  • The quality assurance program of a panoramic radiography is been discussed in two ways: phantom image tests and clinical image quality assurance tests. Methods and standards of each of the test are presented, which efforts guarantee a proper quality of image, improve standard of dental care and decrease radiation exposure.

하악골 골절에 대한 파노라마방사선사진의 진단능 (Diagnostic ability of panoramic radiography for mandibular fractures)

  • Purpose : The purpose of this study was to evaluate the diagnostic efficacy of panoramic radiographs for detection of mandibular fractures. Materials and Methods : The sample was comprised of 65 patients (55 fractured, 10 non-fractured) with 92 fracture sites confirmed by multi-detector computed tomography (CT). Panoramic radiographs were evaluated for mandibular fractures by six examiners; two oral & maxillofacial radiologists (observer A&B), two oral & maxillofacial surgeons (observer C&D), and two general dentists (observer E&F). Results : Sensitivity of panoramic radiography for mandibular fractures was 95.7% in observer A&B, 93.5% in observer C&D and 80.4% in observer E&F. The lowest sensitivity was shown in symphyseal/parasymphyseal areas, followed by subcondylar/condylar regions. Conclusion : Panoramic radiography is adequate for detection of mandibular fractures. However, additional multidetector CT is recommended to ascertain some indecisive fractures of symphysis and condyle, and in complicated fractures.

Cone beam형 전산화단층촬영에 의한 흡수선량 (Radiation absorbed doses of cone beam computed tomography)

  • Purpose: To measure the absorbed doses of cone beam computed tomography (CBCT), which is recently being more frequently used, and to compare them with those of panoramic radiography. Materials and Methods: To measure the absorbed doses of CBCT ($PSR-9000N^{TM}$, Asahi Roentgen Ind. Co., Japan), we placed TLD chips on the skin regions above the parotid and thyroid glands, and on the dorsum of tongue in a dental head phantom. We used two image acquisition modes of the Dental and Panoramic modes of CBCT, which differed in the field of view. Also, panoramic radiographs (Auto IIIN, Asahi Roentgen Ind. Co., Japan) were taken to compare with the absorbed doses of CBCT. Result: In the Dental mode of CBCT, the absorbed doses of the parotid gland, dorsum of tongue, and thyroid gland were 3.53, 3.13, and 0.36 mGy, respectively. In the Panoramic mode of CBCT, they were 9.57, 9.15, and 0.85 mGy, respectively. The panoramic mode showed higher absorbed doses than those of the Dental mode. In the panoramic radiography, the absorbed doses of the parotid gland, dorsum of tongue, and thyroid gland were 1.21, 1.19, and 0.16 mGy, respectively. And they were about 1/3 of the Dental mode and 1/9 of the Panoramic mode of CBCT. Conclusion: Absorbed doses of CBCT are higher than those of panoramic radiography, and dependent upon the field of view.

3D CT를 이용한 파노라마 방사선 사진상 상악 매복 견치 위치 정보의 유용성 평가 (Evaluation or potency or panoramic radiography for estimating the position of maxillary impacted canines using 3D CT)

  • 3D CT를 이용하여 파노라마에서 얻어진 상악 매복 견치 위치 정보의 유용성을 평가하기 위해, 상악 견치 매복을 주소로 경북대학교병원에 내원한 환자를 대상으로 파노라마 방사선 사진과 3D CT에서의 매복 견치의 위치를 평가하였다. 대상은 25명(남자 7명, 여자 18명)으로 평균 나이는 10.9세(범위 : 8.2 - 15.7세)이며, 35개의 상악 매복 견치증례를 분석하여 다음의 결과를 얻었다. 첫째, 파노라마 방사선 사진은 확대되어 촬영되므로 3D CT의 계측치보다 상악 견치의 tooth length, crown width, vertical distance 그리고 lateral shift값에서 큰 값을 가졌다. 단, angulation to occlusal plane은 파노라마에서 더 작은 값으로 계측되었다. 둘째, 파노라마 방사선 사진상 상악 견치가 구개측으로 매복된 경우, angulation to occlusal plane은 3D CT보다 작게 측정되고 vertical distance는 더 크게 측정되었다. 셋째, 파노라마 방사선 사진에서 상악 매복 견치의 tooth length, crown width 그리고 순측으로 매복된 경우 상악 견치의 angulation to occlusal plane은 3D CT와 근접된 계측치를 가졌다. 또한 CT와 비교하였을 때, 치근 흡수 정도 평가에 대한 파노라마 방사선 사진의 감수성은 33.3%인 것으로 측정되었다. 파노라마 방사선 사진은 협측 매복견치의 위치는 실제와 유사하다고 생각되나 구개측 매복의 경우 3D CT보다 더 높게, 각도는 더 작게 나타나는 등 상당한 차이가 있어 CT를 이용한 추가적인 검사가 필요하다고 생각된다.

한국인의 디지털파노라마 방사선영상에서 하악관에 관한 연구 (A study of the mandibular canal in digital panoramic radiographic images of a selected Korean population)

  • Purpose : To determine the more valuable information to detect the mandibular canal and the mental foramen in panoramic radiographs of a selected Korean population for the implant. Materials and Methods : This study analysed 288 panoramic radiographic images of patients taken at the Dental hospital of Chosun University retrospectively. Indirect digital panoramic X-ray machine (ProlineXC, PLANMECA, Finland) with processing by using Directview $CR950^@$ (Kodak, U.S.A.) and Direct digital panoramic X-ray machine (Promax, PLANMECA, Finland) were used for all exposures. All images were converted into Dicom format. Results : The common position of the mental foramen was in line with the longitudinal axis of the second premolar (68.1%). The mental foramen was symmetrical in 81.8% of cases. The mandibular canal was not identified at anterior portion and discontinued with the mental foramen in 27.8% of all cases, in 42.4% identified with lower border line continued with the mental foramen, in 14.6% with both upper and lower border lines, and in 15.3% unilaterally identified with lower border line. Conclusion : Clinicians can estimate the upper border line of the mandibular canal from the confirmation of the mental foramen and the lower border line of the mandibular canal symmetrically on the panoramic radiography taken in adjusted midsaggital plane of patient's head.

상악동저와 상악 대구치 치근의 위치 관계: 파노라마방사선사진과 Cone beam형 전산화단층영상의 비교 (Comparison of panoramic radiography and cone beam computed tomography for assessing the relationship between the maxillary sinus floor and maxillary molars)

  • Purpose: This study compared panoramic radiography and cone beam computed tomography (CBCT) for evaluating the relationship between the maxillary sinus floor and the roots of maxillary molars. Materials and Methods: Paired panoramic radiographs and CBCT images from 97 subjects were analysed. This analysis classified 388 maxillary molars according to their relationship to the maxillary sinus floor on panoramic radiograph and CBCT. Correlations between these two radiographic techniques were examined. Results: Maxillary molar roots that were separate from the sinus floor showed the same classification in 100% of the cases when using these two imaging techniques. The corresponding percentage for such roots that were in contact with the sinus floor was 75%. When roots overlapped the maxillary sinus floor on panoramic radiographs, only 26.4% of maxillary first molars and 60.0% of second molars showed protrusion of roots into the sinus with CBCT. Conclusion : The results of the study suggest that roots projecting into the sinus on panoramic radiographs require a three-dimensional image in order to analyze the proximity of their apex to the sinus floor. 최근 개발된 cone beam computed tomography의 흡수선량 및 유효선량 평가 (Absorbed and effective dose from newly developed cone beam computed tomography in Korea)

  • Purpose: Cone beam computed tomography (CBCT) provides a lower dose and cost alternative to conventional CT, promising to revolutionize the practice of oral and maxillofacial radiology. The purpose of this study was to evaluate the absorbed and effective doses of Implagraphy and VCT (Vatech Co., Hwasung, Korea) and compare them with those of panoramic radiography. Materials and Methods: Thermoluminescent dosimeter (TLD) chips were placed at 27 sites throughout the layers of Female ART Head and Neck Phantom for dosimetry. Implagraphy, VCT units, and Planmeca Proline XC panoramic unit were used for radiation exposures. Radiation weighted doses and effective doses were measured and calculated using 1990 and 2005 ICRP tissue weighting factors. Results: Effective doses in Sv (ICRP 2005, ICRP 1990) were 90.19, 61.62 for Implagraphy at maxillay molar area, 123.20, 90.02 for Implagraphy at mandibular molar area, 183.55, 139.26 for VCT and 40.92, 27.16 for panoramic radiography. Conclusion: Effective doses for VCT and Implagraphy were only about 2.2 to 4.5 times greater than those for panoramic radiography. VCT and Implagraphy, CBCT machines recently developed in Korea, showed moderately low effective doses.

상악동의 점막비후에 대한 WATERS방사선사진과 파노라마방사선사진 및 전산화단층사진의 진단능 비교 (Comparison of Waters′ radiography, panoramic radiography, and computed tomography in the diagnosis of antral mucosal thickening)

  • With the CT findings as gold standard, the sensitivity, the specificity, and the diagnostic accuracy of Waters' radiography and Waters' radiography with panoramic radiography were compared in the diagnosis of antral mucosal thickening of 16 patients. Three oral radiologists and three non-oral radiologists interpreted the Waters' radiographs and after 4 weeks, interpreted the Waters' radiographs and panoramic radiographs simultaneously. The interpretation point was the existence or the non-existence of the mucosal thickening on the medial, the posterolateral, the floor, and the roof of maxillary sinus. The obtained results were as followed : 1. In oral radiologist group, the sensitivity, specificity and diagnostic accuracy of Waters' film were 0.7250, 0.8489 and 0.7578 respectively. 2. The sensitivity and the diagnostic accuracy in oral radiologist group were higher than those of non-oral radiologist group (P<0.05), but there was no significant difference between two groups in the specificity (P>0.05), 3. There was no significant difference of the diagnostic abilities except the specificity in oral radiologist group between Waters' radiography and Waters' radiography with panoramic radiography (P>0.05). 4. The sensitivity and the diagnostic accuracy were the highest in the case of medial wall interpretation, the specificity was the highest in the posterolateral wall. 5. In the posterolateral wall and the floor, the sensitivity and the diagnostic accuracy of oral radiologist group were higher than those of non-oral radiologist group (P<0.05).

파노라마 및 경두개 방사선사진에서 하악과두의 조사각도 (Projection angles of mandibular condyles in panoramic and transcranial radiographs)

  • Purpose : To evaluate the true projection angles of film-side mandibular condyles in panoramic and transcranial radiographs. Materials and Methods : 52 panoramic and transcranial radiographs of 4 condyles from two human dry mandibles with gradual horizontal and vertical angle changes were taken. The results were compared with the standard panoramic and transcranial radiographs and the identical pairs were selected. Results and Conclusion : Panoramic radiography projected 10 degrees to the film-sided condyles both horizontally and vertically. Transcranial radiography projected 15 degrees to the film-sided condyles vertically. The medial and lateral poles were not forming the outline of condylar images in both projections when the horizontal angles of condyles were not sufficiently big enough.

A new bite block for panoramic radiographs of anterior edentulous patients: A technical report

  • Purpose: Panoramic radiographs taken using conventional chin-support devices have often presented problems with positioning accuracy and reproducibility. The aim of this report was to propose a new bite block for panoramic radiographs of anterior edentulous patients that better addresses these two issues. Materials and Methods: A new panoramic radiography bite block similar to the bite block for dentulous patients was developed to enable proper positioning stability for edentulous patients. The new bite block was designed and implemented in light of previous studies. The height of the new bite block was 18 mm and to compensate for the horizontal edentulous space, its horizontal width was 7 mm. The panoramic radiographs using the new bite block were compared with those using the conventional chin-support device. Results: Panoramic radiographs taken with the new bite block showed better stability and bilateral symmetry than those taken with the conventional chin-support device. Patients also showed less movement and more stable positioning during panoramic radiography with the new bite block. Conclusion: Conventional errors in panoramic radiographs of edentulous patients could be caused by unreliability of the chin-support device. The newly proposed bite block for panoramic radiographs of edentulous patients showed better reliability. Further study is required to evaluate the image quality and reproducibility of images with the new bite block.
