A Case of Columnar Cell Variant of Papillary Thyroid Carcinoma (갑상선 유두암 원주세포변형 1예)

  • Columnar cell variant of papillary thyroid carcinoma (CCV-PTC) is a rare variant representing 0.15-0.2% of all PTCs. The CCV is aggressive, due to its rapid growth, high local recurrence rate, and frequent lung, brain and bone metastasis. Aggressive surgical and medical management are recommended for these neoplasias. The authors experienced a case of CCV-PTC in a 45-year-old man. We performed total thyroidectomy with neck dissection. The patient received radiation and radioactive iodine therapy. There were no recurrences or complications in the following 24 months after the operation. The patient will closely undergo continuous follow up. We present the clinical characteristics, pathology, treatment, and prognosis of the tumor with a review of the literature.

Cu-64 as a Cancer Theranostics Agent

  • Theranostics, a composite word of therapy and diagnosis, is known as personalized medicine and the concept of diagnosis and treatment at the same time. In nuclear medicine, it means performing both therapeutic and diagnostic radioisotope therapy using the same target molecule. The increased production and utilization of 64Cu opens a new era of theranostics. The studies introduced here have shown that 64CuCl2 and various compounds or biomolecules labeled with 64Cu are unique radiopharmaceuticals with physiological properties suitable for use as diagnostic and therapeutic agents. So far, these two abilities have been described only for radioactive iodine. Although 64Cu has complex chemical properties compared to other PET radioisotopes such as 68Ga, it has an appropriate half-life and enables high-quality PET images similar to 18F, which is an advantage in terms of diagnosis. In addition, since it also has therapeutic properties through the release of β- particles and Auger electrons by electron capture, radiopharmaceuticals using 64Cu stand for innovative radiopharmaceuticals for theranostic purposes. Therefore, based on the initial results obtained using 64Cu as a therapeutic agent, it is expected that additional research on the application of 64Cu will lead to a new era in the theranostics field.

Permanent Hypothyroidism after Radioactive Iodine($^{131}I$) Treatment in Diffuse Toxic Goiter (독성미만성갑상선종환자에서 방사성옥소(放射性沃素)($^{131}I$)투여후(投與後) 발생(發生)하는 영구적갑상선기능저하증(永久的甲狀腺機能低下症))

  • Radioactive iodine(RAI), principally $^{131}I$, effectively controls hyperthyroidism in the majority of patients. The subsequent development of hypothyroidism, however, has been of increasing concern since it was first pointed out by Chapman and Maloof in 1955. And the steady increase of late hypothyroidism during the passage of time was known with its relation with dosage of RAI. The authors have investigated the development of hypothyroidism in 935 patients with diffuse toxic goiter(DTG) who were treated with ($RAI^{131}I$) at the Seoul National University Hospital from 1960 to 1977 to reveal its relation with the number of RAI treatments, dosage of RAI, age of patients and exophthalmos with the following results. 1) The incidence of hypothyroidism by year after RAI therapy among 631 patients with DTG who were treated with single RAI regimen was 7.4%(1 year), 11.8%(2 year), 16.2%(3 year), 22.1%(4 year) and 25.5%(5 year), and that among 163 patients given multiple RAI treatments was 8.6%(1 year), 10.4%(2 year), 13.3%(3 year), 29.1%(4 year), and 54.1%(5 year)respectively showing much higher year1y increments from 4 years after RAI treatment. in comparison with the former. 2) Among 550 patients in the lower dose group treated with single RAI regimen less than 5.0mCi ($Mean{\pm}S.D.:\;4.3{\pm}0.6mCi$), the incidence of hypothyroidism by year after RAI treatment was 6.8%(1 year), 11.4%(2 year), 15.4%(3 year), while among 81 patients in the higher dose group given single RAI treatment not less than 5.5 mCi ($Mean{\pm}S.D.:\;6.3{\pm}0.5mCi$) it was 12.0%(1 year), 15.4%(2 year) and 20.4%(3 year) respectively. However, the duration till euthyroid state after RAI therapy in the two groups was $5.1{\pm}3.6$ months and $4.8{\pm}2.8$ months respectively showing no statistically signficant difference (p>0.1). 4) The incidence of hypothyroidism after RAI treatment in patients younger than 30 years of age was 4.3%(1 year) and 7.7%(2 year); in patients from 30 years to 49 years of age, 5.8%(1 year) and 11.1%(2 year); and in those older than 50 years, 11.0%(1 year) and 14.4%(2 year). The data revealed rising incidence of hypothyroidism with increase of patients' age. 4) Among 116 patients with exophthalmos the incidence of hypothyroidism by year after RAI treatment was 7.1%(1 year) and 12.1%(2 year) while that among 184 patients without exophthalmos was 7.3%(1 year) and 12.2%(2 year) respectively. With the above data the authors could conclude that the hypothyroidism in patients with DTG who were treated by RAI developed more frequently than reported by others in Korea till now, and increased with the passage of time, the yearly increments from 4 years after RAI treatment increasing markedly in the multiple dose group, and the incidence could be reduced by decreasing the administered RAI doe not increasing the duration till euthyroid state after RAI therapy.

A Case Study about Counting Uncertainty of Radioactive Iodine (131I) in Public Waters by Using Gamma Spectrometry (감마분광분석을 이용한 환경 중 방사성요오드(131I)의 측정 불확도에 관한 사례 연구)

  • The radioactive iodine ($^{131}I$) presents in the environment through the excrete process of nuclear medicine patients. In the detecting of low level of $^{131}I$ in the public water, the counting uncertainty has an effect on the accuracy and reliability of detecting $^{131}I$ radioactivity concentration. In this study, the contribution of sample amount, radioactivity concentration and counting time to the uncertainty was investigated in the case of public water sample. Sampling points are public water and the effluents of a sewage treatment plant at Sapkyocheon stream, Geumgang river. In each point, 1, 10 and 20 L of liquid samples were collected and prepared by evaporation method. The HPGe (High Purity Germanium) detector was used to detect and analyze emitted gamma-ray from samples. The radioactivity concentration of $^{131}I$ were in the range of 0.03 to 1.8 Bq/L. The comparison of the counting uncertainty of the sample amount, 1 L sample is unable to verify the existence of the $^{131}I$ under 0.5 Bq/L radioactivity concentration. Considering the short half-life of $^{131}I$ (8.03 days), a method for measuring 1 L sample was used. However comparing the detecting and preparing time of 1, 10 L respectively, detecting 10 L sample would be an appropriate method to distinguish $^{131}I$ concentration in the public water.

The Changes of Serum TSH in Various States of Thyroid Function (갑상선(甲狀腺) 기능(機能)상태에 따른 혈청(血淸) 갑상선자극(甲狀腺刺戟)홀몬의 변동(變動)에 관(關)한 연구(硏究))

  • The serum concentrations of thyrotropin (TSH) were measured by means of radioimmunoassay, in 98 cases of normal controls, 51 cases of hyperthyroidism, 80 cases of primary hypothyroidism and 4 cases of secondary hypothyroidism to evaluate the diagnostic significance in various functional states of the thyroid. The obtained data were analyzed in correlation with other thyroid function test values in various phases of the functional thyroid diseases. The results were as follows: 1) The serum TSH concentration in normal control group was $<1.3{\sim}8.0{\mu}U/ml$. 2) The measurement of serum TSH was more significant in diagnostic accuracy compared with that of serum $T_4(75.0{\pm}12.2%)$. Free $T_4$ Index ($64.2{\pm}15.2%$), serum $T_3(41.0{\pm}21.0%)\;or\;T_3$ resin uptake ($41.1{\pm}15.8%$) in evaluation of primary hypothyroidism. 3) In case of overt hypothyroidism, the serum TSH and $T_4$ were both abnormal, compatible with the clinical diagnosis, while in case of preclinical or mild hypothyroidism, the serum $T_4(41.2{\pm}23.8%)\;or\;50.0{\pm}25.0%)$ was much less reliable than serum TSH. 4) In the treatment of primary hypothyroidism with desiccated thyroid, the administration of 1 grain of the hormone per day was sufficient to suppress the serum concentration of TSH to normal range. It showed that the measurement of serum TSH concentration was a significant criteria in evaluating the efficiency of the treatment of hypothyroidism. 5) The measurement of serum TSH concentration is a very significant method in the early detection of hypothyroidism induced during or after the treatment of the hyperthyroidism with antithyroid drugs or radioactive Iodine ($^{131}I$).

Suggestion of A Practical Simple Calculation Method for Safe Transportation Time after Radioactive Iodine Treatment in Patients with Thyroid Cancer (갑상선암 환자에서 방사성옥소치료 후 안전하게 이동할 수 있는 시간을 계산하기 위한 실용적인 간편계산법 제안)

  • When a patient with thyroid cancer is released from isolation after I-131 treatment and return to home using a vehicle, travel time should be controlled to reduce the amount of radiation to accompanying person. As the calculation of appropriate travel time is difficult, there is no patient-specific guideline until now. If we assume that there is no excretion and no physical decay during the relatively short travel time, calculation become quite simple; total radiation dose = dose rate ${\times}$ travel time. Results of this simple calculation and conventional calculation were compared using datum from 120 patients. Travel time calculated by simple method was 56% of conventional method in 0.3 m, 91% in 0.5 m and 96% in 1 m. Simple method was safe. It can be applied easily and also can be applied to the patients with hyperthyroidism treated by I-131.

Removal of Radioactive Ions from Contaminated Water by Ion Exchange Resin (오염된 물로부터 이온교환수지를 이용한 방사성이온 제거)

  • In this study, we used three kinds of commercially available cation, anion, and mixed-ion exchange resins to separate radioactive ions from a polluted water containing Cs, I, and other radioactive ions. The experiment was conducted at a room temperature with a batch method, and a comparative analysis on the decontamination ability of each resin for the removal of Cs and I was performed by using different quantities of resins. The concentration was analyzed using ion chromatography and the ion exchange resin product from company D showed an overall high ion exchange ability. However, for most of the experiments when the amount of ion exchange resin was decreased, the decontamination ability of the resins against mass increased. When the mass of company D's cation exchange resin was small, the ion exchange ability against Cs and I ions were measured as 0.199 and 0.344 meq/g, respectively. When the mixed ion exchange resin was used, the ion exchange ability against I ions was measured as 0.33 meq/g. All in all, company D's ion exchange resins exhibited a relatively higher ion exchange ability particularly against I ions than that of other companies' exchange ions.

Results of Radioiodine Treatment for Distant Metastases of Differentiated Thyroid Carcinoma (분화성 갑상선암의 원격 전이에서 방사성옥소 치료 성적)

  • Purpose: To evaluate the effectiveness of radioiodine treatment for metastatic thyroid carcinoma, we reviewed results of radioactive iodine treatment in patients with functional lung or bone metastases. Materials and Methods: Of 760 patients who were treated for differentiated thyroid cancer between 1984 and 1998, we detected pulmonary metastases and bone metastases in 76 patients (10.0%) and 20 patients (2.6%), respectively. Among them, we could evaluate the effectiveness of I-131 therapy in 53 patients with lung metastases and 15 patients with bone metastases. Results: Of 53 patients who received I-131 therapy with a mean cumulative dose of 26.2 GBq (1.1-84.4 GBq) for pulmonary metastases, metastatic lung lesions completely resolved in 19 patients (35.8%) and improved in 22 patients (41.5%). In 13 of 19 patients with complete remission of pulmonary metastases, the total accumulated dose of I-131 was less than 18.5 GBq. We found 43 sites of metastatic bone lesions in 15 patients with bone metastases. Of 29 lesions which received I-131 therapy, metastatic lesions improved in 14 sites (48.3%), but did not change or progress in 15 sites (51.7%) despite the I-131 therapy. Three lesions were completely cured with a combination treatment of surgery(${\pm}$ external radiotherapy) and I-131 therapy, and the other 11 lesions improved. Conclusion: Radioactive iodine treatment gives favorable results for pulmonary metastases. However, for bone metastases, there might be a need to use combination therapy including I-131 and surgery or external irradiation.

Preliminary study on the efficacy of xerostomia treatment with sialocentesis targeting thyroid disease patients given radioiodine therapy

  • Background: Radioiodine therapy has been widely used for thyroid disease patients, but hyposalivation and xerostomia may occur in 10~30% of patients. Sialocentesis is a procedure that removes inflammatory substances in the salivary duct and expands the duct for the secretion and delivery of saliva. In this study, thyroid disease patients treated with radioactive iodine were selected among the patients with xerostomia who visited the hospital, and the effect of sialocentesis was compared and analyzed. And then, comparison between the radioiodine therapy-experienced group and the non-radioiodine therapy-experienced group was conducted. Results: In this study, we studied xerostomia patients who underwent radioiodine therapy due to thyroid diseases and who underwent sialocentesis at the Korea University Anam Hospital. Sialocentesis is conducted by one surgeon. The study also compares the clinical symptoms before and after the surgery. After the procedure, the discomfort due to xerostomia was reduced, and the symptom was improved effectively. Conclusions: The results of this study showed that sialocentesis has a clinical effect in the treatment of xerostomia, which is a side effect of radioiodine therapy. In addition, the possibility of further clinical application of sialocentesis in the future is found.

The Perceptual and Consonant Analysis for the Voice with Hypothyroidism (갑상선 기능저하 음성에 대한 청지각적 및 파열음 분석에 대한 연구)

  • Background and Objectives : The main purpose of this study is to clarify perceptual and acoustic analysis for the patients with hypothyroidism after thyroidectomy especially focused on the characteristics of speech articulation with special reference to the consonant production. Materials and Methods : The subjects of the research were 40 male and female adults (males : 5, females : 35). They were all received radioactive iodine treatment which after total thyroidectomy. Voice samples were collected during the three stages of after surgery, pre-radioisotope treatment (RIT), and post-RIT. The acoustic analysis was conducted by using Pratt (ver.5.2.21) after measuring voice onset time (VOT). The subjective evaluation of the voices used CAPE-V. Results : A significant decrease in overall severity was displayed in the CAPE-V following RIT. It may be conjectured that this is connected to the change in voice following RIT. The loudness of the sound displayed a significant decrease in the CAPE-V following RIT. It is conjectured that this is connected to the decrease in vocal intensity following RIT. No statistically significant results were revealed for the comparative analysis on the voice onset time (VOT) in all plosives during the three periods. Conclusion : Perceptually, the overall severity of the voice with hypothyroidism was changed significantly before and after RIT. Eventhough VOT were not significantly changed, it tended to decrease VOT in patients with hypothyroidism.
