Objective: To assess the role of arterial spin-labeling (ASL) perfusion MRI in identifying cerebral perfusion changes after indirect revascularization in children with moyamoya disease. Materials and Methods: We included pre- and postoperative perfusion MRI data of 30 children with moyamoya disease (13 boys and 17 girls; mean age ± standard deviation, 6.3± 3.0 years) who underwent indirect revascularization between June 2016 and August 2017. Relative cerebral blood flow (rCBF) and qualitative perfusion scores for arterial transit time (ATT) effects were evaluated in the middle cerebral artery (MCA) territory on ASL perfusion MRI. The rCBF and relative time-to-peak (rTTP) values were also measured using dynamic susceptibility contrast (DSC) perfusion MRI. Each perfusion change on ASL and DSC perfusion MRI was analyzed using the paired t test. We analyzed the correlation between perfusion changes on ASL and DSC images using Spearman's correlation coefficient. Results: The ASL rCBF values improved at both the ganglionic and supraganglionic levels of the MCA territory after surgery (p = 0.040 and p = 0.003, respectively). The ATT perfusion scores also improved at both levels (p < 0.001 and p < 0.001, respectively). The rCBF and rTTP values on DSC MRI showed significant improvement at both levels of the MCA territory of the operated side (all p < 0.05). There was no significant correlation between the improvements in rCBF values on the two perfusion images (r = 0.195, p = 0.303); however, there was a correlation between the change in perfusion scores on ASL and rTTP on DSC MRI (r = 0.701, p < 0.001). Conclusion: Recognizing the effects of ATT on ASL perfusion MRI may help monitor cerebral perfusion changes and complement quantitative rCBF assessment using ASL perfusion MRI in patients with moyamoya disease after indirect revascularization.
모야모야 질병에 있어서 펄스 동맥 스핀표지 영상기법을 이용한 뇌혈류량 영상과 고식적 관류 자기공명 영상을 전대뇌동맥, 중대뇌동맥, 후대뇌동맥을 좌우 6개 영역으로 나누어 시각적 평가와 정량적 평가를 하여 펄스 동맥 스핀표지 영상기법의 유용성을 알아보고자 하였다. 시각적인 평가에서는 펄스 동맥 스핀표지 혈류량 영상과 고식적인 관류 자기공명 뇌혈류량 영상에서 관류가 감소된 범위가 자기공명 혈관조영술에서 결손된 위치와 일치하였다. 정량적인 영상 평가는 펄스 동맥 스핀표지 뇌혈류량 영상과 고식적인 관류 자기공명 뇌혈류량 영상에서 관류가 감소된 범위와 위치는 일치하였지만 혈류의 양을 추정할 수 있는 평가에서는 다소간의 차이를 보였다.
Sequential brain SPECT imaging has been used to assess the cerebral perfusion reserve(CPR) in cerebrovascular diseases(UD). We have realized parametric images of CPR using deadtime correction of gamma camera and normalized difference ratio. For the anatomical localization of CPR, the parametric images were registered to the contours of the cerebral regions using optimal threshold method, which showed to reflect the CPR more reliably and distinctively than the simple subtraction. We conclude that the quantitative estimation of CPR using normalized difference ratio image could be useflll for the diagnosis and prognostic assessment of CVD.
고양이 뇌 지방색전증을 유발한 후 자기공명 관류영상기법을 이용하여 대뇌혈류량을 정량적으로 분석하고 동적특성 변화를 구현 할 수 있는 기법을 제안한다. 고양이 20마리를 대상으로 한쪽 내경동맥에 리노레익 (n=11)을 주입하여 뇌 지방색전을 유발시켰고, 대조군으로는 이바론 입자 (n=9)를 주입하여 색전이 유발되게 하였다. 그 후 30분과 2시간에 각각 T2강조, 확산강조영상을 획득하고 가장 색전이 많이 일어난 부위에서 관류강조영상을 획득하였다. 획득한 데이터는 IDL 소프트웨어와 자체 개발한 영상처리 알고리즘을 이용하여 신호강도 곡선을 ${\Delta}R_2^*$ 곡선으로 변환한 후 적분하여 뇌혈류량을 측정하였다. 실험군에서 병변부위의 뇌혈류량은 정상부위에 비해 감소하였으며, 뇌혈류량 비는 시간경과에 따라 유의한 차이가 있었다 (P<0.005). 뇌 지방색전증의 초기에는 혈류량이 감소하였으나, 2시간에서는 30분에 비해 뇌혈류량이 다소 증가됨을 관찰 할 수 있었다. 뇌경색 발생시 조기에 자기공명확산 및 관류강조영상을 획득하여 개발한 영상처리 알고리즘을 적용하여 뇌 혈류량의 다양한 동적변화 특성 및 혈류역학적 변화를 상대적 관류도 맵으로 얻을 수 있었다.
Objective : Perfusion computed tomography (PCT) has the ability to measure quantitative value and produce maps of mean transit time (MTT), cerebral blood flow (CBF), and cerebral blood volume (CBV). We assessed cerebral hemodynamics by using these parameters and acetazolamide (ACZ) challenge for pre- and post-procedural evaluation in patients with unilateral cerebrovascular stenotic disease. Methods : Thirty patients underwent pre-procedural PCT with ACZ challenge, and 24 patients (80%) was conducted follow up PCT after angioplasty with same protocol. The mean MTT, CBF, and CBV were measured and compared in both middle cerebral arterial (MCA) territories before and after ACZ challenge. Hemispheric ratio and percent change after ACZ challenge were calculated before and after angioplasty. Results : The mean stenosis rate was 76.6%. Significant increases in MTT (32.6%, p=0.000) and significant decreases in CBF (-14.2%, p=0.000) were found in stenotic side MCA territories. After ACZ challenge, there were significant changes in MTT (37.4%, p=0.000), CBF (-13.1%, p=0.000), and CBV (-10.5%, p=0.001) in pre-procedural perfusion study. However, no significant increases were found in MTT, or decreases in CBF and CBV in post-procedural study. There were no significant changes after ACZ challenge also. In addition, the degrees of these changes (before and after ACZ challenge) were highly correlated with the stenotic degrees in pre-procedural perfusion study. Conclusion : PCT with ACZ challenge appears to be a useful tool to assess the cerebral perfusion status especially in patients with unilateral symptomatic stenotic disease.
자기공명 관류영상기법을 이용하여 고양이 뇌 지방색전증 유발한 후 대뇌혈류량을 정량적으로 분석하고 동적특성 변화를 구현하고자 하였다. 고양이 44마리를 대상으로 일측 내경동맥에 트리올레인 (n=15), 올레익 (n=9) 및 리노레익 (n=11)을 주입하여 뇌 지방색전을 유발시켰고, 대조군으로는 이바론 입자 (n=9)를 주입하여 색전이 유발되게 하였다. 그 후 30분과 2시간에 각각 T2강조, 확산강조영상을 획득하고 가장 색전이 많이 일어난 부위에서 관류강조영상을 얻었다. 획득한 데이터는 자체 개발한 영상처리 프로그램과 IDL 소프트웨어를 이용하여 신호강도 곡선을 ΔR$_2$* 곡선으로 변환한 후 적분하여 뇌혈류량을 측정하였다. 모든 군에서 병변부위의 뇌혈류량은 정상부위에 비해 감소하였다. 실험 각 군별 뇌혈류량 비는 시간경과에 따라 유의한 차이가 있었으며 (P<0.005), 2시간에서는 30분에 비해 뇌혈류량이 다소 회복되었다. 뇌 지방색전증의 초기에는 혈류량이 감소하였다. 뇌경색 발생시 조기에 DWI와 PWI을 획득하여 뇌 혈류량의 다양한 동적변화 특성 및 혈류역학적 변화에 대한 정보를 임상적으로 제공 할 수 있으리라 여겨진다.

Objective: To determine the correlation between cerebral blood flow (CBF) on arterial spin labeling (ASL) MRI and the degree of postoperative revascularization assessed on digital subtraction angiography in children with moyamoya disease (MMD). Materials and Methods: Twenty-one children (9 boys and 12 girls; mean age, 8.4 ± 3.6 years; age range, 3-16 years) with MMD who underwent both pseudocontinuous ASL MRI at 1.5T and catheter angiography before and after superficial temporal artery encephaloduroarteriosynangiosis were included in this retrospective study. The degree of revascularization in the middle cerebral artery (MCA) territory was evaluated on external carotid angiography and was graded on a 3-point scale. On ASL CBF maps, regions of interest were manually drawn over the MCA territory of the operated side at the level of the centrum semi-ovale and over the cerebellum. The normalized CBF (nCBF) was calculated by dividing the CBF of the MCA territory by the CBF of the cerebellum. Changes in nCBFs were calculated by subtracting the preoperative nCBF values from the postoperative nCBF values. The correlation between nCBF changes measured with ASL and the revascularization grade from direct angiography was evaluated. Results: The nCBF value on the operated side increased after the operation (p = 0.001). The higher the degree of revascularization, the greater the nCBF change was: poor revascularization (grade 1), -0.043 ± 0.212; fair revascularization (grade 2), 0.345 ± 0.176; good revascularization (grade 3), 0.453 ± 0.182 (p = 0.005, Jockheere-Terpstra test). The interobserver agreement was excellent for the measured CBF values of the three readers (0.91-0.97). Conclusion: The nCBF values of the MCA territory obtained from ASL MRI increased after the revascularization procedure in children with MMD, and the degree of nCBF change showed a significant correlation with the degree of collateral formation evaluated via catheter angiography.
Regional cerebral blood flow were measured in 10 patients with Parkinson's disease and 12 normal persons using $^{99m}Tc-HMPAO$ SPECT. Reconstructed images were interpreted qualitatively and were compared with those findings of CT. For the quantitative analysis, six pairs of region of interest matched with the perfusion territories of large cerebral arteries and cerebellar hemisphere were determined. From the count values, indices showing the degree of asymmetry between right and left cerebral or cerebellar hemisphere, cerebral asymmetry index (ASI) and percent index of cerebellar asymmetry (PIA), and an index showing change of each region, region to cerebellum ratio (RCR) were obtained. ASI of normal persons and patients were $0.082{\pm}0.033$ and $0.108{\pm}0.062$, respectively and PIA were $-0.4{\pm}0.7%$ and $-0.7{\pm}1.0%$, respectively, which showed no statistically significant difference between normal persons and patients. Among 10 RCR's, those of both regions of basal ganglia and both regions of anterior cerebral artery were significantly reduced. We concluded that the most significant reduction of regional cerebral blood flow in patients with Parkinson's disease was observed in the regions of basal ganglia and in the regions of anterior cerebral artery, and the degree of change in hemispheric blood flow was similar in both hemisphere.
Objectives: This study investigated alterations in regional cerebral blood flow (rCBF) in patients with transient global amnesia (TGA) using statistical parametric mapping 99 (SPM99). Methods: Noninvasive rCBF measurements using 99mTc-ethyl cysteinate dimer (ECD) SPECT were performed on 8 patients with TGA who have ongoing symptoms and 17 age matched controls. The relative rCBF maps in patients with TGA and controls were compared. Results: In patients with TGA, significant decreased rCBF was found along the L superior temporal extending to L parietal region of the brain and L thalamus. There were areas of increased rCBF in the R temporal, R frontal region and R thalamus. Conclusion: We could demonstrate decreased perfusion in left cerebral hemisphere and increased perfusion in right cerebral hemisphere in patients with TGA using SPM99. The imbalanced change of rCBF between bilateral cerebral hemisphere in patients with TGA might suggest that imbalanced neuronal activity between the bilateral hemispheres may have strong relationship to the pathogenesis of the TGA. For quantitative SPECT analysis in TGA patients, we recommend SPM99 rather than the ROI method because of its definitive advantages.
