고유수용성신경근촉진법의 팔 폄 패턴이 반대편 다리의 근활성도에 미치는 영향 (The Effect of Arm Extension Patterns of PNF on Muscle Activity of Opposite Lower Extremity)

  • Purpose: The purpose of this study is to analyze the effect of the arm extension pattern of proprioceptive exercise program on the muscle activity of the opposite lower extremity. Methods: In this study, electromyogram MP150(Biopac system, USA) was applied to 20 healthy male subjects. Arm extension-adduction-internal rotation pattern was applied within initial, mid and end range in sling position and supine position for measurement. And the effect on the activity of rectus femoris and tibialis anterior muscle of the opposite lower extremity was compared and analyzed. Results: The results of this study were summarized as follows: First, there was a statistically significant difference of the activity of the tibialis anterior muscle within the Initial range in sling position and supine position(P<0.05). Second, there was a statistically significant difference of the activity of the tibialis anterior muscle within the end range in sling position and supine position(P<0.05). Conclusion: Rectus femoris and tibialis anterior muscles shows the higher activity in the supine position than in the sling position. Therefore, the supine position is more appropriate than the sling position to make irradiation on lower extremity muscle with the extension pattern.

  • This study reviewed a broad range of existing reform options, and on the basis of this review proposed a number of initiatives for institutional reform in developing countries. This strategies were intended to help FAO staff to provide guidance to the developing countries for the reform of their agricultural and rural extension systems. The paper defines the distinction among extension as a function, agricultural extension as part of a larger knowledge triangle, and agricultural and rural extension as an expanded concept of knowledge and information systems. The focus is on new measures that promote food security, market-driven system, and highly competitive agribusiness enterprises. Moreover, this study focus on the tensions that exist between the modern force of globalization and the traditional forces of culture, geography, and community.

  • This study undertakes a critical survey of literatures on the nest development in agricultural extension system; T&V(Traininig and Visit Extension), FSR&E(Fanning Systems Research and Extension), and FF(Farmer First). The study compares the three extension systems in their establishment, objectives, charactersistics, and methods. to find the minor factors for the betterment of agricultural extension system. Those are; 1) client-oriented program, 2) educational service, 3) comprehensive goal and work, 4) benefit to wide range of customers, 5) democratic program, 6) participatory approach, 7) four based experiment, 8) linkages among participants, 9) specialization of extensionists, 10) autonomy of extension offices, 11) humanstic program.

  • North Dakota State University (NDSU) Extension Service's purpose was to create a learning partnership that helped adults and youth enhance their lives and communities. NDSU Extension Service has maintained a strong blend of county, regional and state staff to support their program delivery. Experiment Station and Extension Service were integrated both in their job duties and at the administrative level. While researchers at the campus and center carried out both research and outreach activities with producers, Extension staff both at county and center carried out some research activities as well as outreach activities. The strong county-based Extension network was the main avenue for program delivery. Extension agents also provide programs on a multi-country basis. Program planning includes county advisory councils, multi-county advisory councils, support groups, and commodity groups, such as farmers and business people. Planning was used to shape their long-range plan of work along with adjustments to their annual activities. Funding of the NDSU Extension Service has been a blend of federal, state, and county dollars. In the past few years, grant dollars and agency partnerships have increased. Local input into extension programs, combined with support and funding from state and federal partners, enabled the extension service to truly meet the needs of people.

Effects of Hold-Relax and Active Range of Motion on Thoracic Spine Mobility

  • Few studies address the use of manual muscle stretching to improve spinal active range of motion(AROM). There is evidence that' Hold-Relax'(HR) is effective for increasing ROM in the extremities, which leads the researchers to anticipate similar benefits in the spine. The purpose of this study is to investigate the effects of HR(trunk flexors) and active thoracic flexion and extension on thoracic mobility, specifically flexion and extension in healthy individuals. A convenience sample of 30 physical therapy students(22-38 years) were randomly assigned to intervention sequence 'A-B' or 'B-A', with at least 7 days between interventions. Intervention' A' consisted of HR of the ventral trunk musculature while 'B' consisted of thoracic flexion-extension AROM. Thoracic flexion and extension AROM were measured before and after each intervention using the double inclinometer method. Paired t-tests were used to compare AROM pre and post-intervention for both groups, and to test for carry-over and learning effects. There was a statistically significant increase(mean=$3^{\circ}$ ; p=0.006) in thoracic extension following HR of the trunk flexors. There were no significant changes in thoracic flexion following HR, or in flexion or extension following the AROM intervention. No carryover or learning effects were identified. HR may be an effective tool for improving AROM in the thoracic spine in pain free individuals. Further investigation is warranted with symptomatic populations and to define the minimal clinical difference(MCD) for thoracic spine mobility.

퇴행성관절염 노인을 위한 온요법의 적용시기에 따른 효과 (Effects of Heat Therapy according to the Application Time among the Elderly with Osteoarthritis)

  • This study was to investigate the effects(pain, discomfort, and range of motion) of heat therapy according to the application time among the elderly with osteoarthritis. Study participants were 27 elderly women, who were diagnosed as osteoarthritis, suffered from it for more than 6 months, and who were staying at a nursing home in Busan. The independent variable was heat therapy, which was applied for 20minutes, one time per week according to 3 timetable(before waking up, while in daily living, before going to bed) over 6 weeks. The dependent variables were pain and discomfort measured by 20 points visual analog scales, and range of extension and flexion measured by goniometery. The data were analyzed by descriptive statistics and repeated measures ANOVA. The results were as follows ; 1) There were significant differences for pain(F=9.77 p=.0001), discomfort(F=8.07 p=.001), range of extension(F=3.05 p=.05), and flexion(F=9.67 p=.0001) among heat therapy application times. 2) There were significant differences for pain(F=58.18 p=.0001), discomfort(F=63.68 p=.0001), range of extension(F=11.59 p=.001), and flexion(F=17.59 p=.0001) between before and after applying heat therapy. 3) There were not statistically significant differences for pain(F=.64 p=.531), discomfort(F=.18 p=.836), range of extension(F=1.33 p=.270), and fiexion(F=.26 p=.773) between before and after applying heat therapy according to the heat therapy application times. In conclusion, heat therapy was effective in reducing pain and discomfort, and in improving ROM for the elderly with osteoarthritis, but the effect of it was not different according to the time of application. We recommended further studies with larger sample size, longer and more repeatedly applied to investigate the effect of heat therapy according to the time of application.

The Effect of Squat Exercise Using a Reformer on Muscle Strength, Range of Motion, and Gait in Patients who Underwent Total Hip Replacement Surgery : A Pilot Study

  • 본 연구의 목적은 리포머를 이용한 스쿼트 운동이 엉덩관절전치환술 환자의 근력, 관절가동범위 그리고 보행 능력에 미치는 영향을 알아보고자 하였다. 본 연구는 G시 소재의 재활병원에 입원 중인 환자 20명을 대상으로 실험을 실시하였다. 중재 방법으로 주 7회 2주간 실험군은 리포머를 이용한 스쿼트 운동을, 대조군은 스쿼트 운동을 시행하였다. 리포머군의 그룹 내 비교에서는 엉덩관절 굽힘, 폄, 벌림 근력에서 유의한 차이가 있었다(p<0.05). 리포머군의 엉덩관절 폄과 벌림의 관절가동범위와 보행에서도 그룹 내 유의한 차이가 있었다(p<0.05). 그룹 간 비교에서는 엉덩관절 폄 근력, 엉덩관절 폄, 벌림 관절가동범위 그리고 보행에서 유의한 차이가 발생하였다(p<0.05).

Combination Effects of Self-Volar Gliding Using a Strap and Wrist Distraction on Pain and Wrist Extension Range of Motion in Subjects with Dorsal Wrist Pain

  • Purpose: This study examined the effects of self-volar gliding combined with a strap and wrist distraction on pain and the active and passive wrist extension range of motion (ROM) in subjects with dorsal wrist pain during partial weight bearing of the hand. Methods: Thirty subjects (14 males and 16 females) with dorsal wrist pain during partial weight bearing through the hand participated in this study. The two different self-volar gliding techniques were performed for each group. Self-volar gliding using a strap (SVGS) and SVGS and wrist distraction (SVGSD) were performed five times for one week for each group. The active and passive ROM of wrist extension and the peak pressure pushed by the hand at pain (PPHP) were measured. An independent t-test was used to compare the improvements of these elements between the two different self-volar gliding techniques. The level of statistical significance was at ${\alpha}=0.05$. Results: The active and passive ROM of wrist extension and PPHP were greater in both self-volar gliding groups after the one week intervention. On the other hand, these parameters were greater in the SVGSD group than in the SVGS group (p<0.05) Conclusion: SVGSD is recommended to improve the active and passive ROM of wrist extension and PPHP in subjects with dorsal wrist pain during partial weight bearing of the hand.

연속적 능동무릎폄 검사가 뒤넙다리근의 유연성에 미치는 영향 (Influence of successive active knee extension tests on hamstring flexibility)

  • Background: In clinical practice, active knee extension (AKE) test has been widely used to measure hamstring flexibility. During the AKE test, the knee extension is repeated six times. The first five repetitions are considered as warm-up, and the sixth is used as baseline. In order to accurately measure the subject's inherent flexibility, warm-up trials have been proposed as mentioned above; but currently, the evidence is insufficient. Design: Cross-sectional study. Methods: Forty-three healthy adults participated in this study. The AKE was performed slowly after flexing the hip and knee joints by 90° in a supine position. The knee was extended as far as could be done without causing discomfort or pain. When reaching the end range, knee flexion was performed without stopping. As per standard protocol, the subjects performed the knee extension six times. Results: There was no significant difference between the AKE value in the first trial with those in the other five trials. During the repetitions, the average value in the group with higher flexibility tended to decrease and the Intraclass correlation coefficient gradually decreased. Conclusion: Successive repetitions of the AKE test can misrepresent an individual's inherent hamstring flexibility. In order to avoid a decrease in hamstring flexibility, it is strongly recommended that individuals with high initial hamstring flexibility avoid unnecessary repetitions of the AKE test.

Comparing the Immediate Effectiveness of Lumbar Flexion and Extension Exercise With Regards to Pain, Range of Motion, Pelvic Tilt, and Functional Gait Ability in Patients With Lumbar Spinal Stenosis

  • Background: In patients with lumbar spinal stenosis (LSS), lumbar flexion exercise (LFE) is considered a standard therapeutic exercise that widens the space between the spinal canal and intervertebral foramen. However, some researchers have reported that lumbar extension exercise (LEE) may improve lumbar pain and functional ability in patients with LSS. Although exercise intervention methods for patients with LSS have been widely applied in clinical settings, few studies have conducted comparative analysis of these exercise methods. Objects: This study aimed to compare the effects of LFE, LEE, and lumbar flexion combined with lumbar flexion-extension exercise (LFEE) on pain, range of motion (ROM), pelvic tilt angle, and functional gait ability in patients with LSS. Methods: A total of 30 patients with LSS, LFE (n1=10), LEE (n2=10), and LFEE (n3=10) were assigned to each of the three exercise groups. The numerical pain rating scale (NPRS), modified-modified schober test (MMST)-flexion, MMST-extension, pelvic tilt inclinometer, and 6-minute walking test (6MWT) were measured. Results: After the intervention, statistically significant differences were observed in the NPRS (p=.043), MMST-flexion (p<.001), MMST-extension (p<.001), and 6MWT (p=.005) between groups. According to the post hoc test, the NPRS was statistically significant difference between the LFEE and LEE groups (p=.034). The MMST-flexion was statistically significantly different between the LFE and LEE (p=.000), LFE and LFEE (p=.001), and LEE and LFEE (p=.001) groups. The MMST-extension was statistically significantly different between the LFE and LEE (p<.001), LFE and LFEE (p=.002), and LEE and LFEE (p=.008) groups. The 6MWT was statistically significantly different between the LFE and LFEE (p=.042) and the LEE and LFEE (p=.004) groups. Conclusion: This study suggested that LFEE was the most effective exercise for pain and functional gait ability in patients with LSS, LFE was the most effective exercise for lumbar flexion ROM, and LEE was the most effective exercise for lumbar extension ROM.
