이식된 자가골의 종류와 형태에 따른 임플란트 변연골 흡수량에 관한 후향적 연구 (The retrospective study of marginal bone loss around dental implants according to different autogenous bone grafts)

  • Introduction: This study examined the cumulative resorption of implants placed in a severely atrophic mandible and analyzed the radiologic bone resorption in the marginal bone, after an autogenous bone graft including both block and particulates that had been harvested from the ramus and iliac crest. Materials and Methods: A retrospective study was performed on patients who had bone grafts for augmentation followed by implant installation in the mandible area from 2003 to 2008. Twelve patients (6 men and 6 women) who received 34 implants in the augmented sites were evaluated. Cumulative radiologic resorption around the implants was measured immediately, 3 months, 6 months and 12 months after implant installation surgery. Results: The installed implant in grafted bone showed 0.84 mm marginal bone resorption after 3 months and 50% total cumulative resorption after 1 year. The mean marginal bone resorption around the implant installed in the grafted bone was 0.44 mm after 3 months, 0.52 mm after 1 year, after which it stabilized. The implant survival rate was 97% (failed implant was 1/34). Marginal bone resorption of the installed implant in the autogenous onlay block bone grafts was 0.98 mm after 3 months, which was significantly higher than that of a particulated bone graft (0.74 mm) (P <0.05). Conclusion: An autogenous graft including block type and particulate type is a predictable procedure for the use of dental implants in a severely atrophic mandible. Implant placement in augmented areas show a relatively high survival and minimal bone loss, as revealed by a radiologic evaluation.

종골 부정 유합에 대한 두개의 골편을 이용한 거골하 관절 신연 유합술 (Subtalar Distraction Two Bone-Block Arthrodesis for Calcaneal Malunion)

  • Purpose: This study was designed to evaluate the results and efficacy of subtalar distraction two bone-block arthrodesis for calcaneal malunion. Materials and Methods: From January 2004 to June 2007, we operated on 8 patients (12 cases). There were 7 male patients and 1 female patient; their mean age was 42 years. 8 cases among them were operated initially. The period between initial injury and arthrodesis was 21 months. At an average follow up was 19 months. In operation, we used extensile lateral approach and arthrodesis was performed through tricortical two bone-block and cannulated screws. The Ankle-Hindfoot scale was used for clinical evaluation. In radiologic analysis, plain X-ray and CT were examined union and various parameters. Results: All cases achieved radiologic union at the final follow-up. The mean Ankle-hindfoot scale (maximum of 94 points) increased from 43.4 points preoperatively to 84 points at the final follow-up. The radiologic analysis of the pre- and postoperative standing lateral radiographs showed and average increase of 5.5 mm in talo-calcaneal height, $5.1^{\circ}$ in talocalcaneal angle, $6.1^{\circ}$ in talar declination angle and decrease of $5.7^{\circ}$ in talo-first metatarsal angle. Conclusion: The short term results of subtalar distraction two bone-block arthrodesis is promising, but longer follow-up was needed.

초기 발견에 실패한 후두-환추-축추 불안전성 환자: 증례보고 (Missed Detection of an Occipito-atlanto-axial Instability: A Case Report)

  • Occipito-atlanto-axial subluxation is a type of high cervical injury. Misdiagnosis of this instability might cause several complications, including neurologic deficits. A radiologic evaluation must be performed and specific parameters that imply cervical instability must be confirmed. Full understanding of these parameters may prevent not only delayed diagnosis but also unexpected complication. We report a case of delayed detection of a cervical instability and emphasize the role of proper diagnosis.

소아에서의 캡슐내시경 (Capsule Endoscopy in Children)

  • The small bowel is the most difficult intestinal segment to examine. Radiologic tests are mostly insensitive and double-balloon enteroscopy is unsuitable for the younger child. Capsule endoscopy is a novel wireless method of investigation of the small bowel. The primary indications for capsule endoscopy include evaluation of obscure gastrointestinal bleeding, small bowel Crohn's disease, and polyposis syndromes. Capsule endoscopy offers an accurate and effective means of investigating the small bowel in children. Capsule retention is a potential complication of capsule endoscopy. This review provides the indications, safety, and limitations of wireless capsule endoscopy in children.

  • Multi-detector row CT (MDCT) provides faster speed, longer coverage in conjunction with thin slices, improved spatial resolution, and ability to produce high quality muliplanar and three-dimensional (3D) images. MDCT has revolutionized the non-invasive evaluation of the central airways. Simultaneous display of axial, multiplanar, and 3D images raises precision and accuracy of the radiologic diagnosis of central airway disease. This article introduces central airway imaging with MDCT emphasizing on the emerging role of multiplanar and 3D reconstruction.

방사선학과 교육과정 개선을 위한 현 교육과정 평가 (Evaluation of Present Curriculum for Devlopment of Dept. of Radiological Science Curriculum)

  • 시대와 사회적인 변화에 따라서 교육과정은 변화의 요구를 받게 된다. 교육과정의 변화를 요구받는 현시점에서 교과과정을 개선하기 위한 선행단계인 현재의 교과과정을 분석, 평가하고자 한다. 교육전문가 집단의 설문과 현재 방사선학과가 개설되어 있는 21개 대학 중 19개 대학 방사선학과의 교육과정 및 참고문헌을 중심으로 연구하였으며, 연구내용은 방사선사 업무범위와 교과과정, 방사선과학 교육과 교과과정, 디지털과 국제화시대의 교과과정, 방사선과학에 대한 학문적인 발전과 교과과정 등의 내용을 중심으로 연구하였다. 업무범위에서는 혈관조영학 및 중재적검사학은 6 ~ 8개교, 투시영상학은 4개교, 초음파영상학 및 실습은 6개교, 자기공명영상학은 2개교 등에서 미개설된 것으로 나타났다. 기초의학 교과목인 인체생리학, 인체해부학 및 실습, 의학용어 등은 대부분의 대학에서 개설되어 있었으나, 병리학 5개교, 영상해부학 6개교, 임상의학 11개교 등에서 미개설 되어 있는 것으로 나타났다. 기초이공학 교과목 중에서 일반생물학 및 실습은 11개교, 일반물리학 및 실습은 14개교, 일반화학 및 실습은 8개교 등 약 절반정도의 대학에서 개설된 것으로 나타났다. 전공기초에서 디지털화 교과목인 보건전산학, 컴퓨터프로그램밍, PACS 등이 개설된 대학은 4 ~ 5개교에 불과하였다. 방사선과학 교육은 확대된 업무범위를 포함하여, 방사선과학의 학문적 발전과 디지털화 교육 및 국제화 그리고 방사선과학 분야의 미래지향적인 교육의 토대를 마련하기 위해서는 현재 각 대학에서 개설, 운영되고 있는 교과과정은 일정부분 표준화의 필요성을 인식하였으며, 이에 방사선학과 교육과정은 인증제 도입의 필요성을 제시한다.

골다공증과 동반된 족관절 외과 골절의 지연나사를 이용한 치료 (Treatment of Ankle Lateral Malleolar Fractures Accompanying Osteoporosis using Lag Screw)

  • Purpose: To evaluate the results of ankle lateral malleolar fractures classified as Danis-Weber type B accompanying osteoporosis that were treated with lag screw. Materials and Methods: 15 cases of Danis-Weber type B ankle lateral malleolar fractures that had T score of less than -2.5 in BMD(bone mineral density) test were selected from June 2003 to December 2005. 10 cases were males and 5 cases were females with mean age of 59 years. The main injury mechanism was supination and external rotation. Mean follow-up period was 16 months. Clinical and radiologic evaluation was done according to Meyer and Kumler's criteria. Results: All cases showed satisfying result and mean radiologic bone union period was 3 months. Anatomic reduction and bone union was acquired in all cases without complications including wound infection, skin necrosis, delayed union and nonunion Conclusions: Lag screw fixation seem to be excellent treatment of Danis-Weber type B lateral malleolar fractures with osteoporosis as it can minimize soft tissue injury and enable anatomic reduction with firm fixation.

일반엑스선영상검사의 인자에 따른 산란방사선량 평가 및 분석 (Evaluation and Analysis of Scattered Radiation Dose according to Factors in General X-ray Examination)

  • Control of scattered radiation is one of very important factors in the use of medical radiation. In general X-ray exam, the causes, measurement methods, and the kind of detectors of scattered rays within the radiation area are diverse. In this study, the dose of scattered ray was measured by changing the thickness of the polycarbonate phantom and the tube voltage. As a result of measurement of scattered radiation, the results show that the scattered dose significantly(p<.05) increased with growing of thickness of phantom in the tube voltage 40, 50 and 60 kVp(F(p)<.05, R2>64%). As tube voltage increased at all phantom thicknesses, the scattered dose also significantly(p<.05) increased(F(p)<.05, R2>69%). In cases where a significant correlation was shown, the coefficient of determination of more than 60% was shown in regression analysis. The results of this study can be used as data on scattered radiation dose according to the tube voltage and the object thickness in general X-ray imaging exam.

소아백혈병 환자의 전신방사선조사 시 조직보상체의 재질변화에 따른 선량평가 (In Pediatric Leukemia, Dose Evaluation according to the Type of Compensators in Total Body Irradiation)

  • 소아백혈병의 치료방법 인 조혈모세포이식법의 전처치 방법으로서 항암제와 함께 대표적으로 사용되고 있는 전신방사선조사에 대하여 치료를 시행하기 전 정량적인 인체장기에 대한 선량을 평가하고자 하였다. 모의실험 프로그램 MCNPX를 사용하여 가상의 공간에서 소아용 모의피폭체를 대상으로 조직보상체의 재질을 변화시켜 실험을 진행하였다. 그 결과 첫째, 평균피부선량은 조직보상체의 재질에 따라 Plexiglass는 74.60 mGy/min, Al은 73.96 mGy/min, Cu는 72.26 mGy/min, Pb의 경우 67.90 mGy/min을 보였다. 둘째, 심부선량은 조직보상체 재질에 상관없이 갑상선, 생식선, 소화기계, 머리, 폐, 신장의 순으로 높게 나타났다. 끝으로 조직보상체와 환자와의 거리는 50 cm 이격시켰을 때가 이상적인 것으로 분석되었다. 본 연구결과를 토대로 할 때, 조직보상체 Al, Cu, Pb은 현재 사용되고 있는 Plexiglass 재질을 대체할 수 있을 것으로 판단된다.

Alpha-fetoprotein (AFP) Elevation Gastric Adenocarcinoma and Importance of AFP Change in Tumor Response Evaluation

  • Background: Elevated serum alpha-fetoprotein (AFP) levels in adults are considered abnormal. This parameter is used mostly in the diagnosis and follow-up of hepatocellular carcinomas and yolk sac tumors. Among the other rare tumors accompanied with elevated serum AFP levels, gastric cancer is the most common. In this study, we evaluated the follow-up and comparison of the treatment and marker response of patients with metastatic gastric cancer who had elevated serum AFP levels. Materials and Methods: We performed a retrospective study, including all consecutive patients with advanced gastric cancer, who received systemic chemotherapy with elevated AFP level. Results: Seventeen metastatic gastric cancer patients with elevated AFP levels at the time of diagnosis were evaluated. Fourteen (82.4%) were males and three (17.6%) were females. The primary tumor localization was the gastric body in 8 (76.4%), cardia in 7 (41.2%), and antrum in 2 (11.8%). Hepatic metastasis was observed in 13 (76.4%) at the time of diagnosis. When the relationship of AFP levels and carcinoembryonic antigen (CEA) response of the patients with their radiologic responses was evaluated, it was found that the radiologic response was compatible with AFP response in 16 (94.1%) patients and with CEA response in 12 (70.6%); however, in 5 (29.4%) patients no accordance was observed between radiological and CEA responses. Conclusions: Follow-up of AFP levels in metastatic gastric cancer patients with elevated AFP levels may allow prediction of early treatment response and could be more useful than the CEA marker for follow-up in response evaluation.
