A Clinical Study on Patients with Traumatic Rupture of Intervertebral Disc Treated by Oriental Medicine Treatment in Combination with Soyeom Pharmacopuncture Therapy (추간판 외상성 파열 환자에 대한 소염약침 병행 치험 2례)

  • Objectives: This study is to observe the effect of oriental medicine treatment combined with Soyeom pharmacopuncture therapy on traumatic rupture of intervertebral disc patients caused by traffic accidents. Methods: The patients who have traumatic rupture in lumbar(L)-spine(case 1) and cervical(C)-spine(case 2) were treated by oriental medicine treatment in combination with Soyeom pharmacopunture therapy. VAS(visual analog scale), ROM(range of motion), and physical exam were checked on a daily basis while NDI(neck disability index) and ODI(Oswestry disability index) were examined for 3 times at 7 day interval. Results: 1. Fer 8-9 days from the admission day, the sole use of oriental medicine treatment did not make many improvements in the case of two patients' symptoms. After combining with Soyeom pharmacopuncture therapy, chief complaints were improved significantly. 2. In case 1, lumbago decreased from VAS 10 to VAS 5 and lumbar ROM got better than before. ODl score cropped from 42 to 27. 3. In case 2, left shoulder pain and nuchal pain lowered from VAS 10 to VAS 4, left upper limb numbness and weakness were improved, The patient showed nearly fun ROM. NDI score decreased from 26 to 19. Conclusions: Oriental medicine treatment in combination with Soyeom pharmacopuncture therapy is proved to be helpful to improve the symptoms of the traumatic rupture of intervertebral else patients caused by traffic accident.

Effects of Cervical Stretching Program on Pain, Fatigue and Range of Motion of Nursing College Students Overusing Smartphones (경부스트레칭프로그램이 스마트폰 과다 사용하는 간호대학생의 경부통증, 피로, 관절운동범위에 미치는 효과)

  • The purpose of this study was to evaluate the effect of cervical stretching program on neck pain, fatigue and range of motion caused by overuse of smartphone. Methods: We treated stretching program to measure the pain, fatigue, and range of motion of the neck before and after cervical stretching program for experiment group (n=25), control group(n=27) nursing college students using the smartphone. The general characteristics of the subjects and the range of neck pain, fatigue, and cervical range of motion were analyzed using descriptive statistics. The results of the cervical stretching program for neck pain, fatigue, and range of motion of the neck were analyzed using independent t-test. Results: After cervical stretching program, the pain of the neck(t=2.86, p=.006), fatigue(t=5.44, p<.001). flexion(t=-2.10, p=.041), extension(t=-2.84, p=.006), right lateral flexion(t=-2.4, p=.047), left lateral flexion(t=-2.05, p=0.46) and right rotation(t=-4.48, p<.001), left rotation(t=-3.38, p=.001) range of rotation were improved. Conclusion: The results of this study suggest that a cervical stretching program can be used as an effective intervention for health problems in musculoskeletal disorders due to smartphone overuse.

The Effects of Mulligan Mobilization with Movement and McKenize Exercise on Pain, Balance, Range of Motion in Patients with Knee Pain (무릎관절 통증 환자에서 멀리건 관절가동술과 맥켄지 운동이 통증과, 균형, 관절가동범위에 미치는 영향)

  • Background: This study aimed to compare the effects of the Mulligan mobilization with movement and McKenzie exercise after applying conservative physical therapy to patients with knee pain. Methods: Patients were randomly allocatied into two groups: the Mulligan mobilization with movement (10 subjects) and the McKenzie technique (10 subjects). Each group was givenr conservative physical therapy and manual therapy sessions, three times week, for four weeks. The pain intensity was measured using the visual analogue scale (VAS). The cervical range of motion (ROM) was measured with a goniometer. Balance was measured using the modified Berg balance scale (BBS). Results: After four weeks of therapy, VAS (p<.05) decreased significantly, and ROM and balance increased siginficantly in both groups(p<.05). There was a significant improvement in knee extension (p<.05) in the McKenzie group compared to the Mulligan group. No intergroup differences were found with respect to the knee flex, VAS, and BBS (p>.05). Conclusion: The McKenzie exercises are more effective than Mulligan mobilization with movement for improving knee extension. Both interventions have the same effects on pain relief, in increasing knee flexion ROM and improving balance in patients with knee pain.

The Effect of Mulligan Technique And TENS on cervical ROM in Persons with Neck Pain (경부통 증상자에게 Mulligan technique과 TENS 적용 후 관절가동범위에 미치는 영향)

  • Purpose : The purpose of this study was to compare Mulligan technique with Transcutaneous electrical nerve stimulation(TENS) in persons with neck pain. Methods : Twenty subjects with neck pain participated in the experiment. All subjects randomly assigned to the Mulligan technique group and TENS group. Both groups receive treatment(Mulligan technique: all areas 8 times once, TENS : 100Hz 15 minute once) 3 times during 2 weeks. And LEX was used to measure range of motion of neck. All measurement of each subject were measured at pre-treatment and post-treatment. Results : 1. All areas of range of motion of neck were significantly increased (p<0.05). 2. These data suggest that Mulligan technique and TENS are beneficial to increase all areas of range of motion of neck. 3. Comparing with two groups, Mulligan technique increases all areas of range of motion of neck more than TENS and appeared significant difference statistically. Conclusion : Mulligan technique is more effective than TENS to increse range of motion in persons with neck pain.

  • 목적 : 이 연구의 목적은 스트레칭이 노인의 경부 통증과 관절가동범위에 미치는 영향을 알아보는데 있다. 방법 : 치료에 따른 전, 후 경추 굴곡 신전 패턴을 이용한 스트레칭이 목의 관절가동범위 변화와 통증변화에 미치는 영향을 알아보기 위해 20명(남자 10명, 여자 10명)을 대상으로 치료 전, 치료 4주 후에 얻은 측정치를 대응 t-검정을 사용하여 분석하였다. 결과 : 치료 4주후 통증수치, 경추 관절 굴곡, 신전, 좌회전, 우회전 변화는 유의한 감소를 보였다. 결론 : 스트레칭이 노인 경부근막동통증후군 환자의 관절가동범위 증가와 통증 감소에 효과적이라는 것을 확인하였다.

The Effects of Aroma Therapy on the Pain, Range of Motion and the Tiredness of the 20's Female University Student with Headache (아로마 테라피가 두통을 가진 20대 여대생의 통증과 ROM 및 피로도에 미치는 영향)

  • Purpose : The purpose of this study was to exam effects of aroma therapy on the woman with headache in range of motion, results based on Pain and the tiredness index. we applied this study in female patients with headache. tiredness, stress, pain to examine effects of easing off headache, Tiredness. Methods : The study has been conducted with three different groups of "H" University in Gwangyang, the forty-five 20's female university student with headache were randomly assigned to groups each groups did a survey to evaluate their tiredness, VAS, VRS before this study and after there are three groups: 15 people gets a swedish massage by using almond oil(I Group, n= 15), 15 people got a swedish massage by using lavender oil(II Group, n=15) and 15 people got a swedish massage by using lavender oil, smalled at the scent of lavender candle(III Group, n=15). I, II, III Group common characteristics(gender, age, height, weight) were measured by using goniometer to measured flexion extension, left right flexion, left right rotation of cervical range of motion. the measured data were analyzed by using repeated ANOVA. Especially in order to investigate the effect of Aroma therapy by using lavender candle this study is progressed 20 minutes, 3 times per week for a term of 3weeks(total 9). Results : The results of this study were as follows : 1) the VAS and VRS showed significant different result. 2) in the range of motion, cervical extension between Group I and II and left right flexion between Group I and III showed quite motional difference. 3) in the tiredness, there were no significant factors after examination. Conclusion : Therefore, the results of this study demonstration that the effect of Aroma therapy by using lavender candle didn't show prominent effects.

The clinical study on one case of cervical sprain patient due to traffic accident in pregnancy (임신 중 교통사고로 인한 경추 염좌 환자 1례 증례보고)

  • Purpose : With the development of society, traffic accident(TA) rate is increasing accordingly. And the number of women in pregnancy who are damaged by TA is increasing as more women are involved in economical and social activities. TA victim in pregnancy cannot get proper treatment(Tx) in western medicine. Oriental medicine Tx is noted as alternative in treating TA victims recently. The main purpose of this clinical study is to make a survey of effective way of the oriental gynecological Tx about TA victim in pregnancy. Methods : The patient in this case, 28-year-old, was admitted for 14days(27th/May/2004 - 9th/June/2004) to cure severe neck pain and limitaion of cervical ROM(range of motion) occured from TA on the 4th week of pregnancy. This patient was treated with the acupunture, herbal medicine and physical treatment. Result and Conclusion : The symptoms of patient in this case are remarkable alleviated. And we think that the TA victims in pregnancy can be managed effectively by oriental gynecological Tx due to consider preservation of fetus and pregnant woman. After this, further study, approach and case will be needed about oriental gynecological Tx of TA victims in pregnancy.

Comparative Analysis between Total Disc Replacement and Posterior Foraminotomy for Posterolateral Soft Disc Herniation with Unilateral Radiculopathy : Clinical and Biomechanical Results of a Minimum 5 Years Follow-up

  • Objective : To compare the clinical outcomes and biomechanical effects of total disc replacement (TDR) and posterior cervical foraminotomy (PCF) and to propose relative inclusion criteria. Methods : Thirty-five patients who underwent surgery between 2006 and 2008 were included. All patients had single-level disease and only radiculopathy. The overall sagittal balance and angle and height of a functional segmental unit (FSU; upper and lower vertebral body of the operative lesion) were assessed by preoperative and follow-up radiographs. C2-7 range of motion (ROM), FSU, and the adjacent segment were also checked. Results : The clinical outcome of TDR (group A) was tended to be superior to that of PCF (group B) without statistical significance. In the group A, preoperative and postoperative upper adjacent segment level motion values were $8.6{\pm}2.3$ and $8.4{\pm}2.0$, and lower level motion values were $8.4{\pm}2.2$ and $8.3{\pm}1.9$. Preoperative and postoperative FSU heights were $37.0{\pm}2.1$ and $37.1{\pm}1.8$. In the group B, upper level adjacent segment motion values were $8.1{\pm}2.6$ and $8.2{\pm}2.8$, and lower level motion values were $6.5{\pm}3.3$ and $6.3{\pm}3.1$. FSU heights were $37.1{\pm}2.0$ and $36.2{\pm}1.8$. The postoperative FSU motion and height changes were significant (p<0.05). The patient's satisfaction rates for surgery were 88.2% in group A and 88.8% in group B. Conclusion : TDR and PCF have favorable outcomes in patients with unilateral soft disc herniation. However, patients have different biomechanical backgrounds, so the patient's biomechanical characteristics and economic status should be understood and treated using the optimal procedure.

The Effects on Flexion-Extension Craniocervical Joint by Proprioceptive Neuromuscular Facilitation with Hold-Relax Technique (고유수용성 신경근 촉진법 굴곡 신전 패턴이 두경부에 미치는 영향)

  • Purpose : To identify the effects of Hold-Relax of proprioceptive neuromuscular facilitation (PNF) on the functional improvement of patients with neck myofascial pain syndrome. Methods : The present research investigated 30 patients with neck myofascial syndrome, dividing them into a group doing Hold-Relax of proprioceptive neuromuscular facilitation. This study examined degree of recovery from neck pain by comparing their neck myofascial pain syndrome before and after the treatment, and compared to find difference in the degree of recovery from myofascial pain syndrome. Results : The results are as follows. For the Hold-Relax of proprioceptive neuromuscular facilitation(PNF), the visual analogue scale(VAS) decreased significantly for six weeks treatment, and the range of motion(ROM) of cervical vertebrae increased significantly(p<.05). Conlusion : This study suggest that Hold-Relax of proprioceptive neuromuscular facilitation have an effect on the functional improvement of patients with neck myofascial pain syndrome.

Sonographic Diagnosis of Cervical Lymph Node Metastasis in Patients with Thyroid Cancer and Comparison of European and Korean Guidelines for Stratifying the Risk of Malignant Lymph Node

  • Objective: To evaluate the ultrasonography (US) features for diagnosing metastasis in cervical lymph nodes (LNs) in patients with thyroid cancer and compare the US classification of risk of LN metastasis between European and Korean guidelines. Materials and Methods: From January 2014 to December 2018, US-guided fine-needle aspiration was performed on 836 LNs from 714 patients for the preoperative nodal staging of thyroid cancer. The US features of LNs were retrospectively reviewed for the following features: size, presence of hilum, margin, orientation, cystic change, punctate echogenic foci (PEF), large echogenic foci, eccentric cortical thickening, abnormal vascularity, and cortical hyperechogenicity. A multiple logistic regression analysis was performed to identify the independent US features for the diagnosis of metastatic LNs. The diagnostic performance of independent US features was subsequently evaluated. LNs were categorized according to the Korean Thyroid Imaging Reporting and Data System (K-TIRADS) and European Thyroid Association (ETA) guidelines, and the correlation between the two sets of classifications was assessed. Results: Absence of the hilum, presence of cystic changes, PEF, abnormal vascularity, and cortical hyperechogenicity were independent US features of metastatic LNs. Cystic changes, PEF, abnormal vascularity, and cortical hyperechogenicity showed high specificity (86.8%-99.6%). The absence of the hilum had the highest sensitivity yet low specificity (66.4%). When LNs were classified according to the ETA guidelines and K-TIRADS, they yielded similar categorizations of malignancy risks and were strongly correlated (Spearman coefficient, 0.9766 [95% confidence interval, 0.973-0.979]). According to the ETA guidelines, 9.8% (82/836) of LNs were classified as "not specified." Conclusion: Absence of hilum, cystic changes, PEF, abnormal vascularity, and cortical hyperechogenicity were independent US features suggestive of metastatic LNs in thyroid cancer. Both K-TIRADS and the ETA guidelines provided similar risk stratification for metastatic LNs with a high correlation; however, the ETA guidelines failed to classify 9.8% of LNs into a specific risk stratum. These results may provide a basis for revising LN classification in future guidelines.
