Radical Radiotherapy for Carcinoma of the Prostate (전립선암의 근치적 방사선치료)

  • Purpose : To evaluate effect and tolerance of external beam radiotherapy for carcinoma of the prostate and define the optimal radiotherapeutic regimen. Materials and methods : We retrospectively analyzed the records of 60 patients with prostate cancer who were treated with external beam radiotherapy with curative intent in our institution between September, 1987 and March, 2000. Histologic diagnosis was established by transurethral resection or ultrasonography guided biopsy. The major presenting symptoms were a nodule at routine prostatic examination and frequency and urgency of urination, along with dysuria. The median age was 63 years with range of 51 to 87 years. There were 6 patients in Stage A, 20 in Stage 8, 26 in Stage C, and 8 in Stage Dl. All patients were treated with megavoltage equipment producing 10 MV photons. The 4 field pelvic brick technique was used to a dose of 45 Gy or 50.4 Gy at 1.8 Gy per day in 5 to 6 weeks, after which a small boost field was delivered 2.0 Gy per day to a total dose of 66 to 70 Gy. The follow-up period ranged from 1 to 8 years. Results : Actuarial 5-year and 7-year survival rates for Stage A, B, C, and D1 were $100\%\;and\;84\%$, $83\%\;and\;72\%$, $67\%\;and\;54\%$, and $v$, respectively. The corresponding 5-year and 7-year relapse free survival rates were $84\%\;and\;84\%$, $77\%\;and\;67\%$, $48\%\;and\;40\%$, and $33\%\;and\;25\%$, respectively. Relapse free 5-year survival rates for Stage B were $80\%,\;80\%,\;and\;50\%$ for well, moderately, and poorly differentiated tumors, respectively. These were $64\%,\;44\%,\;and\;33\%$ for Stage C, respectively. The local control rates at 5 years were $84\%,\;85\%,\;78\%,\;and\;60\%$ for Stage A, B, C, and D1, respectively. Mild to moderate complications were observed in $22\%$ of patients. Severe complications requiring surgical procedures were documented in only $3\%$ of patients. Conclusion : This study confirms that external beam irradiation is an effective and safe treatment for prostatic cancer, providing long-term local control and good survival with acceptable complications.

Evaluation of Image Quality in Micro-CT System Using Constrained Total Variation (TV) Minimization (Micro-CT 시스템에서 제한된 조건의 Total Variation (TV) Minimization을 이용한 영상화질 평가)

  • The reduction of radiation dose from x-ray is a main concern in computed tomography (CT) imaging due to the side-effect of the dose on human body. Recently, the various methods for dose reduction have been studied in CT and one of the method is a iterative reconstruction based on total variation (TV) minimization at few-views data. In this paper, we evaluated the image quality between total variation (TV) minimization algorithm and Feldkam-Davis-kress (FDK) algorithm in micro computed tomography (CT). To evaluate the effect of TV minimization algorithm, we produced a cylindrical phantom including contrast media, water, air inserts. We can acquire maximum 400 projection views per rotation of the x-ray tube and detector. 20, 50, 90, 180 projection data were chosen for evaluating the level of image restoration by TV minimization. The phantom and mouse image reconstructed with FDK algorithm at 400 projection data used as a reference image for comparing with TV minimization and FDK algorithm at few-views. Contrast-to-noise ratio (CNR), Universal quality index (UQI) were used as a image evaluation metric. When projection data are not insufficient, our results show that the image quality of reconstructed with TV minimization is similar to reconstructed image with FDK at 400 view. In the cylindrical phantom study, the CNR of TV image was 5.86, FDK image was 5.65 and FDK-reference was 5.98 at 90-views. The CNR of TV image 0.21 higher than FDK image CNR at 90-views. UQI of TV image was 0.99 and FDK image was 0.81 at 90-views. where, the number of projection is 90, the UQI of TV image 0.18 higher than FDK image at 90-views. In the mouse study UQI of TV image was 0.91, FDK was 0.83 at 90-views. the UQI of TV image 0.08 higher than FDK image at 90-views. In cylindrical phantom image and mouse image study, TV minimization algorithm shows the best performance in artifact reduction and preserving edges at few view data. Therefore, TV minimization can potentially be expected to reduce patient dose in clinics.

The Effect of Genistein on Melanin Synthesis and In vivo Whitening (제니스테인의 멜라닌 생성 억제 및 In vivo 미백 효과)

  • The effect of genistein on melanin synthesis was studied using in vitro and in vivo model. Genistein inhibited melanin synthesis in cultured melan-a cells dose dependently. Tyrosinase activity was decreased by genistein treatment in melan-a cells, but genistein did not inhibit tyrosinase directly. Genistein did not affect the expression of tyrosinase in melan-a cells. Genistein inhibited the activity of ${\alpha}$-glucosidase in virtro and the glycosylation of tyrosinase in melan-a cells. The resulting unsaturated glycosylation of tyrosinase makes it unstable and disturb correct transportation. To further clarify the effect of genistein on the melanogenesis, we established UVB-induced hyperpigmentation on the shaved backs of brown guinea pigs. The animals were exposed to UVB radiation once a week for three consecutive weeks. Genistein (1 and 2%) or vehicle alone as a control were then topically applied to the hyperpigmented areas daily. Genistein showed significant lightening effect on the UVB-induced hyperpigmentation in five weeks. Depigmenting effect was prominent in 2% genistein treatment with Fontana-Masson staining. In conclusion, genistein may be a useful agent for skin whitening.

Effect of retinoic acid on the radiosensitivity of normal human oral keratinocyte (Retinoic acid가 사람 정상 구강각화세포의 방사선감수성에 미치는 영향에 관한 연구)

  • Purpose : To evaluate the effect of all-trans-retinoic acid (ATRA) on the radiosensitivity of normal human oral keratinocyte (NHOK). Materials and methods: Relative cell survival fraction including SF2 (survival fraction at 2 Gy) was calculated on the basis of colony formation assay. Data were fitted to the linear-quadratic model to establish the survival curve and calculate α and β values. Using flow cytometry at 1, 2, 3, 4, and 5 days after exposure to 2 and 10 Gy irradiation, cell cycle arrest and apoptosis were analysed. To understand the molecular mechanism of the radiosensitization of ATRA on NHOK, proteins related with apoptosis and cell cycle arrest were investigated by Western blot analysis. Results: Treatment with ATRA resulted in a significant decrease of SF2 value for NHOK from 0.63 to 0.27, and increased α and β value, indicating that ATRA increased radiosensitivity of NHOK. ATRA increased LDH significantly, but increasing irradiation dose decreased LDH, suggesting that the radiosensitizing effect of ATRA is not directly related with increasing cell necrosis by ATRA. ATRA did not induce appotosis but increased G2 arrest after 10 Gy irradiation, implying that the increased radiosensitivity of NHOK may be due to a decrease in mitosis casued by increasing G2 arrest. ATRA inhibited the reduction of p53 at 3 days after l0Gy irradiation and increased p21 at 1 day after 10 Gy irradiation. Further study is required to determine the precise relationship between this effect and the radiosensitizing effect of A TRA. Conclusion: These results suggested that ATRA increase radiosensitivity by inhibiting mitosis caused by increasing G2 arrest.

Concurrent Chemoradiotherapy in Elderly Patients with Locally Advanced Esophageal Carcinoma (고령의 국소 진행된 식도암환자에서 동시 항암화학방사선치료)

  • Purpose: The effect of concurrent chemoradiotherapy was analyzed in elderly patients when used in the treatment of locally advanced esophageal cancer. Materials and Methods: The retrospective analysis included 28 elderly patients aged 65 or older, with histopathologically confirmed squamous cell carcinoma of the esophagus, underwent concurrent chemoradiotherapy from January 2001 to July 2007. The squamous cell carcinoma disease stages included 8 patients (28.8%) in stage IIa, 10 patients (35.7%) in stage IIb, and 10 patients (35.7%) in stage III. Fractionated radiotherapy was performed with a 6 MV or 10 MV X-ray for 45~63 Gy (median: 59.4 Gy). Chemotherapy was applied concurrently with the initiation of radiotherapy. A 75 mg/$m^2$ dose of Cisplatin was intravenously administered on day 1. Further, 5-FU 1,000 mg/$m^2$ was continuously administered intravenously from days 1 to 4. This regimen was performed twice at 3-week intervals during radiotherapy. Two cycles of consolidation chemotherapy was performed after radiotherapy. Results: The follow-up period was 3~72 months (median: 19 months). The treatment responses after concurrent chemoradiotherapy included a complete response in 11 patients (39.3%), a partial response in 14 patients (50.0%), and no response in 3 patients (10.7%). The overall response rate was 89.3% (25 patients). The overall 1-, 2- and 3-year survival rates were 55.9%, 34.6% and 24.2%, respectively. The median survival time was 15 months. Two-year survival rates of patients with a complete response, partial response, and no response were 46.2%, 33.0%, and 0%, respectively. The stage and tumor response after concurrent chemoradiotherapy were statistically significant prognostic factors related with survival. No treatment-related deaths occurred in this study. Conclusion: Concurrent chemoradiotherapy is a relatively effective treatment without serious complications in elderly patients with locally-advanced esophageal cancer.

Palliative Effect of Radiation Therapy in Management of Symptomatic Osseous Metastases (골 전이암에서 고식적 방사선치료의 효과)

  • Bone metastases represent an important and frequent clinical problem in patients with advanced cancers. Especially, painful bone metastases are common features in these patients. Radiotherapy is an effective tool for palliative aim of painful metastatic osseous lesions. Various treatment results have been previously reported. The present retrospective study was aimed to evaluate the efficacy of palliative irradiation on pain relief, with the goal of selecting appropriate irradiation dose schedule. Radiotherapy consisted of 5times a week with a various fractional dose between 180 and 400cGy. The response of pain relief and the survival time after completion of radiotherapy are related to total dose and most of the patients have shown a similar response by the end of radiotherapy. The higher dose and the more aggressive multimodality treatment, the better pain control and the longer survival time.

Gamma Irradiation Effects on Conchospores of Porphyra Species 2. The Effects of High Gamma Irradiation on Germination and Growth of Conchospores of Two Varieties (김의 각포자에 대한 r-선의 조사효과 2. 두 품종의 각포자의 발아생장에 미치는 고선량 r-선의 조사효과)

  • For the fundamental studies of radiation breeding in edible marine algae, the biological effects on conchospores of Porphyra species by gamma-irradiation were examined. Two varieties, Keun-cham-gim (Porphyra tenera Kjell. form tamatsuensis Miura) and Saga No.5, were chosen for this study, and their conchospores after r-irradiation($5.0{\sim}20.0$ KR) were cultured for 50 days. The results obtained were summarized as follows. 1. Gamma-irradiation in less than the dose of 20KR did not affect germination of conchospores, and almost all spores grew into two cells germ in 24 hours after irradiation, but withering germs were gradually increased in number according to higher exposure within 5 days old culture. 2. High irradiation caused the induction of giant cells, abnormal useless growth of hold-fast, lumpish thalli and callus-like lumpy tissues. 3. The liberation of neutral spores from young germs and carpospores from mature thalli were observed on the frond exposed at $10{\sim}20$ KR irradiation. All spores were normal in division and its size. 4. The best irradiation effect on growth of Keun-cham-gim was observed at 10 KR dose, whose growth-rates were $140\%$ in wet weight and $108\%$ in mean frond area, but only $48\%$ was recorded in wet weight at 20 KR exposure. Saga No.5 were in contrast with Keun-cham-gim, and their most growth-rate was $400\%$ in wet weight ($258\%$ in frond area) at 10 KR irradiation and the worst was $20\%$ at the dose of 20 KR. 5. The withering phenomenon to death by treatment of gamma-ray presented substantial difference between two varieties. Survival rate compared with control in Keun-cham-gim was $70.7\%$ at 20 KR, but that in Saga No.5 recorded $47.0\%$ at same dose. 6. Synthesizing the results of high and low r-irradiation, it was suggested tat high r-irradiation in more than 5.0 KR inhibited conspicuously the growth of germs derived from conchospores, and about half of them withered at 15.0 KR dose or more.

Effect of UV-B on fatty Acid Composition, Lipid Peroxidation and Polyamine in Kidney Bean(Phaseolus vulgaris L.) (UV-B가 강낭콩(Phaseolus vulgaris L.)의 지방산 구성, 지질과산화 및 polyamine 함량에 미치는 영향)

  • To investigate the effects of UV-B on fatty acid composition, lipid peroxidation and biochemical defense responses of plant, kidney bean (Phaseolus vulgaris L.) was subjected to enhanced UV-B irradiation [daily dose : 0.02.(No UV-B) and 11.36 (enhanced UV-B) $kJ\;m^{-2};UV-B_{BE}$] for 3 weeks. UV-B drastically inhibited both height and dry weight of kidney bean. The content of malondialdehyde significantly increased by about 50% after 3 weeks of UV-B irradiation. The ratio of unsaturated to saturated fatty acids of kidney bean was increased by UV-B irradiation. Three major polyamines of kidney bean leaves : putrescine, spermidine and spermine, were observed. All of the polyamine contents were increased with UV-B irradiation. These results suggested that enhanced UV-B radiation caused oxidative stress on lipids and biochemical protection responses might be activated to prevent from damaging effects of oxidative stress generated by UV-B irradiation.

Some Aspects of Dietary Garlic, Selenium and Tocopherol, in the Nutrition of Animal (마늘, Se 및 비타민 E가 동물영양에 미치는 효과)

  • Tocopherol, Se and garlic powder were dieted to hatched chick breeding. The Se content of certain organs is influenced by garlic powder supply. The high Se content for the testis was a function of the vitamin E uptake. Effect of low dose of Se on the growth and survival of rat were examined under the diet of $2\;{\mu}g/ml$ of Se in drinking water either in the form of $Na_2SeO_3$ or $Na_2SeO_4$. The females were dead in early ages while the males were not influenced by dieting the selenite did not make males dying rapidly at early ages and males were less growth depressed. The previously known fact that garlic act as a tonics may be attributable to its high contents of Se and sulfur-containing amino acids which are closely related to vitamin E. Further details on the dietary mechanisms of the Se, vitamin E, and garlic powder are described in this paper.

Effect of Single or Fractionated X-Irradiation on the NP-SH and NP-SS of Rabbit Liver and Lung Tissues (단회(單回) 및 분획(分劃) X선(線) 조사(照射)가 가토(家兎)의 간(肝) 및 폐조직(肺組織)의 NP-SH 및 NP-SS에 미치는 영향(影響))

  • In view of the importance of sulfhydryl groups in producing a chemical protection against ionizing radiation, an attempt was made to evaluate the changes of intrinsic non-protein sulfhydryl (NP-SH) and non-Protein disulfide(NP-SS) of the lung and liver tissues of rabbits following the whole body X-irradiation with 900 r either in single or fractionated $(300\;r{\times}3)$ dose. NP-SH was measured by Ellman's method, and NP-SS was measured by the electrolytic reduction method described by Dohan. Experiment was performed at 1,3,5,24 and 48 hours post·irradiation, and the results were compared with the control. The results thus obtained are summarized as follows; 1) Intrinsic levels of NP-SH and NP-SS of normal rabbits were $0.77{\pm}0.10$ and $0.61{\pm}0.07\;{\mu}mol/gm$ wet weight in liver, and $0.28{\pm}0.03$ and $0.54{\pm}0.03$ in lung tissues respectively. 2) NP-SH of liver after single X-irradiation showed no significant change in general, but at 48 hours post·irradiation, it was elevated comparing with the normal value. 3) Levels of NP-SS in liver was decreased than the normal value in the irradiated groups, and the lowest level was observed at 3 hours after single X-irradiation and at 5 hours after fractionated irradiation. 4) In lung tissues, levels of NP-SS showed no significant change from the control at earlier experimental hours, but a great decrease was observed at later Part of the experiment.
