자궁경부암에서 대동맥 임파절 전이의 치료 (Radiotherapy of Para-Aortic Node Metastases in Carcinoma of the Uterine Cervix)

  • Forty one patients with para-aortic node metastases from carcinoma of the uterine cervix treated with radiotherapy at Department of Radiation Oncology, Yonsei University, College of Medicine from January 1982 to December 1987 were retrospectively analyzed. Eleven patients were diagnosed at the time of diagnosis of carcinoma of the cervix (early diagnosis) and 30 patients were diagnosed during follow up period after definitive radiotherapy of primary site (late diagnosis). The most important factors affecting the survival in this study were time of diagnosis and dose of irradiation. Overall 5 year actuarial survival rate of 41 patients was $25.7\%$. Five year survival rate for early diagnosis was $60.3\%$, but late diagnosis was $16.9\%$. And survival rate for high dose (over 4000 cGy) radiation group and low dose radiation group were $42.2\%\;and\;8.9\%$ respectively The most leading cause of death was para-aortic node failure, so early diagnosis and maximum palliation with full dose radiotherapy (over 4000 cGy) is necessary to improve the survival and the quality of life.

진단용 방사선 관련 업무 종사자의 피폭관리에 관한 연구 (A Study on the Management of Exposure of Workers and Assistants Related to Diagnostic Radiation)

  • 의료기관에는 진단용 방사선 발생장치를 취급하는 방사선사, 의사, 치과의사, 치과위생사 등 방사선 관계 종사자가 있다. 그리고 간호사, 간호조무사 등 방사선진료를 보조하거나 방사선 검사실로 환자이송 등을 하는 업무 보조자들이 있다. 방사선 관계 종사자는「의료법」 등에 의해 방사선 피폭관리가 이루어지고 있으나 방사선 진료업무 보조자 등은 이에 대한 법적 근거가 없는 실정이다. 또한 진단용 방사선 피폭관리는 의료법령에 의해 규율되고 있고, 치료용 방사선과 핵의학검사에 의한 방사선 피폭관리는「원자력안전법」의 규율을 받고 있다. 이에 진단용 방사선에 의한 피폭관리를 개선하기 위하여「의료법」 상 진단용 방사선 피폭관리에 관한 규정과「원자력안전법」 상 관련 규정들을 비교·검토하여 보았다. 그 결과로 얻은 주요 내용은 다음과 같다. 첫째, 진단용 방사선에 의한 피폭관리 대상으로 방사선 관계 종사자 외에 방사선 피폭 우려가 있는 간호사, 간호조무사, 임상실습 학생 등을 포함시켜 입법적으로 해결할 필요가 있다. 둘째,「원자력안전법」에서처럼 진단용 방사선 관계 종사자가 임신이 확인된 경우에는 피폭선량 한도를 명문으로 규정해야 한다. 셋째,「진단용 방사선 발생장치의 안전관리에 관한 규칙」의 개인피폭선량계의 종류에 관한 규정을 현실에 맞게 개정할 필요가 있다. 넷째, 방사선 관계 종사자, 방사선작업종사자와 수시출입자에 대한 건강진단의 검사항목은 동일해야 할 것으로 보인다. 다섯째, 의료기관에서 진단용 방사선뿐만 아니라 치료용 방사선과 핵의학을 포함한 의료용 방사선 전체를 하나의 법체계에서 통일하여 규율하는 것이 필요하다고 본다.

진단용 방사선 안전관리 법령의 문제점에 관한 연구 (Problems of the Act and Subordinate Statutes Related to the Regulation of Radiation Safety for Diagnosis)

  • 의료기관에서 진단용 방사선의 이용은 매년 급속히 증가하고 있다. 또한 집단 유효선량도 매년 증가하고 있다. 그러므로 국민에 대한 방사선 피폭을 최대한 적게 낮추어야 하며, 진단용 방사선 안전 관리를 효율적으로 할 수 있도록 제도적으로 확립하여야 한다. 이에 진단용 방사선 안전관리에 대한 법적 규율이 법령체계상 맞지 않거나 내용에 있어서 현실과 괴리가 없는지 문제점을 파악하고 그에 대한 개선 방향을 모색하여 다음과 같은 결과를 얻었다. 첫째, 「의료법」에는 검사·측정기관에 대한 근거 규정도 없고 행정처분에 대한 위임규정도 없다. 그러므로 「의료법」 에 검사·측정기관에 대한 근거 규정과 이들 기관들에 대한 행정처분의 근거 규정을 두어 법적 정당성을 확보해야 한다. 둘째, 진단용 방사선 특수의료장비에 대해서는 「특수의료장비의 설치 및 운영에 관한 규칙」과 「진단용 방사선 발생장치 안전관리에 관한 규칙」을 통합하여 신고 등 행정적 절차를 일원화해야 한다. 셋째, 「진단용 방사선 발생장치 안전관리에 관한 규칙」 상 진단용 방사선 안전관리기준을 위반한 경우에 행정처분 등 제재가 미비된 사항들을 보완할 필요가 있다. 넷째, 의료기관에서 이용하는 진단용 방사선과 치료용 방사선에 대하여 「의료법」과 「원자력안전법」의 이원적인 법령 체계로 규율하는 것은 진단용 방사선 안전관리에 있어서도 효율적이지 못하다. 따라서 진단용 방사선뿐만 아니라 치료용 방사선, 핵의학을 포함한 의료용 방사선 전체를 「의료법」 체계에서 통일하여 규율하는 것이 필요하다고 본다.

방사선안전을 위한 보건/비보건계열의 의료방사선 인식도 분석 (A Study on recognition for Medical radiation of health and non-health for radiation safety)

  • This study implemented the survey and analysis of the exposure to radiation by using the questionnaire targeting H Health College, located in Daejeon from September 1st, 2014 to October 15th. A total of 400 copies of the questionnaire was distributed and among them, 385 copies, excluding 15 omitted ones (total collection ratio: 96.3%), were used for the analysis. The analysis results are as follows. For the harmfulness of radiation for diagnosis, the average of the health-related was 3.15 and the average of the health-unrelated was 2.82, which the health-related students recognized the harmfulness of radiation for diagnosis higher (p<.001). The necessity of radiation for diagnosis was appeared higher from the health-related students as the average of the health-related was 3.07 and 2.52 for the average of the health-unrelated (p<.001). The recognition on the prevention of the exposure to radiation was higher from the health-related students as the average of the health-related was 3.13 and 1.84 for the average of the health-unrelated (p<.001). From this study, the necessity of using radiation from the medical field and the recognition on its harmfulness appeared to have a big difference between the health-related and health-unrelated. For such, the accurate understanding of the recognition on radiation and the education to improve recognition on radiation are considered to be required.

이동형 X선 장치 차폐도구 제작을 통한 표면선량 분포 측정 (Measurement of Skin Dose Distribution for the Mobile X-ray Unit Collimator Shielding Device)

  • Opened a court in February 10, 2006, a rule of safety management of the diagnosis radiation system was promulgated for safety of the radiation worker, patients and patients' family members. The purpose of this rule is to minimize the risk of being exposed to radiation during the process of handling X-ray. For this reason, we manufactured shielding device of mobile X-ray unit collimator for diminution of skin dose. Shielding device is made to a thickness of Pb 0.375mm. For portable chest radiography, we measured skin dose 50cm from center ray to 200cm at intervals of 20cm by Unfors Xi detector. As a result, a rule of safety management of the diagnosis radiation system has been strengthened. But there are exceptions, such as ER, OR, ICU to this rule. So shielding device could contribute to protect unnecessary radiation exposure and improve nation's health.

진단용 방사선발생장치의 검사기준을 적용한 동물병원의 방사선 안전관리 (The Radiation Safety Management in the Animal Hospital Using Inspection Standard of Diagnosis Radiation System)

  • The purpose of this study was to investigate the actual conditions of radiation safety supervision in animal clinics using inspection standard of X-ray generator for diagnosis. The surveys for inspection standard system, equipment condition, and safety supervision were carried out in 18 animal clinics randomly. The inspection standard included reproducibility of dose exposure, kVp, mAs, collimator accuracy test, collimator luminance test, X-ray view box luminance test, grounding system equipment test and external leakage current test. The surveys of equipment condition and safety supervision used one-on-one interview with 5 points measurement. As a result, 44.44% of reproducibility of dose exposure was proper, 81.25% of kVp test was good, and 100% of mAs test was appropriate. Also, 66.66% of collimator accuracy test was proper, 61.11% of collimator luminance test was good, 53.13% of X-ray view box luminance test was suitable. In addition, only 5.55% of grounding system equipment and ground resistance was proper, 63.64% of external leakage current test was appropriate in grounding system equipment test. The 100mA electric capacity of X-ray generator for diagnosis was popular with 44.44%, and its 55.56% was purchased used equipment. Monthly average of less than 50 times (61.11%) was top frequency in use, and no animal clinic had a thermo-luminescence dosimeter(TLD). The 16 animal clinics with radiation safety zone and 2 without radiation safety zone were appeared.

원발성 접형동암 (Primary Carcinoma of the Sphenoid Sinus)

  • Four cases of sphenoid sinus carcinoma have been observed for last 10 years and we reviewed English literatures about sphenoid sinus carcinoma. The sphenoid sinus carcinoma is rare and the diagnosis is difficult. In the early stage, the non-specific deep constant headache is the only symptom but if the sinus wall is penetrated, the neuro-ophthalmologic symptoms and signs may appear. The extension of lesion is identified by radiologic imaging and the diagnosis requires direct biopsy. In case of deep constant headache combined with neuro-ophthalmologic symptoms and signs the sphenoid sinus carcinoma should be considered. Our small data reveals that the radiation treatment offers a possibility of relatively good outcome, although most of the cases are advanced already on initial diagnosis.

Fate of pulmonary nodules detected by computer-aided diagnosis and physician review on the computed tomography simulation images for hepatocellular carcinoma

  • Purpose: To investigate the frequency and clinical significance of detected incidental lung nodules found on computed tomography (CT) simulation images for hepatocellular carcinoma (HCC) using computer-aided diagnosis (CAD) and a physician review. Materials and Methods: Sixty-seven treatment-$na{\ddot{i}}ve$ HCC patients treated with transcatheter arterial chemoembolization and radiotherapy (RT) were included for the study. Portal phase of simulation CT images was used for CAD analysis and a physician review for lung nodule detection. For automated nodule detection, a commercially available CAD system was used. To assess the performance of lung nodule detection for lung metastasis, the sensitivity, negative predictive value (NPV), and positive predictive value (PPV) were calculated. Results: Forty-six patients had incidental nodules detected by CAD with a total of 109 nodules. Only 20 (18.3%) nodules were considered to be significant nodules by a physician review. The number of significant nodules detected by both of CAD or a physician review was 24 in 9 patients. Lung metastases developed in 11 of 46 patients who had any type of nodule. The sensitivities were 58.3% and 100% based on patient number and on the number of nodules, respectively. The NPVs were 91.4% and 100%, respectively. And the PPVs were 77.8% and 91.7%, respectively. Conclusion: Incidental detection of metastatic nodules was not an uncommon event. From our study, CAD could be applied to CT simulation images allowing for an increase in detection of metastatic nodules.

Lack of any Association between Season of Diagnosis and Survival of Gastric Cancer Cases in Kayseri, Turkey

  • Background: The influence of season of diagnosis on cancer survival has been an interesting issue for many years. Most studies have shown a possible association between seasonality and survival in some cancers. We aimed to investigate whether there is an association between season of diagnosis and survival in patients with gastric cancer. Materials and Methods: We reviewed retrospectively the files of 279 histologically proven gastric cancer patients. According to diagnosis date, the patients were grouped into 4 seasons of diagnosis groups, spring, summer, autumn, and winter. Results: There was no significant differences when the overall survival rates of the patients were compared according to the patients' season of diagnosis (p: 0.871). Median overall survival rates were 22.0 (14.5-29.5) months for the patients who were diagnosed in spring, 24.0 (12.4-35.6) for summer, 18.0 (9.96-26.0) for autumn and 21.0 (16.3-25.7) for winter. Median disease-free survival rates were 66.0 (44.1-68.1) months for the patients who were diagnosed in spring, 28.0 (17.0-39.0) for summer, 22.0 (0-46.4) for autumn and 23.0 (17.5-28.5) for winter. While the rate was best for the patients diagnosed in spring the differences were not statistically significant (p= 0.382). Conclusions: On the basis of the above results the season was not suggested as contributing to prognosis in gastric cancer cases in Kayseri, Turkey.

Low-Dose Abdominal CT for Evaluating Suspected Appendicitis in Adolescents and Young Adults: Review of Evidence

  • Due to its excellent diagnostic performance, CT is the mainstay of diagnostic test in adults with suspected acute appendicitis in many countries. Although debatable, extensive epidemiological studies have suggested that CT radiation is carcinogenic, at least in children and adolescents. Setting aside the debate over the carcinogenic risk of CT radiation, the value of judicious use of CT radiation cannot be overstated for the diagnosis of appendicitis, considering that appendicitis is a very common disease, and that the vast majority of patients with suspected acute appendicitis are adolescents and young adults with average life expectancies. Given the accumulated evidence justifying the use of low-dose CT (LDCT) of only 2 mSv, there is no reasonable basis to insist on using radiation dose of multi-purpose abdominal CT for the diagnosis of appendicitis, particularly in adolescents and young adults. Published data strongly suggest that LDCT is comparable to conventional dose CT in terms of clinical outcomes and diagnostic performance. In this narrative review, we will discuss such evidence for reducing CT radiation in adolescents and young adults with suspected appendicitis.
