stenosis. So we used a silastic endotracheal tube and guiding wire as stylet of a T-tube. During insertion, ventilation was normally maintained through the lumen of endotracheal tube. This provided rapid relief from airway obstruction and asphyxation and is a easy, safe and effective method to restore patency of the major airways.

  • Co-flow laminar diffusion flames' temperature has been studied experimentally for ethylene$(C_2H_4)$ using a co-flow burner in order to investigate the characteristics of diffusion flame's temperature distribution. The temperature distributions in the flame were measured by rapid insertion of a R-type thermocouple. The measurement area was divided into three zones. 1st area was expect to created PAH zone, Il nd area was expect to form soot zone, which is known to generate most soot volume fraction, and III rd area was expect to from soot oxidization zone. Also The temperature along the flame y-axis as a fuel quantity was measured. As a results, we have measured temperature neglecting the effect of soot particles attached to the thermocouple junction, which is close to the nozzle and upstream zone has a unstable flow in co-flow diffusion flame and acquires that the flame y-axis temperature has a uniform temperature in the generated soot volume fraction zone(II nd).

Pull-in 봉합술을 이용한 수지건 원위 부착부 파열의 치료 (Pull-in Suture Technique for the Disinsertion of the Phalangeal Tendon Distal Insertion)

  • Purpose: The disinsertion of the phalangeal tendon distal insertion has difficulties in ordinary tenorrhaphy operation for the anatomical features, and still has controversy between non-surgical and surgical management. The purpose of this study is to select treatment for the injury of the phalangeal tendon distal insertion, as we've had a good results from operation treatment with Pull-in suture technique. Methods: We reviewed the hospital records of 12 patients treated with Pull-in suture technique with disinsertion of the phalangeal extensor or flexor tendon distal insertion from June 2006 to June 2007. Eight patients were involved with the tendon disinsertion without bone fracture, and 4 patients were involved with the fracture of the phalangeal tendon distal insertion site. After removal of the K-wire in week 6, active physical exercises were commenced immediately. The mean follow-up period was 12.4 months. Results: All the patients who had tendon disinsertion with bone fracture had IIB, by Wehbe and Schneider's classification 2, and we evaluated the results comparing the same finger of the other hand according to Crawford's evaluation criteria 5. The nine excellent and three good results were obtained and there were no limitation of motor for the patient who had operation for the rupture of flexor tendon as well. There were no particular complications during the follow-up period. Conclusion: The most important thing for the disinsertion of the phalangeal tendon distal insertion is to maintain an accurate and durable reduction state keeping the tension of tendon. At this point, after removal of the K-wire, the Pull-in suture technique allows accurate realignment of the tendon-bone unit without any specific instrumentation under the more stable state. The Pull-in suture technique seems to be a strong alternative for the treatment of disinsertion of the phalangeal tendon distal insertion, with successful treatment outcome(rapid functional recovery and high patient satisfaction).

  • The temperature and soot particle measurement technique in a laminar diffusion flame has been studied to investigate the characteristics of soot particle with temperature using a co-flow burner. The temperature distribution in the flame were measured by rapid insertion of a R-type thermocouple and the soot particles by LEM/LIS techniques. In these measurement, soot volume fraction, number density and soot diameters were analyzed experimentally. As a results, the spacial distributions of particle volume fraction, soot diameter, and number density are mapped throughout the flame using the Rayleigh theory for the scattering of light by particles. A laser extinction method was used to measure the soot volume fraction and laser induced scattering method was used to measure the soot particle diameter and number density. Also, we measured temperature without the effect of soot particles attached to the thermocouple junction, which is close to the nozzle. In this result, we found that upstream zone has a unstable flowing in co-flow diffusion flame and the y-axis temperature of flame has a uniform temperature distribution in the most soot volume fraction zone.

동축 이중 에틸렌 확산화염의 매연 농도분포 및 온도 측정 (Soot Concentration and Temperature Measurements in Laminar Ethylene Jet Double-concentric Diffusion Flames)

  • Experiments were performed with double-concentric diffusion flame(DDF) in order to investigate the characteristics of soot formation and temperature distributions. The flame size and shape of the DDF are similar to those of the well-known normal co-flow diffusion flame(WF), except the formation of a tiny inverse flame near the central tube exit. A laser light extinction technique was used to measure the soot volume fractions. The temperature distributions in the flame were measured by rapid insertion of a R-type thermocouple. Soot concentrations along the flame axis of the DDF were higher than those of the NDF. However, the maximum soot volume fraction of the DDF along the periphery of the flame was lower than that of the NDF. It is mainly due to the effect of nitrogen-dilution from the inner air. Measured temperature distribution explains these trends of soot concentration. The temperature along the flame axis was also higher in DDF than that of the NDF. However, the flame temperatures at the flame front of the two flames were almost same regardless of the inner flame. This phenomenon means that the inverse flame inside the DDF did not affect on the flame structure including the temperature and soot concentration, except the region around the flame axis.

The role of rapid tissue expansion in separating xipho-omphalopagus conjoined twins in Vietnam

  • Conjoined twins are rare, and each set of conjoined twins has a unique conjoined anatomy. It is necessary to perform separation to increase the chance of patient survival. Tissue expansion is an advanced technique for providing sufficient soft tissue and skin for wound closure. We report the successful application of rapid tissue expansion in 10-month-old xipho-omphalopagus conjoined twins in Vietnam. A tissue expander was placed on the anterior body between the sternum and umbilicus with a baseline of 70 mL sterile saline (0.9% NaCl). The first injection into the tissue expander began on the 6th day after expander insertion, and injections continued every 2 days with approximately 30-70 mL per injection according to the expansion of the skin. The expander reached 335 mL after six injections and within 10 days. In order to prepare for surgical separation, expansion was completed on the 15th day after insertion. The expanded skin area was estimated to be 180 cm2, which was sufficient to cover both patients' skin deficiencies. The twins presented for surgical separation 6 days following the completion of tissue expansion. Both babies were discharged in good health 1 month after separation.

  • Flame temperatures were measured and compared using a rapid insertion technique and a two-color pyrometry with Abel inversion process in co-flow ethylene diffusion flames. The measured line-of-sight temperature showed very limited usefulness in understanding the detailed soot formation/oxidation process in a co-flow diffusion flame. The flame temperatures could be measured with reasonable accuracy for the soot laden regions in ethylene diffusion flames using two-color pyrometry with an Abel inversion technique. Two-color-pyrometry with Abel inversion was demonstrated as a useful temperature measurement technique for co-flow diffusion flames, expecially under pressure conditions, where a thermocouple is not applicable. The soot volume fraction could be also obtained using tow-color pyrometry with Abel inversion, which provides important information for understanding the soot formation/oxidation mechanism in diffusion flames.
