A Study on the Reduction of Scattered Ray in Outside Radiation Field (조사야 외부의 산란선량 감소 방법에 관한 연구)

  • In this research, The way to decrease a patient's exposure dose by reducing the scattered radiation dosage outside a radiation field with an diagnosis X-ray was examined. The scattered radiation dosage reaching other parts outside the radiation field was to be reduced by attaching a self-produced $150{\times}190mm^2$ lead plate to the lower part of a collimator. When a lead plate was inserted additionally and the scattered radiation dosage of the X axis was measured in the direction of the central X-ray axis, It was found out to have been decreased by 26 to 36%, and in the direction of Y axis, which was vertical direction from the central axis, The scattered radiation dosage depending on whether a lead plate was used or not displayed no large differences. These results shows that the impact of the scattered radiation by the off focus X-ray that was generated around the focus was bigger than that generated by the shutter of the collimator. Therefore it has been concluded that installing an additional lead plate in the lower part of the existing collimator can decrease the scattered radiation dosage outside a radiation field.

  • 1. Purpose : 30 patients with uterine cervical cancer who had been treated at Department of radiation Oncology of Korea university Hospital from June, 1, 1994. to December. 31, 1994. were analyzed to the radiation field accuracy using verification films. 2. Methods and Materials : 30 patients were divided by 6 groups, 5 patients respectively : Group (1) : Standard physique, supine not-fixed Group (2) : Standard physique, supine fixed Group (3) : Obesity physique, supine, not-fixed Group (4) : Obesity physique, supine, not-fixed Group (5) : Obesity physique, prone, not-fixed Group (6) : Obesity physique, prone, fixed All groups were taken verification fimls during the whole treatment period, and it compared with simulation films and portal fimls, 3. Results : This results suggest that all patients should be immovilized and especially obesity physique patients should be treated by prone position in order to reduce radiation field discrepancy. In addition, this results should help to prescribe appropriate safty margins for patients simulation.

A Study of Image Quality and Exposed Dose by Field Size Changing on CBCT (CBCT 촬영 시 조사야 조절에 따른 영상의 최적화 및 피폭선량에 관한 고찰)

  • Purpose: Modern radiation therapy technique such as IGRT has become a routine clinical practice on LINAC for decrease patient's set-up error. CBCT can be used to adjust patient set-up error and treat patient more accurately. The Purpose of this study is to evaluate field size of CBCT for improving Image quality and suggest reference date of CBCT field size. Materials and Methods: Image date were acquired using KV CBCT and Catphan phantom (Half fan and full fan mode were scanned from 2 ~16 cm, at intervals of 2 cm). Field size were categorized by Small field size (2 cm, 4 cm), Medium field size (8 cm, 10 cm), Large field size (more than 14 cm) and evaluate. To estimated the CTDi using CTDi phantom and Ion chamber. Results: CT number linearity of Small and Large field size are greater than Medium field size. Spatial resolution are not significantly different without Small field size. But half fan mode is more different than full fan mode. In full fan, except Medium field size, all field size exceed recommendation for HU uniformity. But half pan has stability for all field except Small field size. CTDi makes radical sign function graph in Medium field size. Conclusion: The worst result was given by Small field size for Image quality and practically. Medium field size can be useful to prevent patient from radiation exposure and give better Image quality. So this study recommends that Medium field size (8~10 cm) is more suitable for CBCT.

Field-in-Field Technique to Improve Dose Distribution in the Junction of the Field with Head & Neck Cancer (Field-in-Field Technique을 이용한 두경부암의 접합부위 선량개선에 관한 고찰)

  • Purpose: In treating head and neck cancer, it is very important to irradiate uniform dose on the junction of the bilateral irradiation field of the upper head and neck and the anterior irradiation field of the lower neck. In order to improve dose distribution on the junction, this study attempted to correct non uniform dose resulting from under dose and over dose using the field-in-field technique in treating the anterior irradiation field of the lower neck and to apply the technique to the treatment of head and neck cancer through comparison with conventional treatment. Materials and Methods: In order to examine dose difference between the entry point and the exit point where beam diffusion happens in bilateral irradiation on the upper head and neck, we used an anthropomorphic phantom. Computer Tomography was applied to the anthropomorphic phantom, the dose of interest points was compared in radiation treatment planning, and it was corrected by calculating the dose ratio at the junction of the lower neck. Dose distribution on the junction of the irradiated field was determined by placing low-sensitivity film on the junction of the lower neck and measuring dose distribution on the conventional bilateral irradiation of the upper head and neck and on the anterior irradiation of the lower neck. In addition, using the field-in-field technique, which takes into account beam diffusion resulting from the bilateral irradiation of the upper head and neck, we measured difference in dose distribution on the junction in the anterior irradiation of the lower neck. In order to examine the dose at interest points on the junction, we compared and analyzed the change of dose at the interest points on the anthropomorphic phantom using a thermoluminescence dosimeter. Results: In case of dose sum with the bilateral irradiation of the upper head and neck when the field-in-field technique is applied to the junction of the lower neck in radiation treatment planning, The dose of under dose areas increased by 4.7~8.65%. The dose of over dose areas also decreased by 2.75~10.45%. Moreover, in the measurement using low-sensitivity film, the dose of under dose areas increased by 11.3%, and that of over dose areas decreased by 5.3%. In the measurement of interest point dose using a thermoluminescence dosimeter, the application of the field-in-field technique corrected under dose by minimum 7.5% and maximum 17.6%. Thus, with the technique, we could improve non.uniform dose distribution. Conclusion: By applying the field-in-field technique, which takes into account beam divergence in radiation treatment planning, we could reduce cold spots and hot spots through the correction of dose on the junction and, in particular, we could correct under dose at the entry point resulting from beam divergence. This study suggests that the clinical application of the field-in-field technique may reduce the risk of lymph node metastasis caused by under dose on the cervical lymph node.

  • Purpose: Digital medical image commenced with an introduction of PACS has become more popular today in the radiation diagnosis and medical treatment and made great progress, in particular, for medical testing field, whereas it has made slow progress for radiation therapy area. In order to accommodate the current trend of digital from analog, a spherical type mechanical check device (SMCD) that is form of spherical differing from the existing form of flat or cube has been designed and tested its practicability to replace the part in mechanical check with digital image from QA operation. Materials and Methods: If the distance maintains constant between source(target) and image detector with constant distance to the center of spherical type mechanical check device(SMCD), the size will be shown as a constant image at all times regardless of its direction exposed. For the test, two accurate hemispheres are made and put together which results in a sphere of the equilateral circle. Results: It enables a variety of implementation of the existing mechanical check using digital image as follows: congruity level of radiation field and light field, size accuracy of radiation field and collimation field, gantry rotation isocenter check, collimation rotation isocenter check, room laser accuracy check, collimation rotation angle check, couch rotation angle check, and more. Conclusion: It has proved its practicability in checking isocenter congruity level as real time at the time of simultaneous rotation between gantry and couch that is applied to the non-coplanar field, which had been hard to apply as a device formed of existing flat or cube.

A Non-contact Detection Method for Smelting in Submerged Arc Furnace based on Magnetic Field Radiation

  • This paper demonstrates the key parameter detection for smelting of submerged arc furnace (SAF) based on magnetic field radiation. A magnetic field radiation model for the inner structure of SAF is established based on relative theory of electromagnetic field. A simple equipment of 3D magnetic field detection system is developed by theoretical derivation and simulation. The experiments are carried out under the environment of industrial field and AC magnetic field generated by electrode currents and molten currents in the furnace is reflected outside of the furnace. The experimental results show that the key parameters of smelting including the position of electrode tip, the length of electric arc, and the liquid level of molten bath can be achieved. The computed tomography for SAF can be realized by the detection for smelting.

Effect of skin dose by materials located in treatment field (방사선 치료 시 조사야 내에 위치할 수 있는 이물질이 체표선량에 미치는 영향)

  • Purpose : In radiotherapy, various materials are used to located in treatment field unintentionally. It increases the dose delivered to the skin by interactions of the X-ray within the materials and occurs unwanted skin reaction.(due to the dose build-up effect) This aim of the this study is to measure the increase in skin dose when 13 materials are located in treatment field. Methods : Photon beam measurements were made using an plane-parallel chamber (Markus, PTW-Freiburg) in a polystyrene phantom. skin dose were measured using various overlaying 13 materials. a fixed geometry of a $10{\times}10cm$ field, a SSD=100cm and photon energy 4MV on Varian CLINAC 600C accelerator were used for all measurements. Results : There is an increase in skin dose for all materials($16.4{\sim}160.1\%$). As a percentage of maximum dose, the lowest skin dose were measured for the underwear with silk($43.2\%$) and the highest were measured for the 100m1 fluid-bag($96.6\%$) Conclusion : There is a significant increase in skin dose with 13 materials in the treatment field. a significant increase in skin dose can occur which could produce unwanted skin reaction. considerations for placement of 13 materials to be outside the treatment field whenever possible should be used to keep skin dose to a minimum level.

Study on the Improvement of the Radiation Work Field Classification System in Republic of Korea (국내 방사선종사자 피폭 분류체계 개선에 관한 연구)

  • Occupational exposure records are subject of global interest, and analysis of radiation workers in work categories is being conducted. In Rep. of Korea, according to relevant ministries, the MOHW(Ministry of Health and Welfare), the MAFRA(Ministry of Agriculture, Food and Rural Affairs), and the NSSC(Nuclear Safety and Security Commission) collect and analyze records of occupational exposure by dividing them into 11 work categories. However, this classification system lacks consistency with the systems of major countries, including the UNSCEAR(United Nations Scientific Committee on the Effects of Atomic Radiation). The domestic radiation work field classification system does not have clear classification criteria and does not reflect the characteristics of the radiation work field. Through the analysis of the classification system of the UNSCEAR, we suggested the five main categories(nuclear cycle, medical, industrial, others(education/research, military/public) field and several sub-categories according to each radiation work field.

Economic Scale of Radiation Application in Japan

  • The economic scale of nuclear application is a good indicator to show how the radiation technology is useful and contribute to improve public welfare and living standard. Recent research in Japan shows that the economic scale of nuclear field was 4,112 B¥ for radiation application(46%) and 4,741 B¥ for nuclear energy (54%) playing a role of "two wheels of one cart" in nuclear field and the total 8,853 B¥ constitutes 1.8% of gross domestic products (GDP). The radiation application consisted of 2,295 B¥ (56%) in industry (semiconductor, sterilization, nondestructive testing, radiation processing of tires, etc.), 1,538 B¥ (37%) in medicine (therapy and diagnosis such as X-ray, nuclear medicine, computed tomography, etc.) and 279 B¥ (7%) in agriculture (mutation breeding, food irradiation, sterile insect technique, etc.). Radiation application by ${\gamma}$-ray, electron beam and ion beam is steadily increasing in Japan.

A Trial of 6 MV Linear Accelerator Radiation Therapy (RT) for Breast Cancer (6 MV 선형가속기를 사용한 유방암 치료)

  • Radiation Therapy(RT) has been used in the treatment of breast cancer for over 80 years. Technically, it should include a part or all of such areas as chest wall or breast, axilla, internal mammary nodes(IM) and supraclavicular nodes (SCL). Authors tried three-field technique for the treatment of breast cancer using 6 MV linear accelerator, exclusively the department of Radiology, Kang-Nam St. Mary's Hospital, at Catholic Medical College. The field junction was checked by a phantom study and radiation doses measured by film densitometry and TLD. The 3 fields we used in this study were two isocentric opposing tangential fields encompassing the breast, chest wall and occasionally IM and one single anterior field encompassing the axilla and SCL. Using appropriate beam blocks and blouses, we were able to avoid unwanted intrinsic divergency of photon beam. Blocking also enabled us to set-up precise radiation field with ease.
