  • Objective: The shock index (SI), as a trauma triage tool, is a capable clinical indicator of hemodynamic instability and hypovolemic shock, but the conception of SI is contradictory to shock. The reverse shock index (RSI) was introduced recently, but its utility has not been sufficiently proven. Methods: This study examined the RSI utility by evaluating the procedures performed at an emergency department (ED) and the associated outcomes when the RSI is used alone or in combination with the Korean Triage and Acuity Scale (KTAS). This was a retrospective study conducted by including data of 4,789 adult trauma patients for a year. The clinical variables, procedures performed on patients, and outcomes were investigated. The median RSI was 0.9 in the RSI<1 group. Results: Patients in the RSI<1 group had a higher odds of requiring procedures at the ED and for experiencing worse outcomes: intubation (odds ratio [OR], 5.4; 95% confidence interval [CI], 2.3-13.1; P<0.001), chest tube insertion (OR, 6.5; 95% CI, 0.4-111.84; P<0.001), use of emergency drugs (OR, 3.6; 95% CI, 1.5-8.5; P<0.001), circulatory support (OR, 5.4; 95% CI, 2.3-12.9; P<0.001), intensive care unit admission (OR, 3.5; 95% CI, 1.8-6.8; P<0.001), and mortality during the ED stay (OR, 20.4; 95% CI, 5.5-75.7; P<0.001). In the group with KTAS 1-3, trends similar to those in the RSI<1 group were observed. Patients with RSI<1 had more severe injuries and poorer outcomes than those with $RSI{\geq}1$, regardless of whether the RSI was used alone or in combination with KTAS. Conclusion: RSI can provide an appropriate triage with concurrent KTAS use.

  • In this paper, we propose a robustness sensitivity index (RSI) of highway networks to analyze the effect of congestion in a specific section on the entire highway. The newly proposed RSI is defined as the change in the total mileage of the transportation network per extended unit length when the length of a particular section is extended. When the RSI value is large, traffic congestion in the section has a worse effect on the entire network than in other sections. The existing network robustness index (NRI) simply observes changes in transportation networks with and without specific sections, but the RSI proposed in this study is a kind of performance indicator that allows quantitative analysis of the ripple effect of the entire network according to the degree of congestion in a specific section. While changing the degree of congestion in a particular section, it is possible to calculate how the traffic volume increases, decreases, and the size and location of the congestion section change. This analysis proves the superiority of RSI as it cannot be analyzed with NRI. Various properties of RSI are analyzed using data from the domestic highway network. In addition, using the RSI concept, it is shown that the ripple effect on other sections in which a change in the degree of congestion of a specific section occurs can be analyzed.

  • This study tries to measure the direct and indirect effects of the Regional Strategic Industry (RSI) promotion projects in Chungbuk Province in Korea. In specific, it critically examines whether there exists policy consistency and connectivity between the hardware-oriented Stage I (2002~2007) and the software-centered Stage II (2008~2012) RSI promotion projects. Applying System Dynamics (SD) techniques, this study examines complex system characteristics of RSI promotion projects, all of which have been derived from the causal and stock-flow models and their simulated results. Major findings are as follows: Firstly, 'the continuous investment' is regarded as the most crucial policy leverage for the strategic industry promotion and regional economic growth. Secondly, without exceptions, the RSI promotion projects should switch their evaluation criteria to performance-oriented ones. Thirdly, in selecting their subprojects, the RSI promotion projects should pay due attention to evaluating technology value and marketability. Fourthly, it should put policy priority in strengthening cluster networking and interconnectivity among projects, inevitably supporting a selective number of virtuous network systems. Fifthly, auxiliary projects such as marketing, technology aid, and knowledge-based services should not be overlooked.

  • This study investigates a rocking seismic isolation (RSI) system as a seismic protection measure against narrow-band ground-motions generated by earthquakes. Structures supported over RSIs are considered capable of reducing the lateral demands and damage of the main structural system through lifting and rocking. This lifting and rocking during earthquake activity is provided by free-standing columns. A single-degree-of-freedom (SDOF) system supported on a RSI system is subjected to narrow-band seismic motions and its response is compared to an analog system without RSI. The comparison is then extended to reinforced concrete linear frames with and without RSI; three-bay frames with 11 and 17 storeys are considered. It is found that the RSI systems significantly reduce acceleration and displacement demands in the main structural frames, more noticeably if the first structural mode dominates the response and for ratios of the predominant frequency of the ground motion to the predominant frequency of the main frame near one. It is also found that the RSI system is more effective in reducing lateral accelerations and displacements of the main structure when the aspect ratio, b/h, and size, R, of the free-standing columns decrease, although the rocking stability of the RSI system is also reduced.

  • Background and Objectives : Proton pump inhibitors(PPIs) improve the symptoms of laryngopharyngeal reflux(LPR). But there is little reports about the changes of each items in reflux symptom index (RSI) and reflux finding score (RFS) after PPIstreatment. The purpose of this study is to analyze the changes of pre- and post-treatment score in each RSI and RFS items after 8 weeks medication with proton pump inhibitors in laryngopharyngeal reflux patients. Methods : Prospective study. Among the patients who had visited the department of otolaryngology from January 2007 to December 2008, 91 patients who had shown scores greater than 13 on the RSI and/or 7 on the RFS were studied. All patients received PPIs once daily before breakfast for 8 weeks. RSI and RFS were assessed at initial, four weeks and eight weeks after medication. Result: All RSI items were improved (p < 0.05). The globus sense followed by throat clearing, heartburns and hoarseness showed high initial RSI score than other items. And globus sense, throat clearing, hoarseness and heartburn were improved significantly more than others items. But only posterior commissure hypertrophy of RFS was improved significantly more than others items. Conclusion: Empiric PPIs therapy reduced the RSI scores and more effective for symptoms such as globus sense, throat clearing, hoarseness and heartburn among suspected LPR patients. In RFS, only posterior commissure hypertrophy has improved significantly. However, the changes of each categories of RFS were minimal (average: 0.16), therefore clinical significance is restricted in RFS.

  • This research proposes a novel trading method based on sample entropy for the FTSE China A50 Index. The approach is used to determine the points at which the index should be bought and sold for various holding durations. The findings are then compared to three other trading strategies: buying and holding the index for the entire time period, using the Relative Strength Index (RSI), and using the Moving Average Convergence Divergence (MACD) as buying/selling signaling tools. The unique entropy trading method, which used 90-day holding periods and was called StEn(90), produced the highest cumulative return: 25.66 percent. Regular buy and hold, RSI, and MACD were all outperformed by this strategy. In fact, when applied to the same time periods, RSI and MACD had negative returns for the FTSE China A50 Index. Regular purchase and hold yielded a 6% positive return, whereas RSI yielded a 28.56 percent negative return and MACD yielded a 33.33 percent negative return.

  • The preparation of Streptococcus faecalis RSI is used as a medicinal preparation for human intestinal disorders. But the microbe in this preparation is very sensitive to rifampicin. If this preparation is taken with rifampicin, its therapeutic effect can not be expected. To develope rifampicin resistant mutants, the rifampicin sensitive strain S. faecalis RSI was treated with Nmethyl-N'-nitro-N-nitrosoguanidine(MNNG). Twelve strains of the MNNG-induced mutants showed distinct resistance to rifampicin and five mutants were selected for further studies. They also exhibited identical characteristics with the parent S. faecalis RSI when they were tested for lactic acid formation and growth inhibition of E. coli. From in vitro test, it was identified that rifampicin is not inactivated by certain factors of the rifampicin resistant mutants. Conclusively, the rifampicin resistant mutants are efficient strains that have insensitivity against rifampicin and original biochemical characteristics of the parent strain.

  • Research was carried out to ascertain whether or not the volume of saliva flowing into the rumen regulates dry forage intake in ruminants. Goats with a parotid fistula were fed roughly crushed alfalfa hay cubes, concentrated beef cattle feed and $NaHCO_3$ wice daily (10:00-12:00, 16:00-18:00). Except for the days on which experiments were conducted, the animals were free access to drinking water. The animals were intraruminally infused every day prior to the morning feeding period with parotid saliva collected from the parotid fistula over a 24 h period. The present experiment consisted of three treatments, non-infusion (NI), intraruminal infusion of parotid saliva (RSI), and intraruminal infusion of warm water (RWI). In the RSI treatment, approximately 4-5 kg of parotid saliva (280-290 mOsm/l) collected over a 24 h period was intraruminally infused 1 h prior to the commencement of morning feeding. In the RWI treatment, parotid saliva was substituted for warm water ($36^{\circ}C$). After infusions, the animals were fed on roughly crushed alfalfa hay cubes for 2 h. During feeding, eating and saliva secretion rates were measured. Blood samples were also periodically collected from the jugular vein. After 2 h feeding, water intake was measured for 30 min. These measurements were used to define thirst levels. On the day of the experiment, the animals were not access to drinking water during the morning feeding. It is thought that rumen fill in RSI and RWI treatments was higher than the NI treatment. In comparison with the NI treatment however, cumulative feed intake increased by 39.3% with RSI treatment and by 45.9% with RWI treatment after completion of the 2 h feeding period. After 2 h feeding, thirst level in the RSI treatment showed only a 10% decrease compared to the NI treatment, but thirst level in the RWI treatment decreased 49.8%. Despite the significant differences in thirst levels between RSI and RWI treatments, the cumulative feed intake in both treatments was similar. When comparing accumulated saliva secretion volumes 2 h after feeding, volumes in the RSI treatment were significantly 35.9% lower than the NI treatment while volumes in the RWI treatment were unchanged. However, the volumes of saliva and fluid flowing into the rumen were greater in both RSI and RWI treatments when compared to the NI treatment. The results indicate that the amount of saliva flowing into the rumen is a factor regulating feed intake in ruminants fed on dry forage.

  • To investigate the diagnostic accuracy and applicability of barium enema(BE) and rectal suction biopsy with acetyl cholinesterase(AChE) histochemistry in the diagnosis of neonatal Hirschsprung's disease(HD), we retrospectively reviewed the findings of BE and AChE staining in 96 neonates with suspected HD during a 10-year period from January 1991 to December 2000. Sixty-nine cases of HD(58 males and 11 females) and 27 cases of non-HD are included in this study. In regard to BE, HD was based on definite transitional zone, suspicious HD on reversed rectosigmoid index(RSI <1), and non-HD on normal RSI(RSI>1). The histochemical criterion used for the diagnosis of HD was that of Chow et al(1977), i.e., the presence of many coarse discrete cholinergic nerve fibers in the muscularis mucosae and in the immediately subjacent submucosa regardless of infiltration of cholinergic nerve fibers in the lamina propria. Of 66 neonates with HD who underwent BE, transitional zone was identified in 33 cases(50 %) and reversed RSI in 19 cases(21 %), microcolon in 4 cases and normal finding in 10 cases(15 %) while of 27 neonates with non-HD, there was normal finding in 16 cases and reversed RSI in 9 cases(41 %). Thus diagnostic accuracy based on transitional zone was 64 %. The positive predictive value of reversed RSI for the diagnosis of HD was 68 %. Of 42 neonates with HD who underwent AChE histochemistry, there were 41 AChE-positive reactions and one AChE-negative reaction in a neonate with total colonic aganglionosis, while of 27 cases of non-HD, there were one equivocal AChE-positive reaction and 26 AChE-negative reactions. Thus AChE histochemical study showed a 97 % diagnostic accuracy with a 98 % sensitivity and a 96 % specificity. In conclusion, we believe that BE is valuable as a first diagnostic step since about 80 % of neonates with HD show significant radiologic findings such as a transitional zone or reversed RSI. AChE histochemical study was a more reliable diagnos tic tool showing a 97 % diagnostic accuracy, and is part.
