Arthroscopic Excision of Delayed Diagnosed Intra-articular Osteoid Osteoma of the Elbow: A Case Report

  • An intra-articular osteoid osteoma is a very rare cause of elbow pain, and its diagnosis and treatment remain challenging. Delayed diagnosis may lead to arthritic change of the joint. In this study, the authors present the occurrence of intra-articular osteoid osteoma in the right elbow of a 15-year-old male patient who presented with prolonged pain and limited motion owing to delayed diagnosis. After confirming the nidus of osteoid osteoma from radiographic evaluation, the lesion was completely removed arthroscopically. The patient presented a complete relief of symptoms and full range of motion. This is the first domestic report of successful arthroscopic treatment of an intra-articular osteoid osteoma of the elbow.

THE EFFECT OF INTRA-ARTICULAR INJECTION OF HYALURONIC ACID AFTER ARTHROCENTESIS IN TREATMENT OF INTERNAL DERANGEMENTS OF THE TMJ (악관절 내장증 치료를 위한 악관절 세정술 후 관절강내 Hyaluronic Acid 주입 효과)

  • This study was designed to investigate the effect of intra-articular injection of hyaluronic acid for the treatment of internal derangements of the temporomandibular joint(TMJ). Sixteen patients with internal derangements of TMJ in 1 male and 15 females aged 68 to 18 years comprised the study material. The patients' complaint was limited mouth opening and pain during function. Patients were divided into 3 groups(articular disc displacement with reduction, articular disc displacement without reduction, osteoarthritis group). The preauricular area was disinfected anesthetized locally with 2% lidocaine hydrochloride. Arthrocentesis was performed. Hyaluronic acid(1.5 ㏄) was then injected into the superior compartment of the TMJ. Active range of motion exercises were instituted at approximately 24 hours postoperatively. Antibiotics and NSAID, three times daily by mouth, was prescribed for 3 days. Clinical evaluation of the patients was done before the procedure, immediately after the procedure, final follow-up visit postoperatively. Pain during function was assessed using visual analog scales(VAS). Maximal mouth opening was recorded as a distance between upper incisal edge and lower incisal edge. Intra-articular injection of hyaluronic acid caused significant reduction of pain during function, significant increase of maximal opening range. These findings suggest that intra-articular injection of hyaluronic acid is useful for decreasing patient reports of pain while increasing functional mobility of the mandible in internal derangements of the TMJ.

A Case Report of Intra-articular Bee Venom Pharmacopuncture for Adhesive Capsulitis (견관절 유착성 관절낭염에 대한 관절강 내 봉약침 적용 1례)

  • Objectives : This study was performed to report a case of adhesive capsulitis treated by intra-articular bee venom pharmacopuncture. Methods : Patient was treated by intra-articular bee venom pharmacopuncture four times during two weeks, and after that the patient had intermission period for 2 weeks. The same treatment was administered four times during two weeks after the intermission. The effect of treatment was evaluated by visual analog scale(VAS), range of motion(ROM). Results : After treatment, VAS score declined from 7 to 4, ROM increased. Conclusions : Intra-articular bee venom pharmacopuncture was effective for adhesive capsulitis.

The Effect of Intra-articular Bee Venom Injection on Meniscal Injury : Four Cases Report (반월상 연골 손상에 대한 관절강내 봉약침 치료 증례보고 4례)

  • According to previous reports, intra-articular bee venom injection is very effective for synovitis, intra-articular lesions. In this article, we report 4 cases of well-treated meniscal tear with intra-articular bee venom injection. We used intra-articular bee venom injection on meniscal tear diagnosed by MR imaging and prescribed herbal medication, physiotherapy if necessary. Outcomes were measured by Visual Analogue Scale(VAS) and Lysholm Knee Scoring Scale. Patients who are treated by intra-articular bee venom injection had a significant effect on the pain reduction, improved range of motion and knee function. Further well-designed, controlled studies and more cases are needed to define the effect of intra-articular bee venom injection on knee.

Changes in Range of Motion after Intra-Articular Corticosteroid Injection in Frozen Shoulder: A Retrospective 3-Month Follow-Up Study (동결견 환자에서 관절강내 스테로이드 주사 후 관절가동범위의 변화)

  • Objective: To examine (1) the degree of reduction of passive range of motion (PROM) on the affected side compared to that on the unaffected side and (2) the degree of increase in PROM following intra-articular corticosteroid injection (IACI) in patients with frozen shoulder. Method: The medical records of 120 patients with frozen shoulder were retrospectively reviewed. PROM of the unaffected and affected shoulder (flexion, extension, abduction, internal rotation, external rotation) was compared, and changes in PROM of the affected shoulder after a single IACI (triamcinolone 20 mg) were evaluated after 12 weeks. Results: At the time of diagnosis, PROM of the affected shoulder was most limited in external rotation, followed by internal rotation, abduction, extension, and flexion, compared to that of the unaffected shoulder. Compared to before IACI, PROM of external rotation demonstrated the greatest increase compared to all the other movements after IACI. Conclusion: Limitation in PROM of the frozen shoulder at the time of diagnosis was greatest for external rotation. Moreover, external rotation experienced the greatest improvement after IACI. Our findings should help to further clarify the clinical characteristics of frozen shoulder, aid in its diagnosis, and allow the prediction of the effects of IACI.

Results of Operative Treatment for Large Osteochondral Lesion of Medial Talar Dome (내측 거골 체에 발생한 비교적 큰 골연골 병변에 대한 수술적 치료 결과)

  • Purpose: The purpose of this study is to assess the results of the autologous osteochondral grafting harvested from medial side of talus for relatively large osteochondral lesion of the medial talar dome. Materials and Methods: From October 2004 to September 2005, 12 patients with osteochondral lesion measured more than 10 mm in axial MRI who were followed up more than 1 year after operation were analyzed. We evaluated postoperative symptoms by Mann and Reynolds scale, morbidity of donor site, and compared the range of both ankle motion. We also evaluated the union at the medial malleolar osteotomy site, trabecular connection between the grafted osteochondral mass and talus, irregularity of the articular surface in lesion. Results: Clinical results were rated as excellent in 4, good in 7, fair in 1. The mean angle of the total range of motion in affected ankle was decreased by 3 degrees compared to that in unaffected ankle. We did not observe abnormal findings at donor site. The osteotomized bone was united at mean 9 weeks (range, 8-12 weeks). We observed trabecular connection between grafted osteochondral mass and talus at mean 14 weeks (range, 12-16 weeks). We also observed irregular articular surface in osteochondral lesions in 6, smooth articular surface in 6. Conclusion: The local autologous osteochondral graft for relatively large osteochondral lesion of the medial talar dome is useful operative method with advantages of wide operative field, low morbidity of donor site, and high satisfaction rate.

Effect of Intra-articular Bee Venom Injection on Acute Traumatic Arthritis of Elbow Joint : Two Cases Report (급성 외상성 주관절염에 대한 관절강내 봉약침 치험 2례)

  • Objectives : We got a good effect in two patients not by subcutaneous but by subcutaneous but by intra-articular bee venom injection treatments at traumatic arthritis of elbow joint, so report it. Methods : We used only intra-articular bee venom injection treatments at acute traumatic arthritis of elbow joint. After measured the VAS(visual analogue) and ROM(range of motion) of elbow joint, we bee venom injection effect. Results and Conclusions : The symptoms(swelling, pain) and the functions of elbow joint were quickly improved through intra-articular bee venom injection treatments. One case was improved that Rt. elbow joint flexion was increased from $60^{\circ}$ to $150^{\circ}$, extension was increased from $20^{\circ}$ to $0^{\circ}$. The other case was improved that Lt. elbow joint flexion was increased from $90^{\circ}$ to $150^{\circ}$, extension was increased from $30^{\circ}$ to $0^{\circ}$, and both were decreased in VAS. The results suggest that bee venom intra-articular injection has acute pain reduction, anti-inflammation effect.

Effects of Taping Therapy and Passive Range of Motion Exercises on Shoulder joint, Hand dexterity in the Elderly (테이핑요법과 수동적 관절범위운동이 노인의 견관절과 손의 민첩성에 미치는 영향)

  • The purpose of this study was to investigate the effects of taping therapy and passive range of motion exercises for the elderly aged 65 above with shoulder pain and limited of range of motion. This study was a nonequivalent control group pretest-posttest design. Data were collected about shoulder pain, range of motion, hand dexterity from 40 of elderly(exp.:20, con.:20) in B city from August to September, 2009. Collected data were analyzed using SPSS Win 12.0. Shoulder pain was significantly decreased. Range of motion which was flexion, extension, abduction, horizontal abduction. and hand dexterity were significantly increased in the taping group and PROM group. Shoulder pain and hand dextrity was non significantly between taping group and PROM group. Range of motion which was extension more increased taping group than PROM group. It was proven that taping therapy and PROM was effective for shoulder pain, articular range of motion and hand dexterity.

Effect of Korean Medicine Treatments in Patients with Proximal Tibia Fracture: A Retrospective Observational Study (경골 근위부 골절 환자에 대한 한의학적 치료 효과: 후향적 관찰 연구)

  • Objectives The purpose of this study is to find the effect of Korean medicine treatments on pain reduction and range of motion recovery in patients with proximal tibia fractures. Methods We studied 15 patients who had been diagnosed as proximal tibia fracture. This study was conducted as retrospective observational study which analyzed patient's medical records with IBM SPSS statistics 25 program. We used numeric rating scale to evaluate pain reduction and range of motion to observe the patient's recovery. Results The average of numeric rating scale reduced statistically significantly from 5.26±1.38 to 2.73±1.17 (p<0.001). The average of knee flexion range of motion increased statistically significantly from 81.11±34.34 to 117.66±19.01 (p<0.01). Conclusions We found that Korean medicine treatments have a positive effect on pain reduction and knee rom improvement in patients with proximal tibia fractures.

Studies on the Knee Joint Pain (무릎관절의 통증에 관한 연구)

  • It is general knowledge that knee joint pain can be attributed to trauma and degenerative change around the knee joint. However most patients who have suffered from pain or limited range of motion of the knee joint show no definite pathology on X-ray or laboratory examination. We examined 242 patients with knee joint pain and found compression or entrapment of the articular nerve fiber by the tissue around the knee joint resulted in pain in almost all cases. Conclusion: by relieving the compression of the articular nerve fiber with just physical therapy and LASER stimulation on the identified trigger points, in conjunction with NSAIDs, muscle relaxants, were found to be very effective in the treatment of knee joint pain.
