Objectives : The purpose of this survey is the development on the neck pain for acupuncture and moxibustion clinical guideline. Methods : The survey questionnaire was developed by the committee of experts who major in acupuncture & moxibustion or statistics for acupuncture clinical trial protocol development. The questionnaires were distributed via e-mail to 75 members of Korean Acupuncture & moxibustion society. 57 members completed answers, and the computerized data were analyzed by SPSS 17.0 statistical program. Results and Conclusions : 1. The first selected pattern identification on the neck pain This study shows that the meridian pattern identification was selected 35 times(61.4%), the pattern identification based on cause of disease was selected 8 times(14.0%), the visceral pattern identification was selected 7 times(12.3%), the other pattern identification was selected 4 times(7.0%), Qi blood yin yang pattern identification was selected 2 times(3.5%), according to symptoms was selected 1 time(2.4%). 2. Meridian pattern identification Small intestine meridian of hand Taeyang was used 39 times(18.1%), Large intestine meridian of hand Yangmyeong and Bladder meridian of foot Taeyang was used 34 times(15.7%), Gall-bladder meridian of foot Soyang was 32 times(14.8%), Tripple energizer meridian of hand Soyang was used 31 times(14.4%), Governor meridian was used 30 times(13.9%), Lung meridian of hand Taeeum was used 8 times(3.7%), Heart meridian of hand Soeum and Pericarduim meridian of hand Gworeum was used 4 times(1.9%). 3. Pattern identification based on cause of disease Wind-Cold-Dampness was used 31 times(17.5%), Accumulation of the collateral by Phelgm-Dampness was used 16 times(14.0%), affection by exopathogen Wind-Cold(stiff neck, sprain of cervical) was used 13 times (11.4%), Defecient-Cold was used 10 times(8.8%), affection by exopathogen Wind-Dampness was used 9 times(7.9%), Deep Invasion by Wind-Cold was used 8 times(7.0%), Wind-Cold was used 7 times (6.1%), Wind-Cold was used 6 times(5.3%), Accumulation in the Center by Phelgm-Dampness, Imparement of bou fluid by Pathogenic Heat, Wind-Heat with Dampness was used 5 times(4.4%), affection by exopathogen Wind-Dampness and Accumulation of the collateral by Wind-Cold was used 4 times(3.5%), Invasion of Dampness-Heat was used 2 times(1.8%). 4. Visceral pattern identification Rising of the Liver yang was used 16 times(41%), Yin deficiency of Liver and Kidney+pathogens was used 15 times(38.5%), Yin deficiency of Liver and Kidney was used 8 times(20.5%) on this survey.
Objectives : This trial were conducted to evaluate the efficacy and adverse events of Bofutsusho-san(BTS) and Boiogiot-tang(BOT) by oriental obesity pattern identification on obese subjects, as compared to placebo. Methods : 166 subjects(body mass index ${\geq}25kg/m^2$) were recruited and randomized to receive BT(n=55), FH(n=55) or placebo(n=56) for 8 weeks. Anthropometric factors, serum lipid, glucose, blood pressure(BP), pulse rate, resting metabolic rate and oriental obesity pattern identification questionnaire were measured at baseline and 8 weeks. Adverse events and safety outcome variables were also checked during trials. Results : The frequency of top-scored oriental obesity pattern was ordered by indigestion(食積) > stagnation of the liver qi(肝鬱) > yang deficiency(陽虛) > spleen deficiency(脾虛) > phlegm(痰飮) > blood stasis(瘀血) in subjects. BTS group significantly decreased body weight, body mass index(BMI), waist circumference(WC), body fat mass, total cholesterol and HDL-cholesterol in stagnation of the liver qi(肝鬱) and WC in indigestion(食積). BOT group showed significant decrease of body weight, BMI, WC, and body fat mass in indigestion(食積) not in deficiency(虛症). Adverse events were reported most frequently in yang deficiency(陽虛) by BT group and stagnation of the liver qi(肝鬱) by BOT group. Conclusions : Bofu-tsusho-san was effective in treating obesity with stagnation of the liver qi(肝鬱). Obesity pattern identification could be a useful diagnostic tool predicting treatment effects and adverse events.
The association of cold-heat (CH) pattern and anthropometry/body composition has been suggested in that they are related to thermoregulation. We aimed to study the association of CH pattern and anthropometry/body composition. A total of 1479 individuals aged 50-80 years were included in the study, and their CH pattern were evaluated by a self-administered questionnaire. After adjustment for age and sex, the CH score were significantly correlated with weight, BMI (body mass index), body surface area, waist-hip ratio, fat free mass, body fat mass, body cell mass, intracellular water, extracellular water, and basal metabolic rate; however, the correlation coefficients were mostly low (0.15-0.24). The selected variables for predicting CH score were various according to the methods used for variable selection; however, the adjusted R-squared of the final models were all around 0.12. Thus the most parsimonious model could be the one that includes sex and BMI. In conclusion, various anthropometry and body composition indices were associated with CH pattern. Future studies are necessary to improve the performance of the prediction model.
본 연구는 과학의 본성에 대한 관점을 개선하기 위해 개발된 활동들인 암상자 활동과 정육면체 활동을 가지고 수업을 한 후, 학생들의 관점의 변화를 알아본 것이다. 이 활동들이 과학 이론의 정의, 과학 지식의 변화가능성, 과학 가설, 이론, 법칙의 차이, 모델 형성, 그리고 실험 및 조사에서 상상력과 창의성의 필요에 대한 이해를 향상하는지를 조사하였다. 이 내용들은 10학년 과학 탐구 단원에서 강조하는 요소들이다. 4명의 학생들을 대상으로 변화를 살펴 본 결과, 2명은 수업 후 전문가적인 관점을 가진 반면, 다른 두 명은 여전히 초보적인 관점에 머물렀다. 이 연구는 암상자 활동과 정육면체 활동이 학생들의 과학의 본성을 이해하는데 도움을 줄 수 있는 잠재성을 갖고 있음을 보여주었다. 학생에 따른 차이는 수업 방법, 학생들의 기존 지식 및 동기 부여와 관련하여 설명할 수 있다. 이 활동을 통한 과학의 본성에 대한 인식의 개선은 이 활동들이 직접 과학 지식의 본성을 가르치는 데 초점을 맞추고 있기 때문이라고 보여진다. 과학 지식의 변화가능성과 실험 및 조사에서 창의력과 상상력의 필요는 거의 모든 학생들에 의해 전문가적 관점을 보이지 않은 것으로서 그 요소를 포함한 수업 자료의 개발이 필요할 것으로 보인다.
Objectives Recently the fuzzy logic is widely used in the decision making, identification, pattern recognition, optimization in various fields. In this study, we propose the fuzzy logic as the objective method of distinguishing hot and cold, the basis of diagnosis in Korean medicine. Methods We developed fuzzy inference system to distinguish whether the subjects had hot or cold. The cold and hot questionnaire of Korean traditional university textbook, the pulse rate and the DITI value of face used in the system. These three kinds of information were defined as 'fuzzy sets,' and 54 fuzzy rules were established on the basis of clinical practitioners' knowledge. The fuzzy inference was performed by using the Mamdani's method. To evaluate the usefulness of the fuzzy inference system, 200 cases of data measured in the Woosuk university hospital of oriental medicine were used to compare the determining hot, normal, cold results obtained from the experts and from the proposed system. Results As a result, 100 cases of "cold", 54 cases of "normal", and 34 cases of "hot" were matched between the experts and the proposed system. This fuzzy system showed the conformity degree of 94%(${\kappa}=0.853$). Conclusions In this study, we could express the process of distinguishing hot-cold using the fuzzy logic for objectification and quantification of hot-cold identification. This is the first study that introduce a fuzzy logic for distinguish pattern identification. The degree of the heat characteristic of the patients inferred by this system could provide a more objective basis for diagnosing the hot-cold of patients.
Objective : The aim of this study was to survey the present utilization of pattern identification(PI) in the clinical field among Korean Medicine Doctors. Method : This survey was conducted from Oct. 1 to Oct. 31, 2014 by face-to-face interview using structured questionnaire. The subjects were 400 korean medicine doctors who worked in hospitals or clinics. The questionnaire consisted of two sections - a demographics section(i.e., sex, age, clinical experience, place of work, residence and so on) and the main research section(i.e., the PI utilization rate, the utilization of PI systems, the utilization of PI in the clinical domain and in the treatment domain, the correlation between utilization PI and so on). Results : This study revealed that subjects used the PI to 66.7% of their patients on average. The utilization rate of PI in patients was significantly higher for female physicians than for male physicians, for physicians worked in clinics than for physicians who worked in hospitals and for physicians with more than 10 years clinical experience than for physicians with less than 10 years clinical experience. In the utilization of PI systems, Visceral PI was most as 92.0%. There were significant difference on the PI utilization rate according to utilization of Meridian and Collateral PI, Triple Energizer PI, Defense, Qi, Nutrient and Blood PI, Six-Meridian PI and Six Excesses PI. As a result of analysing the PI utilization rate by clinical domain and in the treatment domain, the Treatment and the Drug Treatment showed the highest values. Among utilized PI systems, the correlation coefficients between Defense, Qi, Nutrient and Blood PI and Triple Energizer PI showed the highest value, but the correlation coefficients between Sasang Constitutional Medicine with the each PI showed substantially lower value. Conclusion : The results of this study demonstrate that usage of PI was higher than usage of U code(in KCD) usage in clinical field. we suggested that additional studies on using PI and developing more appropriate standardized tool should be conducted to widen scope of PI's utilization.
Objective: The aims of this study were to analyze the deficiency-excess pattern identification (虛實辨證) and compare it to the sputum cytokines of asthma patients. Method: 50 asthma patients who met the inclusion and exclusion criteria were included in this study. They were divided into two groups: deficiency and excess syndrome groups. Sputum examinations were performed including $TNF-{\alpha}$, Interleukin (IL)-4, IL-5, IL-10, and IL-13. The Quality of Life Questionnaire for Adult Korean Asthmatics (QLQAKA), the Visual Analog Scale(VAS), and heart rate variability (HRV) were also measured. We also conducted laboratory tests, including the hematological indexes. Results: Based on the pattern identification, 50 asthma patients can be divided into two categories of groups: the deficiency syndrome group (N=24) and the excess syndrome group (N=26). In the analysis of sputum cytokines, although the $TNF-{\alpha}$, IL-4 and IL-13 were at a higher level in the deficient pattern group than in the excess pattern group, it was insignificantly different. There was a negative correlation in the analysis of QLQAKA and VAS. In the analysis of HRV, although the mean value of VLF, LF, and HF in the deficiency syndrome group was higher than in the excess syndrome group, it was insignificantly different. There was no significant difference in the hematological tests between the deficiency and the excess syndrome group. The mean value of the IgE in the blood tests was five times greater than the reference value. Conclusion: The cytokines of sputum including $TNF-{\alpha}$, IL-4, IL-5, IL-10, and IL-13 were indifferent statistically. Reinforcing the healthy and eliminating the pathogenic factors should be considered.
Objectives : As depression falls into the category of Wuljeung, Gamiguibi-Tang(Jiaweiguipitang) is the standard prescription to cure Wuljeung. This study develops a questionnaire for building the guidelines to administer Gamiguibi-Tang to depression and evaluates reliability and validity of questionnaire. Methods : With extracting the text related to depression and Gamiguibi-Tang through total 9 Korean medicine books and consulting the experts, the study selected 80 items and converted them into a questionnaire. It surveyed the neuropsychiatry professors and the medical specialists three times by Delphi method, and lastly selected 21 final items of a questionnaire. On the basis of the questionnaire, it collected total 216 samples and tested their reliability and validity. Results : 21 items all didn't reduce total Cronbach alpha coefficient and satisfied test-retest reliability. As a result of factor analysis, totally 5 factors were extracted such as mental state, sleep, accompaniment, fatigue and weakness. Finally, in comparing a depression group with a normal control group, two groups all showed meaningful difference in each 21 items' point, the sum of factor 1 to 5 items' points, and the sum of 21 items' points. Conclusions : The questionnaire on the updated depression prescription guideline of Gamiguibi-Tang satisfied both of reliability and validity. Later it can help objectifying to apply Gamiguibi-Tang to depression cure.
This study aimed to investigate heart rate variability (HRV) characteristics of cold pattern with repeated measurement data. Participants were taken from a Daejeon University cohort study from 2015 to 2018. Forty-seven of the participants studied displayed cold pattern while 23 showed signs of non-cold pattern. HRV was measured in supine position for 5 minutes at each year, and an 8-item cold pattern questionnaire was used for the diagnosis of cold pattern. SDNN (standard deviation of the NN intervals) and RMSSD (the square root of the mean squared differences of successive NN intervals) were used as time domain analysis, and TP (total power), VLF (power in very low frequency range), LF (power in low frequency range), HF (power in high frequency range), LF norm (LF power in normalized units), HF norm (HF power in normalized units) and LF/HF were used as frequency domain analysis. In the Mann-Whitney U test, LF norm, HF norm, and LF/HF showed differences between the cold pattern group and non-cold pattern group at every measurement, and in the independent t-test, the differences were also observed at three points except for the baseline (2015). In the repeated measures ANOVA, the interaction effects were not observed in all HRV parameters, but the time period effects were observed in SDNN, RMSSD, TP, VLF, LF and HF. There were significant differences between those two groups in LF norm, HF norm and LF/HF. This study suggests that LF norm, HF norm and LF/HF might be a useful indicator of cold pattern properties.
Objectives : This study was done to assess the effects of the clinical interchange between the Western Medicine and the Oriental Medicine for ischemic stroke patients. The patient outcomes include changes in neurologic function by modified NIH stoke scale, stroke pattern identification scale, and patient satisfaction, Methods : For the assessment of effects, this study was performed with 178 inpatients who had undergone the stroke care at three hospitals (W Hospital adopted western therapy, S Oriental Hospital adopted Sasang constitution medicine therapy, and H Oriental Hospital adopted mixed therapy according to a joint protocol on Western Oriental medical care) from November 1997 to December 1998. Patients were interviewed or written with self-entered questionnaire forms, and clinical data were obtained, Physicians or oriental doctors wrote clinical questionnaire forms according to the care process. Results : The patient outcomes within three hospitals at 2 stages (at admission and discharge in the modified NIH stroke scale. at admission and second weeks during admission in the stroke pattern identification scale) were found to be decreased, Especially in the results of hierarchical multiple regression analysis, the degree of improvement of modified NIH stroke scale of the stroke patients at W Hospital was significant large than it at S Oriental Hospital. Also, the degree of improvement of stroke pattern identification scale at W Hospital was significantly large than it at other two hospitals. However, the patient's satisfaction score at three hospitals wasn't significantly different. Conclusions : The result of this study suggested that the joint clinical research of Western & Oriental medical practitioners was possible even if there was a conflict between Western Medicine and Oriental Medicine. Therefore Western & Oriental medical practitioners share a mutual responsibility to apply evidence-based practice, to seek scientific empirical proof through randomized clinical trials between the multicenter.
