삼차원 뇌혈관조영술에서 테이블 높이와 확대율 조절에 따른 수정체 선량 감소에 대한 연구 (Radiation Dose Reduction of Lens by Adjusting Table Height and Magnification Ratio in 3D Cerebral Angiography)

  • Both angiography and interventional procedures accompanied by angiography provide many diagnostic and therapeutic benefits to patients and are rapidly increasing. However, unlike general radiography or computed tomography using the same X-ray, the amount of radiation is quite high, but the dose range can vary considerably for each patient and operator. The high sensitivity of the lens to radiation during cerebral angiography and neurointervention is already well known, and although there are many related studies, it is insufficient to easily reduce radiation in diagnosis and treatment. In this situation, in particular, by adding three-dimensional rotational angiography (3D-RA) to the existing two-dimensional (2D) angiography, it is now possible to make an accurate diagnosis. However, since this 3D-RA acquires images through projection of more radiation than before, the exposure dose of the lens may be higher. Therefore, we tried to analyze whether the radiation dose of the lens can be reduced by moving the lens out of the field range by adjusting the table height and magnification ratio during the examination using 3D-RA. The surface dose was measured using a rando phantom and a radiophotoluminescent glass dosimeter (PLD) and the radiation dose was compared by adjusting the table height and magnification ratio based on the central point. As a result, it was found that the radiation dose of the lens decreased as the table height increased from the central point, that is, as the lens was out of the field of view. In conclusion, in 3D-RA, moving the table position of about 2 cm in height will make a significant contribution to the dose reduction of the lens, and it was confirmed that adjusting the magnification ratio can also reduce the surface dose of the lens.

  • 흉폐부 또는 흉벽에서 발생하는 악성소세포 종양군인 Ewings sarcoma, 횡문근육종, Askin tumor, 신경아세포종, PNET, 임파종 등은 현미경학적 소견만으로는 감별하기 어렵다. 그러나 최근에는 조직세포화학적검사, 면역세포화학적검사, 세포배양, 세포유전학적 검사등의 도움으로 상기한 악성소세포 종양군들이 모두가 같은 계통의 primitive pluripotent cells로 부터 분화되어 발병되는 것으로 확인되었다. 치료는 외과적 절제술, 방사선치료, 항암요법 등이 시도되고 있으나 예후는 재발 과원격전이로 인하여 불량한 것으로 보고되고 있다. 본 저자들은 본원에서 치료한 예를 보고하면서 흉폐부에서 발생하는 악성소세포 종양군의 조직발생과 감별진단에 대하여 논하고자 한다.

흉부 X선촬영 조건에 따르는 의료피폭에 관한 조사연구 (A Survey on Medical Radiation Dose by the Radiographic Conditions of Chest)

  • It is a matter of common knowledge that madical radiation is most accented for of radiation is doses applied to the whole of people, and of them the radation dose by radiography diagnosis is mainly prevalent. In applying X-rays to a certain man for radiography diagnosis a radiologyist will have to have an absolute sense of mission concerning the reduction and prevention of the patient's radiation dose as the radiologyist obligation. Accordingly, the radiography conditions of the patient's chest employed 197 medical facilites were surveyed and skin dose was computated by the IPH Bit system and examined. As a result, it was shown that the average skin dose was $288\;{\mu}Sv$, its minimum value was $1600\;{\mu}Sv$, which was over 32 times its minimum value. This shows that the appropriate radiography method has not been applied at applying X-ray to the patient. It comes from the performance of X-ray equipment, the choice of auxiliary equipment materials etc. But the most important thing is to master the appropriate radiography condition, and therefore this point will have to be kept in mind.

방사선검사 시 의료방사선 안전성에 대한 인식도 조사 (Research of Awareness for Medical Radiation Safety in Radiography)

  • Most patients and parents and guardians display frequent anxiety due to radiation exposure during outpatient, ward, and pediatric general radiographic examinations. This is a behavior that perceives only the harmfulness of radiation. For the recognition of medical radiation, we conduct surveys on outpatients, inpatients, and pediatric parents and guardians to identify their awareness, and then use the radiation dose promotional materials After providing accurate information on the use of radiation, the outpatient, inpatient, and pediatric parents and guardians were asked to explain the change in awareness. The questionnaire items were classified into five categories: repetitive radiation awareness for diagnosis, awareness of exposure dose, availability of exposure information, awareness of radiation risk, and awareness of health problems caused by radiation. There was a statistically significant difference in the items of recognition result of medical radiation, although there was a slight difference in the individual items in the pre and post-recognition results of providing information about the radiologists of the protector and the outpatient(p<0.05). Therefore, through the installation of these promotional materials, we will improve our awareness of medical radiation safety during general radiography surveillance in the Department of Radiology to provide better quality medical information and medical services, thereby contributing to strengthening the competitiveness of the hospital.

Radiation Induced Lung Injury: Prediction, Assessment and Management

  • Radiation induced lung injury has long been considered a treatment limiting factor for patients requiring thoracic radiation. This radiation induced lung injury happens early as well as late. Radiation induced lung injury can occur in two phases viz. early (< 6 months) when it is called radiation pneumonitis and late (>6 months) when it is called radiation induced lung fibrosis. There are multiple factors that can be patient, disease or treatment related that predict the incidence and severity of radiation pneumonitis. Radiation induced damage to the type I pneumocytes is the triggering factor to initiate such reactions. Over the years, radiation therapy has witnessed a paradigm shift in radiation planning and delivery and successfully reduced the incidence of lung injury. Radiation pneumonitis is usually a diagnosis of exclusion. Steroids, ACE inhibitors and pentoxyphylline constitute the cornerstone of therapy. Radiation induced lung fibrosis is another challenging aspect. The pathophysiology of radiation fibrosis includes continuing inflammation and microvascular changes due to pro-angiogenic and profibrogenic stimuli resembling those in adult bronchiectasis. General supportive management, mobilization of airway secretions, anti-inflammatory therapy and management of acute exacerbations remains the treatment option. Radiation induced lung injury is an inevitable accompaniment of thoracic radiation.

Season of Diagnosis and Survival of Advanced Lung Cancer Cases - Any Correlation?

  • Introduction: The influence of season at diagnosis on cancer survival has been an intriguing issue for many years. Most studies have shown a possible correlation in between the seasonality and some cancer type survival. With short expected survival, lung cancer is an arena that still is in need of new prognostic factors and models. We aimed to investigate the effect of season of diagnosis on 3 months, 1 and 2 years survival rates and overall survival of non small cell lung cancer patients. Materials and Methods: The files of non small cell lung cancer patients that were stages IIIB and IV at diagnosis were reviewed retrospectively. According to diagnosis date, the patients were grouped into 4 season groups, autumn, winter, spring and summer. Results: A total of 279 advanced non small cell lung cancer patients' files were reviewed. Median overall survival was 15 months in the entire population. Overall 3 months, 1 and 2 years survival rates were 91.0%, 58.2% and 31.2% respectively. The season of diagnosis was significantly correlated with 3 months survival rates, being diagnosed in spring being associated with better survival. Also the season was significantly correlated with T stage of the disease. For 1 and 2 years survival rates and overall survival, the season of diagnosis was not significantly correlated. There was no correlation detected between season and overall survivals according to histological subtypes of non small cell lung cancer. Conclusion: As a new finding in advanced non small cell lung cancer patients, it can be concluded that being diagnosed in spring can be a favorable prognostic factor for short term survival.

Evaluation of Prognostic Factors and Survival Results in Pancreatic Carcinomas in Turkey

  • Background: The goal of this retrospective study was to evaluate patient characteristics, treatment modalities and prognostic factors in Turkish patients with pancreatic cancer. Materials and Methods: Between January 1997 and December 2012, 64 patients who presented to the Department of Radiation Oncology, Karadeniz Technical University, Faculty of Medicine with a diagnosis of pancreatic cancer were evaluated. The E/K ratio of the cases was 2.4/1 and the median age was 59.6 (32-80) years, respectively. Some 11 cases (18%) were stage 1, 21 (34.4%) were stage 2, 10 (16.4%) were stage 3, and 19 (31.1%) were metastatic. Results: The mean follow-up time was 15.7 months (0.7-117.5) and loco-regional recurrence was noted in 11 (40.7%) who underwent surgery while metastases were observed in 41 patients (66.1%). The median overall survival (OS) was 11.2 months and the 1, 3 and 5-year OS rates were 41.7%, 9.9% and 7.9% respectively. The median disease-free survival (DFS) was 5.2 month and the1, 2 and 5 year DFS were 22.6%, 7.6% and 3.8% respectively. On univariate analysis, prognostic factors affecting OS included status of the operation (p<0.001), tumor stage (p=0.008), ECOG performance status (p=0.005) and CEA level (p=0.017).On multivariate analysis, prognostic factors affecting survival included status of the operation (p=0.033) and age (p= 0.023). Conclusions: In the current study, age and operation status were independent prognostic factors for overall survival with pancreatic patients. Thus, the patients early diagnosis and treatment ars essential. However, prospective studies with more patients are needed for confirmation.

Researches of Epigenetic Epidemiology for Infections and Radiation as Carcinogen

  • In recent years, a number of studies have been reported on the various types of cancer arising from epigenetic alterations, including reports that these epigenetic alterations occur as a result of radiation exposure or infection. Thyroid cancer and breast cancer, in particular, have high cancer burden, and it has been confirmed that radiation exposure or onco-viral infection are linked to increased risk of development of these two types of cancer, respectively. Thus, the environment-epigenetic alteration-cancer occurrence (EEC) hypothesis has been suggested. This paper reviews the trends in research supporting this hypothesis for radiation exposure and onco-viral infection. If more evidences accumulate for the EEC hypothesis from future research, those findings may greatly aid in the prevention, early diagnosis, treatment, and prognosis of the thyroid cancer and breast cancer.

  • Machine Learning has introduced many solutions in data science, but its application in IoT faces significant challenges, due to the limitations in memory size and processing capability of constrained devices. In this paper we design an automatic gamma radiation detection and identification embedded system that exploits the power of TinyML in a SiPM micro radiation sensor leveraging the Edge Impulse platform. The model is trained using real gamma source data enhanced by software augmentation algorithms. Tests show high accuracy in real time processing. This design has promising applications in general-purpose radiation detection and identification, nuclear safety, medical diagnosis and it is also amenable for deployment in small satellites.

The Study of Opportunistic Infection in the Medical Radiation Environment

  • With the development of medical technologies and the growth of household incomes, most of the people have become to be interested in health as it leads to significant early detection and cure of diseases. However the pattern of disease becomes diverse and it makes the imaging diagnosis department crowded with many patients. Therefore the opportunistic infection could be serious there. The hospital becomes the place which provides high risks of infection danger but it is true that the opportunistic infection of the hospital employees is not still recognized well. The imaging diagnosis department has developed a lot but it becomes the medium of secondary infection of patients and employees. So this study analyzesthe status of bacteria infection of the examination table, the handle of diagnostic equipment and the hands of radiation staffs in imaging diagnosis department of the general hospitals and individual ones. And the result shows that some bacteria were found even though it's little. We are trying to make the hospitals to be free of secondary infection securing safety measure to prevent the secondaryinfection from occurring to patients and hospital employees.
