Comparison of CT Image Performance with or without Tin Filter based on Blind Image Quality Evaluation Method (블라인드 품질 평가 방법을 사용한 주석필터 사용 유무에 따른 CT 영상 특성 비교)

  • The use of tin filters as a way to reduce the medical radiation in computed tomography (CT). However, due to the changed X-ray spectrum with the use of tin filters, disease diagnosis could be affected because it appears as images of different impressions from previous images. Therefore, this study evaluates the changes in images when using tin filter and high pitch in chest low-dose CT. In this study, images were acquired in groups of three for comparison. Group 1 did not apply to tin filter, and used the existing pitch 0.8. Group 2 used a tin filter, pitch 0.8, Group 3 used a tin filter, and pitch 2.5. To compare the image quality, the natural image quality evaluator (NIQE) and the blind/referenceless image quality evaluator (BRISQUE) were used among the blind quality evaluation factors depended on a no-reference basis. As a result, the NIQE values were low in the order of Group 1, Group 3, and Group 2. BRISQUE values were low in the order of Group 3, Group 2 and Group 1. This study confirms the superiority of images of tin filter and high pitch techniques in chest low-dose CT, which is considered to be a fundamental study for acquiring accurate images of patients with difficult breathing control.

Case study on the effects of VR educational media on oral imaging practice (치위생학과 구강영상학실습 수업에서의 VR활용에 관한 사례 연구)

  • Objectives: This study aims to confirm the educational necessity and utilization of VR media. And it was conducted to prepare basic data necessary for the use of VR in various dental hygiene education in the future and the development of innovative practical training courses. Methods: Before and after using VR in oral radiology practice classes, learning interest (4 items), learning commitment (9 items), learning motivation (5 items), educational media preference (4 items), and satisfaction (10 items) were investigated and analyzed. Friedman two way ANOVA by ran a nonparametric analysis corresponding to repeated measures ANOVA was performed. The statistical significance level was 0.05. Results: It was found that there were statistically significant differences in learning interest, learning immersion, and learning motivation according to the type of oral radiology practice education medium (p<0.05). Conclusions: VR is expected that the use of learning media using VR will lead to students' interest, immersion, and learning motivation in class, and that positive learning effects on VR education media can be sufficiently obtained.

A Study on the Usefulness of an Ankle Joint Examination Assistive Device using a 3D Printing (3D 프린터를 이용한 발목관절 검사 보조기구의 유용성연구)

  • The mortise view radiography procedure is an ankle joint examination and observes the presence of trauma, sprain, or dislocation suspected in the ankle joint. The auxiliary equipment used during the mortise view radiography procedure can generate artifacts in the radiograph images and is not diverse enough to be custom-made for each patient; not cost-efficient. The purpose of this study is to create a custom assistive device to support mortise view radiography procedure. This study utilized 3D printing technology to create the mortise view radiography procedure assistive device (ShinHan Device; SHD). The lengths of the tibiotalar joint (TTJ), talar calcaneal joint (TCJ), and medial joint (MJ) were measured and evaluated by five researchers using both SHD and the prototype Hologic tool. The mean ranges were found to be 39.42-39.47 mm for TTJ, 31.41-31.57 mm for TCJ, and 21.21-21.23 mm for MJ while using SHD device. On the other hand, the measurements showed mean ranges of 39.73-39.79 mm for TTJ, 31.46-31.50 mm for TCJ, and 21.31-21.35 mm for MJ while using the Hologic tool. Based on this study results, the error ranges at all positions decreased by 24% for TTJ, 17% for TCJ, and 36% for MJ when using SHD device compared to the Hologic tool. Moreover, when SHD was used, it allowed for a highly reproducible examination posture (ICC = 0.99), and it enabled the acquisition of radiograph images without artifacts, which were present in the Hologic tool.

Analysis of Absorbed Dose on the Nucleus Size Change of Single Cells using Therapeutic Radioisotopes (치료용 방사성동위원소 사용 시 단일 세포의 핵 크기 변화에 따른 흡수선량 분석)

  • Targeted radionuclide therapy (TRT) is a method of treating tumor cells using radiopharmaceuticals. Cells and nuclei constituting tissues of the human body are composed of spherical and oval shapes, but cancer cells are composed of various cell types. Therefore, this study analyzed the absorbed dose for each organelle according to the change in the size of the cell nucleus for beta-emitting nuclides during targeted radionuclide therapy through the Monte Carlo method. Cells were set in two sphere shapes, 5 ㎛ and 10 ㎛, and the internal structure was divided into cell nucleus, cytoplasm, and cell surface. Next, the absorbed dose according to the increase in the size of the cell nucleus was evaluated. As a result, 177Lu among the target radionuclides showed the highest dose in all cell compartments. As the ratio of the nucleus in the cell increased, the absorbed dose on the cell surface increased, but the absorbed dose in the cytoplasm and nucleus tended to decrease. Accordingly, it is judged that it is important to select a radionuclide considering the size of cancer cells and determine an appropriate amount of radioactivity during targeted radionuclide treatment.

  • To develop the Korean-Version-Improvement program for ordinary people, we investigate how Korean-High-Intelligent people use their cognitive function, intellectual capacity and Intelligent Quotient test. Using Mensa iq test, we classify intelligent examination tool into various components; visual, geometrical component and cognitive interpretation component. Also we organized each component, got database of intelligent subjects sample, classified itemized availability. Based on these things, we prove efficiency of intelligent examination tool. We found out that high intelligent people showed high Intelligent Index using visual cognitive stimulation with cognitive function when subjects were exposed to certain situation; solving the problem. This intelligent examination tool is low price and safe and easily carrying out with personal computer. If we develop intelligence quotient examination customized with mental retardation populations, a variety of subjects will improve their cognition with efficiency and inexpensively.

A Study on Evaluation of Portal Vein by Utilizing MIP Reconstruction in the PC Environment after Abdomen CT of Hepatic Artery Embolization Patients (간동맥 색전술 환자의 복부단층촬영 후 PC 환경에서 MIP재구성영상을 이용한 간문맥평가에 관한 고찰)

  • When most patients are diagnosed with the quiet progressed hepatoma which often would make the operation impossible, the Interventional Radiology hepatic artery embolization is an extremely useful method for such patients. An existence of the malfunction is evaluated by gaining a portal vein image as a delayed phase image after injecting a contrast media into the superior mesenteric artery. However, it is difficult to make a definite judgement due to the extended exposure time with the peristalsis and the intestine gas obstructing the sharpness of the image when the Patient exposure time increases and due to the increased usage of contrast media and its side effect. The portal vein can be evaluated by obtaining the MIP image after reconstructing a 3-dimensional personal computer setting using the 2-dimensional from an enhancement abdomen CT image that is almost a requisite in operation to a hepatoma patient. Such method nay prevent a decrease in the quality of image based upon the time delay and intestine gas; also, because the patient exposure dose and contrast media usage may be reduced, it is a new, valuable way to decide the operational matter of hepatic artery embolization on a pre-angiography.

Evaluation of Absorbed Dose According to Nanoparticle Density During the Breast Cancer Brachytherapy (유방암 근접치료 시 나노입자의 밀도에 따른 흡수선량 평가)

  • The purpose of this study was to evaluate the efficacy of brachytherapy of breast cancer by dose assessment which a steady increased in Korea women. The dose assessment was performed using the MCNPX program, a MonteCarlo simulation technique. The sources used for brachytherapy was 192Ir. And nanoparticle which used for dose enhancement was gold. The density of nanoparticle was 7, 18 and 30 mg. Evaluation of absorbed dose according to distance is measured at a distance of 30, 50, 100 and 200 cm from the patient. As a result, The breast absorbed dose results increased in proportion to the density of nanoparticle. And the surrounding organs were not significantly different according to the density. But, in some organs, the absorbed dose decreased as the density of nanoparticles increased. Absorbed dose according to the distance was in inverse proportion to distance.

  • Medical institutions are abundant in human resources and have a unique structure with diverse professions and various organizational cultures coexist. This organizational culture influences organizational effectiveness and influences the effectiveness of the organization as a factor of leadership and organizational culture. Leadership is an important factor in linking employee behavior to organizational commitment to achieve job satisfaction. The purpose of this study was to analyze the effect of organizational effectiveness on organizational culture, leadership perception, and job stress in a special organization called radiologist. As a result, organizational culture and transformational leadership showed high correlation with 0.627. Among the organizational effectiveness variables, organizational commitment showed the highest influence with transformational leadership(${\beta}=0.284$, p<0.001), and job satisfaction was also the highest with transformational leadership (${\beta}=0.440$, p<0.001). The results showed that the expectation of transformational leadership affects organizational commitment and job satisfaction. In order to expect a positive change in the effectiveness of the organization, the radiologist culture requires an organizational culture that can fully demonstrate autonomy and creativity, and transformational leadership is required for this.

Volumetric change of the latissimus dorsi muscle after postoperative chemotherapy and radiotherapy in immediate breast reconstruction with an extended latissimus dorsi musculocutaneous flap: final results from serial studies

  • Background Breast reconstruction using an extended latissimus dorsi (eLD) flap can supplement more volume than reconstruction using various local flaps after partial mastectomy, and it is a valuable surgical method since the reconstruction area is not limited. However, when performing reconstruction, the surgeon should consider latissimus dorsi (LD) volume reduction due to postoperative chemotherapy (POCTx) and postoperative radiotherapy (PORTx). To evaluate the effect of POCTx and PORTx on LD volume reduction, the effects of each therapy-both separately and jointly-need to be demonstrated. The present study quantified LD volume reduction in patients who underwent POCTx and PORTx after receiving breast-conserving surgery (BCS) with an eLD flap. Methods This study included 48 patients who received immediate breast reconstruction using an eLD flap from January 2013 to March 2017, had chest computed tomography (CT) 7-10 days after surgery and 10-14 months after radiotherapy completion, and were observed for more than 3 years postoperatively. One surgeon performed the breast reconstruction procedures, and measurements of breast volume were obtained from axial CT views, using a picture archiving and communication system. A P-value <0.05 was the threshold for statistical significance. Results The average volume reduction of LD at 10-14 months after completing POCTx and PORTx was 64.5% (range, 42.8%-81.4%) in comparison to the volume measured 7-10 days after surgery. This change was statistically significant (P<0.05). Conclusions Based on the findings of this study, when harvesting an eLD flap, surgeons should anticipate an average LD volume reduction of 64.5% if chemotherapy and radiotherapy are scheduled after BCS with an eLD flap.

A Study on the Application of Deep Learning Model by Using ACR Phantom in CT Quality Control (CT 정도관리에서 ACR 팬텀을 이용한 딥러닝 모델 적용에 관한 연구)

  • This study aimed to implement a deep learning model that can perform quantitative quality control through ACTS software used for quantitative evaluation of ACR phantom in CT quality control and evaluate its usefulness. By changing the scanning conditions, images of three modules of the ACR phantom's slice thickness (ST), low contrast resolution (LC), and high contrast resolution (HC) were obtained and classified as ACTS software. The deep learning model used ResNet18, implementing three models in which ST, HC, and LC were learned with epoch 50 and an integrated model in which three modules were learned with Epoch 10, 30, and 50 at once. The performance of each model was evaluated through Accuracy and Loss. When comparing and evaluating the accuracy and loss function values of the deep learning models by ST, LC, and HC modules, the Accuracy and Loss of the HC model were the best with 100% and 0.0081, and in the integrated model according to the Epoch value, Accuracy and Loss with epoch 50 were the best with 96.29% and 0.1856. This paper showed that quantitative quality control is possible through a deep learning model, and it can be used as a basis and evidence for applying deep learning to the CT quality control.
