테이핑이 발목의 관절가동범위와 고유수용성감각에 미치는 영향 (The Effect of Taping on the Range of Motion and Proprioception at the Ankle Joint)

  • This study was designed to determine the effect of ankle taping and short period of walking on the treadmill on the range of motion (ROM) and proprioception at the ankle joint. Twenty healthy male subjects (mean age=24.2 yr) participated in this study. Goniometry and videotape replaying method were used to measure the ankle ROM. Passive sagittal and frontal plane motions were measured. The difference in degree between the stimulus point and the reproduced point was defined as an angular error. The measurements were performed at four different phases: pre-taping (PRT), post-taping immediately (POT), post-5 minute walking with taping (P5M), and post-10 minute walking with taping (P10M). The ankle of dominant limb was taped by a certified athletic trainer using a closed basket weave technique. Participants walked on the treadmill at 2.5 mph. The results showed that the mean of the sagittal plane motion at PRT, POT, P5M, and P10M was 53.0, 30.5, 36.2, and 40.2 degrees, respectively. The frontal plane motion at PRT, POT, P5M, and P10M was 33.6, 13.9, 15.7, and 18.6 degrees, respectively. The angular error at PRT, POT, P5M, and P10M was 5.5, 1.6, 1.8, and 1.9 degrees, respectively. After 10 minutes of walking, the sagittal plane motion and frontal plane motion was increased by 9.7 and 4.7 degrees compared with POT, respectively. The proprioception was significantly improved after the application of ankle taping. Both the restriction of frontal plane motion and proprioception improvement at the ankle joint may contribute to ankle stability during walking.

  • Background: This study examined the effects of adding fibular repositioning taping (FRT) to ankle mobilization with movement taping (AMT) on the ankle range of motion (ROM), balance, and gait performance in patients with chronic stroke with limited ankle dorsiflexion. Methods: The participants were randomized into the control (n=15) and AMT+FRT groups (n=15). The control groups applied only non-elastic taping on the affected ankle, and the AMT+FRT groups also applied non-elastic taping to the inferior tibiofibular joint. Both groups performed treadmill walking for 10 minutes. The ankle dorsiflexion passive ROM, balance, gait velocity, and cadence were measured before and after the intervention. Results: Both groups showed a significant difference after the intervention in the dorsiflexion ROM (p<.01), balance (p<.01), and gait performance (p<.01). On the other hand, no significant difference was observed between the two groups (p>.05). Conclusion: AMT improved ankle dorsiflexion passive ROM, balance, gait velocity, and cadence in patients with chronic stroke, but there was no difference between the two groups. Therefore, the addition of FRT to AMT does not influence the ankle dorsiflexion ROM, balance, and gait performance in patients with chronic stroke.

  • Background: Ankle sprains are a common clinical ankle disorder and alternations in tibiofibular joint biomechanics along with the talus are thought to contribute to its occurrence. During ankle joint dorsi flexion, proper movement requires the talus to glide posteriorly. Due to the wider front of the talus head, achieving the end range of dorsi flexion necessitates both superior and posterior glide of the distal fibula and anterior glide of the proximal fibula. The purpose of this study was to investigate the effects of tibiofibular joint mobilization on pain, range of motion, and balance in patients with lateral ankle sprains. Methods: Participants were randomly assigned to a control group (n=33) or an experimental group (n=31). Both groups underwent ankle joint mobilization three times a week for two weeks. Additionally, the experimental group received proximal and distal tibiofibular joint mobilization three times a week for two weeks. Measurements were obtained pre-intervention and post-intervention (after 2 weeks). Results: Evaluation parameters included the visual analog scale score (VAS), range of motion (ROM), and one-leg standing test (OLS). Post-intervention, both groups demonstrated significantly improved results for all assessments (p<.01). A significant intergroup difference was observed only in the ROM (p<.01) and OLS (p<.05). Conclusion: Our findings suggest that tibiofibular joint mobilization, combined with ankle joint mobilization, may be beneficial in enhancing outcomes for individuals with lateral ankle sprains.

The Relationship between Functional Movement Screen and Ankle Dysfunctions with Chronic Ankle Instability

  • Purpose: The purpose of this study was to investigate the correlations between functional movement screen (FMS) and ankle dysfunctions in subjects with chronic ankle instability (CAI). Methods: This study was a cross-sectional study of 20 participants with CAI. The ankle dorsiflexion range of motion (ROM), Foot and Ankle Disability Index (FADI), center of pressure (COP) path length, and COP velocity for ankle dysfunction were measured in all the subjects. All the subjects underwent the FMS concerned with ankle functions consisted of deep squats, hurdle steps and in-line lunges. The Spearman rank-order correlation coefficient was used to determine relationship between the ankle ROM, FADI, COP and FMS. Results: The results of the deep squat and in-line lunge exercises revealed a significant correlation with the ankle dorsiflexion ROM, FADI, COP path length, and COP velocity. The hurdle step showed no correlation with the ankle dorsiflexion ROM and FADI but a significant relationship with the COP path length and COP velocity. Conclusion: The results of this study showed that relationship deep squat and in-line lunge and it is suggested that an assessment tool using ankle dorsiflexion ROM and ankle instability would be clinically effective.

치료적 운동이 앞 발목 충돌 증후군 환자의 통증, 관절가동범위, 근력 및 균형능력에 미치는 영향: 증례보고 (Effects of Therapeutic Exercise on Pain, Range of Motion, Strength and Balance Ability in a Patient with Anterior Ankle Impingement: A Case Study)

  • PURPOSE: The purpose of this study is to determine the effect of therapeutic exercise on pain, range of motion and strength, and balance ability in a patient with an anterior ankle impingement (AAI). METHODS: A-32-year-old male presented limited ankle motion and pain with forced dorsiflexion at both ankle joints. In response, a therapeutic exercise program consisted of ankle joint mobilization, strength exercises using an elastic band, and proprioceptive exercises including semi-squats and a one-legged standing exercise with open and closed eyes. The program was performed for 40 min/day, twice per week, for 8 weeks. Pain, range of motion (ROM), and muscle strength (ankle dorsiflexion, plantarflexion, inversion, and eversion), as well as a one-legged standing test of both ankles, were measured before and after 4 and 8 weeks of therapeutic exercise. RESULTS: VAS decreased in both ankles after 8 weeks of exercise, respectively, compared to baseline levels. Range of motion and strength increased in both ankles for dorsiflexion, plantarflexion, inversion, and eversion after 8 weeks of exercise compared to baseline levels. In addition, the ability to perform a one-legged standing test with eyes opened and closed improved in both legs after 8 week of exercise compared to baseline levels. CONCLUSION: These results suggest that therapeutic exercise improves pain, ROM, muscle strength, and balancing ability in patients with AAI.

  • Objectives: The Purpose of this study is to report the effects of Korean medical treatment on patient of peroneal neuropathy caused by sitting cross-legged. Methods: The patient diagnosed as peroneal neuropathy was treated by Acupuncture, Bee Venom therapy, Herbal medicine. To evaluate the effects of Korean treatment, we used Range of Motion(ROM) of Ankle, Visual Analog Scale(VAS), Numeric Rating Scale(NRS), Ankle-Hind-foot Scale(AHS), Digital Infrared Thermal Imaging(DITI). Results: After treatment, ROM of Ankle, VAS, NRS, AHS were improved. Temperature Difference(${\Delta}T$) was decreased on DITI. Conclusions: This study suggests that Korean medical treatment might be effective on sitting cross-legged patient diagnosed as Peroneal Neuropathy.

  • Objectives : The purpose of this study is to report the effect of acupotomy for patients with chronic arthritis of ankle. Methods : We treated Two patients who have chronic arthritis of ankle with acupotomy. To check the effect and satisfaction of acupotomy we used visual analog scale(VAS) and Five-point Likert scale. At the same time, we measured Change of ankle range of motion(ROM). Results : As a result of evaluation by using VAS, after treatment VAS score decreased(VAS3, VAS5). ROM of ankle widened. And the Likert scale point were checked 3 and 4. Conclusions : This study shows acupotomy has useful effect on chronic arthritis of ankle. An acupotomy is an alternative effective treatment modality for chronic arthritis of ankle.

  • This study aims at measuring and comparing the joint range of motion(ROM) of the elderly, quantitatively understanding the joint ROM and duration for their dressing and undressing of pants with 3D motion analysis equipment, and thereby providing basic data necessary for the future development of clothes carrying functional designs. The findings are as follows. As for the study method, the 9-item joint ROM measurements were conducted with goniometer, and the questionnaire analyses were carried out for t-test, ANOVA, and regression analysis with spss12.0 program. The 3D motion analyses were handled with 3D Motion Analysis Package Version 3.1 Program. The findings are as follows. First, the ROM was shown to be significantly low, as the arthritis-pain consciousness level was felt higher by the subject than the average one. Seven ROM variables, such as hip flexion, hip adduction, hip internal rotation, hip external rotation, knee flexion, ankle dorsiflexion, and ankle plantar flexion, were shown to significantly affect the discomfort level experienced at the time of dressing or undressing. Second, in the motion of inserting the remaining leg into the pant crotch part(3e), the difference of angles in the hip joint and knee joint was the largest between the women in their 20s and the elderly women with arthritis. The Immediate Effect of Ankle Balance Taping using Kinesiology Tape on the Weight-bearing Ankle Dorsiflexion Range of Motion and the Dynamic Balance in Asymptomatic Subjects

  • PURPOSE: The purpose of this study was to examine the immediate changes in the weight-bearing ankle dorsiflexion range of motion (ROM) and the dynamic balance in asymptomatic subjects using the modified Star Excursion Balance Test (SEBT) after ankle balance taping (ABT) and placebo ABT with kinesiology tape METHODS: A total of 23 active participants (11 men, 12 women) volunteered for this study. Ankle flexibility was assessed using the weight-bearing lunge test, and dynamic balance was assessed using the modified SEBT. Participants were asked to respond to questions regarding their perception of stability, reassurance, and confidence when performing modified SEBT. RESULTS: The weight-bearing ankle dorsiflexion ROM did not show a significant decrease after real ABT or placebo ABT compared to the ROM prior to ABT. The anterior, posterolateral, and posteromedial reach distances of SEBT did not increase significantly after real ABT or placebo ABT compared to the distances prior to ABT. However, the participants' perception of stability, reassurance, and confidence, when performing SEBT with real ABT, was increased compared to that during the control trial. CONCLUSION: This study showed that although real ABT did not immediately improve the reach distances in the 3 directions during modified SEBT, it improved the participants' perception of stability, reassurance, and confidence without decreasing weight-bearing ankle dorsiflexion ROM.

만성 편마비 환자의 발목에 적용한 능동운동을 동반한 관절가동술이 하지근력과 보행의 시공간적 변수에 미치는 영향 (Effects of Ankle Joint Mobilization With Movement on Lower Extremity Muscle Strength and Spatiotemporal Gait Parameters in Chronic Hemiplegic Patients)

  • The purpose of this study was to determine the effect of ankle joint mobilization with movement (MWM) on the range of motion (ROM) in the ankle, on the muscle strength of lower extremities, and on spatiotemporal gait parameters in chronic hemiplegic patients. Fifteen subjects with chronic stroke were divided into two groups: an experimental group (8 subjects) and a control group (7 subjects). Both groups attended two or three sessions of physical therapy each week. The experimental group also attended additional MWM training sessions three times a week for five weeks. For both groups, the ROM of the ankle, the muscle strength of the lower extremities, and the spatiotemporal gait parameters in paretic limbs were evaluated before and after the training period. The results showed that the experimental group experienced more significant increases than did the control group in terms of passive (6.10%) and active (21.96%) ROM of the ankle, gait velocity (12.96%), and peak torque, of the knee flexor (81.39%), the knee extensor (24.88%), and the ankle plantar flexor (41.75%)(p<.05). These results suggest that MWM training in patients with chronic stroke may be beneficial in increasing ROM in the ankle, muscle strength in the lower extremities, and gait speed.
