Background: To evaluate survival outcomes and prognostic factors for overall survival (OS) in patients with metastatic renal cell carcinoma (mRCC) who received sunitinib (SU) and pazopanib (PZ) as first-line therapy in real-world Korean clinical practice. Methods: Data of 554 patients with mRCC who received SU or PZ at eight institutions between 2012 and 2016 were retrospectively reviewed. Based on the targeted therapy, the patients were divided into SU (n = 293) or PZ (n = 261) groups, and the clinicopathological variables and survival rates of the two groups were compared. A multivariable Cox proportional hazard model was used to determine the prognostic factors for OS. Results: The median follow-up was 16.4 months (interquartile range, 8.3-31.3). Patients in the PZ group were older, and no significant difference was observed in the performance status (PS) between the two groups. In the SU group, the dose reduction rate was higher and the incidence of grade 3 toxicity was more frequent. The objective response rates were comparable between the two groups (SU, 32.1% vs. PZ, 36.4%). OS did not differ significantly between the two groups (SU, 36.5 months vs. PZ, 40.2 months; log-rank, P = 0.955). Body mass index, Eastern Cooperative Oncology Group PS > 2, synchronous metastasis, poor Heng risk criteria, and liver and bone metastases were associated with a shorter OS. Conclusion: Our real-world data of Korean patients with mRCC suggested that SU and PZ had similar efficacies as first-line therapy for mRCC. However, PZ was better tolerated than SU in Korean patients.

High-temperature mechanical behaviors of Type 316L stainless steel (SS), which is considered as one of the major structural materials of Generation-IV nuclear reactors, were investigated through the tension and creep tests at elevated temperatures. The tension tests were performed under the strain rate of 6.67×10-4 (1/s) from room temperature to 650℃, and the creep tests were conducted under different applied stresses at 550℃, 600℃, 650℃, and 700℃. The tensile behavior was investigated, and the modeling equations for tensile strengths and elongation were proposed as a function of temperature. The creep behavior was analyzed in terms of various creep equations: Norton's power law, modified Monkman-Grant relation, damage tolerance factor(λ), and Z-parameter, and the creep constants were proposed. In addition, the tested tensile and creep strengths were compared with those of RCC-MRx. Results showed that creep exponent value decreased from n=13.55 to n=7.58 with increasing temperature, λ = 6.3, and Z-parameter obeyed well a power-law form of Z=5.79E52(σ/E)9.12. RCC-MRx showed lower creep strength and marginally different in creep strain rate, compared to the tested results. Same creep deformation was operative for dislocation movement regardless of the temperatures.
Background: In clinical trials with no upper age limit, the proportion of older patients is usually small, probably reflecting the more conservative approach adopted by clinicians when treating the elderly. An exploratory analysis of elderly patients in the RECORD-1 Trial showed that patients ${\geq}$ 65 y.o. had superior median PFS than overall RECORD-1 population (5.4 months and 4.9 months, respectively). We investigated the efficacy, relative benefit and safety of Everolimus (EVE) as sequential therapy after failure of VEGFr-TKI therapy for older patients with metastatic renal cell cancer (mRCC), in daily practice. Materials and Methods: 172 consecutive IRB approved patients with mRCC (median age 65, M:F 135/37, 78% clear cell) who received salvage EVE at 39 tertiary institutions between October 2009 and August 2011 were included in this analysis. Some 31% had progressed on sunitinib, 22% on sorafenib, 1% on axitinib, 41% on sequential therapy, and 5% had received other therapy. Patients with brain metastases were not included and 95% of the patients had a ECOG (Eastern Cooperative Oncology Group) performance status (PS) of 0 or 1. Previous radiotherapy was an exclusion criterion, but prior chemotherapy was permitted. Adequate organ function and hematologic parameters were mandatory. EVE administration was approved by the institutional review board at each participating institution and signed informed consent was obtained from all patients. Results: Median time of the whole cohort to last follow-up was 3.5 months (range 0.4-15.2 months). Forty four percent were continuing to take EVE at last followup. There were 86 (50%) patients ${\geq}$ 65 y.o. and 86 (50%) <65 y.o. The percentage of patients who showed PR/SD was higher in the older group than in the younger one (5.9%/61.2% vs 1.2%/46.5%, respectively). Median survival of older patients was also significantly longer (3.5 +/- 0.31 vs 3.1 +/- 0.34, hazard ratio=0.45, CI; 0.255-0.802). Analysis using Cox regression model adjusted for gender, PS, number of metastases, site of metastases, histology, smoking history and age detected an association between age and PFS (p=0.011). The frequency of adverse events in elderly patients treated with EVE was no greater than that in younger patients, although such toxicity may have had a greater impact on their quality of life. Conclusions: Older patients should not generally be excluded from accepted therapies (mTOR inhibitors after failure of VEGFr-TKI therapy) for mRCC.
Purpose: Renal cell carcinoma (RCC) and melanoma have been considered 'radioresistant' due to the fact that they do not respond to conventionally fractionated radiation therapy. Stereotactic radiosurgery (SRS) provides high-dose radiation to a defined target volume and a limited number of studies have suggested the potential effectiveness of SRS in radioresistant histologies. We sought to determine the effectiveness of SRS for the treatment of patients with radioresistant brain metastases. Materials and Methods: We performed a retrospective review of our institutional database to identify patients with RCC or melanoma brain metastases treated with SRS. Treatment response were determined in accordance with the Response Evaluation Criteria in Solid Tumors. Results: We identified 53 radioresistant brain metastases (28% RCC and 72% melanoma) treated in 18 patients. The mean target volume and coverage was 6.2 ± 9.5 mL and 95.5% ± 2.9%, respectively. The mean prescription dose was 20 ± 4.9 Gy. Forty lesions (75%) demonstrated a complete/partial response and 13 lesions (24%) with progressive/stable disease. Smaller target volume (p < 0.001), larger SRS dose (p < 0.001), and coverage (p = 0.008) were found to be positive predictors of complete response to SRS. Conclusion: SRS is an effective management option with up to 75% response rate for radioresistant brain metastases. Tumor volume and radiation dose are predictors of response and can be used to guide the decision-making for patients with radioresistant brain metastases.

Background: Long-term survival is a problem with locally advanced and metastatic renal cell carcinomas. Sunitinib malate is an oral multitargeted tyrosine kinase inhibitor, but data on sunitinib use as a second line treatment in metastatic renal cell carcinoma (mRCC) are limited. Prognostic and predictive value of peripheral blood markers has been shown for many cancers. Materials and Methods: Efficacy and safety profiles of sunitinib after interferon alpha (IFN-${\alpha}$) were evaluated based on retrospective data for 23 patients with mRCC. Hematological parameters (neutrophils, lymphocytes, platelets, mean platelet volume, neutrophil/lymphocyte ratio, platelet/lymphocyte ratio) were recorded at the time of metastasis. It was evaluated whether hematological parameters were prognostic and predictive factors. Results: Median progression-free survival (PFS) time was 16.5 months (95%CI: 0-34.5). Median overall survival (OS) time was 25.7 months (95%CI: 10.8-40.0). Most common side effects were neutropenia (52.2%), stomatitis (26.1%) and hand-food syndrome (26.1%). PFS was found 3.13 vs 17.1 months in patients with neutrophil / lymphocyte ratio (NLR)>3 vs $NLR{\leq}3$ (p:0.012). Median OS was 6.96 vs 27.1 months in patients with NLR>3 vs $NLR{\leq}3$ (p:0.001).While 75% of patients who responded to sunitinib had $NLR{\leq}3$, in 72% of patients with no response to sunitinib NLR>3 was detected (p:0.036). The association between the Memorial Sloan-Kettering Cancer Center (MSKCC) criteria and NLR was statistically significant (p:0.022). Conclusions: Data on second line sunitinib treatment following cytokine in mRCC are limited. In our study, we observed second line sunitinib treatment following IFN-${\alpha}$ to be effective and tolerable. NLRappeared to have prognostic and predictive value.

The aim of this study is to define a correlation between spatial distribution characteristics of FFG(Functional Feeding Groups) of aquatic insects and related environmental factors in the Geum River based on the theory of RCC(River Continuum Concept). For that objective we had used SMRA(Stepwise Multiple Regression Analysis) method to analyze close relationship between the distribution of aquatic insects and the physical and chemical factors that may affect their inhabiting environment in the study area. And then, a probabilistic method named Frequency Ratio Model(FRM) and spatial analysis function of GIS were applied to produce a predictive distribution map of biota community considering their distribution characteristics according to the environmental factors as related variables. As a result of SMRA, the values of decision coefficient for factors of elevation, stream width, flow velocity, conductivity, temperature and percentage of sand showed higher than 0.5. Therefore these 6 environmental factors were considered as major factors that might affect the distribution characteristics of aquatic insects. Finally, we had calculated RMSE(Root Mean Square Error) between the predicted distribution map and prior survey database from other researches to verify the result of this study. The values of RMSE were calculated from 0.1892 to 0.4242 according to each FFG so we could find out a high reliability of this study. The results of this study might be used to develop a new estimation method for aquatic ecosystem with macro invertebrate community and also be used as preliminary data for conservation and restoration of stream habitats.

Since extensometers are used to determine the absolute deformation of foundation and abutments and all results are obtained in reference to the base rod, the accuracy of these results has been constantly a subject of debate. In this regard, locating and installing extensometers outside the range of effect zone is also another challenge. The main purpose of this paper is to investigate and modify extensometers results based on the mentioned issues. For this aim, the concrete RCC Zhaveh dam in Iran was selected as the case study. To study the results of extensometers installed in this dam, first, the 3DEC_DP 5.00 software was applied for numerical modeling. Parameters such as discontinuities, dead load and piezometric pressure in the interface of concrete and rock were considered. Next, using the results obtained from 6 extensometers in foundation and abutments and 4 clinometers in dam body, the numerical model was calibrated through back analysis method. The results indicate that the base rod is moved and is not recommended being used as the base point. In other words, because installation of base anchor outside the range of effect zone is not possible due to the operational and economic considerations, the obtained results are not accurate enough. The results indicate a considerable 2-3 mm displacement of the base rod (location of the base anchor) in reference to the real zero point location, which must be added to the base rod results.

Objective : Rathke's cleft cysts (RCCs) are nonneoplastic cysts. Most of them are asymptomatic and stable; when symptomatic, RCCs are surgically fenestrated and drained. However, the outcomes remain unclear. The authors evaluated the outcomes of RCC decompression. Methods : Between 2004 and 2019, 32 RCCs were decompressed in a single tertiary institution. The clinical characteristics, intraoperative findings, postoperative complications, and endocrinological and surgical outcomes were retrospectively reviewed. Patients who underwent sequential imaging at least twice and at least 12 months after surgery were included in the analysis. Results : Patients' mean age was 40.8±14.9 years, and 62.5% were women. The mean follow-up duration was 62.3±48.6 months. In 21 patients (65.6%), no residual cysts were identified on postoperative magnetic resonance imaging. Of the 18 patients with preoperative visual field defects, 17 (94.4%) experienced postoperative visual improvement. Postoperative complications included endocrinological deterioration in 11 patients (34.4%), permanent diabetes insipidus in 11 (34.4%), infection in four (12.5%), intrasellar hemorrhage in three (9.4%), and cerebrospinal fluid leak in two (6.3%). Follow-up images revealed cyst recurrence in nine patients (28.1%), an average of 20.4 months after surgery; in three patients, the cysts were symptomatic, and resection was repeated. Multivariable analysis revealed that postoperative endocrinological deterioration was the only independent factor associated with cyst recurrence (p=0.028; hazard ratio, 6.800). Conclusion : Our findings showed that although only cyst fenestration for decompression was performed to preserve pituitary function, more pituitary dysfunction occurred than expected. Besides, the postoperative hormonal deterioration itself acted as a risk factor for cyst recurrence. In conclusion, surgery for RCC should be more careful.

RCC consumes large quantities of natural resources like gravel stone and steel, and there is a need to investigate on an innovative material that utilizes limited quantities of natural resources but should have good mechanical strength. This study deals with the experimental investigation of strength evaluation of cementitious composites reinforced with polyolefin fibers from 0% to 2.5% (with interval of 0.5%), namely Polyolefin Fiber Reinforced Cementitious Composites (PL-FRCC) and developing statistical regression models for compressive strength, splitting-tensile strength, flexural strength and impact strength of PL-FRCC. Paired t-tests (for each PL fiber percentage 0 to 2.5%) bring out that there is significant difference in compressive and splitting-tensile strength when curing periods (3, 7, 28 days) are varied. Also, a strong relationship exists between the compressive and flexural strength of PL-FRCC. The proposed mathematical models developed in this study will be helpful to ascertain the mechanical strength of FRCC, especially, when the fiber reinforcing index is varied.

In this study, the JIP test was exploited to assess drought-tolerance of transgenic rice overexpressing OsNAC10. Two types of promoters, RCc3 (root-specific) and GOS2 (constitutive), were used to drive the transcription factor OsNAC10, a gene involved in diverse functions including stress responses. Three-month-old plants were exposed to drought for 1 week and their fluorescence kinetics was evaluated. Our results showed that drought-treated non-transgenic plants (NT) have higher fluorescence intensity at the J phase (2 ms) compared to transgenic plants, indicating a decline in electron transport beyond the reduced plastoquinone ($Q_A^-$). As manifested by negative L bands, transgenic plants also showed higher energetic connectivity and stability over NT plants under drought conditions. Also, the pool size of the end electron acceptor at the photosystem I was reduced more in NT than in transgenic plants under drought conditions. Furthermore, the transgenic plants had higher $PI_{total}$, a combined parameter that reflects all the driving forces considered in JIP test, than NT plants under drought conditions. In particular, the $PI_{total}$ of the RCc3:OsNAC10 plants was higher than that of NT plants, which was in good agreement with their differences in grain yield. Thus, the JIP test proved to be practical for evaluating drought-tolerance of transgenic plants.
