요양병원 간호사의 간호행위와 위임실태 (Nursing Activities and Delegation in Long-term Care Settings)

  • 본 연구는 요양병원 간호사의 간호행위와 위임실태를 파악하여 의료서비스의 질향상에 기초적 자료를 제공하고자 하였다. 연구 대상자는 요양병원 3년 이상의 근무경력을 가진 간호사 140명을 대상으로 2013년 7월 26일부터 12월 23일까지 자료수집하였다. 수집된 자료는 SPSS 20.0 통계프로그램의 빈도분석, 평균과 표준편차, Independentent t-test와 One way ANOVA로 분석하였다. 본 연구결과 간호행위 위임실태로는 간호행위 위임절차의 프로토콜이 없는 경우가 프로토콜이 있는 경우보다 높게 나타났다. 이 연구를 통해 요양병원 간호사의 간호행위는 위임에 대한 명시된 기준이나 절차가 없는 상황에서는 의료서비스의 질을 떨어뜨리는 주요요인이 될 수 있으므로 간호업무위임의 법칙, 제도적 근거와 간호업무 위임행위에 대한 절차와 방법, 지침이 마련되어짐이 요구된다.

의료기관 리모델링의 법적 제문제 (Legal Issues of Medical Institution Remodeling)

  • 의료기관은 환자의 만족도 충족과 타 의료기관과의 경쟁력 향상을 위해 신축보다는 기존 병원 건물을 리모델링하게 된다. 의료기관 리모델링은 의료의 질과 환자 안전의 수준 향상을 위해 강화된 법과 제도로 인하여 점차 증가하고 있다. 다만 선행연구들은 의료기관 리모델링에 대해 건축적 요소 등 제한된 범위에서만 논의가 이루어졌다. 이에 본 연구는 의료기관 리모델링의 법적 제문제를 다각적으로 검토함으로써 향후 리모델링을 계획하는 의료기관에게 기본적인 자료를 제공하고자 한다. 의료기관 리모델링은 공사 여건의 제한으로 공사 기간이 길어지고, 환자에게 소음·분진 등의 피해를 발생시킨다. 따라서 사전에 환자의 피해를 예방하고 적정한 의료기관의 리모델링을 지원하는 방안이 필요하다. 이에 의료기관 리모델링 활성화 및 지원에 관한 법률 마련이 필요하다. 그리고 의료기관은 공사기준 자율 준수 체크리스트, 사전 방지조치 및 사후 개선조치, 병원 내 감염과 방사선 노출에 대한 대책을 마련해야 한다.

  • Today, although hospital infectious diseases are readily diagnosed, are treatable and preventable, many of these continues to be a major health problem in the developing countries, as well as the advanced nations. In the advanced countries efforts for hospital infection control has been presented but in Korea. The importance of being knowledgeble concerning hospital infection control is not much recognized yet. Presently in Korea good quality of care and services in the hospital is a main issue of discussion, therefore the subject of hospital infection control can't be over emphasized. Hospital infection control measures ranged from almost non existent to none when the pathogen transmission were not fully understood. As the knowledge of the transmission and contraction of the diseases expanded, newer and more effective procedures evolved. To be vital it is required to have good system for hospital infection control and inspection, rules and regulations and many numbers of persons with dedication. The strategy has been applied for hospital infection control standards as outlined by the centers for disease control and prevention(CDC). The hospital infection control committee is the factor to be well managed. Especially nurses are the important part of any hospital infection control program because they are the one who makes function properly. It is also required the responsibility of every employer who has employees who are exposed to blood, blood products or other potentially hospital infectious materials. Laws enacted by agencies of the federal government but the emphasis, and the demands for initiating and maintaining these control measures should be practiced on a routine and daily basis. The forgoing facts and requirements will assist us in assuring our hospital infection control program is successful.

병원모바일앱 품질요인이 이용자의 지속이용의도에 미치는 영향: 정보시스템성공모형과 기대일치모형의 통합적 접근 (The Effect of Hospital Mobile App Quality Factors on Users ' Continuous Use Intention: An Integrated Approach of Information Systems Success and Expectation-Confirmation Models)

  • 정보통신기술 기반의 '스마트병원'과 '디지털 헬스케어'가 의료분야의 화두가 되면서 병원모바일앱이 주목받고 있지만, 홍보 부족, 불안정한 시스템, 늦은 업데이트 등의 이유로 이용률은 저조한 편이다. 이러한 상황에서 병원모바일앱의 이용률을 높이기 위한 체계적인 연구가 필요하지만, 관련 연구는 다소 부족한 실정이다. 이에, 본 연구에서는 기술적 관점의 정보시스템성공모형과 인지적 관점의 기대일치모형을 통합하여, 병원모바일앱 지속이용의도에 대한 영향 메커니즘을 실증하고자 한다. 이를 위해 병원모바일앱 이용 경험이 있는 국내 성인 181명을 대상으로 온라인 설문조사를 실시하였다. 구조방정식모형 분석 결과, 대부분의 품질요인이 기대일치와 인지된 유용성 및 만족도에 유의미한 영향을 미치는 것으로 나타났으며, 기대일치는 인지된 유용성과 만족도에 유의한 영향을 주는 것으로 나타났다. 또한, 인지된 유용성과 만족도는 병원모바일앱의 지속이용의도에 유의한 영향을 미친다는 사실을 밝혀냈다. 본 연구는 기대일치모형과 정보시스템성공모형을 통합하여 의료분야, 그 가운데에서도 이용률이 저조한 병원모바일앱 사용 맥락에 적용한 연구라는 점에서 의의가 있으며, 연구결과를 통해 병원모바일앱의 이용률 증대 및 효과적·효율적인 서비스 운영을 위한 실무적 시사점을 제시하였다.

종합병원 경비업무의 발전방안에 관한 연구 (A Study on development of general hospital security service)

  • 본 연구는 종합병원 경비업무현장에서 병원 시설의 경비업무와 의료진, 환자 및 보호자 등의 내방객의 안전에까지 종합적인 경비업무를 수행 하고 있는 종합병원 경비업무 경비원들이 현재 인지하고 있는 문제점들을 바탕으로 종합병원 경비업무의 발전방안을 모색하는데 목적이 있다. 이러한 연구의 목적을 달성하기 위해 종합병원 경비업무에 6년 이상 종사하고 있는 현장전문가 9명에게 면담조사를 실시하여 자료를 분석하였다. 종합병원 경비업무를 수행하고 있는 종합병원 경비업무경비원들은 종합병원 경비업무의 발전방안을 다음과 같이 제시하였다. 첫째, 종합병원 경비업무 경비원들은 업무현장의 성격에 알맞은 신임교육 프로그램을 이수하여야 한다. 둘째, 종합병원 경비업무 경비원 직무교육도 실질적인 상황에 맞는 교육내용으로 프로그램의 개선이 필요하다. 셋째, 종합병원 경비업무 경비원들에게는 추가적으로 CS교육과 CPR 및 제세동기 사용법등의 응급상황 대처요령과 전염성 질병대처교육 등이 진행되어져야 한다. 넷째, 종합병원에서의 난동 및 폭력 행사자에 대한 대응범위가 구체적으로 명시되어져야 할 것이다. 다섯째, 경비협회에서는 종합병원경비업무의 발전을 위하여 경비원들을 위한 적극적인 프로그램 개발 및 다양한 노력이 필요하다. 여섯째, 종합병원 경비업무에 대한 인식 전환이 필요하며, 하도급에 의한 계약방식보다는 직접고용의 형태가 필요시 되어 진다.

교통사고 환자 131례에 대한 한방치료 만족도 조사 (Survey on Satisfaction of Korean Medicine Treatment in 131 Cases by Traffic Accident)

  • Objectives : We investigate to identify the satisfaction degree of patients by traffic accident(TA) for improving the quality of services and fortifying the competitiveness of Korean medical hospital on TA patients. Methods : The survey used by preceding articles was accepted for assessment of the satisfaction degree of patients in this study. All data were coded by SPSS v18.0 and analyzed by descriptive maneuver, ${\chi}^2$-test, T-test and correlation. Results : According to the survey, 84% of patients included this study were hospitalized in the Korean medical hospital within 2 weeks from TA and 92.3% were satisfied with the current medical service. The answer that Korean medicine is superior to western medicine for TA treatment was the main reason for selecting Korean medical hospital. 35.1% of patients satisfied Chuna treatment and 42.9% unsatisfied herbal medicine. There were no differences in the satisfaction degree with sex, age, education and occupation. The satisfaction degree of the current Korean medical service was significantly correlated with doctor's service, use the procedure and reuse intention. Conclusions : It is necessary to develop and to apply the customer satisfaction program for the Korean medicine hospital on TA patients.

Oncology Nurses Knowledge and Attitudes Regarding Cancer Pain Management

  • Background: Oncology nurses play a crucial role in cancer pain management and must be highly informed to ensure their effective practice in the cancer setting. The aim of this study was to determine the baseline level of knowledge and attitudes of oncology nurses regarding cancer pain management. Materials and Methods: A cross-sectional survey research design was employed. The sample comprised 58 cancer nurses working in Shahid Sadoughi hospital, Yazd, Iran. The "Nurses Knowledge and Attitudes Survey Regarding Pain" (NKAS) tool and a demographic form were utilized to ascertain the knowledge and attitudes of oncology nurses working in oncology settings. Results: The average correct response rate for oncology nurses was 66.6%, ranging from 12.1% to 94.8%. The nurses mean score on the knowledge and attitudes survey regarding pain management was 28.5%. Results revealed that the mean percentage score overall was 65.7%. Only 8.6% of nurse participants obtained a passing score of 75% or greater. Widespread knowledge deficits and poor attitudes were noted in this study, particularly regard pharmacological management of pain. Conclusions: The present study provides important information about knowledge deficits in pain management among oncology nurses and limited training regarding pain management. Our results support the universal concern of inadequate knowledge and attitudes of nurses regarding cancer pain. It is suggested educational and quality improvement initiatives in pain management could enhance nurses knowledge in the area of pain and possibly improve practice.

재원환자의 질환군에 따른 가정간호 요구도 (Needs of Home Care Nursing Services for Hospital Patients)

  • Purpose: The purpose of this study was to investigate the needs of home care nursing services in relation to the patients in hospital. Method: Subjects were 129 patients who admitted hospital and were selected through convenience sampling. Data were collected using the Home Health Care Need Assessment Questionnaire constructed by the Korea Health Industry Development Institute. Data were analyzed using SAS 8.12 program by applying percentage. Results: 1. According to the diagnosis of the subjects, the majority had cancer (25.0%), followed by musculoskeletal disease (15,6%), neuro/cerebral vascular disease (14.1%), digestive (10.9%) and respiratory disease (10.9%). 2. With regard to fundamental nursing service, subjects wanted to receive home care services for the following reasons: Problem identification and diagnosis (77.5%), vital sign check (49.6%); and intake and output measure (20.9%). 3. With regard to clinical laboratory tests, 62.8% wanted to receive blood tests, and followed by urine tests 26.4%, and wound drainage 26.4%. 4. With regard to medication and treatment service, 40.3% of the subjects wanted to receive intravenous fluid therapy, 26.4% intravenous antibiotics, and 26.4% the monitoring of fluid therapy. 5. With regard to therapeutic nursing service, 33.3% wanted to receive wound care, 26.4% ROM exercise, and 27.9% foley catheter change and care. 6. With regard to educational needs, 42.6% wanted education on infection monitoring, 41.4% on medication, and 34.9% on diet. 7. With regard to counseling needs, 65.9% wanted to receive telephone counseling about patient condition, 52.7% counseling about re-admission and 51.9% direct counseling about patient condition. In the group of injury and toxicity, and cardiovascular/circulatory diseases, 100% wanted telephone and direct counseling about the patient condition. Conclusion: Therefore, in order to improve the quality of hospital based home health care services, various factors that affect to the need of home health care should be analyzed and specified nursing care should be looked into.

병원직원의 노동조합몰입에 영향을 미치는 결정요인분석 (Determinants Influencing Labor Union Commitment of Hospital Employees)

  • The purpose of this study was to provide basic materials needed to enhance quality of organizational life by identifying the improvements of labor union management in the perspective of general hospital organization management. The subjects of this study were 428 employees in 8 Hospitals in Metro Capital including Seoul. Materials were collected from administrators, nurses and medical technicians in target hospitals from November 10 to November 30, 2006 through survey questionnaires. The main results of this study were as follows: 1. The commitment level of the subjects according to their characteristics was higher in older employees than the younger ones, large family to support than small family to support and those who had higher positions in labor union. 2. The commitment level of the subjects according to the Job and role related variables were higher those who had higher satisfaction level to their job, role conflict in all hospitals. 3. The commitment level of the subjects according to union related variables, variables jointly controlled by union and employer was statistically significant positive correlation. 4. The results of multiple regression analysis shows that formal and informal socialization, satisfaction with the labor union's were all found as important antecedents of labor union commitment. 5. The results of AMOS shows that structure characteristics of hospital, Job and manager satisfaction, socialization were statistically significant labor union satisfaction. The satisfaction level of labor union was statistically significant labor union commitment To summarize study results, the level of commitment in labor union depends on job satisfaction, managers' attitudes, union satisfaction factors, their colleagues attitudes toward union. Therefore hospital managers should have democratic and flexible attitudes toward labor union. Additionally, as formal and informal socialization is important determinant in union commitment, hospital managers should have countermeasures to enhance the colleague attitude and job satisfaction level of hospital employees. Moreover, as managerial factors of the principal of hospital influence union commitment directly, the attitudes of hospital managers toward union and transparency of hospital management should be improved.

환자안전사건으로 인한 제1의 피해자 심리사회적 지원 프로그램 개발을 위한 유사 프로그램 검토 (Review of Similar Programs for the Development of a Support Program for First Victims Due to Patient Safety Incidents)

  • Purpose:In this study, we reviewed existing victim support programs implemented in Korea to aid in the development of psychosocial support programs for patients and guardians who have experienced patient safety incidents. Methods: We reviewed similar programs: a support program for suicide survivors operated by the Korea Psychological Autopsy Center (Korea Foundation for Suicide Prevention), a family harmony program for workers in industrial accidents operated by the National Center for Forest Therapy, and the support services for crime victims provided by the Korean Crime Victims Support Association. We reviewed the contents of each website and conducted interviews with key personnel from each institution. Results: The support program for families who have experienced suicide was developed based on the suicide prevention project at the Central Psychological Autopsy Center. The family harmony program for workers who suffered industrial accidents is operated by the National Center for Forest Therapy at the behest of the Korean Workers' Compensation and Welfare Service. The Korean Crime Victims Support Association was established by the Ministry of Justice in accordance with the Crime Victim Protection Act and provides support to victims of crime. Each program was designed and implemented considering the objectives and goals, defining their recruitment plans as well as the selection criteria for their participants, and creating quality content that adequately addressed the struggles of their participants. Conclusion: The summarization of the various types of victim support programs in this study can be helpful in the future development of psychosocial support programs for victims of patient safety incidents.
