Review on Efficacy and Safety of Manipulation Therapy including Chuna Manipulation (추나요법을 포함한 수기치료의 효과 및 안전성에 관한 문헌고찰)

  • Objectives : This study aims to educate clinical doctors of the valuable practice of Manipulative Therapy(MT) as an opportunity for evidence-based medicine. Consequently it also serves to review the effectiveness and safety of MT. Methods : The literature studies of overseas were done by Cochrane Library and Medline website; those of domestic researches were completed by utilizing the sources which are gained from KERIS, KISS, DBpia, Kisti, and domestic institutes related with MT. Results : Eight papers related to the effectiveness of MT were published in South Korea, which concluded with the positive effects of MT. None of them are, however, well-designed randomized controlled trials(RCT). On the other hand, fifteen cases of nine articles indicate the adverse reaction of MT, and numbers of the researches in overseas revealed the side effects of MT in order of cervical, lumbar, and thoracic vertebrae. A vascular adverse reaction such as vertebral and carotid artery dissection was a highly reported cervical adverse reaction; in the thoracic and lumbar regions, neurological adverse reaction in terms of disc hemiation was frequently discovered. Conclusions : Henceforth, highly qualitative studies are required developing the effective outcomes and preventing any possible complications of MT. Therefore, systemic curriculums in institutions and sufficient clinical training in the filed are strongly recommended.

A Review of the Domestic Trends of Korean Traditional Medicine for Idiopathic Scoliosis (특발성 척추측만증의 한의학적 치료에 대한 국내 연구 동향: 국내 학술지를 중심으로)

  • Objectives This study aimed to analyze the trends of Korean traditional medicine treatment for idiopathic scoliosis. Methods We searched clinical studies used Korean traditional medicine for idiopathic scoliosis in 4 Korean online databases 'Koreanstudies Information Service System', 'Earticle', 'Oriental Medicine Advanced Searching Integrated System', 'Research Information Sharing Service'. We selected 18 studies and analyzed their characteristics according to author, publication year, number/sex/age of patient, treatment period, intervention, outcome measure and result. Results Eighteen case report studies were selected by criteria and randomized controlled trials were not found. The most frequently used therapy was Chuna therapy. Cobb's angle was most commonly used outcome measure. Conclusions In this study, we reviewed studies used Korean traditional medicine treatments for idiopathic scoliosis. However, evidence is limited because all selected studies were case reports. This study suggests that further clinical studies are needed to provide higher evidence about Korean traditional medicine treatment for idiopathic scoliosis.

Acupuncture Treatment for Ankylosing Spondylitis Using China National Knowledge Infrastructure: A Literature Review (강직성 척추염의 침 치료에 대한 최근 연구동향 분석: China National Knowledge Infrastructure 검색을 중심으로)

  • Objectives This study was performed to research the trends of Chinese acupuncture treatment for ankylosing spondylitis (AS). Methods We searched clinical studies about Chinese acupuncture treatment for AS through China National Knowledge Infrastructure. We analyzed the characteristics of selected studies according to research design, sample size, publication year, treatment methods, evaluation criteria and adverse events. Results 16 studies published from 2015 to 2020 were selected. They consisted of 11 randomized controlled trials and 5 case reports. In these studies, several types of acupuncture treatments were performed and especially there were many treatments that applied thermal stimulation of needles. The most commonly used evaluation criteria was effective rate. Conclusions This study showed that acupuncture treatment could be effective for AS. It suggested that various studies should be conducted to provide reliable evidence about acupuncture treatment for AS in Korea.

EVIDENCE-BASED PEDIATRIC DENTISTRY : CONCEPTS AND APPLICATIONS (근거중심 소아치과학의 개념과 응용)

  • The purpose of this study is to establish the concept of the evidence-based pediatric dentistry and to investigate the application methods of it. The definition of the evidence-based pediatric dentistry was suggested as 'the integration of the best scientific research evidence with the professional expertise of pediatric dentist and the value of pediatric patients and their caregivers in dental practice for the child and adolescent'. The methods of practising evidence-based pediatric dentistry were investigated, the recent conclusions of the evidence-based pediatric dentistry were summarized by topics, the research methods of systemic review and clinical practice guidlines were analyzed, and the individual research methods as randomized controlled trials were investigated. The author proposed increasing the understanding and consensus of the necessity of evidence-based pediatric dentistry, education about the evidence-based pediatric dentistry, computerization of all published articles of Korean pediatrtic dentistry, continuing research and popularization of systemic research and clinical practice guidlines, production of Korea's own evidences of pediatric dentistry, and the improvement of soical environment of pediatric practice.

A Review on the Korean Medicine Treatment for Thoracic Outlet Syndrome (흉곽출구 증후군의 한의학적 치료에 대한 국내외 연구동향 분석)

  • Objectives: The objective of this study was to review the trends of Korean medicine-based treatment for thoracic outlet syndrome(TOS). Methods: Korean and foreign on-line databases(Pubmed, Cochran Library, CNKI, NDSL KISS and OASIS) were researched for articles discussing Korean medicine-based treatment for TOS. Repeated articles, review articles, commentaries, and those not relevant to the topic of study were excluded. Results: The total number of studies selected was 26, which included 18 case reports and 8 randomized controlled trials(RCT). In these studies, acupuncture, pharmacopuncture, acupotomy, and warm acupuncture were used as interventions for TOS.Chuna therapy was found to be the most commonly used combination treatment. Despite the high prevalence of TOS, only 3 case reports of the 26 studies selected in this review were published in Korean academic journals. Conclusions: In this study, we analyzed the trends of traditional Korean medicine-based treatment for TOS. The results showed that Korean medicine-based treatment could be an effective method for treating TOS.

A Meta Analysis on the Effects of Exercise on Bone Mineral Density among Middle-aged and Older Women (운동이 중년이후 여성의 골밀도에 미치는 효과에 대한 메타 분석)

  • Purpose: This study was to examine the effectiveness of exercise for bone mineral density (BMD). Methods: Four investigators reviewed English articles from Pub Med and CINAHL, selecting randomized controlled trials on exercise programs for middle-aged and older women. Out of 25 studies identified, 14 that satisfied with the inclusion criteria were included in the meta-analysis. The quality of the studies was assessed using recognized methods and the effect size was calculated as a Hedges'g using Comprehensive Meta-analysis Version 2.0. Primary outcomes were changes in BMD at femoral neck, trochanter, and lumbar spine. Subgroup analysis included changes in BMD according to exercise style. Results: Weight bearing exercise was effective(Q=20.1, p>.05, ES=0.32), and resistance exercise was effective in case of comparing to pre and post intervention (Q=4.15, p=.98, ES=0.14). At the femoral neck, 9 study groups were homogeneous and the experimental groups demonstrated a positive effect on BMD (Q=19.5, p>.05, ES=0.33). In contrast, marked heterogeneity (Q=33.3, p<0.01) was apparent in 7 study groups evaluating trochanter. Conclusion: These findings suggest that weight bearing is effective for BMD of the femoral neck, and is relevant to the non-pharmacological treatment of bone loss for middle-aged and older women.

Interventions Using Technologies for Older Adults in Long-term Care Facilities: A Systematic Review (장기요양시설 거주 노인 대상 테크놀로지 활용 중재에 관한 체계적 문헌고찰)

  • Purpose: Although innovative interventions using technologies have been introduced in long-term care settings, available evidence is still anecdotal. The purpose of this study is to investigate and synthesize the outcomes of interventions using technologies delivered to nursing home residents. Methods: Published clinical trials were identified through PubMed, CINHAL, Cochrane and PsycINFO databases and manually hand-searching. Eligible studies were articles published between 1997-2016 in English or Korean with a randomized controlled trial or quasi-experimental design in which interventions using technologies were delivered to nursing home residents. Results: A total of 20 studies were selected for this review. Types of interventions using technologies were classified into the electronic documentation technology (n=1), the clinical decision support system (n=1), the safety technology (n=1), the health and wellness technology (n=10), and the social connectedness technology (n=7). Overall resident outcomes indicated that interventions using technologies improved behavioral symptoms and psycho-social outcomes, but mixed results were shown in the aspects of physical function, cognitive function, social relationship and quality of service. Conclusion: This review demonstrates that incorporating technologies into nursing home care have positive effects on residents' psycho-social outcomes and behavioral symptoms. To disseminate the effectiveness of interventions using technologies, further research is needed to determine what mechanisms underlying such relationships exist.

Effectiveness of Smartphone Addiction Intervention for University Students: A Systematic Review and Meta Analysis (대학생 스마트폰 중독 중재프로그램의 효과: 체계적 문헌고찰 및 메타분석)

  • This study aimed to evaluate the effectiveness of smartphone addiction interventions among university students. Selection criteria included university students, smartphone addiction interventions, smartphone addiction, depression, anxiety, self-control and time of smartphone usage. A total of 915 articles were retrieved. From those, 15 studies from 13 articles fulfilled the inclusion criteria. A meta-analysis showed that smartphone addiction interventions undergone by university students had statistical significance in alleviating smartphone addiction (d=-1.57, 95% CI:-2.84, -0.30), depression (d=-3.73, 95% CI:-4.97, -2.48), and self-control (d=7.28, 95% CI:2.76, 11.79); but were not significantly effective in improving anxiety and reducing time of smartphone usage. The systematic review and meta-analysis suggested that smartphone addiction interventions boost the alleviation of smartphone addiction. However, it remains necessary to conduct well-designed randomized controlled trials, including the consideration of various cultural characteristics.

Analysis of Clinical Researches on Traditional Medicine Treatment for Behcet's Disease in Korea, China, and Japan (한중일의 베체트병에 대한 한의약 임상연구 분석)

  • Objective : To find a treatment strategy, this study aimed to review and compare the clinical studies of Korea, China, and Japan on traditional medicine treatment for Behcet's disease. Method : Literature searches were performed on six databases in each country and English databases, such as OASIS, KoreanTK, CNKI, J-STAGE, PubMed, and Cochrane CENTRAL. In addition, using the limited keywords, the area was restricted to traditional medicine treatment, but the period was not limited. Studies were categorized according to the study design and we analyzed the difference of treatment among three countries. Results : One hundred and forty-one clinical studies met our inclusion criteria: 11 studies in Korea, 15 in Japan, 115 in China. Randomized controlled trials (RCT) have increased in China, however, no RCT was reported in Korea and Japan. The method of treatment was mainly traditional herbal medicine in all the three countries, but there were significant differences in the field of syndrome identification and prescription among countries. The studies in China made use of most various treatment modalities and reported the highest effectiveness. Conclusion : Traditional medicine treatment showed a considerable effect for improving various symptoms of Behcet's disease. However, more research for answering the safety and well-designed studies considering characteristics of Behcet's disease should be performed to evaluate the effectiveness of traditional medicine treatment objectively in the future.

Current Research Trend on the Acupuncture and Moxibustion Treatment for Pancreatitis (국내외 췌장염 관련 침구치료 연구 현황)

  • Objectives : The aim of this study is to review the current research trend on the acupuncture and moxibustion treatment for pancreatitis. Methods : Through nine search engines, articles were collected, which had involved the use of acupuncture and moxibustion treatment for pancreatitis. Data were extracted from the selected articles regarding the year, country, type of pancreatitis, study design, evaluation variable, intervention, point for acupuncture and moxibustion, and effectiveness of treatment. Results : Total 56 articles were reviewed. The number of studies increased gradually every ten years. Fifty-one studies(91%) were conducted in China. Most of clinical studies and animal experimentations reported on acute pancreatitis. Thirty-nine studies were randomized controlled trials(RCTs), but most of them were assessed as uncertain in the risk of bias evaluation. The degree of overall improvement(79%) was used mainly as evaluation variable of clinical studies. In animal experimentations, the examination of pancreatitis tissue(77%) and blood test(77%) were used the most. Most of studies were conducted with the acupuncture, one with moxibustion. Among them, electroacupuncture was the most common intervention of treatment group. ST36 was the most frequently used acupuncture point in the included studies. Effective rate increased and serum amylase, clinical symptoms, recovery time of gastrointestinal function decreased statistically significantly in the treatment group of clinical studies. Conclusions : The result of this study could be used for the future practice and research about the acupuncture and moxibustion treatment for pancreatitis.
