Influence of Transcranial Direct Current Stimulation on Lower Limb Muscle Activation and Balance Ability in Soccer Player

  • Purpose: This study is to investigate influence of tDCS on lower limb muscle activity and balance ability in soccer player. Methods: Sessions were conducted with 15 subjects in tDCS group and 15 in action observation training group for 20 minutes, 5 sessions a week, for 8 weeks. All soccer players underwent 30 minutes of plyometric training before main exercise. To evaluate lower limb muscle activation, rectus femoris and biceps femoris were taken measure using surface electromyogram system and to evaluate balance ability, surface area, whole path length, limited of stability were measured using biorescue. Results: Regarding balance shown in surface area, whole path length, limited of stability and muscle activation in rectus femoris and biceps femoris, tDCS group showed more significant change than action bservation training group. Conclusion: Therefore, intervention using tDCS is more effective in improving lower limb muscle activation and balance ability than action observation training.

슬관절 각도에 따른 교각운동이 하지근 활성도에 미치는 영향 (The Effect of Lower Extremity Muscle Activity on Bridging Exercise According to the Knee Joint Angle)

  • Purpose : The purpose of this study was to assess the effects of the lower extremity muscle activity on bridging exercise according to the knee joint angle. Methods : Twenty-five healthy adults volunteered to participate in this study. Subjects were required to complete following four bridging exercises; knee joint flexion $120^{\circ}$, $90^{\circ}$, $60^{\circ}$, $45^{\circ}$. Surface electromyography from selected lower extremity muscles was normalized to maximum voluntary isometric contraction. Muscle activity was measured by QEMG-4 system (LXM 3204, Laxtha Korea). A repeated measures of one-way ANOVA was used to determine the influence of bridging exercise on muscle activity for each muscle and descriptive statistics was used to determine muscle ratio. Results : The biceps femoris of all bridging exercises showed significantly(p<.05). The vastus medialis and lateralis of all bridging exercises showed significant excepted $120^{\circ}$(p<.05). The rectus femoris of all bridging exercises showed no significant. Median of vastus medialis/rectus femoris ratio of $120^{\circ}$ was 2.03, $90^{\circ}$ was 2.16, $60^{\circ}$ was 2.67, $45^{\circ}$ was 4.10. Median of vastus lateralis/rectus femoris ratio of $120^{\circ}$ was 1.70, $90^{\circ}$ was 1.70, $60^{\circ}$ was 2.08, $45^{\circ}$ was 2.58. Median of vastus medialis/vastus lateralis ratio of $120^{\circ}$ was 1.26, $90^{\circ}$ was 1.50, $60^{\circ}$ was 1.52, $45^{\circ}$ was 1.47. Conclusion : Angular motion decreasing with knee joint flexion made increase biceps femoris and vastus medialis activation. This result will be use knee joint stabilizing exercises during bridging or unstable surface training and biceps femoris strength training.

이침을 병행한 고유수용성신경근촉진법이 뇌졸중 환자의 다리 근활성도에 미치는 영향 (Effects of Proprioceptive Neuromuscular Facilitation Combined with Auricular Acupuncture on Activation of the Leg Muscles of Strok e Patients)

  • PURPOSE: This study analyzed how proprioceptive neuromuscular facilitation (PNF) combined with auricular acupuncture affected the activation of the leg muscles of stroke patients and measured the effects of this combination to provide clinical data. METHODS: The subjects were divided randomly into experimental group I, which received PNF combined with auricular acupuncture, and experimental group II, which received PNF alone. Each group had ten members. A 30-minute intervention was performed four days a week for six weeks. Before the six weeks of intervention, pre-tests were conducted to measure muscle activation in the legs. After six weeks, post-tests were also conducted to measure muscle activation in the legs. RESULTS: Experimental group I showed a statistically significant difference in muscle rectus femoris, muscle biceps femoris, muscle tibialis anterior, and muscle soleus. Experimental group II also showed a statistically significant difference in the muscle rectus femoris, muscle biceps femoris, muscle tibialis anterior, and muscle soleus (p < .05). In a between-group comparison of the changes, a statistically significant difference was observed between the two groups in terms of muscle rectus femoris, muscle biceps femoris, muscle tibialis anterior, and muscle soleus (p < .05). CONCLUSION: Intervention in experimental group I increased the activation of the leg muscles more effectively because auricular acupuncture was applied to various spots on the ear corresponding to the spleen, liver, kidney, pelvis, knee, ankle, and toe. Auricular acupuncture is expected to be used more widely in the future because it is a safe way of stimulating muscle activation.

구두 굽의 높이가 한다리 서기 자세에서 배곧은근, 척추세움근, 넙다리곧은근, 뒤넙다리근의 근활성도에 미치는 영향 (Effects of Height of the Shoe Heel in a Static Posture on Muscle Activity of the Rectus Abdominis, Erector Spinae, Rectus Femoris and Hamstring)

  • 본 연구의 목적은 젊은 여성을 대상으로 구두 굽 높이에 따라 한다리 서기 자세를 수행하는 동안 배곧은근, 척추세움근, 넙다리곧은근, 뒤넙다리근의 근활성도를 비교하기 위해 연구를 실시하였다. 본 연구는 건강한 20대 여성 20명을 대상으로 0cm, 3cm, 7cm 의 각각 다른 높이의 굽이 있는 구두를 착용하고 안정한 지지면 위에서 정적 균형검사 중 하나인 오른쪽 다리로 한다리 서기를 하는 동안 표면 근전도 장비를 이용하여 오른쪽 배곧은근, 척추세움근, 넙다리곧은근, 뒤넙다리근의 근활성도를 측정하여 비교 분석하였으며 연구 결과 척추세움근과 넙다리곧은근의 근활성도는 굽높이가 증가함에 따라 유의한 증가가 있었고(p<0.05), 배곧은근, 뒤넙다리근의 근활성도는 유의한 차이가 없었다(p>0.05). 구두 굽 높이의 증가는 척추세움근 및 넙다리곧은근의 근활성도를 증가시키며 이로 인한 허리 앞굽음과 골반의 앞쪽 기울임 증가는 신체분절의 위치나 무게 중심의 변화를 일으켜 허리 골반 부위 및 엉덩관절, 무릎관절에 근골격계 통증과 같은 문제를 일으킬 수 있을 것이다.

The Effects of Direction Changes on the Muscular Activity of the Lower Extremities During Seated Reaching Exercises

  • Purpose: Although multi-directional reaching exercises are commonly used clinically, the effects of specific movement directions on the muscle systems of the lower extremities have not been explored. We therefore investigated lower extremity muscle activity during reaching exercises with different sagittal and horizontal plane movements. Methods: The surface electromyography responses of the bilateral rectus femoris, tibialis anterior, peroneus longus, and gastrocnemius muscles were measured during reaching exercises in three directions in the horizontal plane (neutral, $45^{\circ}$ horizontal shoulder adduction, and $45^{\circ}$ abduction) and three directions in the sagittal plane (neutral, $120^{\circ}$ flexion, and $60^{\circ}$ flexion). A total of 20 healthy, physically active participants completed six sets of reaching exercises. Two-way repeated ANOVA was performed: body side (ipsilateral and contralateral) was set as the intra-subject factor and direction of reach as the inter-subject factor. Results: Reaching at $45^{\circ}$ horizontal shoulder adduction significantly increased the activity of the contralateral rectus femoris and gastrocnemius muscles, while $45^{\circ}$ horizontal shoulder abduction activated the ipsilateral rectus femoris and gastrocnemius muscles. The rectus femoris activity was significantly higher with reaching at a $120^{\circ}$ shoulder flexion compared to the other conditions. The gastrocnemius activity decreased significantly as the shoulder elevation angle increased from $60^{\circ}$ to $120^{\circ}$. Conclusion: Our results suggest that multi-directional reaching stimulates the lower extremity muscles depending on the movement direction. The muscles acting on two different joints responded to the changes in reaching direction, whereas the muscles acting on one joint were not activated with changes in reaching direction.

편마비 환자에게 적용된 경두개직류자극이 하지 근 활성도 및 보행능력에 미치는 영향 (Effect of Trans Cranial Directed Current Stimulus on Lower Extremity Muscle Activation and Walking Capacity for Hemiparalysis Patients)

  • Purpose: The purpose of this study is to investigate the effect of non-invasive transcranial direct current stimulation (tDCS) on muscle activity, including 10 m WT, TUG, and BBS, in hemiplegic stroke patients. Methods: This study was conducted on 42 inpatients diagnosed with hemiplegia due to stroke at hospital B in Daejeon for more than 6 months. Walking training was conducted for six weeks, five times a week for 30 minutes, with a general walking group (14 people), tDCS walking group (14 people), and tDCS (sham) walking group (14 people). Results: As a result of the study, the change in the muscle activity before and after tDCS intervention was significantly increased in the tibialis anterior muscle in the CG group. In the EG group, the erector spine (lumbar), rectus femoris, and tibialis anterior muscles significantly increased. In the SEG group, significant increases were observed in the rectus femoris and tibialis anterior muscles. Significant differences were found in the rectus femoris and tibialis anterior muscles in the comparison between groups after intervention according to tDCS application. Also, 10 m WT, TUG, and BBS were significantly increased in the CG, EG, and SEG groups after intervention, and there were significant differences in 10 m WT, TUG, and BBS in comparison between groups after intervention according to tDCS application. Conclusion: As a result, tDCS is an effective in improving the walking ability of stroke patients, and in particular, it effectively increases the muscle activity of the rectus femoris and tibialis anterior muscles, which act directly on walking, and also improves the speed and stability of walking. It is considered being an effective method to increase the gait of stroke patients by combining it with the existing gait training.

고유수용성 신경근 촉진 스트레칭 기법과 탄성 스트레칭 기법이 엉덩관절 유연성 및 근 긴장도에 미치는 영향 (The Effects of Proprioceptive Neuromuscular Facilitation Stretching and Ballistic Stretching on Hip Joint Flexibility and Muscle Tone)

  • Purpose: The purpose of this study is to compare the effects of proprioceptive neuromuscular facilitation (PNF) stretching, based on ballistic stretching and the contract-relax technique, on hip joint flexibility and muscle tone in adults with shortened rectus femoris muscles. Methods: The study involved 40 adults with shortened rectus femoris muscles, identified using the modified Thomas test. Participants were randomly divided into two groups: PNF stretching, employing the contract-relax technique, and ballistic stretching. Measurements included muscle tension, hip joint range of motion, and muscle characteristics. The rectus femoris muscle shortening effect was confirmed by the modified Thomas test, while the flexibility effect was assessed through hip joint motion range. The muscle tension effect was determined using Myoton-PRO. Results: Both stretching methods resulted in significant improvements in modified Thomas test angles and frequency, with the PNF stretching group showing notably greater changes. However, neither stretching method significantly affected decrement or stiffness measurements. These findings suggest that PNF stretching may be more effective for certain outcomes compared to ballistic stretching. Conclusion: In summary, both stretching methods positively influenced flexibility and muscle tension, with PNF stretching showing a greater impact. These findings highlight the importance of selecting the appropriate stretching technique for achieving functional improvements in muscles, which could serve as valuable indicators for preventing and treating muscle injuries in both sports and daily activities.

A Comparative of Knee Joint Instability and Muscle Activity Due to Medial or Lateral Deviation of Knee while Lunge Exercise

  • Objective: The purpose of this study was to compare the muscle activity of the vastus lateralis, vastus medialis, rectus femoris, and gluteus medius muscles according to the medial/lateral bias of the knee during lunge exercise. Design: Crossed-control group study Methods: This study recruited 20 healthy men and women in their 20s who were capable of lunge exercise and had no musculoskeletal or neurological abnormalities. All three postures were performed three times each during lunge exercise. In each posture, surface electromyography (EMG) equipment was used to measure muscle activity of the vastus lateralis, vastus medialis, rectus femoris, and gluteus medius during lunge exercise. Results: As a result of comparing lower extremity muscle activity in normal, medial and lateral knee-deviation postures according to lunge exercise, the muscle activity of the vastus medialis was 107.09±13.90% in the normal posture, 79.24±5.26% in the medial- deviation posture and 125.73±14.30% in the lateral- deviation posture, which was a statistically significant difference (p<0.05). However, In the case of the vastus lateralis, rectus femoris and gluteus medius there was no statistically significant difference in muscle activity in the medial and lateral deviation positions(p>0.05). Conclusions: As a result, it was found that the muscle activity of the vastus medialis during lunge exercise significantly increased in the lateral deviation posture compared to the normal and medial deviation postures. In the case of the vastus lateralis, rectus femoris and gluteus medius there was no significant difference in muscle activity for medial and lateral knee deviation.

Comparative Analysis of Preoperative and Postoperative Muscle Mass around Hip Joint by Computed Tomography in Patients with Hip Fracture

  • Purpose: This study was conducted in order to assess changes in hip muscles by comparing results of preoperative and postoperative computed tomography (CT) in older patients who underwent surgery for treatment of hip fracture. Materials and Methods: A total of 50 patients (aged ≥65 years) who underwent surgery for treatment of intertrochanteric fractures (25 patients) and femoral neck fractures (25 patients) between February 2013 and February 2019 and underwent preoperative and postoperative pelvic CT were enrolled in the study. The cross-sectional area, attenuation and estimates of muscle mass of the gluteus medius, gluteus minimus, iliopsoas, and rectus femoris on the uninjured side were measured. Basic patient data (sex, age, height, weight, body mass index [BMI], bone mineral density [BMD], Harris hip score [HHS], and length of follow-up) were collected from medical records. Results: No significant differences in sex, age, height, weight, BMI, BMD, HHS, and length of follow-up were observed between the two groups. No significant difference in the cross-sectional areas and attenuations of gluteus medius and gluteus minimus was observed after surgery; however, a statistically significant decrease was observed in those of iliopsoas and rectus femoris after surgery. Lower estimates with statistical significance of muscle mass of the iliopsoas and rectus femoris were observed on postoperative CT. Conclusion: Muscle mass of the hip flexor (iliopsoas, rectus femoris) showed significant decreases on postoperative CT compared with preoperative CT. Based on these findings, selective strengthening exercise for hip flexor should be beneficial in rehabilitation of hip fractures.

엉덩관절 굽힘 각도에 따른 쪼그려 앉기 운동 시 하지의 근활성도 비교 (Comparison of Low Limb Muscle Activity during Squat Exercise according to Hip Joint Flexion Angle)

  • Background: The purpose of this study was to determine if there is a difference in activation of the rectus femoris, vastus medialis oblique, vastus lateralis, semitendinosus, and biceps femoris when performing normal free squat with standinding position and free squat with $30^{\circ}$ flexed hip joint. Methods: Electromyograph surface electrodes were placed on the rectus femoris, vastus medialis oblique, vastus lateralis, semitendinosus, and biceps femoris of 19 healthy college students. The participants performed standing bilateral squats and standing bilateral squats with $30^{\circ}$ flexed hip joint with EMG measures taken upon initiation of muscle activity as confirmed by an electronic goniometer. Participants completed one trial with the EMG time measurements on each type. Results: There was a significant difference between normal squats(standing squats) and normal squats with $30^{\circ}$ flexed hip joint. The normal squat exercise was statistically higer than normal squat exercise with $30^{\circ}$ flexed trunk except for semitendinosus and biceps femoris that shown slightly high. Conclusions: As a result of this study, there were increases of muscle activity in both ways. In particular, it may be more beneficial for knee joint stabilization to perform normal squat exercise with standing position relatively.
